0:00 Introduction
0:46 Uses of Alprazolam
1:10 Potential side effects of Alprazolam
2:30 Important considerations

Today, we are delving into the world of anxiety medications, with a specific focus on Alprazolam. Commonly known by its brand name Xanax, Alprazolam is one of the most prescribed medications for managing anxiety and panic disorders. In this blog, we'll explore its uses, potential side effects, and important considerations to be aware of when using this medication. So, let's dive right in!

What is Alprazolam?
Alprazolam is a benzodiazepine drug that belongs to the class of central nervous system depressants. It works by enhancing the effects of a neurotransmitter called gamma-aminobutyric acid (GABA), which helps calm down the brain and nervous system, reducing anxiety and promoting relaxation.

Common Uses of Alprazolam:
Alprazolam is primarily prescribed to treat generalized anxiety disorder (GAD), panic disorder, and social anxiety disorder. It's usually prescribed for short-term relief of anxiety symptoms and is not typically recommended for long-term use due to the risk of dependence and tolerance.

Potential Side Effects:
Like all medications, Alprazolam can cause side effects, which may include drowsiness, dizziness, headaches, and dry mouth. It's essential to be aware of these potential side effects and discuss them with your healthcare provider.

Important Considerations:
Before taking Alprazolam, it's crucial to inform your doctor about any pre-existing medical conditions or medications you're currently taking. Alprazolam may interact with certain drugs, including other medications and substances that depress the central nervous system, such as alcohol.

Risk of Dependence and Withdrawal:
Due to the potential for dependence, Alprazolam should be used only as prescribed and not in larger amounts or for longer than recommended. Abruptly stopping the medication can lead to withdrawal symptoms, so it's essential to follow your doctor's guidance when discontinuing its use.

Alprazolam and Mental Health:
While Alprazolam can provide relief for those struggling with anxiety, it's not a cure for underlying mental health conditions. It's crucial to combine medication with therapy and lifestyle changes to address the root causes of anxiety.

Alprazolam and Pregnancy:
If you are pregnant or planning to become pregnant, discuss the use of Alprazolam with your healthcare provider. The medication may pose risks to the developing fetus, and the benefits and potential risks must be carefully considered.

Managing Anxiety Holistically:
While Alprazolam can be beneficial for managing acute anxiety symptoms, it's important to explore other coping mechanisms and lifestyle changes to support overall mental well-being. Techniques such as mindfulness, meditation, regular exercise, and a balanced diet can all contribute to anxiety reduction.

Talking to Your Doctor:
Always consult with your healthcare provider before starting or stopping any medication, including Alprazolam. They can guide you on the appropriate dosage, duration of use, and any potential interactions with other medications.

Conclusion:
Alprazolam can be a useful tool in managing anxiety and panic disorders, but it's essential to use it responsibly and under the guidance of a qualified healthcare professional. Remember that no medication is a substitute for comprehensive mental health care, so consider combining medication with therapy and healthy lifestyle choices for long-term well-being. As always, prioritize open communication with your healthcare provider to ensure your treatment plan aligns with your individual needs and goals. Thank you for joining us today, and we hope this blog has provided valuable insights into Alprazolam and its role in anxiety management.

8 Weird and Unexpected Side Effects of Pantoprazole (Skin, Infections, Nutrient Deficiencies).

Have you ever wondered why your skin feels different after starting a medication? Could your frequent infections be linked to the pill you’re taking every morning? Or why you're suddenly experiencing symptoms of nutrient deficiencies, despite eating a balanced diet?
Pantoprazole, a widely used acid reflux medication, has some unusual side effects that most people don’t know about. From unexpected changes in your skin to recurrent infections and hidden nutrient imbalances, these effects might seem random, but they’re deeply connected to how this medication works in your body. Also, note that these side effects may not be experienced by everyone and only a few individuals might experience them in long-term use.
In this video, I’ll reveal 8 strange side effects of pantoprazole, explain the science behind why they happen, and share actionable tips to help you minimize or even prevent these issues. Whether you're currently on pantoprazole or simply curious about its effects, this is information you can’t afford to miss—so let’s dive in!

