Pioglitazone, sold under the brand name Actos among others, is an anti-diabetic medication used to treat type 2 diabetes.[2] It may be used with metformin, a sulfonylurea, or insulin.[2][3] Use is recommended together with exercise and diet.[3] It is not recommended in type 1 diabetes.[3] It is taken by mouth.[3]

Common side effects include headaches, muscle pains, inflammation of the throat, and swelling.[3] Serious side effects may include bladder cancer, low blood sugar, heart failure, and osteoporosis.[3][2] Use is not recommended in pregnancy or breastfeeding.[2] It is in the thiazolidinedione (TZD) class and works by improving sensitivity of tissues to insulin.[2]

Pioglitazone was patented in 1985, and came into medical use in 1999.[4] It is available as a generic medication.[2] In 2019, it was the 179th most commonly prescribed medication in the United States, with more than 3 million prescriptions.[5][6] It was withdrawn in France and Germany in 2011.[7][8][9] Pioglitazone is used to lower blood glucose levels in type 2 diabetes either alone or in combination with a sulfonylurea, metformin, or insulin.[1] The effects of Plioglitazone have been compared in a Cochrane systematic review to that of other blood sugar lowering-medicine, including metformin, acarbose, and repaglinide, as well as with appropriate diet and exercise, not showing any benefit in reducing the chance of developing type 2 diabetes in people at risk.[10] It did, however, show reduction of risk of developing type 2 diabetes when compared to a placebo or to no treatment.[10] These results should be interpreted considering that most of the data of the studies included in this review were of low or very-low certainty.

While pioglitazone does decrease blood sugar levels, the main study that looked at the medication found no difference in the main cardiovascular outcomes that were looked at.[11] The secondary outcome of death from all causes, myocardial infarction, and stroke were lower.[11]

Pioglitazone has been found to reduce all-cause mortality in type 2 diabetic patients compared to other therapies, with a 60% reduction in mortality in those exposed to pioglitazone, compared to those never exposed.[12] Another study found an all-cause mortality hazard ratio of 0.33 for pioglitazone after adjusting for 40 covariates, compared to insulin.[13] Due to insufficient data on all-cause mortality, cardiovascular mortality, myocardial infarction and stroke, this was not possible to compare in a more recent review.[10]

Phenytoin is also known as the brand name: Dilantin and Phenytek

Phenytoin comes in a capsule, oral suspension, chewable tablet, and injection dosage forms.

Phenytoin is most commonly used for the control and prevention of seizures. No matter what your doctor had you taking it for phenytoin is usually taken by mouth three times daily. DO NOT stop taking phenytoin at once because it can actually increase your risk of having a seizure.

Standard dosing of phenytoin ranges from: 100 mg to 300 mg

Some common side effects with phenytoin include decreased coordination, involuntary eye movement, confusion, slurred speech.
Some less common side effects with phenytoin include unusual hair growth, changes in muscle tone, blood disorders, facial changes, constipation, dizziness, enlargement of lips, fever, headache, trouble falling asleep, joint pain, nausea, nervousness, sore or swollen gums, skin peeling, skin rash, tremors, twitching, vomiting, or yellowing of skin and eyes.

Do not take phenytoin if you are hypersensitive to phenytoin or other hydantoins.

While taking phenytoin, remember it is important to tell your doctor or pharmacist if you:

• Have signs of an allergic reaction, such as rash, itching, swelling, or tightness of the chest
• Have a severe skin rash that is reddish or purplish.
• Are pregnant or plan to become pregnant.
• Have yellowing of skin and eyes as this could be a sign of liver disease.

