Apalutamide for High-Risk Prostate Cancer | Learn About Clinical Trials Side Effects 1 Views • 13 days ago

PCRI's Executive Director, Mark Scholz, MD, explains a clinical trial (NCT03767244) investigating the use of hormone therapy before, during, and after a radical prostatectomy in men with high-risk (Azure) prostate cancer.

0:10 Historically, adjunctive hormone therapy has only been used in men who were being treated with radiation. Also, radical prostatectomy has historically performed poorly in treating high-risk prostate cancer. Recent studies, however, have demonstrated a benefit in men with high-risk prostate cancer who were treated by radical prostatectomy and concurrent hormone therapy.

1:18 Janssen Pharmaceuticals is funding this clinical trial that is investigating 1500 men with high-risk prostate cancer. Half of the study's participants will receive Lupron plus radical prostatectomy and the other half will receive Lupron and Erleada (apalutamide) plus radical prostatectomy.

2:18 The study is designed so that a patient will receive hormone therapy for 6 months, undergo surgery, and then continue on hormone therapy for another 6 months (for a total of 12 months of hormone therapy).

3:17 If the study successfully demonstrates a benefit to using Lupron and Erleada concurrently with radical prostatectomy, then this likely become the standard of treatment for men with high-risk disease who choose radical prostatectomy over radiation.

The CGRP injectable migraine prevention treatments, Aimovig (erenumab), Ajovy (fremanezumab), and Emgality (erenumab) are transforming the lives of people living with migraine by dramatically reducing the frequency of migraine attacks by 50, 75 or even 100 percent.

Migraine prevention is the most important thing you can do if you have migraine. You should consider migraine prevention if you have 4 or more days of migraine (not attacks of migraine - a migraine attack can last 1 or more days) a month. You should commit to migraine prevention if you have 2 or more days a month where you missing work or school, or leaving early, because of a migraine attack. Other reasons to prevent migraines include:

- Going to the ED, Urgent Care, or your doctor's office for a migraine emergency.
- Acute migraine treatments cause awful side effects or don't work.
- Personal preference.

Until 2018, all migraine prevention treatments were drugs that were invented for other problems and "accidentally" found to be helpful for migraine prevention. These drugs include the platform migraine treatments amitriptyline, topiramate (Topamax), divalproex / valproic acid (Depakote) , and propranolol. These drugs are still good basic choices for prevention. But, they may not work well enough for some people, and they can have a lot of side effects and unwanted interactions with other medicines.

The CGRP injectables have changed the picture of migraine prevention. Aimovig, Ajovy and Emgality are injectable monoclonal antibodies. Their mechanism of action is to prevent CGRP from sticking to CGRP receptors. This stops the nervous system's "migraine amplification circuit", the trigeminovascular pathway.

Aimovig, Ajovy and Emgality have a lot in common. They are all injections into the skin with a fairly small needle. The injection sites and techniques are similar. They all can be taken once a month. They all need refrigeration. They all seem to work both quickly and well. They all share two common, and common-sense side effects. They all have no interactions with other drugs.

Because these three treatments have never been tested against each other, no one can really say which one is best. (And I have seen cases where people have not responded to one, but have responded to another.)

If you have been prescribed Aimovig, the brand name for erenumab, you should have received training from your healthcare professional on how to give yourself the injection safely. In this video, we will review step-by-step how to give yourself an Aimovig injection using the pre-filled autoinjector.

Before using Aimovig, review the Instructions for Use provided with your prescription. When you are ready to inject your dose, gather your materials. You will need: Your Aimovig autoinjector that has been at room temperature for at least 30 minutes, a piece of gauze or a cotton ball, an alcohol swab, and a sharps container.

Now that you are ready to inject your dose, let’s review the steps on how to inject your Aimovig autoinjector.

Step One: Check your pen to make sure the medication isn’t expired, frozen, or cloudy. It should look clear and colorless to slightly yellow, and shouldn’t have particles in it.
Step two: Wash your hands with soap and water.
Step three: Choose your injection site. If you are injecting Aimovig by yourself, you can inject it into your stomach, 2 inches away from your belly button, or into the front of your thigh. If someone else is helping you, they can inject it into the back of your upper arm. Clean your chosen injection site with an alcohol swab.
Step four: Remove the white cap for 70mg pen or orange cap for 140mg pen and either stretch or pinch your skin.
Step five: Place the injection end straight onto your skin, making sure you can see the window.
Step six: Push the autoinjector firmly against your skin which unlocks the start button. While still holding the autoinjector firmly against your skin, press the start button. You might hear or feel a click. Continue holding the autoinjector against your skin until the window is fully yellow - this may take up to 15 seconds, you might hear or feel a click.
Step seven: Remove the autoinjector from your skin and place it directly into a sharps container.
Step eight: If you have any bleeding at the injection site, you can press a cotton ball or gauze over the injection site until the bleeding has stopped.

Aimovig is a brand-name and the generic is Erenumab. In May 2018 it was the first CGRP which Works on receptor site. It blocks CGRP thereby blocks a migraine. It is given once a month at 70 to 140 mg subcutaneously that is under the skin. The half-life is 28 days. The side effects are Injection site reaction, constipation, some hypertension. Contraindications are hypersensitive reaction to the medicine or anaphylaxis and then you edema for me to discontinue the medicine and seek medical advice right away. Constipation is a major problem. You can keep it in the refrigerator but if you leave it outside the room temperature for seven days it’s OK beyond that you need to discard the medicine. It has excellent track record of preventing the migraine. It is a monoclonal antibody. It is for migraine prophylaxis or migraine prevention.

Aimovig is something that you actually have to inject and you do that subcutaneously. Sometimes people get freaked out when they find out it's an injectable. But it's really not that bad if you do it the right way.

Now when it comes to side effects Aimovig is actually really well tolerated.

Most Likely Side Effects:

Constipation: the most likely side effect of a Aimovig is going to be constipation.

It occurs in about 1-3% of people and the severity tends to vary.

Immune System Issues: the other side effect that you can see happen is antibody development.

What that means is when you give Aimovig your body can actually develop antibodies to it making it less effective.

In a small number of people the antibodies that develop are called neutralizing antibodies.

 In about 1% of people maybe even less they can develop these neutralizing antibodies and that will make Aimovig completely ineffective. But again, it's not very common.

How should this medicine be used?


Advair Diskus comes as a powder and as an inhalation solution to inhale by mouth using a specially designed inhaler. It is usually used twice a day, in the morning and evening, about 12 hours apart. Use Advair Diskus at around the same times every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Use Advair Diskus exactly as directed. Do not use more or less of it or use it more often than prescribed by your doctor.

Talk to your doctor about how you should take your other oral or inhaled medications for asthma during your treatment with salmeterol and fluticasone inhalation. If you were using a short-acting beta agonist inhaler such as albuterol (Proventil, Ventolin) on a regular basis, your doctor will probably tell you to stop using it regularly but to continue to use it to treat sudden attacks of asthma symptoms.
