Bone Fracture. Risks of PPIs for Acid Reflux/GERD/Heartburn and bones.

Dr. Waks discuses the FDA communication that links fracture risks to proton pumps inhibitors. Proton Pump Inhibitors are the drugs commonly used to relieve Acid reflux, Heartburn or GERD.
proton pump inhibitors side effects: decreased calcium absorption and fragile bones, osteoporosis.
"There are limited studies on the effects of PPIs on bone health in the younger populations. As peak bone mass is an important determinant of bone health, it is important to know whether PPI use will affect peak bone mass attainment and osteoporosis risk later in adulthood."

Long-term PPI therapy is reported to be associated with decreased bone mineral density (BMD). Reduction in BMD and deterioration of bone microstructure are characteristics of osteoporosis, a metabolic bone disease. Osteoporosis ultimately leads to decreased bone strength and susceptibility to fracture. Osteoporotic fracture is becoming a major health concern in the rapidly aging society, owing to the tremendous economic burden, mortality, and morbidity associated with it. In fact, hip fracture is considered as the most common cause of elevated mortality and dependency in elderly patients.

Summary:
1. May interfere with other medication
Do you take over the counter or prescription medication with dietary supplements? You may be putting yourself at risk! For some people, these supplements increase the effectiveness of other medication, on the other hand it may decrease it as well.

2. Acne
Biotin is known for adding volume to hair and making nails strong, but is acne the price you really want to pay? The body absorbs biotin the same way it absorbs other vitamins and minerals - through the gastro system. This is also the system that absorbs acne causing agents present in the skin. But, in case of biotin overload, this absorption gets interrupted. In the end, this results in less acne fighting vitamins and of course, the return of the dreaded acne! If you are taking biotin regularly and notice some acne, especially along your chin and jawline, then biotin may be doing more than beautifying your hair.

3. Skin rash
This just keeps getting worse! If you thought that acne was horrible then let me tell you, the skin problems related to biotin don't stop there. Skin rash is a common side effect of too much biotin in your system.

4. Upset stomach
The moment you feel like your stomach health is being compromised, stop! Unhealthy gut health is a breeding ground for several diseases and a weakened immune system. Basically, a happy gut is the secret to a happy life.

Lisinopril (Prinivil) - Uses, Dosage, Side Effects - Doctor Explains
Lisinopril (Prinivil) - Uses, Dosage, Side Effects - Doctor Explains Side Effects 8 Views • 5 months ago

In this video we will cover everything you need to know about Lisinopril. We will discuss how and when to use it, we will cover its side effects, dosage and much, much more.

You can find more useful videos on cardiovascular diseases and their treatments here:
https://www.youtube.com/playli....st?list=PLaG3Bo3L0gX

Generic name: Lisinopril.
Brand names: Prinivil.
Available as a tablet.

How to use it?
- Tablet: Just swallow it with half a glass of water.
It is usually best to take this drug at a fixed time. If you experience dizziness as a side effect, you could best take it in the evening before going to bed.

When to use it?
Lisinopril is part of a group of drugs called angiotensin converting enzyme inhibitors. These drugs widen the blood vessels, thus lowering the blood pressure and lowering the straint on the heart.
Therefore Lisinopril can be used to treat; a high blood pressure and heart failure.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating a high blood pressure in adults, most doctors will start with an initial dose of 10mg 1x/day. Which if necessary can be slowly increased up to a max. dose of 20-80 mg 1/day.
- When treating heart failure, the initial dose is much lower, and needs to be build up over a long time. Ultimately the maintenance dose will be about; 20-35mg 1x/day.

What are the side-effects?
- Common (1-10%): low blood pressure, dizziness, headache, cough, diarrhoea, vomiting and renal impairment.
- Uncommon (0.1-1%): Palpitations, rapid heartbeat, Raynaud's phenomenon, heart attack, cerebral haemorrhage, nausea, abdominal pain, fatigue, taste disturbance, mood swings, hallucinations, sleep disorder, impotence, rash and itching.
- Rare (0.01-0.1%): rash, hair loss, psoriasis, allergic reactions, acute renal failure, confusion, dry mouth, breast formation and blood count abnormalities.
- Very rare (0.01%): several major side effects.

