New approaches to moderate to severe psoriasis unburden even the most stubborn cases.  This progression stems from recent understanding of the disease as an immune abnormality leading to chronic skin inflammation.  Over the past decade “biologics” have supplanted topical steroids and tar ointments when the disease covers more than 10% of the skin’s surface.
 
Early biologics including Humira, Enbrel and Remicade offer moderate relief but targeted a wider swath of the immune system.  Newer drugs narrow their focus and offer a greater likelihood of complete skin clearance within a relatively short timeframe.  Among the new generation are Cosentyx, Taltz and now Tremfya.
 
Tremfya is the first drug to specifically attack a critical messenger in the inflammatory cascade.  It blocks the ability of IL-23 to initiate the process leading to the psoriatic  manifestations of redness, itching and skin discomfort.
 
Treatment may be administered by either the patient or health care practitioner.  After an initial subcutaneous injection and another 1 month later, therapy continues with a single shot every 8 weeks.
 
First the good news – after 16 weeks, about 85% of patients initially suffering moderate to severe disease can expect clear or almost completely clear skin.  The bad news – the cash price for the drug is well is excess of $10,000.00 a syringe or $60,000.00 each year.  For those paying full price this averages $225 every day.
 

Fortunately the drug company whose sales exceed $200,000,000.00 each day are kind enough to offer a copay card that limits your out of pocket cost to $5 per syringe.  They seem content to reap their rewards from the insurance companies who in turn raise their premiums on everyone to cover these inflated charges.

Penn Medicine's Trauma Center uses all means necessary to quickly find and stop bleeding, as uncontrolled bleeding is the leading cause of death in patients who have been severely injured. Depending on the location and severity of bleeding, a patient may require specific medications, red blood cell transfusion, invasive catheter procedures, plasma or platelets, or surgery.

Patrick Kim, MD, trauma surgeon and the Trauma Program Medical Director at Penn Medicine's Level 1 Trauma Center, discusses life-threatening bleeding and the use of tranexamic acid (TXA), a medication that helps control bleeding after trauma which has been used to treat bleeding on a wide varity of patients in the operating room and Emergency Department.

"It has the greatest impact of survival on the patients who are bleeding the most," Dr. Kim explains. "In one study, TXA improved survival by 14% -- which is remarkable for any single medical treatment. TXA is an important part of our Trauma Center strategy to stop bleeding and save lives. Although it's not a replacement for blood transfusion or prompt surgery, it is an important addition to these treatments." Tranexamic acid is a synthetic derivative of lysine, classified as an antifibrinolytic agent.
It works by inhibiting the breakdown of fibrin clots, thereby promoting hemostasis.
It is commonly used to reduce bleeding in various clinical settings, including the following.
Trauma-related bleeding.
Surgical bleeding prevention in various types of surgeries, including Cardiothoracic surgeries, Orthopedic surgeries, Obstetric, and gynecologic surgeries.
In the treatment of heavy menstrual bleeding.
Hereditary angioedema.
Liver disease.
Hemophilia adjunct therapy during dental extractions.
And in the treatment of disseminated intravascular coagulation.
Tranexamic acid inhibits fibrinolysis by Competitive Inhibition of Plasminogen Activation.
It binds reversibly to lysine-binding sites on plasminogen, preventing its activation to plasmin.
Plasmin is the enzyme responsible for breaking down fibrin clots.
Tranexamic acid also weakly inhibits plasmin activity, further reducing fibrinolysis.
By stabilizing fibrin clots, it reduces bleeding without directly affecting platelet function or coagulation factors.
Tranexamic acid can be administered through various routes.
Intravenous route allows rapid onset of action and is commonly used in acute settings, such as trauma or perioperative bleeding.
Oral tranexamic acid is used for conditions like heavy menstrual bleeding or prophylaxis in hereditary angioedema.
It has a low bioavailability of approximately 30 to 50%.
Topical tranexamic acid preparations are also available and applied locally to surgical sites or wounds to control localized bleeding such as dental extractions and minor surgical procedures.
It is also available as solutions, sprays, or gels.
Intramuscular route is less commonly used.
Tranexamic acid is minimally metabolized and most of the drug is excreted unchanged via the kidneys.
The most common adverse effects of tranexamic acid include gastrointestinal disturbances such as nausea, vomiting, and diarrhea.
Dizziness, and headache.
Rare but Serious side effects include Thromboembolic events such as deep vein thrombosis, and pulmonary embolism.
Visual disturbances.
Allergic reactions such as rash, pruritus, or anaphylaxis.
And Hypotension, especially with rapid intravenous administration.
Absolute contraindications for the use of tranexamic acid include Active thromboembolic disease such as deep vein thrombosis, pulmonary embolism, or cerebral thrombosis.
Acquired defective color vision.
And history of hypersensitivity to tranexamic acid.
Relative contraindications include Renal impairment, History of thromboembolic events or clotting disorders, and Concurrent use of other antifibrinolytics or procoagulants.

