On this episode I breakdown nitrofurantoin pharmacology. Nitrofurantoin is an antibiotic that is used in the treatment of UTI’s.
Nitrofurantoin has activity by creating reactive metabolites that can disrupt protein synthesis and ultimately kills the bacteria causing the infection.
It is important to remember with nitrofurantoin, that in the setting of systemic infection and/or kidney involvement, the drug may not be very effective.
Nitrofurantoin can potentially reduce the effectiveness of certain vaccines. Be sure to look this up prior to administration.
Nitrofurantoin can cause pulmonary symptoms as well as neuropathy. This is very important to monitor for.

La nitrofurantoína es un antibiótico derivado del nitrofurano. Es activa sobre un número importante de bacterias. No obstante, y al igual que cualquier otro antibiótico, no tiene ningún efecto frente a infecciones causadas por virus, como gripe, resfriado, COVID-19 o la mayoría de las infecciones de garganta.
Está indicado para el tratamiento de la cistitis aguda en niñas mayores de 3 meses, adolescentes y mujeres adultas.
La nitrofurantoína está disponible tanto en forma de comprimidos como en suspensión oral. La dosis recomendada para el tratamiento de la cistitis aguda no complicada en mujeres adultas es de 50-100 mg cada 8 horas durante 5-7 días.
Para niñas mayores de 3 meses y adolescentes, la dosis recomendada es de 5-7 mg/kg de peso por día, sin superar la dosis de adulto, repartidas en cuatro tomas durante 5-7 días. En el caso de niñas menores de 6 años, se debe usar la suspensión oral.
Se recomienda tomar nitrofurantoína durante la comida o con un vaso de leche.
Es importante usar nitrofurantoína solo para el tratamiento curativo de cistitis agudas y no como profilaxis, con una duración del tratamiento limitada a un máximo de 7 días puesto que un tratamiento prolongado puede tener riesgos pulmonares, hepáticos, alérgicos y neurológicos.
Las reacciones adversas más frecuentes en tratamientos con nitrofurantoína es la aparición de reacciones cutáneas alérgicas como prurito y urticaria; mareos; y problemas gastrointestinales como náuseas, vómitos, dolor abdominal y diarrea.
Pero el mayor riesgo de nitrofurantoína se debe al mal uso y al abuso que hacemos de los antibióticos, y es la aparición de resistencias, que pueden hacer que este antibiótico deje de ser eficaz. Para evitar o combatir este problema y reducir el riesgo de resistencias bacterianas, el Ministerio de Sanidad, a través de la Agencia Española de Medicamentos ha creado un grupo de expertos (Plan Nacional de la Resistencia a Antibióticos o PRAN), en el que participamos los farmacéuticos a través de representantes del Consejo General de Colegios Farmacéuticos.
Entre las principales recomendaciones, es fundamental no usar nitrofurantoína sin receta médica, ya que un antibiótico que te fue útil la última vez no tiene por qué serlo ahora. Además, es importante que tomes el medicamento a las horas y durante el periodo de tiempo que te haya indicado el médico. Y finalmente, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

Calcium channel blockers nursing pharmacology review of the mechanism of action, side effects, nursing implications, and patient education. These reviews will discuss the Dihydropyridine vs Nondihydrophyridine in terms of calcium channel blockers.
Calcium channel blockers block the L-type calcium channels in the vascular smooth muscle, cardiac myocytes, and cardiac nodal tissue.
Therefore, the mechanism of action for calcium channel blockers are to cause vasodilation of the coronary and peripheral arteries, decrease the strength of heart contractions, decrease the function of the SA (slow the heart rate) and AV (slow the speed of electrical conduction) nodes.
In other words, calcium channel blockers have a negative inotropic, negative chronotropic, and negative dromotropic effect on the heart.
It is important to note that some types of calcium channel blockers are more selective to the vascular smooth muscle, while some calcium channel blockers are more selective the myocardium.
Dihydropyridine are more selective to the vascular smooth muscle. This includes amlodipine, felodipine, and nifedipine. Nondihydrophyridine are more selective to the myocardium. This includes Verapamil and Diltiazem.
Side effects of calcium channel blockers include: hypotension, bradycardia, EKG changes (first degree AV block), constipation, reflex tachycardia etc.

It inhibits movement of calcium ions across cell membrane in systemic and coronary vascular smooth muscle, leading to vasodilatation and reduction in peripheral vascular resistance. So it reduces myocardial oxygen demand, and relaxes and prevents coronary artery spasm.
- It is available as 10, 20 mg immediate-release capsules, and 30, 60, and 90 mg extended-release tablets.

Today we’re going to talk about niacin (vitamin B3) and schizophrenia. With schizophrenia, a person may have hallucinations, delusions, paranoia, feelings of disconnection, a loss of interest in life, and may feel emotionless.
The medical theory of what causes schizophrenia is dopamine excess. A person is then given certain drugs to help regulate this. The potential problem with these drugs is that they could have side effects that cause metabolic syndrome and insulin resistance. This has been shown to worsen symptoms of schizophrenia because of what it could do to the brain.
However, Dr. Abram Hoffer had another theory about schizophrenia. It was called the adrenochrome theory. Adrenochrome is basically the byproducts of adrenaline. In his research, he found that the chemical structure of adrenaline is similar to LSD and that in higher amounts, it could have certain effects on the brain. His theory was that schizophrenia comes from excessive amounts of adrenochrome in the brain, which is toxic.
Normally, a person has an enzyme that clears out that chemical. But, people with schizophrenia have a defect with this enzyme. He found that large doses of vitamin B3 every day (straight niacin) may help prevent the conversion of the precursor of adrenaline to adrenaline, making less adrenaline. This could potentially help clear out adrenaline as well as decrease the production of adrenaline.
Other potential benefits of vitamin B3:
• It could help reduce liver toxicity
• It could help reduce oxidative stress in the mitochondria
• It could help the DNA repair
