La gabapentina es un fármaco que se emplea para el tratamiento de la epilepsia y del dolor neuropático.

Gabapentina está disponible en el mercado en forma de cápsulas y comprimidos.

La dosis a emplear y la duración del tratamiento se deben ajustar individualmente y tienen que ser definidas por el médico. De forma general, para el tratamiento tanto de la epilepsia como del dolor neuropático, la dosis inicial va de 300 mg una vez al día a 900 mg al día dividido en tres tomas. Posteriormente, en base a la respuesta individual del paciente y la tolerabilidad, la dosis puede aumentarse en incrementos de 300 mg/día cada 2-3 días hasta una dosis máxima de 3600 mg/día.

En pacientes con insuficiencia renal, es necesario realizar un ajuste individualizado. Gabapentina se puede tomar con o sin alimentos y es aconsejable no consumir alcohol durante el tratamiento.

Entre sus reacciones adversas más frecuentes, destacamos el aumento de apetito, cambios en el estado de ánimo o dolores de cabeza. También es habitual que aparezca somnolencia y mareos, por lo que se debe tener precaución a la hora de conducir o realizar actividades peligrosas que requieran tu atención.

Entre las principales recomendaciones, es fundamental no usar gabapentina sin receta médica. No suspendas el tratamiento hasta que así lo indique tu médico y sigue sus indicaciones para realizar una suspensión gradual. Y finalmente, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

In the podcast this week, I talk about doxylamine pharmacology. Doxylamine is a first-generation antihistamine; it is commonly an active ingredient in night-time medications like Unisom, Nyquil, and Mucinex. The pharmacology of doxylamine is similar to other first-generation antihistamines, it competitively inhibits the binding of histamine at H1 receptors. Its main uses are as sleep aides, in cough-and-cold medications, but doxylamine has also been given with pyridoxine to treat nausea and vomiting during pregnancy.






Doxylamine’s adverse reactions are related to its anticholinergic properties, they include dry eyes, dry mouth, increased fall risk, sedation, urinary retention, constipation, and confusion. Contraindications include concurrent use with a monoamine oxidase inhibitor, known hypersensitivities, concomitant alcohol use, and if the patient has the following conditions: elevated intraocular pressure, narrow-angle glaucoma, asthma, peptic ulcer disease, urinary bladder neck obstruction, or gastric outlet obstruction. It is also a Beer’s list drug due to its anticholinergic effects. The normal dose in adults is 25 mg. In cases of overdosage, the most common manifestation is exacerbations of its anticholinergic effects. The major complications of an overdose include arrhythmia, respiratory failure, seizures, hyperthermia, rhabdomyolysis, and coma. 






When you know a patient is taking doxylamine, it’s important to be cognizant of their occupation, as well as what other conditions they may have. For example, doxylamine should be used with caution in patients that drive heavy machinery due to its sedating properties. You might be able to tell if a patient’s experiencing an adverse reaction exacerbation if they begin having worsening dementia symptoms or increased urinary retention. Other indications include the use of artificial tears, or saliva, or increased complaints of constipation. To monitor for doxylamine, it’s important to monitor the patient’s tolerability. The onset of doxylamine is relatively quick as well, with a peak concentration within 2-4 hours.






For drug-drug interactions, CYP interactions aren’t as concerning as usual. The main interaction to consider when a patient is taking doxylamine is additive anticholinergic effects. Sedative effects can increase when benzodiazepines, skeletal muscle relaxants, opioids, or antihistamines are concurrently taken. Doxylamine can also counteract the usefulness of dementia or BPH medications due to its anticholinergic properties. There is also a risk of increased anticholinergic burden when taken with skeletal muscle relaxants or tricyclic antidepressants.

La nitrofurantoína es un antibiótico derivado del nitrofurano. Es activa sobre un número importante de bacterias. No obstante, y al igual que cualquier otro antibiótico, no tiene ningún efecto frente a infecciones causadas por virus, como gripe, resfriado, COVID-19 o la mayoría de las infecciones de garganta.

Está indicado para el tratamiento de la cistitis aguda en niñas mayores de 3 meses, adolescentes y mujeres adultas.

