Clotrimazole is an imidazole antifungal that exerts its pharmacological effect by inhibiting the synthesis of ergosterol, an essential component of fungal cell membranes. This inhibition compromises membrane integrity, leading to leakage of cellular contents and ultimately fungal cell death.
Clotrimazole is primarily used topically due to poor systemic absorption when administered via the skin or mucous membranes, which limits systemic side effects.
When clotrimazole is used intravaginally or orally in lozenge form, localized concentrations are sufficient to treat mucocutaneous infections without significant systemic exposure.
Pay attention when clotrimazole is used frequently to treat Candida infections as corticosteroids, immunosuppression, and antibiotics may increase the risk of this type of infection.

As Covid Vaccine mandates become more popular, I have a few thoughts as to what is ethically the right move here. Covid vaccination decreases spread of illness, and will decrease likelihood of transmission to patients. However the vaccine mandate may cause more nurses to leave already short staffed units, making working conditions more unsafe. And costing more lives in the end.

Digoxin is a cardiac glycoside that works by increasing the strength of the heartโs contractions and slowing down the heart rate. It is used clinically to treat atrial fibrillation and heart failure. Digoxin has a narrow therapeutic range, and side effects are seen above dose levels of 2 ng/mL. The risk of toxicity is increased with hypokalemia, so patients should have their potassium levels monitored closely. The earliest sign of digoxin toxicity is GI distress, and later signs include bradycardia, vision changes, and arrhythmias. The apical pulse should be monitored for a full minute before administration, and the digoxin dose should not be given if the heart rate is less than 60 minutes beat per minute.
