Remeron mirtazepine for insomnia
Remeron mirtazepine for insomnia Side Effects 2 Views • 5 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses the use of Remeron (Mirtazepine) in insomnia. Remeron is FDA approved for Major Depressive Disorder, and utilizing it for insomnia is off label. With that said, although it has the potential for various side effects, there may be advantages to utilizing this medication as opposed to sedatives that have higher risk for abuse, physiological dependence and withdrawal (like ambien [zolpidem] and lunesta [eszopiclone]). Remeron is unique in that for some individuals it is more effective for insomnia at LOWER doses, as it becomes more adrenergic at higher doses (causing less sedation).

Dr. Pagnani is the founder and medical director of Rittenhouse Psychiatric Associates, an Instructor at The Johns Hopkins Hospital for the Department of Psychiatry, he supervises psychiatric residents at Jefferson Hospital, he serves on the Young Professional Board of Uplift, the Center for Grieving Children and he maintains a private practice of Psychiatry in Philadelphia where he sees patients for one-time psychiatric evaluations and second opinions. He is a licensed physician and surgeon in NJ, PA, NY, MD, DE, FL and CA and can provide consultations to individuals via HIPAA compliant Telehealth visits living in these locations.

Rittenhouse Psychiatric Associates is a private practice of psychiatry consisting of about 25 psychiatrists, psychiatric nurse practitioners and psychotherapists with physical locations in Philadelphia and Paoli, PA and serving all of PA, NJ, NY and FL virtually via HIPAA compliant tele-psychiatry. Individuals seeking not only evaluations but long-term psychiatric care, can contact Rittenhouse Psychiatric Associates directly.

These videos aim to increase awareness and discussion of mental health topics and to encourage struggling individuals to schedule an appointment directly with a mental health provider. Mental health needs to be normalized, and simply speaking about mental health topics openly, is an important first step.

Dr. Pagnani and Rittenhouse Psychiatric Associates are NOT advocating for or against the use of any mentioned psychiatric medications for any specific individual. Additionally, any side effects mentioned are not a complete list of potential side effects or contraindications to taking any mentioned medications, and all risks and benefits of a medication should be discussed with an individual’s provider at an appointment, before starting, stopping or making changes to a medication regimen.

Remeron (mirtazapine) Side Effects 2 Views • 5 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Remeron (mirtazepine) an antidepressant that works slightly different than your SSRI's or SNRI's: it antagonizes alpha-2 adrenergic and serotonin 5-HT2 receptors and is rather anti-histaminic. This medication can cause several side effects, but some of these side effects can actually work to address some of the symptoms of depression (for some individuals), by combatting difficulty with sleep and poor appetite or weight loss.

Metronidazole: A Comprehensive Guide to Uses, Dosage, and Possible Side Effects
Metronidazole: A Comprehensive Guide to Uses, Dosage, and Possible Side Effects Side Effects 3 Views • 5 months ago

What is Metronidazole Used For?. Metronidazole is a powerful antibiotic medication that is primarily used to treat various bacterial and parasitic infections. It is commonly prescribed for conditions such as bacterial vaginosis, trichomoniasis, and certain types of intestinal infections. Metronidazole works by stopping the growth of bacteria and parasites, effectively eliminating the infection.
In addition to its use in treating these common infections, Metronidazole has also shown efficacy in treating dental infections, skin conditions such as rosacea and acne, and even certain types of cancer. It is often used as an adjuvant therapy in combination with other medications for these conditions. The mechanism of action of Metronidazole in these cases is still being researched, but it is believed to be related to its anti-inflammatory properties.
Understanding the Dosage of Metronidazole.
The dosage of Metronidazole varies depending on the type and severity of the infection being treated. It is available in various forms, including tablets, capsules, creams, gels, and intravenous (IV) formulations. The dosage is typically determined by a healthcare professional based on factors such as the patient's age, weight, and the specific infection being treated.

