CAMZYOS® (mavacamten) is a prescription medicine used to treat adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM). CAMZYOS may improve your symptoms and your ability to be active. It is not known if CAMZYOS is safe and effective in children.

WARNING: RISK OF HEART FAILURE

CAMZYOS may cause serious side effects, including:
Heart failure, a condition where the heart cannot pump with enough force, is a serious condition that can lead to death. You must have echocardiograms before you take your first dose and during your treatment with CAMZYOS to help your healthcare provider understand how your heart is responding to CAMZYOS. People who develop a serious infection or irregular heartbeat have a greater risk of heart failure during treatment with CAMZYOS. Tell your healthcare provider or get medical help right away if you develop new or worsening shortness of breath, chest pain, fatigue, swelling in your legs, a racing sensation in your heart (palpitations), or rapid weight gain.

The risk of heart failure is also increased when CAMZYOS is taken with certain other medicines. Tell your healthcare provider about the medicines you take, both prescribed and obtained over-the-counter, before and during treatment with CAMZYOS.

How Mavacamten Works
Mavacamten is a first-in-class cardiac myosin inhibitor.

On January 22, 2021, the Food and Drug Administration approved the combination of nivolumab (Opdivo, Bristol-Myers Squibb Co.) and cabozantinib (Cabometyx, Exelixis) as first-line treatment for patients with advanced renal cell carcinoma (RCC).

Efficacy was evaluated in CHECKMATE-9ER (NCT03141177), a randomized, open-label trial in patients with previously untreated advanced RCC. Patients were randomized to receive either nivolumab 240 mg over 30 minutes every 2 weeks in combination with cabozantinib 40 mg orally once daily (n=323) or sunitinib 50 mg orally daily for the first 4 weeks of a 6-week cycle (4 weeks on treatment followed by 2 weeks off) (n=328).

The trial demonstrated a statistically significant improvement in progression-free survival (PFS), overall survival (OS) and confirmed overall response rate (ORR) for patients treated with nivolumab plus cabozantinib compared with those who received sunitinib. Median PFS per blinded independent central review (BICR) was 16.6 months versus 8.3 months; HR 0.51 (95% CI: 0.41, 0.64). Median OS was not reached in either arm; HR 0.60 (95% CI: 0.40, 0.89). Confirmed ORR per BICR was 55.7% and 27.1% in the nivolumab plus cabozantinib and sunitinib arms, respectively.

The most common adverse reactions (= 20%) in patients receiving the combination of nivolumab and cabozantinib were diarrhea, fatigue, hepatotoxicity, palmar-plantar erythrodysaesthesia syndrome, stomatitis, rash, hypertension, hypothyroidism, musculoskeletal pain, decreased appetite, nausea, dysgeusia, abdominal pain, cough, and upper respiratory tract infection.

The recommended dose is nivolumab 240 mg every 2 weeks (30-minute intravenous infusion) or 480 mg every 4 weeks (30-minute intravenous infusion) in combination with cabozantinib 40 mg orally once daily without food until disease progression or unacceptable toxicity.

The nivolumab review used the Real-Time Oncology Review (RTOR) pilot program, which streamlined data submission prior to the filing of the entire clinical application. Both the nivolumab and the cabozantinib applications used the Assessment Aid, a voluntary submission from the applicant to facilitate the FDA’s assessment. The FDA approved this application approximately one month ahead of the FDA goal date.

Avastin® (Bevacizumab) is a biolgic cancer drug that binds two growth factors in your body to prevent new blood vessels from forming. Some of the major side effects are vascular, including high blood pressure, nose bleeds, and rarely stroke and heart attack. Bowel perforation is also a possible serious side effect.

Very commonly administered drug, Avastin. Bevacizumab, been around about 15 years now. A biologic. The way it works is it binds to growth factors in your body that prevent new blood vessels from forming. So the idea here is that cancers can't grow, they can't metastasize if they can't get access to the blood vessels. And it does do that, but it has side effects and some of the major side effects are vascular side effects, right?

So high blood pressure, nose bleed, rarely stroke, heart attack can be seen. The one we worry about the most around here is bowel perforation or fistulas. So if there's a lot of cancer in your abdominal cavity and the cancer responds, it might leave a hole so the bowel no longer is in tact and it can sow stool contents out into your abdominal cavity. So none of us likes that. Fortunately, all those scary side effects of stroke, heart attack, and fistula bowel perforation are quite rare, so do watch out for them and talk with your doc about them.

So generally speaking, pretty safe drug to add to the other chemotherapy drugs. Doesn't really work by itself, needs to be part of the recipe for the treatment of your cancer.
