Carbamazepine is an anticonvulsant medication.

Carbamazepine is used in epilepsy to prevent grand mal and partial seizures. It is also used to treat nerve damage due to diabetes, manic depression, as well as trigeminal neuralgia, which is a condition where sufferers feel an intense stabbing pain in the face.

Each individual's dosage of Gericarb SR will vary depending on their condition and reaction to the drug.The following is a general dosage guide to Gericarb SR.

* For epilepsy the regular starting dose is 100-200 mg once or twice daily. This dose will gradually be increased or decreased to control your fits.

The usual maintenance dose is 400mg - 600mg morning and evening.

* For trigeminal neuralgia the usual starting dose is between 100 mg and 400 mg a day. This dose can slowly be increased to treat the pain. In some cases, up to 1600 mg.

When the pain goes away, your doctor will reduce the dose. After a few weeks, if the pain does not return, he may stop treatment.

Initial half-life values range from b 25-65 hours, decreasing to 12-17 hours on repeated doses.
Avoid using it in generalize non-convulsive seizures and juvenile Mýoclonic epilepsy. In Asians do check HLA b1502. You can use it in Partial simple/complex seizures, bipolar disorder,Trigeminal neuralgia.

Combining ovarian suppression and an aromatase inhibitor is an efficacious treatment method for premenopausal women with breast cancer. However, the regimen (which is given long-term) can cause some major adverse events, explained Dawn L. Hershman, MD, MS, professor of medicine and epidemiology at the Columbia University Irving Medical Center.

Several trials were published, one the SOFT trial, the other the TEXT trial. Both of them were very similar in a lot of ways, looking at premenopausal women and the effect of ovarian suppression. We discovered that there actually is a benefit to suppressing a woman's ovaries and giving her an aromatase inhibitor along with it, compared to giving tamoxifen alone.

But the biggest problem with doing that is that it's very toxic. We always think about the toxicities of chemotherapy, but the toxicities from endocrine therapy can be far worse, and they go on much longer. We give these treatment for 5, sometimes even 10 years. It's super challenging to keep people on their therapy. We've done research showing that just even with 5 years of tamoxifen, by the end of the 5 years, only about 50% of patients are taking it more than 80% of the time. The reason why people stop their medications is complicated. But one of the most common reasons why people stop taking their medications is side effects. We know that when we take a young woman and we suppress her ovarian function and we give them an aromatase inhibitor, close to 100% have side effects. It can cause depression, hot flashes, difficulty sleeping severe bowl pain, and problems with their mood. It can increase their risk for osteoporosis, and sometimes even cardiovascular disease. So it's not without significant consequences.

In order to better treat young women, we need better risk prediction models to figure out exactly who are the people at highest risk, so that we can target those women and give them more intensive treatment, and spare women who maybe don't have such high risk cancer.

Video abstract of original research paper “CYP2D6 polymorphisms may predict occurrence of adverse effects to tamoxifen: a preliminary retrospective study” published in the open access journal Breast Cancer - Targets and Therapy by authors Wickramage I, Tennekoon KH, Ariyaratne MAY.

Introduction and aims: Tamoxifen is an adjuvant drug effective in treating hormone receptor – positive breast cancer. However, 30%–50% of patients relapse and many develop adverse
effects, such as hot flashes and fatty liver. Allelic variations altering the activity of cytochrome P450-2D6 enzyme affect response to tamoxifen by modulating metabolism of tamoxifen into its pharmacologically active metabolite endoxifen. Although association between CYP2D6 polymorphisms and recurrence of breast cancer in patients on tamoxifen had been reported, little
evidence exists on association between these polymorphisms and adverse effects to tamoxifen. This study explored the association between CYP2D6 polymorphisms and tamoxifen effects,
hitherto not studied in Sri Lanka.
Methods: A retrospective preliminary study was carried out on 24 breast cancer patients on tamoxifen for minimally 3 months attending National Cancer Institute, Maharagama, Sri Lanka.
They were not on CYP2D6-inhibiting drugs, chemotherapy or other endocrine therapy, and had no conditions that could occur as adverse effects to tamoxifen before starting the therapy. Their
blood samples were collected, DNA was extracted and genotyped using SNaPshot Multiplex sequencing based single-nucleotide polymorphism (SNP) assay.
Results: SNP/allele frequencies detected: 1846G greater than A (confirmatory of *4 null allele)=8.3%; 2549delA (confirmatory of *3 null allele)=50%; 100C greater than T (suggestive of *10 reduced functional allele, in addition to other alleles)=0%; combination of 2988G greater than A, -1584C and 2850C greater than T (strongly suggestive of *41 or other reduced functional allele)=4.8%. Occurrence of heterozygous 2988G greater than A SNP with -1584C and 2850C greater than T was significantly higher among those with ultrasound-diagnosed fatty liver following the commencement of tamoxifen therapy (P=0.029). Adverse effects occurred at a significantly higher frequency among postmenopausal women (P=0.041). Three patients who developed recurrence of breast cancer had no association with SNPs tested.
Conclusions: CYP2D6 SNP combination 2988G greater than A, -1584C and 2850C greater than T, strongly suggestive of *41 reduced functional allele, is likely to be useful in predicting occurrence of adverse effect fatty liver in breast cancer patients on tamoxifen, thereby alternative treatment can be considered and lifestyle modifications implemented. Larger sample studies are recommended with the measurement of tamoxifen and metabolite levels. Alternative therapy should be considered for postmenopausal patients.

CINCINNATI (Liz Bonis) -- You may recall Local 12 News reported about a seminar for cancer and memory loss in September 2015. A young breast cancer survivor shared her story. She had a side effect and doctors at the UC Cancer Institute said more women need to know about it. Lisa Elliott said it was the little things she values more than ever right now. Not just going shopping again but actually remembering all the items on her shopping list. Just a few years ago Lisa was diagnosed December 2010 at age 29, I had taken off my bra one day and I saw a little bit of blood, and I thought, Oh, I must have had a bug bite that I scratched? It turned out she had breast cancer. After surgery, radiation and the medication for prevention, Tamoxifen, she did hear from health care providers about a lot of the expected side effects. But one of the things that they didn't talk about was a sort of foggy brain feeling. And she started noticing that. Her foggy brain however turned into a whole lot more, My friends would tease me. I would start to tell them a story and they would say, You just told me this yesterday! Full blown short and long-term memory loss, Dr. Luke Pater, a radiation oncologist, said his team has been looking into the after effect for years, It's seen with a lot of different therapy but most of the studies have resided with breast cancer and with patients with brain tumors. He said the treatments can definitely impact memory; sometimes short term and sometimes long term or even that foggy thinking. The good news is that it means that research shows patients are living longer. Lisa Elliott said people should consider these things and she wants other survivors to know the side effect is real. It doesn't make you feel better because you still forget things. But it makes you feel better that you are not crazy and it's important for me to share my story or experiences so other people don't feel like they are the only ones, said Lisa.