Now what is pantoprazole?
Pantoprazole is a widely used medication prescribed to manage conditions such as gastroesophageal reflux disease (GERD) and Zollinger-Ellison syndrome. It belongs to a class of drugs known as proton pump inhibitors (PPIs) that work by reducing the production of stomach acid. This reduction helps alleviate heartburn and prevents damage to the esophagus caused by excessive acid. Pantoprazole's effectiveness in controlling acid production makes it a popular choice for those suffering from acid-related conditions.
Despite its therapeutic benefits, pantoprazole is not without its downsides. Individuals on this medication may experience a variety of side effects, some of which can be quite unexpected.

Summary:
1. May interfere with other medication
Do you take over the counter or prescription medication with dietary supplements? You may be putting yourself at risk! For some people, these supplements increase the effectiveness of other medication, on the other hand it may decrease it as well.

2. Acne
Biotin is known for adding volume to hair and making nails strong, but is acne the price you really want to pay? The body absorbs biotin the same way it absorbs other vitamins and minerals - through the gastro system. This is also the system that absorbs acne causing agents present in the skin. But, in case of biotin overload, this absorption gets interrupted. In the end, this results in less acne fighting vitamins and of course, the return of the dreaded acne! If you are taking biotin regularly and notice some acne, especially along your chin and jawline, then biotin may be doing more than beautifying your hair.

3. Skin rash
This just keeps getting worse! If you thought that acne was horrible then let me tell you, the skin problems related to biotin don't stop there. Skin rash is a common side effect of too much biotin in your system.

4. Upset stomach
The moment you feel like your stomach health is being compromised, stop! Unhealthy gut health is a breeding ground for several diseases and a weakened immune system. Basically, a happy gut is the secret to a happy life.

Why Caplyta is the Game-Changing Medication for Mental Health: Exploring its Benefits
Why Caplyta is the Game-Changing Medication for Mental Health: Exploring its Benefits Side Effects 6 Views • 3 months ago

Caplyta represents a significant breakthrough in the field of mental health medication. Unlike traditional treatments, Caplyta specifically targets specific receptors in the brain, reducing the risk of unwanted side effects. This targeted approach allows for a more precise and effective treatment, tailored to the individual's unique needs.
One of the key factors that sets Caplyta apart is its ability to deliver therapeutic relief without causing weight gain, sedation, or movement disorders commonly associated with other medications. This is a game-changer for individuals who have struggled with these side effects in the past, as it allows them to focus on their mental well-being without the added burden of physical discomfort.
Additionally, Caplyta's formulation is designed to provide long-lasting symptom relief, reducing the need for frequent dose adjustments or additional medications. This stability in treatment allows individuals to regain control over their lives and pursue their goals and aspirations with confidence.
Benefits of Caplyta medication.
The benefits of Caplyta medication extend beyond its ability to target specific receptors and avoid debilitating side effects. This groundbreaking medication offers a range of advantages that have transformed the lives of countless individuals struggling with mental health conditions.
One of the primary benefits of Caplyta is its effectiveness in treating a variety of symptoms associated with mood disorders, psychosis, and schizophrenia. Clinical trials have shown significant improvements in symptoms such as hallucinations, delusions, disorganized thinking, and emotional instability. This comprehensive approach to treatment allows individuals to experience relief across multiple aspects of their condition, leading to a better quality of life overall.
Another notable benefit of Caplyta is its ability to improve cognitive function in individuals with mental health conditions. Cognitive impairments are common in conditions such as schizophrenia, making it difficult for individuals to think clearly, concentrate, or remember information. Caplyta's unique formulation has been shown to enhance cognitive abilities, enabling individuals to regain control over their thought processes and engage more effectively in their daily lives.
Furthermore, Caplyta has demonstrated a lower risk of drug-drug interactions compared to other medications. This is particularly important for individuals who may be taking multiple medications to manage their mental health condition and other medical issues. With Caplyta, individuals can have peace of mind knowing that their treatment is less likely to be compromised by potential interactions.
Clinical studies and research supporting Caplyta's effectiveness.
The effectiveness of Caplyta as a game-changing medication for mental health treatment is not simply anecdotal. Extensive clinical studies and research have been conducted to validate its efficacy and safety.
Multiple randomized controlled trials involving thousands of participants have consistently shown that Caplyta is superior to placebo in reducing symptoms related to mood disorders, psychosis, and schizophrenia. These studies have included diverse patient populations, ensuring that the benefits of Caplyta extend across different demographics.
In addition to symptom improvement, research has also highlighted the positive impact of Caplyta on overall functioning and quality of life. Individuals taking Caplyta reported significant improvements in their ability to engage in social interactions, maintain employment, and pursue personal goals. This holistic approach to treatment is a testament to the transformative effects of Caplyta on individuals' lives.
How Caplyta is changing the landscape of mental health treatment.
Caplyta's introduction into the field of mental health medication has undoubtedly reshaped the landscape of treatment options. Its unique formulation and targeted approach have challenged traditional methods, offering a new paradigm for individuals and healthcare professionals alike.
The ability of Caplyta to provide relief without debilitating side effects has given individuals hope and confidence in their treatment journey. The fear of weight gain, sedation, or movement disorders no longer hinders individuals from seeking the help they need.