Safety:
- The first days of using Lisinopril it can lead to dizziness or fatigue. Please do not drive while experiencing this.
- Furthermore, alcohol widens the blood vessels. It can therefore increase the side effect of dizziness at the beginning of treatment. Try drinking alcohol in moderation first so you can estimate its effects.
- Lisinopril can also have interactions with other drugs your are taking. Always discuss this with your prescribing doctor.
- Lastly, Lisinopril could be dangerous for pregnant women and women who are breastfeeding. To be sure, always discuss this with your personal doctor.

Hormone Therapy for High-Risk Prostate Cancer | Prostate Cancer Staging Guide
Hormone Therapy for High-Risk Prostate Cancer | Prostate Cancer Staging Guide Side Effects 7 Views • 4 months ago

PCRI’s Executive Director, Mark Scholz, MD, discusses the use of hormone therapy for men with Azure (High-Risk) prostate cancer. He covers why it is necessary for men with high-risk disease and talks about the three categories of hormone therapy.

0:28 Men with Azure are generally given hormone therapy because there is a higher risk of spread outside the prostate. Hormone therapy is a systemic treatment that acts on the entire body, not just the prostate. It is effective, and its side effects are reversible. Compared to chemotherapy, another systemic treatment, it is more effective and has far fewer side effects.

1:23 The three categories of hormone therapy. Hormone therapy, also known as testosterone inactivating pharmaceuticals (TIP) comes in three different strengths. The first and least potent category of hormone therapy is the “anti-androgens.” Common examples of these drugs are Casodex and Nilutamide. The second category and the most commonly used type of hormone therapy are the “LHRH Agonists” drugs such as Lupron, Eligard, and Trelstar. The last category is the second generation hormonal agents that operate differently than the previous categories. Examples of these are Erleada, Xtandi, and Zytiga. You don’t want more hormone therapy than what is necessary because of added side effects.

2:43 Hormone therapy for the different subtypes of Azure. Each subtype of azure requires different amounts of hormone therapy. High-risk prostate cancer should be treated with a combination of radiation, seed implants, and hormone therapy, but the amount of hormone therapy can differ depending on the subtype. Low-Azure only requires 4-6 months of Lupron and Casodex. Basic-Azure needs between 18-24 months for optimal cure rates. And High azure needs 18-24 months possibly with the addition of a second generation TIP such as Erleada, Xtandi, or Zytiga.

4:12 Summary. The amount of hormone therapy depends on the subtype of prostate cancer. Many studies have shown that Low-Azure only requires a short course of hormone therapy and High-Azure can benefit from adding second generation hormone therapy.

Hear From a Doctor: What to Expect With a TD Treatment
Hear From a Doctor: What to Expect With a TD Treatment Side Effects 7 Views • 4 months ago

See Important Safety Information below, & Medication Guide here: https://bit.ly/4ivEe2k. AUSTEDO XR®/AUSTEDO® can cause depression, suicidal thoughts, or actions in patients with HD. Don’t use if you have liver problems; are taking reserpine, tetrabenazine, or valbenazine.

Hear experts discuss the impact of tardive dyskinesia (TD) and why talking to your doctor about symptoms of TD is so important.