Phenytoin is a first-generation anti-epileptic medication used in the treatment of various types of seizures, terminal neuralgia, behavioral disorders, and status epilepticus; a condition characterized by prolonged seizures.
A seizure is a condition where there is uncontrolled, abnormal electrical activity of the brain, causing neurons to fire action potentials more frequently.
Voltage-gated membrane sodium channels of the neurons are responsible for this increased firing of nerve signals.
Phenytoin works by blocking these voltage-gated sodium channels, reducing the frequency of nerve signals.
Phenytoin can be administered orally and intravenously.
Phenytoin achieves higher concentrations in the central nervous system compared to the serum.
Therefore, it is ideal in the treatment of central nervous system disorders.
Phenytoin is metabolized by the cytochrome P450 enzyme system in the liver.
Drugs that alter the function of these enzymes affect the plasma concentrations of phenytoin.
Medications that inhibit these enzymes will increase the plasma concentrations of phenytoin.
Some of these drugs include amiodarone, cimetidine, metronidazole, chloramphenicol, sodium valproate, and sulfonamides.
By contrast, medications that induce the enzyme system will decrease the plasma concentrations of phenytoin.
Some of these drugs include rifampicin, barbiturates, theophylline, and carbamazepine.
So, individuals who are taking these medications concurrently with phenytoin require dose adjustments according to the clinical response.
Major adverse effects of phenytoin include the following.
Gingival hyperplasia.
Hypotension.
Arrhythmias.
Sedation.
Psychosis.
Megaloblastic anemia.
Steven Johnson syndrome.
Decreased bone mineral content with long-term use.
And immunoglobulin A deficiency.
Since phenytoin is a medication with a narrow therapeutic index, it is important to know the toxic effects of the drug.
Phenytoin displays its primary signs of toxicity in the nervous system and cardiovascular system.
Main central nervous system effects include nystagmus, slurred speech, tremors, hyperactivity, seizures, and coma.
In the cardiovascular system, it can cause life-threatening arrhythmias, hypotension, and cardiovascular collapse.
Finally, there is a rare condition associated with phenytoin crystallization within the circulation called “Purple Glove Syndrome”.
This condition is characterized by worsening limb edema and discoloration.
If not identified and corrected early, this can lead to life-threatening ischemia of the limb, which necessitates amputation.

Warnings:

This medication contains acetaminophen pheniramine phenylephrine. Do not take Theraflu Cold & Sore Throat or Theraflu Flu & Sore Throat if you are allergic to acetaminophen pheniramine phenylephrine or any ingredients contained in this drug.

Contraindications:

Hypersensitivity.
Asthma.
Children under 2 years old.
Narrow-angle glaucoma.
Symptomatic prostate hypertrophy.
Bladder-neck obstruction.
Stenosing peptic ulcer.
Use within 14 days of monoamine oxidase inhibitor (MAOI) therapy.
G-6-PD deficiency.
Severe hepatic impairment.

Cautions:

Pheniramine: May cause significant confusional symptoms; not for administration to premature or full-term neonates.
Phenylephrine: Caution in elderly patients, hyperthyroidism, myocardial disease, bradycardia, partial heart block or severe atherosclerosis.
Acetaminophen: Hepatotoxicity possible in chronic alcoholics following various dose levels; severe or recurrent pain or high or continued fever may indicate a serious illness; contained in many OTC products and combined use with these products may result in toxicity due to cumulative doses exceeding recommended maximum dose.
Acetaminophen: Risk for rare, but serious skin reactions that can be fatal; these reactions include Stevens-Johnson Syndrome (SJS), toxic epidermal necrolysis (TEN), and acute generalized exanthematous pustulosis (AGEP); symptoms may include skin redness, blisters and rash.

Pregnancy and Lactation:

Use acetaminophen pheniramine phenylephrine with caution during pregnancy if the benefits outweigh the risks. Animal studies show risk and human studies are not available or neither animal nor human studies were done.
Acetaminophen pheniramine phenylephrine is excreted in breast milk. Use caution while breastfeeding.

Hypotension: Intravenous administration starts with 0.1 to 0.5 mg every 15 minutes, adjusted based on patient response.

Mydriasis: Typically, 1 to 2 drops of 2.5% solution in each eye.

Side Effects
Common: Nausea, vomiting, headache, anxiety, and dry mouth.

Serious: Hypertension, bradycardia, arrhythmias, and tissue necrosis at the injection site.

When it is that time of year and you have a stuffy nose that won’t clear up, over the counter medications at your pharmacy can help. Today I’m going to talk about phenylephrine, also known as Sudafed PE.

Phenylephrine, sold under the name brands Sudafed PE and Sudogest PE, is a nasal decongestant used to treat nasal congestion caused by a cold, infection, or allergies. It is included in many combination OTC medications for cold, flu, and congestion.