स्टीरॉयड इंजेक्शन लेना चाहिए या नहीं ? Steroid injections for Pain Management
#steroids #steroidssideeffects #painkiller #painmanagement

Steroids: What Are They and How Do They Work? Types, Dosage, Benefits, Side Effects, and Alternatives

In this #video we will discuss the topic of steroids - what they are and how they work. We will explore the different types of steroids, their recommended dosage, potential benefits, and the time it takes for them to start acting. Additionally, we will cover the duration of action, potential side effects, whether steroids can be repeated, and if other #painkillers lose their effectiveness after steroid use. We will also touch upon steroid alternatives and address the issue of pain during injections.

Steroids are commonly used as painkillers for various conditions, including #arthritis and #slippeddisc . However, it is crucial to administer steroid injections under proper guidance to avoid any side effects that can potentially damage the tissues at the injection site.

Some commonly used steroids include Dexamethasone, Triamcinolone, and Methylprednisolone. The dosage of steroids plays a significant role, as larger doses can lead to more side effects, while smaller doses may offer limited relief. Therefore, it is important to strike a fine balance based on your specific condition.

The effects of steroids on the body depend on factors such as the location of the injection, correct injection site, and individual variation. Common side effects may include pain at the injection site, steroid flare, and mild fluctuations in sugar levels. However, rare side effects such as changes in skin color, skin dip due to fat melting, glaucoma, mood changes, and menstrual cycle changes can also occur. Oral steroids generally tend to have more side effects compared to injectable steroids.
Can steroids be used repeatedly, or does the efficacy of other painkillers diminish after steroid use? Currently, there is no conclusive research evidence regarding this matter, so there's no need to worry. The frequency of steroid use can range from 2-3 times a year, depending on your condition. However, repeated injections in specific sites like the knee have been associated with injuries, so it is generally advisable to use the minimum effective dosage.

When it comes to alternatives to steroids, options such as platelet-rich plasma, radio-frequency treatment, or knee replacements can be considered for conditions like #kneearthritis . The choice of alternatives will depend on your specific condition and should be decided in consultation with a #painspecialist .

Pain during steroid injections is similar to any other injection and can vary in intensity according to individual perception.

In conclusion, if your doctor has recommended steroid #treatment there is no need to worry. However, it is essential to ensure that the injections are guided by imaging modalities for accuracy. Additionally, be prepared for varying responses to the treatment, and keep in mind that injectable steroids usually have fewer side effects compared to oral steroids. Regarding the dosage of steroids, your #doctor is the best person to make decisions based on your condition. If you are looking for alternatives to steroids, it is advisable to consult a pain specialist who can guide you through the options.

The Truth Behind Ozempic Face (Wegovy) | Doctor Explains
The Truth Behind Ozempic Face (Wegovy) | Doctor Explains Side Effects 6 Views • 3 months ago

In this video, I will share with you the science behind these “new miracle drugs” - Ozempic and Wegovy and whether you can get an unusual appearence called 'Ozempic Face'. These are both a drug called semaglutide, and work by mimicking the effects of a hormone called (GLP-1) in the body, which helps to regulate blood sugar levels and aids in weight loss.

