Phentermine is an FDA approved oral therapy that assists in short-term weight loss in combination with diet and exercise. Phentermine belongs to the appetite suppressing family and is a weak relative of amphetamine. Phentermine may be appropriate when the BMI – body mass index – exceeds 30 or 27 in the presence of other cardiovascular risk factors .

Phentermine is inappropriate for those who desire to lose a few pounds. In selected individuals it should be administered on hour or two before or after breakfast. A split dose of 18.75 mg before breakfast and dinner is also acceptable. Since phentermine functions as a stimulant, a bedtime dose risks insomnia.

Therapy may continue for a maximum of 12 weeks. Unfortunately a regain of weight generally occurs in the absence of a continuous low calorie diet. weight regain occurs after discontinuation of therapy. The greatest weight loss occurs in the first several weeks of therapy. Effectiveness of Phentermine is lost beyond 12 weeks of use.

Contraindications against use of phentermine include coronary artery disease, arrhythmia, uncontrolled blood pressure and heart failure. Hyperthyroidism, glaucoma, an agitated state and pregnancy also exclude Phentermine eligibility. Phentermine should not be prescribed to those with a history of drug abuse.

Phentermine is an old school weight loss medication with some new and interesting research surrounding it. It's been around for over 60 years and has helped people lose weight predominately though appetite suppression. Even though there is a ton of research surrounding phentermine, there is some new research as well. Namely, long term phentermine use. We look at 2 different studies that take a look at off-label phentermine use including some patients who took this medication for 24 months non-stop. The dosing of phentermine is all over the board but one source showed people taking 15mg per day and 30mg per day.

Phentermine, just like all medications, can have side effects. Phentermine side effects include physical and psychological. Phentermine has a number of contraindications so make sure you're working with a qualified medical professional if this is a medication you're interested in.

Dr. Steve Wolf: Phenobarbital is one of our oldest anti-seizure medications that have been around. It's indicated for both seizure types. The major problem with it is sedation, sleepiness. That's why the newer medications have a lot less sleepiness and side effects and waking issues.

Patty McGoldrick: Now it still is used a lot of times in infants, children who present with neonatal seizures, and that's one of the first medications that's given to them. It's also used for people who present in status, which is prolonged seizures, and who may be in the emergency room. Not necessarily as the first medication that's used for that, but somewhere down the line.

Dr. Wolf: The problem is in the emergency room, when you use it, it can really knock you out, and put you to sleep. And of course, if you give too much of it, it actually can affect your respiration.

Patty: Now, the other big issue with phenobarbital is that we've been using it for years to control seizures in newborns and in infants. And actually, the latest research has shown it can cause much more cognitive damage than we initially thought. So we're trying not to use that as rapidly as we did in the past. We use some of the newer medications. We try to get children off phenobarbital pretty quickly if they were using it.

Dr. Wolf: There were studies in the 80's showing that it can affect hyperactivity in young children, as well as reading scores and attention scores. So again, this drug can have a lot of potential cerebral side effects. That's why it's nice about some of the newer medications that are out there that might have less cognitive effects. So it's important to discuss with the people taking care of you whether this is the right medication for you, and what are the other choices.

They should consider and give medicine as per the doctor has advised. As per the doctor’s advice, they should have the medicines on regular times and regular frequency. They should not miss any medication. Even if they miss, they need to give medications immediately after remembering the next dose and if the child is getting medications, they should not discontinue suddenly or without advice from the doctor. They should not stop medication. Only if you stop medication, you should stop slowly after decreasing the dosage and if they are decreasing the dosage and if they are getting medications with seizures, they can have some behavioral problems in children especially drowsiness and sleepiness in the school or behavioral problems like aggressiveness and hyperactivity and this should be monitored with children and this should be reported to parents. Sometimes rarely problems with the kidney and the liver, they should be reported to the doctor. Regarding other precautions, better to avoid swimming in those children having epilepsy because during the episodes and during swimming if the child gets seizures, the chance of drowning is more. So these are some of the precautions parents should be aware when children are suffering from epilepsy.
