Phenytoin (Dilantin) – Side Effects, Uses, Mechanism of Action, Dosage, Interactions, Warnings
Phenytoin (Dilantin) is an anticonvulsant medication used to control seizures, particularly tonic-clonic (grand mal) and complex partial seizures, as well as seizures following neurosurgery. It works by stabilizing neuronal membranes and inhibiting sodium channels, reducing seizure activity in the brain. Common side effects include dizziness, nausea, and gum overgrowth, while serious effects can include severe skin reactions, blood disorders, and liver toxicity. The dosage varies, typically 100-400 mg daily for adults, with therapeutic monitoring required due to its narrow therapeutic range. Phenytoin interacts with many drugs, including contraceptives and warfarin, and poses risks such as cardiovascular issues, teratogenicity, and bone density loss, especially with long-term use. Careful consideration is needed for patients with liver impairment and during pregnancy.

------------
- In this video we'll briefly discuss what is Phenytoin ( Dilantin ), how it works, what it's used for, the dose and side effects, contraindications and precautions.
- Phenytoin is an anticonvulsant agent. It blocks voltage-gated sodium channels by selectively binding to the channel in the inactive state and slowing its rate of recovery. Thereby stabilizing threshold against hyperexcitability.
- It is available as 30 mg, 50 mg and 100 mg tablets. And also available as oral suspension and as injection.
- Phenytoin is used in the treatment of epilepsy, except absence seizure (petit mal).
- it is used in the control of grand mal and psychomotor seizures, prevention and treatment of seizures occurring during or after neurosurgery, control of grand mal type of status epilepticus (using parenteral administration).
Dosage
- Seizures
- Adults: 300 to 400 mg/day (maximum 600 mg/day).
- Children: 3 to 8 mg/kg/day in 2 to 3 divided doses.
Duration
- According to clinical response
Contra-indications
- Do not administer in case of hypersensitivity to phenytoin.
Adverse effects
- May cause:
- gastro-intestinal disturbances: gingival hypertrophy, nausea, vomiting.
- neurological disorders: dizziness, visual disturbances, mental confusion.
- blood disorders: monitor blood counts if possible and administer folic acid in case of prolonged use, as it may cause folic acid deficiency.
- allergic reactions: cutaneous eruption, fever, adenopathy.
Precautions
- Do not stop treatment abruptly, decrease daily doses gradually.
- It is not recommended to combine phenytoin with oral contraceptives, sulphonamides, or chloramphenicol. Combination with other drugs must be closely monitored (diazepam, phenobarbital, digoxin, corticosteroids, etc.).
- Paracetamol may increase hepatotoxicity potential with chronic phenytoin use.
- Pregnancy: avoid
- Breast-feeding: avoid

Pseudoephedrine Vs. Phenylephrine? In this informative video, we will discuss two common medications used for nasal congestion: pseudoephedrine and phenylephrine. Both options are widely available, but they differ in effectiveness and mechanism of action. We will break down how each medication works, their side effects, and the regulations surrounding their use. What Are The Side Effects Of Phenylephrine Eye Drops? In this informative video, we will discuss the side effects associated with the use of phenylephrine eye drops, commonly utilized in optometry for pupil dilation during eye exams and procedures. Understanding the potential reactions to these drops is important for both patients and practitioners. We will cover the typical side effects that many individuals may experience after application, such as sensations of burning or stinging, blurry vision, and increased light sensitivity. Additionally, we will touch on some of the more serious reactions, which, although rare, can occur and require immediate attention.
Patient safety is a priority in optometry, and monitoring for any adverse effects is essential, especially for those with pre-existing health conditions. We will also provide guidance on what patients should do after receiving these drops, particularly regarding activities that require clear vision. If severe reactions occur, alternative dilation options may be available, and we will briefly mention those as well.

Phenylephrine is a selective alpha-1 adrenergic agonist used as a decongestant, vasopressor, and mydriatic agent. It works by causing vasoconstriction, which reduces nasal congestion and raises blood pressure, but should be used cautiously due to risks like hypertension and rebound congestion.

Study this Ephedrine / Phenylephrine NCLEX mnemonic and other mnemonics with Pixorize.
Ephedrine, pseudoephedrine, and phenylephrine are medications that have different effects depending on their route of administration. When given nasally or orally, they can be used to treat congestion. When used as a decongestant, a potential side effect is rebound nasal congestion, so patients should not take these medications for more than 3-5 days and should taper off the drug slowly. When given through an IV, these drugs act systemically as vasopressors, where they constrict the blood vessels to increase blood pressure. Regardless of the route of administration, side effects to look out for include CNS stimulation, which can present as insomnia, restlessness, agitation, or nervousness. These drugs can also cause uncontrolled hypertension, and should not be given concurrently with other drugs that cause hypertension, or to patients who already have high blood pressure and other cardiovascular conditions.

It’s a common decongestant ingredient found in hundreds of over-the-counter medications. But now, the FDA’s Nonprescription Drugs Advisory Committee says phenylephrine doesn’t actually get rid of nasal congestion.