Ozempic was given to Diabetes patients, and after reports of considerable weight loss, the same laboratory created Wegovy, the same medication but a version made for non-diabetes to lose weight. People taking Ozempic/ Wegovy have reported these Unusual symptoms:

- A hollowed-out appearance on their face
- More wrinkles and lines across, making them look older
- Loose and saggy skin

And we hear all over the media the so-called “ozempic face”. However, its more of a side effect of losing weight quickly, here is why...

When we lose weight, we start to lose the stores of fat around the body (the classic belly fat), but we also lose fat around organs and across our face. If the weight loss occurs very quickly, our skin may not have enough time to adjust to the changes in our body, causing loose, sagging skin, especially in areas where there was a lot of fat or muscle mass before the weight loss. This would occur to anyone going to a sudden loss of weight and is not linked to a specific side effect of a drug.

Doctor explains CETIRIZINE | Uses, doses, side effects & more [Educational guide]
Doctor explains CETIRIZINE | Uses, doses, side effects & more [Educational guide] Side Effects 5 Views • 3 months ago

In this video Doctor O'Donovan explains key things you need to know about cetirizine (brand names: Allacan, Benadryl Allergy, Piriteze Allergy, Zyrtek Allergy).

Cetirizine is an antihistamine medicine that helps the symptoms of allergies.

It's used to treat:

- hay fever
- conjunctivitis (red, itchy eye)
- eczema
- hives (urticaria) – an itchy, raised rash that’s usually caused by an allergic reaction
- reactions to insect bites and stings
- some food allergies

Cetirizine is known as a non-drowsy antihistamine. It's much less likely to make you feel sleepy than some other antihistamines.

Cetirizine is available on prescription, and to buy from pharmacies and shops.

It comes as tablets, capsules and as a liquid that you swallow.

Key facts

1. You'll usually take cetirizine once a day. Children aged under 12 years old take a lower dose twice a day.
2. Cetirizine is classed as a non-drowsy antihistamine, but some people still find it makes them feel quite sleepy. Do not drive or ride a bike if it makes you sleepy.
3. Common side effects include headaches, dry mouth, feeling sick, feeling dizzy and diarrhoea.
4. Some brands of cetirizine come as capsules. These may contain soya oil. Do not take cetirizine capsules if you’re allergic to peanuts or soya.

Some Benadryl products do not contain cetirizine, but a different antihistamine such as acrivastine.

**‘Reporting of side effects**

If you get any side effects, talk to your doctor, pharmacist or nurse. This includes any possible side effects not listed in the package leaflet. You can also report side effects directly via the Yellow Card Scheme at [a website address which links directly to the MHRA Yellow Card site].

By reporting side effects, you can help provide more information on the safety of this medicine.’

The Single WORST Side Effect of Cortisone Shots
The Single WORST Side Effect of Cortisone Shots Side Effects 5 Views • 3 months ago

I reveal the single WORST side effect associated with cortisone shots and corticosteroid injections. We review recent clinical trial data that reveals serious consequences for joints and tendons. Cortisone injections can hinder tissue healing, damage collagen, and increase tissue degradation, which poses a problem for orthopedics. Clinical trials show that the short-term pain reduction effect of cortisone injections is reversed in the intermediate and long terms, leading to tendon damage. The injections can also harm cartilage, worsening conditions like osteoarthritis and potentially requiring joint replacement surgery.

Hormone Therapy for High-Risk Prostate Cancer | Prostate Cancer Staging Guide
Hormone Therapy for High-Risk Prostate Cancer | Prostate Cancer Staging Guide Side Effects 4 Views • 3 months ago

PCRI’s Executive Director, Mark Scholz, MD, discusses the use of hormone therapy for men with Azure (High-Risk) prostate cancer. He covers why it is necessary for men with high-risk disease and talks about the three categories of hormone therapy.

0:28 Men with Azure are generally given hormone therapy because there is a higher risk of spread outside the prostate. Hormone therapy is a systemic treatment that acts on the entire body, not just the prostate. It is effective, and its side effects are reversible. Compared to chemotherapy, another systemic treatment, it is more effective and has far fewer side effects.