APPROVED USES
AUSTEDO XR is a prescription medicine that is used to treat:
• the involuntary movements (chorea) of Huntington’s disease (HD). AUSTEDO XR/AUSTEDO does not cure the cause of the involuntary movements, & it does not treat other symptoms of HD, such as problems with thinking or emotions.
• movements in the face, tongue, or other body parts that cannot be controlled TD (tardive dyskinesia).
It is not known if AUSTEDO XR/AUSTEDO is safe & effective in children.
IMPORTANT SAFETY INFORMATION
AUSTEDO XR/AUSTEDO can cause serious side effects in people with HD, including: depression, suicidal thoughts, or suicidal actions. Do not start taking AUSTEDO XR/AUSTEDO if you are depressed (have untreated depression or depression that is not well controlled by medicine) or have suicidal thoughts. Pay close attention to any changes, especially sudden changes, in mood, behaviors, thoughts or feelings. This is especially important when AUSTEDO XR/AUSTEDO is started & when the dose is changed. Call your doctor right away if you become depressed, have unusual changes in mood or behavior, or have thoughts of suicide.
Do not take AUSTEDO XR/AUSTEDO if you:
• have HD & are depressed or have thoughts of suicide.
• have liver problems.
• are taking reserpine. Do not take medicines that contain reserpine with AUSTEDO XR/AUSTEDO. If your doctor plans to switch you from taking reserpine to AUSTEDO XR/AUSTEDO, you must wait at least 20 days after your last dose of reserpine before you start taking AUSTEDO XR/AUSTEDO.
• are taking a monoamine oxidase inhibitor (MAOI) medicine. Do not take an MAOI within 14 days after you stop taking AUSTEDO XR/ AUSTEDO. Do not start AUSTEDO XR/AUSTEDO if you stopped taking an MAOI in the last 14 days. Ask your doctor or pharmacist if you are not sure.
• are taking tetrabenazine. If your doctor plans to switch you from tetrabenazine to AUSTEDO XR/AUSTEDO, take your first dose of AUSTEDO XR/AUSTEDO on the day after your last dose of tetrabenazine.
• are taking valbenazine.
Other possible serious side effects include:
• Irregular heartbeat (QT prolongation). AUSTEDO XR/AUSTEDO increases your chance of having certain changes in the electrical activity in your heart. These changes can lead to a dangerous abnormal heartbeat. Taking AUSTEDO XR/AUSTEDO with certain medicines may increase this chance.
• Neuroleptic Malignant Syndrome. Call your doctor right away & go to the nearest emergency room if you develop these signs & symptoms that do not have another obvious cause: high fever, stiff muscles, problems thinking, very fast or uneven heartbeat, or increased sweating.
• Restlessness. You may get a condition where you feel a strong urge to move. This is called akathisia.
• Parkinsonism. Symptoms include: slight shaking, body stiffness, trouble moving, trouble keeping your balance, or falls.
Sleepiness (sedation) is a common side effect of AUSTEDO XR/AUSTEDO. While taking AUSTEDO XR/AUSTEDO, do not drive a car or operate dangerous machinery until you know how AUSTEDO XR/AUSTEDO affects you. Drinking alcohol & taking other drugs that may also cause sleepiness while you are taking AUSTEDO XR/AUSTEDO may increase any sleepiness caused by AUSTEDO XR/AUSTEDO.
The most common side effects of AUSTEDO in people with HD include sleepiness (sedation), diarrhea, tiredness, & dry mouth.
The most common side effects of AUSTEDO in people with TD include inflammation of the nose & throat (nasopharyngitis) & problems sleeping (insomnia).
The most common side effects of AUSTEDO XR are expected to be similar to AUSTEDO in people with HD or TD. These are not all the possible side effects of AUSTEDO XR/AUSTEDO. Call your doctor for advice about side effects. You are encouraged to report side effects of prescription drugs to the FDA. Visit https://www.fda.gov/medwatch or call 1-800-FDA-1088.

Ovarian Suppression Plus AI Treatment Can Have Major Adverse Events
Ovarian Suppression Plus AI Treatment Can Have Major Adverse Events Side Effects 7 Views • 4 months ago

Combining ovarian suppression and an aromatase inhibitor is an efficacious treatment method for premenopausal women with breast cancer. However, the regimen (which is given long-term) can cause some major adverse events, explained Dawn L. Hershman, MD, MS, professor of medicine and epidemiology at the Columbia University Irving Medical Center.

Transcription

Several trials were published, one the SOFT trial, the other the TEXT trial. Both of them were very similar in a lot of ways, looking at premenopausal women and the effect of ovarian suppression. We discovered that there actually is a benefit to suppressing a woman's ovaries and giving her an aromatase inhibitor along with it, compared to giving tamoxifen alone.

