Before you start taking Tacrolimus, make sure to read the Patient Information Leaflet from your pharmacist. Take this medication exactly as directed by your doctor, usually every 12 hours. It can be taken with or without food, but it's important to choose one way and stick to it to ensure consistent absorption.

If you are taking the capsules, swallow them whole and do not open or crush them. If you are using the granules, be sure to follow the manufacturer's Instructions for Use and use non-plastic cups, spoons, or oral syringes. Do not sprinkle the granules on food and do not prepare the mixture ahead of time.

The dosage of Tacrolimus is based on your weight, medical condition, lab tests, and response to treatment. It's important to take this medication regularly at the same times each day to keep the amount of medicine in your body at a constant level and get the most benefit from it.

Avoid eating grapefruit or drinking grapefruit juice while using this medication unless your doctor or pharmacist says you may do so safely. And if you are pregnant or may become pregnant, do not handle this medication or breathe the dust from the capsules.

Tacrolimus is an immunosuppressive medicine which has been used in kidney transplants with excellent results for a long time. It is 10 times more potent than Cyclosporin which is another similar medicine which has previously been used in difficult patients with Nephrotic syndrome both children and adults. It is used in kidney transplant,Nephrotic syndrome, SLE and other immunological diseases. My research paper, when I was a Professor at SGPGIMS, for the first time proved that this could be a new treatment for these difficult children with nephrotic syndrome. It has a response rate of 80% even n steroid resistant nephrotic syndrome. However it needs close monitoring by an experienced nephrologist.

Here are some key points about tacrolimus:

1. Immunosuppression: Tacrolimus works by inhibiting the activity of calcineurin, an enzyme crucial for the activation of T-lymphocytes (T cells), which are a type of white blood cell involved in immune responses. By inhibiting T-cell activation, tacrolimus helps to reduce the immune system's activity. Where to apply the wipes?

When to apply?

There are some possible side-effects you should keep in mind. It can cause mild erectile dysfunction  due to lack of sensation.

Topical creams:

Like premature ejaculation wipes, many over-the-counter topical anesthetic creams contain a numbing agent that can treat premature ejaculation by reducing sensation and delaying climax. Apply the cream to your penis 10 to 15 minutes before sex for it to be most effective.

It is important to note that you should wipe off all of the cream thoroughly prior to engaging in sexual intercourse and dispose of the cream appropriately. 

The most common side-effect is skin irritation. But again, due to lack of sensitivity, you can have erectile dysfunction like issues.

Lidocaine spray:

Like topical creams, lidocaine spray can help treat premature ejaculation by desensitizing the penis and decreasing hypersensitivity.
Use the spray approximately 15 minutes before sex in order for it to work best. 

Known potential side effects include temporary sensitivity loss and some difficulty maintaining an erection for the 15 minutes before intercourse.

Climax control condom:

In general, condoms can decrease sensitivity and keep you from ejaculating early. But there are also climax control condoms available over the counter that are either made of a thicker latex material or contain a numbing agent meant to delay climax.