1:23 The three categories of hormone therapy. Hormone therapy, also known as testosterone inactivating pharmaceuticals (TIP) comes in three different strengths. The first and least potent category of hormone therapy is the “anti-androgens.” Common examples of these drugs are Casodex and Nilutamide. The second category and the most commonly used type of hormone therapy are the “LHRH Agonists” drugs such as Lupron, Eligard, and Trelstar. The last category is the second generation hormonal agents that operate differently than the previous categories. Examples of these are Erleada, Xtandi, and Zytiga. You don’t want more hormone therapy than what is necessary because of added side effects.

2:43 Hormone therapy for the different subtypes of Azure. Each subtype of azure requires different amounts of hormone therapy. High-risk prostate cancer should be treated with a combination of radiation, seed implants, and hormone therapy, but the amount of hormone therapy can differ depending on the subtype. Low-Azure only requires 4-6 months of Lupron and Casodex. Basic-Azure needs between 18-24 months for optimal cure rates. And High azure needs 18-24 months possibly with the addition of a second generation TIP such as Erleada, Xtandi, or Zytiga.

4:12 Summary. The amount of hormone therapy depends on the subtype of prostate cancer. Many studies have shown that Low-Azure only requires a short course of hormone therapy and High-Azure can benefit from adding second generation hormone therapy.

Help Reduce Risk of Serious RSV Lung Infection for Your Baby
Help Reduce Risk of Serious RSV Lung Infection for Your Baby Side Effects 4 Views • 3 months ago

Did you know while hospitalization is uncommon, respiratory syncytial virus (RSV) is the leading cause of hospitalization for babies under 1? RSV season is typically fall – spring but can vary by local areas and today I'm sharing about a preventative antibody which can help protect against severe RSV lung infections. I'm joined by Shawn Johnson East and Dr. Natalie Barnett, Vice President of Clinical Research at Nanit, to discuss the importance of RSV protection for babies. They share personal experiences, including Shawn's family's journey with RSV, and offer expert insights on how parents can help protect their little ones during RSV season.

They join me to discuss:

• How RSV is common and can be a serious virus for infants.
• Beyfortus (nirsevimab-alip) 50mg and 100mg Injection, a preventative antibody that helps give babies an extra layer of protection against severe RSV lung infection for babies under age 1 born during or entering their first RSV season and certain kids up to 24 months. Your child should not take Beyfortus if your child has a history of serious allergic reactions to nirsevimab-alip or any of the ingredients in Beyfortus. Beyfortus may not protect all children.
• That parenting can be scary but navigating the journey with monitors like Nanit, and knowledge of prevention measures for RSV can be empowering.

To connect with Shawn Johnson East follow her on Instagram @shawnjohnson. Visit Nanit.com to see their products and learn more about their research. For more about Beyfortus visit

INDICATION
Beyfortus is a prescription medicine used to help prevent a serious lung disease caused by Respiratory Syncytial Virus (RSV) in:
• Newborns and babies under 1 year of age born during or entering their first RSV season.
• Children up to 24 months of age who remain at risk of severe RSV disease through their second RSV season.

IMPORTANT SAFETY INFORMATION

Your child should not take Beyfortus if your child has a history of serious allergic reactions to nirsevimab-alip or any of the ingredients in Beyfortus.

Before your child receives Beyfortus, tell your healthcare provider about all of your child’s medical conditions, including if your child:
• has ever had a reaction to Beyfortus.
• has bleeding or bruising problems. If your child has a problem with bleeding or bruises easily, an injection could cause a problem.

Tell your healthcare provider about all the medicines your child takes, including prescription and over-the- counter medicines, vitamins, and herbal supplements. Your infant should not receive a medicine called palivizumab if they have already received Beyfortus in the same RSV season.

Serious allergic reactions have happened with Beyfortus. Get medical help right away if your child has any of the following signs or symptoms of a serious allergic reaction:
• swelling of the face, mouth, or tongue
• difficulty swallowing or breathing
• unresponsiveness
• bluish color of skin, lips, or under fingernails
• muscle weakness
• severe rash, hives, or itching

The most common side effects of Beyfortus include rash and pain, swelling, or hardness at the site of your child’s injection. These are not all the possible side effects of Beyfortus. Call your healthcare provider if you have questions about side effects.

