Dr Jawad Haider Butt (Copenhagen University Hospital, DK) outlines the findings of a patient-level meta-analysis of the DAPA-HF and DELIVER trials, which studied the safety and effectiveness of dapagliflozin on patients with both peripheral arterial disease and heart failure. These trials covered the spectrum of patients with heart failure.
The findings of this meta-analysis suggest that dapagliflozin is effective in reducing the composite of worsening heart failure and cardiovascular death. This beneficial effect is not modified by PAD status. The risk of amputation within this patient cohort was not increased with dapagliflozin treatment as compared to placebo.

Dapagliflozin (brand name Forxiga) is a Sodium-glucose co-transporter 2 (SGLT2) inhibitor and is prescribed for adults with type 2 diabetes mellitus to improve their diabetic control when diet and exercise alone are not enough for good control. It can also be used to treat heart failure and chronic kidney disease (CKD).

In Diabetes, SGLT2 inhibitors are given on their own in patients for whom the use of metformin is considered inappropriate due to reasons of intolerance or contraindications. Discussion on conversion to and from dabigatran to other anticoagulants.
Conversion to and from dabigatran to other anticoagulants depends on the period of activity of the drug currently being used.
Initiate dabigatran 2 hours or more prior to the time of the next scheduled dose of the parenteral anticoagulant like heparin or low molecular weight heparin. If it is from an infusion of heparin that you are trying to convert, initiate the drug at the time of discontinuation of heparin infusion.
If the creatinine clearance is 30ml/min or more, wait 12 hours after the last dose of dabigatran for initiating parenteral anticoagulant.
When dabigatran is used for thromboprophylaxis after orthopaedic surgery, it can be started 24 hours after the last dose of dabigatran, instead of the 12 hours when it is for prevention of embolism in atrial fibrillation.
If the creatinine clearance is less than 30 ml/min, you have to wait for one day instead of the usual 12 hours.
Discontinue warfarin and initiate dabigatran when INR is less than 2.
When to start warfarin while discontinuing dabigatran is depending on the creatinine clearance. If the creatinine clearance is more than 50 ml/min, warfarin can be started 3 days before discontinuation of the drug.
If it is between 31 and 50 ml/min, warfarin is initiated two days before discontinuation of the drug. When the creatinine clearance is between 15 ml/min and 30 ml/min, initiate warfarin one day prior to discontinuation of the drug.
Please note that the effect warfarin on the INR will be better reflected after dabigatran has been discontinued for two or more days.

The FDA recently approved the oral thrombin inhibitor dabigatran (Pradaxa) for the prevention of stroke or systemic embolism. The approval was based on the Randomized Evaluation of Long-Term Anticoagulation Therapy (RE-LY) trial, which found that dabigatran, an agent that does not require monitoring of the INR, was as safe and effective, and at the 150-mg twice daily dose, actually superior to warfarin. A blood thinner is a drug which is prescribed to a patient to help prevent blood clots. One type of this drug is Pradaxa, manufactured by Boehringer Ingelheim Pharmaceuticals, Incorporated. The drug helps those with atrial fibrillation, which is an irregular heartbeat.
There are a few other blood thinners designed for this use, including the prescription drugs Coumadin, Jantoven and Marfarin. These are more traditional type blood thinners, while Pradaxa was touted as being safer for the patient and less reactive to certain types of food. However, with the other drugs, if a bleed occurred as a side effect it was more manageable and able to be stopped. With Pradaxa, there is no real effective antidote and the risk can become very serious and lead to internal and cranial bleed events.
Because the drug has a half-life in the body's system that can last up to 12 hours, it is difficult to stop the bleeding and more drastic measures need to be taken. Dialysis seems to be the only option after a patient suffers significant bleed. The process takes several hours and is used to remove the drug from the patient's system. Patients who already suffer renal and kidney issues may have an even harder time removing the drug from their system, putting them at an even greater risk. See a doctor immediately even any type of bleeding event occurs and be aware that even minor injuries can result in larger bleed events due to the blood thinning nature of the drug.

Pradaxa is a drug used in the treatment and prevention of heart problems such as atrial fibrillation. This is a condition in which the patient may suffer blood clots, heart attacks, and strokes. These types of drugs which include other products like Coumadin and Warfarin can lead to bleeding events within the body. You may suffer gastrointestinal bleeds and bleeding within the brain. The problem with Pradaxa is that the bleeding events caused by the drug are much harder to stop.
While Coumadin and Warfarin can be stopped with vitamin K being given, Pradaxa has no easy means in which to stop the bleeding event. Some cases have needed to use dialysis to stop the issue, but the risk is very high in which someone can suffer permanent damage or even death if the bleeding is not contained. Pradaxa is an anticoagulant medicine used to stop blood clots forming within the blood vessels.
Pradaxa is used in the prevention of dangerous blood clots in the veins in adults who have undergone certain surgeries, blood clots that can cause stroke, as well as recurrent deep vein thrombosis -a condition where blood clots occur within the veins of the legs- and pulmonary embolism -when blood clots occur in the lungs.

How do Direct Thrombin inhibitors (Dabigatran) Work? These direct thrombin inhibitors are anticoagulants that block clotting factor 2 which prevent the production of fibirin. This causes a decrease in blood clots and ultimately "thins" the blood.