But the biggest problem with doing that is that it's very toxic. We always think about the toxicities of chemotherapy, but the toxicities from endocrine therapy can be far worse, and they go on much longer. We give these treatment for 5, sometimes even 10 years. It's super challenging to keep people on their therapy. We've done research showing that just even with 5 years of tamoxifen, by the end of the 5 years, only about 50% of patients are taking it more than 80% of the time. The reason why people stop their medications is complicated. But one of the most common reasons why people stop taking their medications is side effects. We know that when we take a young woman and we suppress her ovarian function and we give them an aromatase inhibitor, close to 100% have side effects. It can cause depression, hot flashes, difficulty sleeping severe bowl pain, and problems with their mood. It can increase their risk for osteoporosis, and sometimes even cardiovascular disease. So it's not without significant consequences.

In order to better treat young women, we need better risk prediction models to figure out exactly who are the people at highest risk, so that we can target those women and give them more intensive treatment, and spare women who maybe don't have such high risk cancer.

Amazing Benefits Of Magnesium Glycinate |The Best Magnesium Supplement |Magnesium Glycinate
Amazing Benefits Of Magnesium Glycinate |The Best Magnesium Supplement |Magnesium Glycinate Side Effects 7 Views • 5 months ago

Magnesium is the fourth most common mineral in the human body.
Normal magnesium content in a healthy adult is around 25 grams, most of which is found in bone tissue.
Magnesium acts as a co-factor for more than 300 enzyme systems necessary for many important functions, including protein synthesis, muscle contraction, nerve conduction, blood glucose control, hormone function, blood pressure regulation, heart function etc.
Magnesium glycinate is a type of magnesium supplement made by combining magnesium with the amino acid glycine.
In addition, several other forms of magnesium supplements are available on the market.
Some of them include magnesium oxide, magnesium sulfate, magnesium hydroxide, and magnesium carbonate.
Unlike those forms of magnesium supplements, magnesium glycinate is well absorbed into the body.
Additionally, it does not cause gastric disturbances such as diarrhea associated with other forms of magnesium supplements.
Magnesium glycinate is helpful for relieving anxiety, improving sleep quality, and maintaining a healthy blood pressure.
Additional health benefits of magnesium glycinate include the following.
Reducing the risk of type 2 diabetes by increasing insulin sensitivity.
Maintaining a regular heart rhythm.
Regulation of calcium in the body, which is helpful for better bone health.
Prevention of kidney stone formation.
And amplification of exercise performance.
Moreover, magnesium glycinate has also been shown to be helpful in patients with migraine headache.
The best time to take magnesium glycinate is just before bed, because it helps you sleep better.
It is best to take the supplement with addition of vitamin D and B6 for better absorption.

86: GLP-1 Medications Explained - Benefits, Risks, What You Need to Know with Dr. Ben Bikman
86: GLP-1 Medications Explained - Benefits, Risks, What You Need to Know with Dr. Ben Bikman Side Effects 7 Views • 5 months ago

Dr. Bikman’s Metabolic Classroom lecture this week explores the physiological role and implications of GLP-1 (glucagon-like peptide-1) and the GLP-1 receptor agonist drugs used for type 2 diabetes management and weight loss.

Ben begins by explaining GLP-1 as a hormone produced in the intestines in response to food. It plays a vital role in slowing gastric emptying, suppressing glucagon, and regulating appetite by signaling fullness to the brain. These mechanisms contribute to its effects on stabilizing blood sugar and enhancing insulin sensitivity.

He then delves into GLP-1 receptor agonists, drugs that mimic GLP-1’s actions, including semaglutide, liraglutide, and others marketed under brand names like Ozempic and Wegovy. These medications have gained fame for their weight loss efficacy, often leading to reductions in body weight by 15% or more.