Common Side Effects of Metronidazole.
Like any medication, Metronidazole can cause side effects in some individuals. The most common side effects include nausea, vomiting, diarrhea, and a metallic taste in the mouth. These side effects are usually mild and tend to resolve on their own within a few days of starting treatment. However, if the side effects persist or worsen, it is important to consult a healthcare professional.
In some cases, Metronidazole can cause allergic reactions, such as itching, rash, or swelling of the face, tongue, or throat. If any signs of an allergic reaction occur, medical attention should be sought immediately. Additionally, some individuals may experience neurological side effects, such as numbness or tingling in the hands or feet, dizziness, or confusion. These side effects are rare but should be reported to a healthcare professional if they occur.

Precautions and Contraindications for Using Metronidazole.
Metronidazole should be used with caution in individuals with certain medical conditions. It is contraindicated in individuals with a history of hypersensitivity to Metronidazole or other nitroimidazole derivatives. Additionally, it should be used with caution in individuals with liver disease, as Metronidazole is metabolized by the liver. Dose adjustments may be necessary in these cases to prevent potential liver toxicity.
Metronidazole should also be used with caution in individuals with blood disorders, as it can potentially affect blood cell counts. Regular monitoring of blood cell counts may be required during treatment. It is important to inform a healthcare professional of any pre-existing medical conditions before starting treatment with Metronidazole.
Interactions with Other Medications.
Metronidazole can interact with certain medications, potentially leading to increased or decreased efficacy or an increased risk of side effects. It is important to inform a healthcare professional of all medications, supplements, and herbal products being taken before starting treatment with Metronidazole. This includes over-the-counter medications, as well as prescription medications.

In conclusion, Metronidazole is a versatile medication used for the treatment of various bacterial and parasitic infections. It has shown efficacy in a wide range of conditions, including dental infections, skin conditions, and certain types of cancer. However, like any medication, Metronidazole comes with potential side effects that should be considered. It is important to consult a healthcare professional before starting treatment with Metronidazole to ensure it is the appropriate medication for your specific needs and to determine the correct dosage and duration of treatment. By following the prescribed treatment plan and utilizing the tips provided, you can effectively manage your condition and minimize the risk of side effects.

▪ Metronidazole antibiotic is basically an antiprotozoal and anti-amebic drug. Metronidazole is a bactericidal antibiotic. This video will help you to understand pharmacology, mechanism of action, side effects and clinical uses of metronidazole. Metronidazole is the drug of choice for the treatment of pseudo membranous colitis caused by the an aerobic, gram-positive bacillus Clostridium difficile and is also effective in the treatment of brain abscesses caused by these organisms.

▪ Mechanism of Action of Metronidazole:
Metronidazole is selectively toxic to anaerobic micro organisms. Metronidazole is a prodrug that is activated by reduction of the nitro group by susceptible organisms. anaerobic and micro aerophilic pathogens contain electron transport components, that have a sufficiently negative redox potential to donate electrons to metronidazole. After entering the cell by diffusion, the nitro group of metronidazole, is reduced by certain redox proteins, operative only in an aerobic microbes to highly reactive nitro radical which exerts cytotoxicity. The nitro group of metronidazole is able to serve as an electron acceptor, forming reduced cytotoxic compounds that bind to proteins and DNA, resulting in cell death.

▪ Pharmacokinetics:
Metronidazole is completely and rapidly absorbed after oral administration. Metronidazole distributes well throughout body tissues and fluids. Less than 20% of the drug is bound to plasma proteins. With the exception of the placenta, metronidazole penetrates well into body tissues and fluids, including vaginal secretions, seminal fluid, saliva, and breast milk. Therapeutic concentrations also are achieved in CSF. It is metabolized in liver primarily by oxidation and glucuronide conjugation. And excreted in urine, with Plasma half life of about 8 hrs.

▪ Side Effects of Metronidazole:
The most common adverse effects are those associated with the gastrointestinal tract. including nausea, vomiting, epigastric distress, and abdominal cramps. An unpleasant, metallic taste is often experienced. Other effects include oral moniliasis which is basically a yeast infection of mouth. And, rarely cause, neuro-toxicologic problems, such as dizziness, vertigo, seizures and peripheral neuropathy.

