There are medications available to prevent and treat osteoporosis. Medications to prevent osteoporosis are only for those at high risk of osteoporosis, this includes those taking high dose of steroids for a long duration. If someone old has a fracture then usually they are started on medications for osteoporosis regardless of T score. Or if they are diagnosed with osteoporosis on with a T score less than 2.5.

Medications include: bisphosphonates, denosumab, SERMS, PTH analogs and sclerosting inhibitors

In this episode, Dr. Doug discusses the popular drug Prolia (Denosumab) used for the treatment of osteoporosis. He focuses on the risk of fracture after stopping the drug and addresses the fear surrounding it. Dr. Doug presents research from the FREEDOM trial, which showed a reduction in vertebral fractures with Prolia but also a reduction in bone building. He then discusses the 10-year data from the trial, which showed an increased risk of vertebral fractures after discontinuing the drug. Dr. Doug reassures listeners that with a solid plan and monitoring, the risk can be managed.

When Jane was diagnosed with osteoporosis, her physician recommended she take Prolia (Denosumab). Unfortunately, the management of her Prolia was not as good as it should have been and Jane experienced a number of problems. Here is her Prolia management story.

There are two major lessons from her experience:

1./ You should not assume that exercises you find online labelled for "osteoporosis" or for "seniors" are safe for you to do. This applies to all social media sources, even those presented by Physical Therapists. Individuals with osteoporosis need an exercise program targeted at their fracture risk; this is particularly true for individuals with compression fractures.

2./ If you are receiving Prolia injections, it is due to the fact that you are at an elevated risk of fracture. I have many clients who need to be on Prolia because their bone density and bone quality is so poor that a fall could lead to a catastrophic fracture. The problem is not the medication. Prolia is very effective. Problems occur when the administration of the drug is not managed as well as it should be. This is particularly true when it comes to the termination of the use of Prolia.

Dexamethasone belongs to a class of medications called corticosteroids.

It works by preventing the release of a particular substance which is responsible for causing inflammation.

Dexamethasone is prescribed for treating rheumatoid arthritis, osteoarthritis, gout, cerebral edema, multiple myelomas, multiple sclerosis, immunosuppression, brain tumor, shock, inflammation of fluid-filled sacs called bursa (bursitis), and tendinitis.

It is also used to treat inflammatory conditions and chemotherapy-induced nausea and vomiting.

Dexamethasone is preferred to be taken in the morning when it is prescribed as a once-daily dose. Intraocular pressure should be monitored if dexamethasone treatment is taken for more than six weeks.

Dexamethasone

Generic Name: dexamethasone

Trade Name: Decadron

Indication: manage cerebral edema, assess for Cushing's Disease Action Suppress inflammation and normal immune response. Used in inflammatory states to decrease inflammation.

Therapeutic Class: antiasthmatics, corticosteroids

Pharmacologic Class: corticosteroids

Nursing Considerations
 excreted by the liver - monitor liver profile
 avoid in active untreated infections
 may cause CNS alterations
 may cause peptic ulcers
 may cause Cushingoid appearance (buffalo hump, moon face)
 weight gain
 osteoporosis
 decrease wound healing
 may elevate blood sugars
 may increase cholesterol and lipid values