La nitrofurantoína está disponible tanto en forma de comprimidos como en suspensión oral. La dosis recomendada para el tratamiento de la cistitis aguda no complicada en mujeres adultas es de 50-100 mg cada 8 horas durante 5-7 días.

Para niñas mayores de 3 meses y adolescentes, la dosis recomendada es de 5-7 mg/kg de peso por día, sin superar la dosis de adulto, repartidas en cuatro tomas durante 5-7 días. En el caso de niñas menores de 6 años, se debe usar la suspensión oral.

Se recomienda tomar nitrofurantoína durante la comida o con un vaso de leche.

Es importante usar nitrofurantoína solo para el tratamiento curativo de cistitis agudas y no como profilaxis, con una duración del tratamiento limitada a un máximo de 7 días puesto que un tratamiento prolongado puede tener riesgos pulmonares, hepáticos, alérgicos y neurológicos.

Las reacciones adversas más frecuentes en tratamientos con nitrofurantoína es la aparición de reacciones cutáneas alérgicas como prurito y urticaria; mareos; y problemas gastrointestinales como náuseas, vómitos, dolor abdominal y diarrea.

Pero el mayor riesgo de nitrofurantoína se debe al mal uso y al abuso que hacemos de los antibióticos, y es la aparición de resistencias, que pueden hacer que este antibiótico deje de ser eficaz. Para evitar o combatir este problema y reducir el riesgo de resistencias bacterianas, el Ministerio de Sanidad, a través de la Agencia Española de Medicamentos ha creado un grupo de expertos (Plan Nacional de la Resistencia a Antibióticos o PRAN), en el que participamos los farmacéuticos a través de representantes del Consejo General de Colegios Farmacéuticos.

Entre las principales recomendaciones, es fundamental no usar nitrofurantoína sin receta médica, ya que un antibiótico que te fue útil la última vez no tiene por qué serlo ahora. Además, es importante que tomes el medicamento a las horas y durante el periodo de tiempo que te haya indicado el médico. Y finalmente, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

MEDICAL ANIMATION TRANSCRIPT: A vaginal yeast infection is a common condition where too much yeast grows in a woman's vagina. Other names for it are vulvovaginal candidiasis and candida vulvovaginitis. Yeast is a type of fungus which is a plant-like organism. The type of yeast that usually causes the infection is called candida albicans. Your vagina is the passage from the uterus to the outside of your body. The surface inside your vagina normally contains small amounts of this yeast, which usually doesn't cause any problems. Other germs, such as the healthy bacterium Lactobacillus, also live in the vagina with Candida. They keep each other in balance. Sometimes conditions change in the vagina that make the yeast grow out of control. When this happens, yeast cells invade the lining of your vagina, called mucosal epithelium. As the yeast grows and damages your cells, it causes immune cells to enter the tissue. This inflames and damages the tissue as your immune cells try to clear the infection. Unfortunately, yeast cells can avoid immune destruction by killing immune cells. Or, yeast cells may grow too large for immune cells to destroy them. This allows the yeast to grow and spread, forming a layer called a biofilm in your vagina. It's not always clear why a person gets a yeast infection. But, you're more likely to get a yeast infection if you are pregnant, have diabetes. wash the inside of your vagina, also known as douching, have a weakened immune system, take birth control pills, or have taken antibiotics or steroid drugs. The most common symptom of a yeast infection is an extreme itchy feeling in your vagina and your vulva, which is the area around the entrance to your vagina. Other symptoms may include redness, burning and swelling in your vagina or vulva, pain during urination or sex, and a thick white odorless discharge from your vagina. You can treat a vaginal yeast infection with antifungal medicines. These medicines may be creams or suppositories that you apply in your vagina. Or, they may be pills you take by mouth. It's important to use an antifungal medication for as long as the directions tell you to. Don't stop using them early, even if your symptoms go away. Some of the ways you can help prevent a vaginal yeast infection include only washing your vulva, not inside your vagina, and not using douches or hygiene sprays. Keeping the genital area clean and dry. Not wearing tight underwear or pants. Wearing underwear with a cotton crotch. Always wiping front to back after using the toilet. Cleaning your vulva after sex. And, keeping your blood sugar under control if you have diabetes. Also, take antibiotics for other infections only as prescribed.
