If you are taking any of the Anti-Cholinergics as a long-term medication, then that is exactly what might be happening. You might be taking something to help you sleep or to lessen muscle spasms, and what it might be doing in the background is increasing your risk of developing dementia. Video abstract of an original research paper “Effects on caregiver burden of a donepezil hydrochloride dosage increase to 10 mg/day in patients with Alzheimer’s disease” published in the open access journal Patient Preference and Adherence by Nakamura K, Watanabe N, Ohkawa H, et al.

Background: In this study, we evaluated changes in functioning and caregiver burden in Alzheimer’s disease (AD) patients after a dosage increase that was made based on pharmacists’ evaluation of AD patients’ behavior in daily life.
Methods: Pharmacists used a checklist, a questionnaire, and the Repetitive Saliva Swallowing Test (RSST) to gather data on the daily life of AD patients taking donepezil 5 mg/day and their caregivers. In 27 cases, pharmacists suggested a dosage change to 10 mg/day to AD patients’ physicians. Pharmacists then evaluated these patients for 16 weeks after the increase to determine changes in functional assessment staging, caregiver burden, and swallowing function.
Results: During the 16-week study, 20 of the 27 patients showed at least one-stage improvement in relation to the five assessed aspects of daily life (time/place, speech, bathing, dressing, and toileting). The mean score for caregiver burden due to personal strain was significantly lower after the dosage increase than before (5.15±3.76 at baseline; from 3.89±3.42 at week 4 to 3.59±3.90 at week 16; P less than 0.05), as was the mean score due to role strain (2.19±2.80 at baseline; 1.56±2.64 at week 8; P less than 0.05). After the dosage increase, the impaired swallowing function that accompanies AD was improved in the patients with swallowing problems, as indicated by a higher mean RSST score (1.22±0.67 at baseline; from 2.78±1.72 at week 4 to 2.78±1.79 at week 16; P less than 0.05).
Conclusion: The dosage increase not only decreased caregiver burden, but also appeared to improve impaired swallowing function. Medication therapy management by pharmacists of AD patients, including the use of a checklist, contributed to the correct use of donepezil and improved quality of life for caregivers.

Presently, Alzheimer's Disease is incurable. However, the U.S. Food and Drug Administration (FDA) has approved six (6) prescription medications for treating some of the memory-loss symptoms associated with cognitive impairment and dementia.

These pharmaceuticals fall into three (3) broad categories.

Firstly, there are acetylcholinesterase inhibitors (AChEI). These are designed to boost the brain's levels of the neurotransmitter acetylcholine by hindering the body's ability to break it down. These drugs include donepezil (Aricept), galantamine (Razadyne/Reminyl), rivastigmine (Exelon), and the now-obsolete tacrine (Cognex).

Secondly, come the NMDA-receptor antagonists (NMDARA). These have to do with the funciton of the N-methyl-D-aspartate receptor and the neurotransmitter glutamate. At the present time, the main offering, here, is memantine (Namenda).

Thirdly, are composite medications that combine the actions of the first two categories. To put it differently, and as of this writing, researchers have put donepezil together with memantine (to create Namzaric).

None of these interventions is curative. They do not reverse Alzheimer's. They may, however, be able to slow down a person's cognitive decline. However, these drugs may have side effects.

A bit more detailed information about Donepezil and Memantine:
Donepezil or Aricept is what’s known as a cholinesterase inhibitor. When a person is suffering from Alzheimer’s disease, they begin to have less and less of a neurotransmitter called acetylcholine who’s job is to carry signals from nerve cells to muscle cells. Acetylcholine plays a vital role in muscle contractions, memory, motivation, sleep and learning. The brain also has an enzyme called acetylcholinesterase. This is the enzyme that helps to break down acetylcholine so that it can be recycled and rebuilt into new neurotransmitters for the next message.
So, the rationale behind Donepezil is relatively simple: if you block the enzyme acetylcholinesterase from breaking down the acetylcholine, then more of it will remain and the brain can continue carrying signals between nerve cells and muscle cells.

Memantine is what’s known as an NMDA antagonist. So, instead of the neurotransmitter acetylcholine, Memantine works by slowing the transmission of a different neurotransmitter called glutamate. Glutamate is the major excitatory neurotransmitter in the central nervous system. It plays an important role in learning and memory. But, it is thought that too much stimulation by glutamate may over-excite nerve cells and cause them to die. So, by blocking the effects of glutamate, Memantine may protect nerve cells from getting overstimulated.

Your medication Donepezil

Donepezil is also known by the brand name: Aricept
Donepezil comes in 5mg, 10mg, and 23mg tablets as well as 5 and 10mg orally disintegrating tablets.
Donepezil is most commonly used for treating dementia and Alzheimer's disease. It may be given for other reasons, talk with your doctor or pharmacist. No matter what your doctor has you taking donepezil for, it is taken by mouth with or without food once nightly at bedtime. If you are taking the orally disintegrating tablet, you should wash and dry your hands first, then place the tablet on the tongue and let it melt.
If you miss a dose of donepezil:
• Skip the missed dose and go back to your normal time
• Do not take 2 doses at the same time or extra doses
• If you miss 7 days of the drug, call your doctor to find out what to do

Standard dosing of donepezil ranges from 5mg to 23mg once nightly.

The best way to store donepezil:
• Store at room temperature.
• Store in a dry place. Do not store in a bathroom or refrigerator.
• Keep all drugs in a safe place. Keep all drugs out of the reach of children and pets.
• Check with your pharmacist about how to throw out unused drugs.

Some common side effects with donepezil include insomnia, nausea, and diarrhea.
Some less common side effects include chest pain, headache, dizziness, fatigue, depression, confusion, and anxiety.