- Tranexamic acid is an antifibrinolytic agent. It competitively and reversibly inhibits the activation of plasminogen, via binding at several distinct sites, knowing that activation of plasminogen to plasmin, degrades and dissolves fibrin clots, so by occupying the necessary binding sites, tranexamic acid prevents the dissolution of fibrin, thereby stabilizing the clot and preventing hemorrhage.

- In other words, it works by blocking the breakdown of blood clots, which prevents bleeding.

- Tranexamic acid is available as 500 mg tablets, and also available as injection.

- It is used for short-term treatment in patients with hemophilia, to reduce or prevent hemorrhage, and to reduce bleeding during, and following a tooth extraction.

- Tranexamic acid may also be used in hereditary angioedema, epistaxis, and in thrombolytic overdose.

The Na+/K+/2Cl- transporter protein, which is found in the walls of the ascending loop of Henle, is inhibited by these medications. These substances decrease salt or NaCl absorption, which dramatically boosts diuresis and increases urine output as a result.
But here is a catch, Blocking of the NKCC2 cotransporter makes potassium unable to be reabsorbed back into the lumen, which results in the loss of calcium and magnesium ions.
A secondary effect of loop diuretics is to increase the production of prostaglandins, which results in vasodilation and increased blood supply to the kidney.
The collective effects of decreased blood volume and vasodilation decrease blood pressure and ameliorate edema.
Several loop diuretics come in IV and oral forms.
Furosemide
Furosemide comes in oral tablet form in 20, 40, and 80 mg dosages. Injectable solutions come in 10 mg/mL doses. Oral solutions come in either 8 or 10 mg/mL doses.
The half-life for furosemide is 1.5 to 2 hours but can be up to 2.6 hours in those with renal/hepatic dysfunction or heart failure.
Torsemide comes in tablet form in 5, 10, 20, or 100 mg doses. The injectable solution is 10 mg/mL dosing. Torsemide is known to have the longest half-life at 3 to 4 hours and can be as long as 5 to 6 hours in patients with renal/hepatic dysfunction or heart failure.
Bumetanide comes in oral tablets of 0.5, 1, and 2 mg. The IV solution is 0.25 mg/mL. Bumetanide has a half-life of 1 hour and can be administered for 1.3 to 1.6 hours in those with renal/hepatic dysfunction or heart failure.
All three loops typically have a similar onset of action. Oral administration time between the three averages 30 to 60 minutes.

Adverse effects of loops Diuretics typically occur from electrolyte imbalances secondary to the diuresis effects, which include: hyponatremia, hypokalemia, hypochloremia, hypomagnesemia, metabolic alkalosis, prerenal azotemia, and dehydration.
The FDA has approved loop diuretics for treating hypertension alone or with the addition of other anti-hypertensives. However, loop diuretics alone are not used for first-line therapy.
