PCRI’s Executive Director, Mark Scholz, MD, explains why there has been a surgery vs. radiation controversy, describes the pertinent risks and considerations when deciding between the two treatments, and explains why he thinks radiation is usually going to be the preferable option, all things considered.
0:43 The main reason why there is a controversy about whether surgery or radiation is better for treating prostate cancer is that as recently as 10-15 years ago the technology for radiation was significantly less effective and more toxic than it is today. There were also fewer options for salvage therapy after radiation in the event of a relapse. Since then, radiation technology has advanced significantly, but the dialogue has been comparatively mired. Today, radiation can be precisely delivered, and radiation is achieving even better cure rates than surgery.
1:59 Surgeons are constrained by the fact that the bladder and the rectum are within millimeters of the prostate. They cannot cut a margin around the gland. Radiation oncologists are not limited in this way.
2:21 External beam radiation has a cure rate about as good as surgery, and radioactive seed implants have even better cure rates than surgery.
2:33 Most men will live for a very long time after being treated for prostate cancer, and so it is important to consider the side effects of each treatment, especially now that they all have comparable cure rates.
2:54 There is a risk of sexual dysfunction with both surgery and radiation, however, the rate of impotence after surgery is significantly higher than after radiation.
3:15 There is a risk of incontinence with surgery that is almost non-existent with radiation. An operation performed by a top surgeon on an ideal candidate has about a 5-10% risk of causing incontinence, and that is with the best possible conditions. Stress incontinence occurs in around 50% of men, and ejaculation of urine occurs in about 20% of men.
4:00 Rectum burns used to be a concerning side effect of radiation, but that risk has been eliminated with the advent of SpaceOAR, a gel that separates the prostate from the rectum and protects it from radiation.

A newer antipsychotic is Caplyta. It works differently than other antipsychotics, and has a unique side effect profile.
Caplyta is used to treat adults with schizophrenia or bipolar depression. It comes as a capsule that’s taken by mouth once a day.
Caplyta works by affecting neurotransmitters in the brain, including dopamine, serotonin, and glutamate. These actions help relieve symptoms such as hallucinations and delusions associated with schizophrenia, and low mood in people with bipolar depression.
Common side effects of Caplyta may include sleepiness, dizziness, nausea, and dry mouth. Caplyta does not usually cause weight gain as a side effect, unlike some other antipsychotics.
In fact, some people have lost weight during treatment with Caplyta. Based on animal studies, this medication might cause fertility problems in males and females.
Caplyta works gradually, so you may not notice immediate effects. Stay consistent—full benefits can take weeks, but taking it daily helps you get the best results of improved symptoms and severity.

Mike has always been an active person, but everything changed when he started noticing that he was frequently short of breath and that he didn’t have the energy to do the things he loved. Kim was always a very active person. She was into sports and dance, and she loved playing softball. But she started to notice symptoms that forced her to adjust her lifestyle. Barbara, who was told as a child she had a heart murmur, was used to living with symptoms such as occasionally becoming out of breath or feeling faint on a very hot day. This was her “normal,” so it didn’t occur to her to see a doctor about it. But one day when she was being prescreened for unrelated back surgery, she was diagnosed with symptomatic obstructive HCM. Stephanie was a marathon runner who had to give up running and gradually lowered her level of activity over the years. Dani has had a long journey with obstructive HCM/HOCM. It took quite a while to discover the root of the problem and receive an obstructive HCM diagnosis. CAMZYOS® (mavacamten) is a prescription medicine used to treat adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM). CAMZYOS may improve your symptoms and your ability to be active. It is not known if CAMZYOS is safe and effective in children.
WARNING: RISK OF HEART FAILURE
CAMZYOS may cause serious side effects, including:
Heart failure, a condition where the heart cannot pump with enough force, is a serious condition that can lead to death. You must have echocardiograms before you take your first dose and during your treatment with CAMZYOS to help your healthcare provider understand how your heart is responding to CAMZYOS. People who develop a serious infection or irregular heartbeat have a greater risk of heart failure during treatment with CAMZYOS. Tell your healthcare provider or get medical help right away if you develop new or worsening shortness of breath, chest pain, fatigue, swelling in your legs, a racing sensation in your heart (palpitations), or rapid weight gain.
The risk of heart failure is also increased when CAMZYOS is taken with certain other medicines. Tell your healthcare provider about the medicines you take, both prescribed and obtained over-the-counter, before and during treatment with CAMZYOS.
Because of the serious risk of heart failure, CAMZYOS is only available through a restricted program called the CAMZYOS Risk Evaluation and Mitigation Strategy (REMS) Program. Before you take CAMZYOS, you must enroll in the CAMZYOS REMS Program.

CALQUENCE is indicated in combination with bendamustine and rituximab (BR) for the treatment of adult patients with previously untreated mantle cell lymphoma (MCL) who are ineligible for autologous hematopoietic stem cell transplantation (HSCT), as monotherapy for the treatment of adult patients with MCL who have received at least one prior therapy, and for the treatment of adult patients with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).
Serious adverse events, including fatal events, have occurred with CALQUENCE, including serious and opportunistic infections, hemorrhage, cytopenias, second primary malignancies, cardiac arrhythmias, and hepatotoxicity, including drug-induced liver injury. The most common adverse reactions (≥30%) are diarrhea, upper respiratory tract infection, headache, musculoskeletal pain, lower respiratory tract infection, and fatigue. The most common Grade 3 or 4 laboratory abnormalities (≥10%) are absolute neutrophil count decreased, absolute lymphocyte count decreased, platelets decreased, and hemoglobin decreased.
