A Lamotrigina é um anticonvulsivante da classe das feniltriazinas, com menos efeitos colaterais que outros fármacos. Ela age bloqueando canais de sódio e cálcio, reduzindo a liberação de glutamato. Isso ajuda a controlar a hiperatividade neuronal e prevenir crises convulsivas.

A Lamotrigina é eficaz na epilepsia e no transtorno bipolar, principalmente na prevenção de episódios depressivos. Também é usada off-label para TEPT, dor neuropática central e dermatotilomania, com evidências promissoras. Seu efeito se deve à modulação do glutamato, ajudando na estabilização do humor e controle de sintomas compulsivos.

A Lamotrigina pode causar reações cutâneas em 6 a 10% dos pacientes, desde erupções leves até condições graves, como Síndrome de Stevens-Johnson e Necrose Epidérmica Tóxica, principalmente em crianças e ao usar Valproato. On this podcast episode, I discuss Clotrimazole pharmacology, adverse effects, indications, administration, and much more.
Clotrimazole is an imidazole antifungal that exerts its pharmacological effect by inhibiting the synthesis of ergosterol, an essential component of fungal cell membranes. This inhibition compromises membrane integrity, leading to leakage of cellular contents and ultimately fungal cell death.
Clotrimazole is primarily used topically due to poor systemic absorption when administered via the skin or mucous membranes, which limits systemic side effects.
When clotrimazole is used intravaginally or orally in lozenge form, localized concentrations are sufficient to treat mucocutaneous infections without significant systemic exposure.
Pay attention when clotrimazole is used frequently to treat Candida infections as corticosteroids, immunosuppression, and antibiotics may increase the risk of this type of infection.

As Covid Vaccine mandates become more popular, I have a few thoughts as to what is ethically the right move here. Covid vaccination decreases spread of illness, and will decrease likelihood of transmission to patients. However the vaccine mandate may cause more nurses to leave already short staffed units, making working conditions more unsafe. And costing more lives in the end.

Trulicity (dulaglutide) and Ozempic (semaglutide) are medications used to treat Type 2 diabetes. They're taken in combination with diet and exercise.
Both medications may also lower the risk of heart attack, stroke, or death related to heart disease. But Trulicity is approved for this use in more people than Ozempic
Both Ozempic and Trulicity are GLP-1 medications that are indicated for the treatment of type 2 diabetes. However, Ozempic has also been approved by the FDA to support chronic weight management. Trulicity is only approved for the treatment of type 2 diabetes in adults.
Ozempic is a once-weekly injection used to treat Type 2 diabetes.

Ocrevus (ocrelizumab) and Kesimpta (ofatumumab) are two drugs for multiple sclerosis which both work on b-cells. Drug Cards Daily | Episode #37: gabapentin (Neurontin) | For Seizures and Neuropathic Pain
Gabapentin has a wide variety of uses but the main indications are for focal seizures in adults and children and for postherpatic neuralgia in adults. There are many commonly used off label indications such as for neuropathic pain, fibromyalgia, and alcohol dependence. Max dosing for gabapentin is around 3600 mg/day. Although the exact mechanism of action is unknown, it is clear that it is similar in chemical structure to GABA. This is important because GABA is responsible for modulating excitatory neurotransmitter release. Gabapentin is poorly protein bound at less than 3% and not metabolized. When discontinuing longer term therapies, doses should be tapered down over at least 7 days. Some of the most common side effects are dizziness, fatigue, and weight gain. The serious side effects are depression, suicidal thoughts and behaviors, and withdrawal related seizures.

Digoxin is a cardiac glycoside that works by increasing the strength of the heart’s contractions and slowing down the heart rate. It is used clinically to treat atrial fibrillation and heart failure. Digoxin has a narrow therapeutic range, and side effects are seen above dose levels of 2 ng/mL. The risk of toxicity is increased with hypokalemia, so patients should have their potassium levels monitored closely. The earliest sign of digoxin toxicity is GI distress, and later signs include bradycardia, vision changes, and arrhythmias. The apical pulse should be monitored for a full minute before administration, and the digoxin dose should not be given if the heart rate is less than 60 minutes beat per minute.