Adults should take one tablet (10 mg) of phenylephrine every 4 hours as needed. Do not take more than six tablets (60mg) in one day and do not take for more than 7 days. Children younger than 12 take a smaller dose based on age.

Talk to your doctor or pharmacist before giving phenylephrine to a child. Like all OTC medications, follow the instructions on the package unless instructed otherwise by your doctor.

Common side effects of phenylephrine include anxiety, dizziness, restlessness, difficulty sleeping, or an upset stomach. Taking this medicine with food if you have an upset stomach. Tell your doctor if you experience signs of high blood pressure, including bad headache, dizziness or passing out. Other symptoms to tell your doctor about include chest pain, fast heartbeat, nervousness, or shakiness.

If you have high blood pressure, thyroid problems, take medicine for mood (including depression and anxiety), or are pregnant or breastfeeding, be sure to talk to your doctor or pharmacist before taking phenylephrine. Do not take this medication if you are allergic to phenylephrine or any related drug, including pseudoephedrine. If you are 65 years or older, check with your doctor or pharmacist before use, as you could be more predisposed to the adverse effects of phenylephrine.

Phenylephrine, or Sudafed PE, is a nasal decongestant you can take it for a stuffy nose. There are other medications available to help when you have a cold, allergies, or a sinus infection. Your pharmacist will be able to help you choose the best over the counter option for you!

What is phenylephrine hydrochloride (hcl)?




Phenylephrine is used for the temporary relief of stuffy nose, sinus, and ear symptoms caused by the common cold, flu, allergies, or other breathing illnesses (such as sinusitis, bronchitis). This medication works by decreasing swelling in the nose and ears, thereby lessening discomfort and making it easier to breathe. Cough-and-cold products have not been shown to be safe or effective in children younger than 6 years. Do not use this product to treat cold symptoms in children younger than 6 years unless specifically directed by the doctor. Some products (such as long-acting tablets/capsules) are not recommended for use in children younger than 12 years. Ask your doctor or pharmacist for more details about using your product safely. These products do not cure or shorten the length of the common cold and may cause serious side effects. To decrease the risk for serious side effects, carefully follow all dosage directions. Do not use this product to make a child sleepy. Do not give other cough-and-cold medication that might contain the same or similar ingredients (see also Drug Interactions section). Ask the doctor or pharmacist about other ways to relieve cough and cold symptoms (such as drinking enough fluids, using a humidifier or saline nose drops/spray).




How to use Phenylephrine hcl 10 Mg Tablet.




If you are using the over-the-counter product, read and follow all directions on the product package before taking this medication.


Take this medication by mouth with or without food or as directed by your doctor. Taking it with food may decrease stomach upset.


If you are using the liquid, carefully measure your prescribed dose using a medication-measuring device or spoon. Do not use a household spoon because you may not get the correct dose.


If you are using chewable tablets, chew each tablet thoroughly before swallowing.


If you are using a product made to dissolve in the mouth (tablets or strips), dry your hands before handling the medication. Place each dose on the tongue and allow to dissolve completely, then swallow it with saliva or with water.


The dosage is based on your age. Do not increase your dose or take this medication more often than directed without your doctor's approval. Improper use (abuse) of this medication may result in serious harm (such as hallucinations, seizure, death).


If your symptoms worsen or do not improve after 7 days, if you develop fever/chills, or if you think you may have a serious medical problem, get medical help right away.




Side Effects.




Mild upset stomach, trouble sleeping, dizziness, lightheadedness, headache, nervousness, shaking, or fast heartbeat may occur. If any of these effects last or get worse, tell your doctor or pharmacist promptly.


This product may reduce blood flow to your hands or feet, causing them to feel cold. Smoking may worsen this effect. Dress warmly and avoid tobacco use.


If your doctor has directed you to use this medication, remember that your doctor has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.


Tell your doctor right away if you have any serious side effects, including: fast/irregular heartbeat, severe/uncontrolled shaking, difficulty urinating, seizures, mental/mood changes (such as anxiety, panic, confusion, unusual thoughts/behavior).


A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Phenylephrine is used to relieve nasal discomfort caused by colds, allergies, and hay fever. It is also used to relieve sinus congestion and pressure. Phenylephrine will relieve symptoms but will not treat the cause of the symptoms or speed recovery.