Vitamin E plays many roles in our body. It’s perhaps best known for its antioxidant effects, protecting your cells from oxidative damage by neutralizing harmful molecules called free radicals. In addition, it’s needed for proper immune function and cellular signalling.
Though vitamin E is often thought of as a single compound, it’s actually a group of eight fat-soluble compounds with powerful antioxidant effects.
Of these eight chemical forms, alpha-tocopherol best meets the dietary requirements of humans.
Vitamin E exists naturally in certain foods, including seeds, nuts, some vegetables, and some fortified products. You can also take it as a dietary supplement.

Entresto is a brand name for a medication called sacubitril/valsartan. Can Hyoscyamine Help With Anxiety? - Pharmaceutical Insights
Can Hyoscyamine Help With Anxiety? - Pharmaceutical Insights Side Effects 4 Views • 3 months ago

Can Hyoscyamine Help With Anxiety? In this informative video, we'll discuss the role of hyoscyamine, a medication primarily used for gastrointestinal disorders, and its potential effects on anxiety. Hyoscyamine is an anticholinergic medication that works by blocking acetylcholine, a neurotransmitter involved in muscle contractions and secretions. While it is effective for conditions like irritable bowel syndrome and bladder spasms, its use for anxiety is not typical or approved.

We'll cover how this medication interacts with the central nervous system and why it may lead to side effects like dizziness or drowsiness. It's important to understand that these side effects are not intended to treat anxiety and can complicate treatment, especially when combined with other medications.

If you or someone you know is dealing with anxiety, seeking advice from a healthcare professional is essential to find the right treatment options. This video aims to clarify the appropriate uses of hyoscyamine and emphasize the importance of consulting with healthcare providers for anxiety management. Heparin Mechanism of Action | Adverse Effects
Heparin Mechanism of Action | Adverse Effects Side Effects 4 Views • 3 months ago

We have brought you another pharmacology lecture on “thrombolytics”, focusing on Heparin, this time. Let us dive into its Heparin classifications, Heparin mechanism of action, pharmacodynamics, and Heparin pharmacokinetics. Cool medical animations and interactive lectures will ensure that you grasp all the concepts about Heparin Pharmacology perfectly!

▬ 📌 What is Heparin?
Heparin happens to be an injectable indirect anticoagulant. It is a heterogeneous group of straight-chain anionic mucopolysaccharides, called glycosaminoglycans.

▬ 📌 Therapeutic Role of Heparin
But what is the therapeutic role of heparin? What does heparin do?

Well, it helps to prevent clot formation (hemostasis). So it is used in the treatment of deep vein thrombosis (DVT), prophylaxis, pulmonary embolism, myocardial infarction, and coagulopathies.

▬ 📌 Heparin Classification
There are three main types of heparin.
🔹Standard heparin (native heparin)- Obtained from porcine mast cells
🔹Low-molecular-weight heparin (fractionated heparin)- produced by cleavage of native heparin
🔹Synthetic heparin (fondaparinux)

▬ 📌 Heparin Mechanism of Action
Heparin is an anticoagulant. So, it has to interfere with some proteins of the coagulation cascade to hinder hemostasis. Its biological activity is dependent upon the endogenous anticoagulant antithrombin. Antithrombin is a circulating glycoprotein capable of inhibiting clotting factor proteases, especially thrombin (factor IIa), factor IXa, and factor Xa.

Learn more about the action of clotting factors in our lecture on hemostasis.

▬ 📌 Heparin vs Warfarin
The other indirect anticoagulant is warfarin. Heparin is much safer than warfarin. Especially in the case of pregnant women, the anticoagulants of choice would be Heparin because they do not cross the placenta, due to their large size and negative charge.

Watch more on the differences between these drugs in our short video: Heparin vs Warfarin

▬ 📌 Heparin Side Effects
The main heparin side‐effect is bleeding. Due to this, heparin should not be given to patients with hemophilia, thrombocytopenia, or peptic ulcers.