▪ Metronidazole is primarily indicated in conditions like:
* Amoebiasis (Drug of First Choice)
* Giardiasis
* Trichomonas vaginitis (Drug of Choice)
* Metronidazole is the drug of choice for the treatment of pseudomembranous colitis.
* Metronidazole also is used in patients with Crohn’s disease, and Helicobacter pylori infection.

1. Mechanism of Action: Metronidazole works by disrupting the DNA structure of bacteria and certain parasites, leading to their death or inhibition of their growth. It is effective against anaerobic bacteria and protozoa that thrive in environments with low oxygen levels.

2. Indications: Metronidazole is used to treat a range of infections, including:
- Bacterial infections: It's often used to treat infections of the gastrointestinal tract, skin, bones, joints, central nervous system, and more.
- Protozoal infections: Metronidazole is effective against parasites such as Giardia, Trichomonas vaginalis, and Entamoeba histolytica.

3. Bacterial Vaginosis and Trichomoniasis: In particular, Metronidazole is commonly prescribed for the treatment of bacterial vaginosis (BV) and trichomoniasis, both of which are sexually transmitted infections. For BV, it helps restore the balance of bacteria in the vagina. In the case of trichomoniasis, it kills the protozoa causing the infection.

4. Precautions and Side Effects: While generally well-tolerated, Metronidazole can have side effects, including nausea, vomiting, diarrhea, abdominal discomfort, and a metallic taste in the mouth. It's important to avoid alcohol consumption while taking Metronidazole and for a certain period after completing the course, as it can cause severe nausea and vomiting due to the interaction between the drug and alcohol.

5. Pregnancy and Breastfeeding: Metronidazole should be used cautiously during pregnancy and breastfeeding, as certain studies have suggested potential risks. However, its use may still be considered if the benefits outweigh the risks, and a healthcare provider's guidance is crucial.

6. Drug Interactions: Metronidazole can interact with other medications, potentially affecting their effectiveness or causing adverse reactions. It's important to inform your healthcare provider about all the medications you are taking.

7. Resistance: Over time, bacteria and parasites can develop resistance to antibiotics, including Metronidazole. It's essential to use this medication only when prescribed by a healthcare professional and to complete the entire course of treatment to minimize the risk of resistance.

8. Dosage and Administration: The dosage and duration of Metronidazole treatment depend on the specific infection being treated. It's important to follow your healthcare provider's instructions and complete the full course of treatment, even if you start feeling better before the medication is finished.

As with any medication, it's crucial to use Metronidazole under the guidance of a healthcare professional. They will consider your medical history, the specific infection being treated, and any potential interactions or contraindications before prescribing this medication.

Metronidazole Tablets: Uses, Dosage, Mechanism, Side Effects, and Important Advice
Metronidazole Tablets: Uses, Dosage, Mechanism, Side Effects, and Important Advice Side Effects 1 Views • 5 months ago

Welcome to MediInsights! In today's video, we'll be diving deep into the world of Metronidazole tablets, a powerful medication that plays a crucial role in combating various infections. Our mission at MediInsights is to unlock the wonders of wellness, and understanding essential medications like Metronidazole is a big part of that journey.

What are Metronidazole Tablets?

Metronidazole tablets are a versatile antibiotic and antiprotozoal medication used to treat a wide range of infections caused by bacteria and protozoa. Whether it's bacterial vaginosis, pelvic inflammatory disease, or infections of the abdomen, skin, joints, or respiratory tract, Metronidazole has proven to be an effective treatment option. Additionally, it's used to combat protozoal infections such as trichomoniasis, giardiasis, and amebiasis, as well as Clostridioides difficile infections causing severe diarrhea and various dental infections.

Dosage of Metronidazole Tablets:

The dosage of Metronidazole can vary depending on the specific condition being treated.
Always consult your healthcare provider for personalized dosing instructions, especially if you have liver or kidney issues that might require dosage adjustments.