However, Ben emphasizes that weight loss involves not only fat but also lean mass, raising concerns about muscle loss and metabolic health. Additionally, he discusses the medications’ side effects, including depression, anhedonia, and potential long-term complications like fat cell development and gallstones.

The lecture also addresses the diminishing returns of these drugs over time, noting that their efficacy in reducing cravings and improving eating control tends to wane after prolonged use. Dr. Bikman highlights that education on healthy eating and resistance training is crucial for maintaining health outcomes, as reliance solely on medication can lead to unintended consequences, such as increased fat regain after discontinuation.

Ben concludes by stressing the importance of combining these drugs with a low-carb diet and resistance training to minimize muscle loss and optimize long-term outcomes. He encourages using the lowest effective dose and underscores the need for lifestyle changes to complement pharmacological interventions.

Enalapril (Vasotec) - Uses, Dosage, Side Effects - Doctor Explains
Enalapril (Vasotec) - Uses, Dosage, Side Effects - Doctor Explains Side Effects 7 Views • 5 months ago

In this video we will cover everything you need to know about Enalapril. We will discuss how and when to use it, we will cover its side effects, dosage and much, much more.

You can find more useful videos on cardiovascular diseases and their treatments here:
https://www.youtube.com/playli....st?list=PLaG3Bo3L0gX

Generic name: Enalapril.
Brand names: Vasotec en Renitec.
Available as a tablet en oral suspension.

How to use it?
- Tablet: Just swallow it with half a glass of water.
- Oral suspension: measure the precise amount you need to take with measuring cup. Take this amount. Afterwards rinse the cup with some water and drink this water as well.

When to use it?
Enalapril is part of a group of drugs called angiotensin converting enzyme inhibitors. These drugs widen the blood vessels, thus lowering the blood pressure and lowering the straint on the heart.
Therefore Enalapril can be used to treat; a high blood pressure and heart failure.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating a high blood pressure in adults, most doctors will start with an initial dose of 5-20mg 1x/day. Which if necessary can be slowly increased up to a max. dose of 40 mg/day.
- When treating heart failure, the initial dose is much lower, and needs to be build up over a longer time. Ultimately the maintenance dose will be about the same; 20mg 1x/day, up to a maximum dose of 20mg 2x/day.

What are the side-effects?
- Very common (10%): Dizziness, blurred vision, cough and nausea.
- Common (1-10%): Low blood pressure, fainting, chest pain, rapid heartbeat, shortness of breath, diarrhea, abdominal pain, rash, itching, swelling in the face or throat, headache, fatigue and depression.
- Uncommon (0.1-1%): Palpitations, heart attack, cerebral haemorrhage, drowsiness, insomnia, dizziness, confusion, sore throat, hoarseness, asthma, dry mouth, decreased hunger, vomiting, constipation, inflammation of the pancreas, impaired kidney function, muscle cramps, tinnitus, fever, hair loss, rash and erectile dysfunction.
- Rare (0.01-0.1%): Several serious adverse reactions.

Safety:
- The first days of using Enalapril it can lead to dizziness or fatigue. Please do not drive while experiencing this.
- Furthermore, alcohol widens the blood vessels. It can therefore increase the side effect of dizziness at the beginning of treatment. Try drinking alcohol in moderation first so you can estimate its effects.
- Enalapril can also have interactions with other drugs your are taking. Always discuss this with your prescribing doctor.
- Lastly, Enalapril could be dangerous for pregnant women, but often be used safely by women who are breastfeeding. To be sure, always discuss this with your personal doctor.

Pharmacology of Bisacodyl (Dulcolax) ; Mechanism of Action, Pharmacokinetics, Uses, Effects
Pharmacology of Bisacodyl (Dulcolax) ; Mechanism of Action, Pharmacokinetics, Uses, Effects Side Effects 7 Views • 5 months ago

Bisacodyl is a stimulant laxative used to treat constipation and for bowel preparation before medical procedures. Here are some key points about its pharmacology:

Mechanism of Action
Stimulant Laxative: Bisacodyl acts directly on the colonic mucosa, stimulating sensory nerve endings in the walls of the intestine and rectum. This induces parasympathetic reflexes, resulting in increased peristaltic contractions of the colon and promoting defecation.