Other side-effects seen with heparin are:
🔸Hemorrhage
🔸Skin necrosis
🔸Hypersensitivity reactions

Watch to get all the knowledge about Heparin.

The amount of calcium in circulation is MAINLY regulated by 2 hormones: parathyroid hormone (PTH) and calcitriol. PTH is produced in the parathyroid gland while calcitriol is made in the kidney. When serum calcium level is low, PTH is UP-regulated. PTH acts to PROMOTE calcium release from bones and REDUCE calcium loss from urine. At the same time, it stimulates production of calcitriol, which promotes absorption of calcium in the small intestine while also INcreases RE-absorption in the kidney. Together, they bring UP calcium levels back to normal. The REVERSE happens when calcium level is high. This feedback loop keeps serum calcium concentrations within the normal range.
HYPERcalcemia is generally defined as serum calcium level GREATER than 2.6 mmol/L. Because the total serum calcium includes albumin-bound and free-ionized calcium, of which only the LATTER is physiologically active, calcium levels must be corrected to account for albumin changes. For example, INcreased albumin levels produce HIGHER serum calcium values but the amount of FREE calcium may STILL be normal. On the other hand, in conditions with low blood pH, albumin binds LESS calcium; releasing MORE FREE calcium while the total serum calcium may appear normal.
Most symptoms of hypercalcemia can be attributed to the effect it has on action potential generation in neurons. High levels of extracellular calcium INHIBIT sodium channels, which are essential for DEpolarization. Hypercalcemia therefore REDUCES neuronal excitability, causing confusion, lethargy, muscle weakness and constipation. In most cases, excess calcium in the blood is a direct result of calcium release from bones as they break down, becoming weak and painful. As the kidneys try to get rid of the extra calcium, MORE water is also removed, resulting in dehydration, excessive thirst and kidney stones. Extremely high extracellular calcium may also affect cardiac action potentials, causing arrhythmias. Typical ECG findings include short QT interval, and in severe cases, presence of Osborn waves.
While hypercalcemia may result from a variety of diseases and factors, HYPERparathyroidism and cancers are responsible for about 90% of cases, with the former being by far the most common cause. In HYPERparathyroidism, PTH is OVERproduced due to benign or malignant growths within the parathyroid gland.
An existing cancer elsewhere in the body can cause hypercalcemia in 2 major ways. First, some cancer cells produce a protein similar to PTH, called PTHrP, which acts like PTH to increase serum calcium. Unlike PTH, however, PTHrP is NOT subject to negative feedback; consequently, calcium levels may keep rising unchecked. Second, cancers may spread to bone tissues, causing bone resorption or osteolysis, and subsequent calcium release into the blood.
Hypercalcemia treatment consists of lowering blood calcium levels with a variety of drugs, and addressing the underlying cause. While treatment outcome for hyperparathyroidism is generally excellent, prognosis for malignancy-related hypercalcemia is poor, possibly because it usually occurs in later stages of cancer.

An Unusual Side Effect of Acyclovir: Junctional Rhythm-Poonam Patel
An Unusual Side Effect of Acyclovir: Junctional Rhythm-Poonam Patel Side Effects 4 Views • 3 months ago

Co-Authors: Vagharsh Antanesian, MD MPH, Internal Medicine, PGY 3,Norton Community Hospital Norton, VA

Introduction: Acyclovir, an antiviral agent primarily targeting Herpes Simplex Virus (HSV) infections, presents predominantly mild adverse effects such as malaise (10%), gastrointestinal symptoms, and elevated liver function tests (1-10%). However, more severe complications, notably associated with intravenous administration, encompass rare occurrences of acute renal failure, neurological toxicity, and sporadic cases of bradycardia. A review of literature revealed 2 instances linking acyclovir to bradycardia, with no documented reports on junctional rhythm. A phase IV clinical study examining FDA data identified 1,424 individuals encountering side effects from Acyclovir, with only 6 individuals (0.42%) reporting bradycardia, primarily observed in males aged 60 and above using the medication for less than a month. Here we report a rare case of junctional rhythm caused by acyclovir.