Mechanism of Action of Metronidazole Tablets:

Metronidazole works by disrupting the DNA synthesis of the target microorganisms. It enters the cells of bacteria and protozoa, where it undergoes reduction to form reactive intermediates. These intermediates bind to the DNA, causing strand breakage and loss of helical structure, ultimately inhibiting nucleic acid synthesis and leading to cell death. This unique mechanism makes Metronidazole highly effective against anaerobic bacteria and protozoa.

Side Effects of Metronidazole Tablets:

Like all medications, Metronidazole can cause side effects. Common side effects include nausea, vomiting, diarrhea, abdominal pain, headache, dizziness, and a metallic taste in the mouth. Some people may also experience skin reactions such as rash or itching. Although rare, more severe side effects can occur, including seizures, peripheral neuropathy (tingling, numbness, or pain in the hands and feet), severe allergic reactions (anaphylaxis), and liver toxicity (hepatotoxicity). Dark urine, caused by the drug’s metabolites, is generally harmless but can be alarming.

Important Advice for Using Metronidazole Tablets:

When taking Metronidazole, it's crucial to follow these guidelines:

Avoid alcohol during treatment and for at least 48 hours after the last dose to prevent a disulfiram-like reaction (nausea, vomiting, flushing, and tachycardia).
Inform your healthcare provider about all other medications and supplements you are taking to avoid potential interactions.
Complete the full prescribed course, even if you start feeling better, to prevent the development of resistant organisms.
Consult your healthcare provider before using Metronidazole if you are pregnant or breastfeeding, as it can cross the placenta and is excreted in breast milk.
Regular monitoring may be required for long-term use to check for signs of toxicity.
Be cautious when driving or operating machinery if you experience dizziness or other neurological side effects.

Metronidazole is an antibiotic.

It's used to treat skin infections, rosacea and mouth infections, including infected gums and dental abscesses. It's also used to treat conditions such as bacterial vaginosis and pelvic inflammatory disease.

It's also used for infected insect bites, skin ulcers, bed sores and wounds, and to treat and prevent bacterial and parasitic infections.

Metronidazole is only available on prescription.

It comes as tablets, gel, vaginal gel, cream, a liquid you drink or a suppository which is a medicine that you push gently into your anus (bottom). It's also given by injection, but this is usually only done in hospital.

Key facts
The most common side effects of metronidazole tablets, liquid, suppositories or vaginal gel are feeling sick (nausea) or being sick (vomiting), diarrhoea, and a slight metallic taste in your mouth.
It's unusual to have any side effects from metronidazole cream or gel.
Do not drink alcohol while taking a course of metronidazole tablets, liquid, suppositories or vaginal gel, or for 2 days after finishing treatment. Alcohol can cause side effects such as feeling and being sick, stomach pain, hot flushes, difficulty breathing, a pounding heartbeat (palpitations) and headaches.
For most infections, you'll start to feel better in a few days, but for some it may take longer. When treating rosacea, you may only notice a difference after several weeks.
It's important to take metronidazole as your doctor tells you, and finish your full course of treatment.

Metronidazole: What Is It And How Does It Treat Bacterial Vaginosis? With Dr Daniel Atkinson
Metronidazole: What Is It And How Does It Treat Bacterial Vaginosis? With Dr Daniel Atkinson Side Effects 3 Views • 5 months ago

Metronidazole is an antibiotic that treats bacterial vaginosis. Here we have Dr Daniel Atkinson, the Clinical Lead at Treated, to tell us what it is and how it works.

Metronidazole is available as oral tablets or a topical gel for bacterial vaginosis. You’ll take the pill twice a day for a week, or use an applicator to apply the gel into the vagina at night for five days. It treats bacterial vaginosis by targeting the bacteria responsible, inhibiting the production of proteins needed for them to grow and spread. This stops the infection in its tracks, and eventually kills off the bacteria – your immune system will take care of the rest.

The Treated team can help you get the bacterial vaginosis treatment that’s best for your life. Subscribe, pause or stop whenever it suits you. Choose better.