Water and Electrolyte Secretion: It increases the secretion of water and electrolytes into the intestinal lumen, which helps soften the stool and facilitates bowel movements.

Pharmacokinetics
Absorption: Approximately 5% of bisacodyl is absorbed after oral administration.

Distribution: It is distributed locally in the intestines.

Metabolism: Bisacodyl is hydrolyzed by intestinal enzymes and bacteria in the bowel to its active desacetyl forms.

Excretion: The drug is primarily excreted in the stool, with a small amount excreted in the urine.

Onset of Action: The onset of action varies depending on the form of administration:

Oral tablets: 6-12 hours

Suppositories: 15-60 minutes

Enema: 5-20 minutes

Clinical Uses
Constipation: Bisacodyl is used to relieve occasional constipation.

Bowel Preparation: It is used for bowel evacuation prior to radiological examinations and surgical procedures.

Side Effects
Common side effects of bisacodyl include:

Abdominal discomfort

Abdominal cramps

Diarrhea

Rectal irritation

Precautions
Contraindications: Bisacodyl should not be used in individuals with hypersensitivity to the drug, intestinal obstruction, severe dehydration, or acute surgical abdominal conditions.

Special Precautions: Use with caution in patients with nausea, vomiting, or abdominal pain. Prolonged use can lead to dependence.

Bisacodyl is a valuable medication for managing constipation and preparing patients for gastrointestinal procedures, offering effective relief with a well-defined mechanism of action and pharmacokinetic profile.

What is Captopril? What is Captopril used for?

Captopril, marketed under the name Capoten®, stands as a pivotal medication in the battle against high blood pressure.

Classified as an ACE inhibitor, it operates by dilating blood vessels to manage blood pressure effectively. Elevated blood pressure can inflict damage on the kidneys and escalate the risk of experiencing a stroke or heart failure. Captopril is dispensed in two forms: a 25-mg tablet and a 1 mg/mL oral liquid, both administered orally.

Potential Side Effects of Captopril:

Dizziness or feeling light-headed, potentially leading to fainting
Persistent cough
Hypotension (low blood pressure)
Reduced kidney function and diminished urine output
Fatigue
Lowered blood counts, increasing susceptibility to infections, bleeding, and anemia
Elevated potassium levels in the blood
Headaches
Swelling of the face, lips, or tongue
Breathing difficulties or swallowing issues
Hoarseness or throat tightening
Swelling in the legs or ankles
Skin rash

These side effects are among the most commonly reported, but others may occur. It's crucial to communicate any adverse effects to your healthcare provider.
Important Guidelines for Captopril Use:

Should you experience swelling in the face, lips, or tongue, hoarseness, or tightness in the throat, or if you encounter difficulties in swallowing or breathing, discontinue captopril immediately and inform your doctor.

Captopril may cause dizziness or lightheadedness. Until you are aware of its effects on you, avoid driving or operating heavy machinery. Prevent dizzy or fainting spells by rising slowly from sitting or lying positions.

Frequent blood pressure monitoring is advised while on captopril, with adjustments to dosage made as necessary based on your readings. Sudden episodes of severe diarrhea, vomiting, or excessive sweating should be reported to your doctor promptly due to the potential for blood pressure drops.

Captopril's interaction with certain medications can alter its effectiveness. This includes ibuprofen, naproxen, heparin, lithium, potassium salts, and some diuretics. Always inform your healthcare provider about any current or new medications.

Avoid salt substitutes, potassium supplements, and potassium-rich foods during captopril therapy. Additionally, consult your doctor before self-medicating for symptoms like coughs, colds, or pain.

If captopril is prescribed in liquid form, ensure to shake the suspension well before use and keep it refrigerated. Always adhere to the expiration date, discarding any unused medication thereafter.

This comprehensive guide aims to enhance understanding and safe usage of captopril, a key medication in managing high blood pressure, thereby mitigating associated health risks.