When taking donepezil, remember it is important to tell your doctor or pharmacist if:
• You have signs of an allergic reaction, such as rash, hives, itching, blistered, or peeling skin with or without fever
• You have very bad dizziness or passing out
• You have a heartbeat that does not feel normal
• You have seizures
• You experience heartburn
• You have very upset stomach or are throwing up

Esomeprazole and Domperidone

Uses:

Esomeprazole and domperidone is a combination medication that is used to treat gastroesophageal reflux disease (GERD) and other conditions that involve excessive stomach acid production. Esomeprazole is a proton pump inhibitor (PPI) that works by reducing the amount of acid produced by the stomach. Domperidone is a dopamine antagonist that helps to improve gastric emptying, which can help to relieve symptoms of GERD such as heartburn and regurgitation.

Doses:

The usual dose of esomeprazole and domperidone is one capsule taken once daily. The capsule should be taken at least one hour before eating.

Missed Doses:

If you miss a dose of esomeprazole and domperidone, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and take your next dose at the regular time. Do not take a double dose to make up for a missed dose.

Storage:

Esomeprazole and domperidone should be stored at room temperature away from light and moisture.

Domperidone is used short-term in breastfeeding mothers of children being treated with the aim of improving breast milk supply.
While using this medicine – Remember
• Domperidone is not recommended for use in mothers with cardiac arrhythmias or mothers taking any medication which may cause cardiac arrhythmias.
• Domperidone in very small amounts enters the breast milk but there have been no known cases of harmful cardiac effects on babies.
• If you are taking any other medication, including over the counter or complementary medicines (also known as traditional or alternative medicines including vitamins, minerals, herbs, aromatherapy and homeopathic products) from a health food store, discuss this with your doctor prior to commencing domperidone
When and how should I take domperidone?
• Domperidone is used together with frequent expressing. It will work best for you if you are expressing regularly with an electric breast pump – this is at least eight times per day with no breaks longer than five hours.
• Domperidone is normally taken by the breastfeeding mother every eight hours (three times per day). A suggested regime would be at 6am, 2pm and 10pm. Check with the lactation consultant or pharmacist on the best way to fit this medicine into your and your baby’s routine.
How much should I take?
• The usual starting dose is 10 mg (one tablet) every eight hours (three times per day e.g. 6 am, 2 pm and 10 pm). The starting dose at the hospital should never exceed 30mg (3 tablets) in each 24 hours.
• Once your milk supply is well established, keep taking the dose of one tablet (10mg) every eight hours (three times per day) for one more week; then it may be possible to gradually reduce the medicine slowly over a few days and then stop. It is advisable to contact the lactation consultant before you plan to stop this medication, to review your milk supply, and then talking to the doctor who prescribed it to give you a weaning plan.
• The dose may change, depending on each woman’s individual response. Rarely, doses of up to 60mg (6 tablets) are used in each 24 hours, prescribed by the GP.
Side effects you must do something about
• Dizziness, fainting or heart palpitations. Stop taking domperidone and seek medical attention
• Muscle spasms
• Restlessness

Common use

Motilium blocks pereferal and central dophamin receptors, removes inhibiting influence of dophamin on motor activity of gastrointestinal tract and increases evacuative and tone of stomach. It has anti-vomiting effect, soothes hiccup and nausea. Motilium is used to treat vomiting of different origin, hiccup, atonic gastrointestinal tract, necessity to accelerate peristalsis for X-ray examination, dispepsia caused by slow stomach, gastroesophageal reflux disease, esophagitis, gas, heartburn with or without passage of gastric contents into the mouth.



Dosage and directions

Take Motilium orally 30 minutes before a meal. In adults and children older than 5 y.o. take 10 mg 3-4 times a day when dispepsia is diagnosed. In patients with severe nausea and vomiting Motilium daily dose is 20 mg 3-4 times a day. In patients with kidney failure correction of the dose is necessary, number of daily intakes should not exceed 1-2 times. This is only general recommendations and for each individual case consult your doctor as different conditions require different correction of the dose.



Precautions

If you are pregnant or think you might be pregnant first consult your doctor or pharmacist before taking Motilium or any other medicine. Do not take Motilium if you are breast-feeding.



Contraindications

Hypersensitivity, bleeding in gastrointestinal tract, mechanical intestinal obstruction, perforation of the stomach or intestines, prolactinoma, infancy (younger than 5 y. o. and ones whose weight is less than 20 kg). Caution should be experienced in patients with renal / hepatic insufficiency, during lactation, pregnancy.



Possible side effect

Among most common side effects these can occur: transient spasms of the intestine, allergic reactions (skin rash, urticaria), hyperprolactinaemia (galactorrhoea, gynecomastia).

Domperidone, sold under the brand name Domstal among others, is a medication used as an antiemetic, gastric prokinetic agent, and galactagogue.

Domstal is a medicine that increases the movements or contractions of the stomach and bowel. Domstal is also used to treat nausea and vomiting caused by other drugs used to treat Parkinson's Disease.
Domperidone is to be given only by or under the immediate supervision of your doctor. Domstal should be used at the lowest effective dose for the shortest duration necessary to control nausea and vomiting.
It is recommended to take oral Domstal tablets before meals. If taken after meals, absorption of the drug is somewhat delayed.
Patients should try to take each dose at scheduled time. If a scheduled dose is missed, the missed dose should be omitted and the usual dosing schedule resumed. The dose should not be doubled to make up for a missed dose.
Usually, the maximum treatment duration should not exceed one week.
Side effects associated with domperidone include dry mouth, abdominal cramps, diarrhea, nausea, rash, itching, hives, and hyperprolactinemia (the symptoms of which may include breast enlargement, galactorrhea, breast pain/tenderness, gynecomastia, hypogonadism, and menstrual irregularities)