Metronidazole - Mechanism of action, Side effects, and Uses
Metronidazole - Mechanism of action, Side effects, and Uses Side Effects 1 Views • 5 months ago

Metronidazole is an antibiotic and antiprotozoal medication that is used to treat a variety of infections caused by certain types of bacteria and parasites. It is available under various brand names, including Flagyl, Metrogel, and Noritate. Here are some key points about Metronidazole:

1. Mechanism of Action: Metronidazole works by interfering with the DNA of microorganisms like bacteria and parasites. It disrupts their DNA structure and prevents them from reproducing and growing, ultimately leading to their death.

2. Indications: Metronidazole is used to treat a range of infections, including:
- Bacterial Infections: It is effective against various anaerobic bacteria, which are bacteria that can survive and thrive in low-oxygen environments. Common infections treated with Metronidazole include bacterial vaginosis, certain dental infections, and some types of skin infections.
- Protozoal Infections: Metronidazole is also used to treat protozoal infections, such as giardiasis, trichomoniasis, and amoebiasis.

3. Dental and Gastrointestinal Uses: In dentistry, Metronidazole is sometimes used in combination with other antibiotics to treat certain oral infections. It can also be used to treat gastrointestinal infections, particularly when caused by anaerobic bacteria.

4. Treatment of Inflammatory Bowel Disease (IBD): Metronidazole may be used as part of the treatment regimen for some forms of IBD, like Crohn's disease, to help control inflammation and manage symptoms.

5. Topical Formulations: Metronidazole is available in topical forms like creams and gels, which are used to treat skin conditions like rosacea and certain types of acne.

6. Precautions: While generally considered safe and effective, Metronidazole may have some side effects, including nausea, diarrhea, and a metallic taste in the mouth. It should be used with caution in individuals with certain medical conditions, and alcohol consumption should be avoided during Metronidazole treatment, as it can lead to severe nausea and vomiting due to a disulfiram-like reaction.

7. Drug Interactions: Metronidazole can interact with other medications, so it's essential to inform your healthcare provider about all the drugs you are taking.

8. Pregnancy and Breastfeeding: It's important to consult a healthcare professional if you are pregnant or breastfeeding before taking Metronidazole, as its safety during pregnancy and lactation may vary depending on the specific situation.

9. Resistance: Like all antibiotics, the overuse or misuse of Metronidazole can lead to antibiotic resistance, where bacteria become less sensitive to its effects. This underscores the importance of using this medication only as prescribed by a healthcare provider.

10. Availability: Metronidazole is available by prescription and comes in various forms, including tablets, capsules, creams, gels, and intravenous (IV) formulations.

It's crucial to follow your healthcare provider's instructions and complete the full course of Metronidazole treatment, even if your symptoms improve before you finish the medication. This helps ensure that the infection is completely cleared and reduces the risk of developing antibiotic resistance. Always consult a healthcare professional for guidance on using Metronidazole or any other medication.

Metronidazole | Bacterial Targets, Mechanism of Action, Adverse Effects
Metronidazole | Bacterial Targets, Mechanism of Action, Adverse Effects Side Effects 3 Views • 5 months ago

Lesson on the antibiotic Metronidazole (Flagyl). Metronidazole is a small sized antibiotic, which allows it to be easily absorbed and diffusely distributed throughout the body. This makes it particularly important in bone and brain infections. Metronidazole has important activity against obligate anaerobes and some protozoal species, but has limited effects on aerobes. Metronidazole’s mechanism of action (which is described in the video) allows it to be an effective bacteriocidal antibiotic. Metronidazole has a wide variety of adverse effects, including GI side effects and specific side effects when alcohol is consumed when taking it.

I hope you find this lesson helpful. If you did, please like and subscribe for more lessons like this one :)

In this video Doctor O'Donovan explains key facts you need to know about Metronidazole - an antibiotic tablet that has many uses - including for rosacea.

What is Metronidazole?

Metronidazole is an antibiotic. You might know it by its brand name - Flagyl. Here in the UK, metronidazole is only available on prescription. It comes as tablets, gel, vaginal gel, cream, a liquid you drink or a suppository which is a medicine that you push gently into your anus (bottom). It's also given by injection, but this is usually only done in hospital.

Metronidazole uses:

It's used to treat skin infections, rosacea and mouth infections, including infected gums and dental abscesses. It's also used to treat conditions such as bacterial vaginosis and pelvic inflammatory disease. It's also used for infected insect bites, skin ulcers, bed sores and wounds, and to treat and prevent bacterial and parasitic infections.

What are the strengths and doses of metronidazole?

This will vary from person to person so always follow the specific instructions given to you by your own doctor or pharmacist.

Can I drink alcohol if I take metronidazole?

According to NHS UK "Do not drink alcohol while taking a course of metronidazole tablets, liquid, suppositories or vaginal gel, or for 2 days after finishing treatment. Alcohol can cause side effects such as feeling and being sick, stomach pain, hot flushes, difficulty breathing, a pounding heartbeat (palpitations) and headaches."

Are there any side effects of metronidazole?

In this video we cover some of the common, plus more rare (but serious) side effects to be aware of. It is always really important to read the information leaflet inside the medication packet before taking any medication. Please remember - this is NOT a clinical advice video but just a general educational video. For specific questions or concerns around allergies ALWAYS speak to your own pharmacist or health provider.

Serious side effects happen in less than 1 in 1000 people but they can include:

- the whites of your eyes turn yellow, or your skin turns yellow although this may be less obvious on brown or black skin – these can be signs of liver or gallbladder problems
- you get unexpected infections, mouth ulcers, bruising, bleeding gums, or extreme tiredness – these can be caused by a blood problem
- you have bad stomach pains which may reach through to your back – this can be a sign of pancreatitis
- you have blurred vision or double vision
- anaphylaxis

Metronidazole/Flagyl Toxicity, Adverse Effects & Thiamine Deficiency
Metronidazole/Flagyl Toxicity, Adverse Effects & Thiamine Deficiency Side Effects 3 Views • 5 months ago

In this video I discuss some of the side effects associated with a commonly prescribed antibiotic called metronidazole, also known as Flagyl.

In many, these adverse effects resolve with discontinuation of the drug. However, some people are left with longstanding illness which continues despite no longer taking this medication.

Research shows that metronidazole can readily convert into a thiamine (vitamin B1) analog which inhibits the activation of this vitamin inside the cell. Most of the symptoms of metronidazole toxicity are identical to neurological beri-beri and Wernicke's Encephalopathy, both conditions which are caused by deficiency of thiamine. Some researchers have concluded that this drug may "reinforce" or unmask an underlying insufficiency, triggering extreme symptom changes.

Many people report improvement in toxicity symptoms through using bioavailble forms of thiamine in conjunction with other B vitamins and magnesium supplementation.

Metoprolol Side Effects - How to manage them?
Metoprolol Side Effects - How to manage them? Side Effects 4 Views • 5 months ago

Metoprolol is a selective beta blocker used to treat hypertension and heart failure.

It is available in two salt forms. Metoprolol succinate and tartrate. Both these salts produce few side effects like fatigue, dizziness, hypotension, and cold hands and feet. The extent of side effects depends on the dose and other risk factors. Reducing the dose and a few lifestyle modifications like good hydration, avoiding alcohol, and limiting caffeine are useful in minimizing adverse effects.

Metoprolol Risks Are You Aware of These Hidden Dangers

Recent pharmacovigilance studies have identified new safety concerns linked to metoprolol, a commonly prescribed beta-blocker for high blood pressure and heart conditions. Analysis of the FDA Adverse Event Reporting System (FAERS) has detected unexpected serious adverse reactions, including orthostatic intolerance (dizziness or fainting when standing), trigemino-cardiac reflex (sudden drop in heart rate and blood pressure), decorticate posture (a neurological response indicating brain injury), and a strong association with suspected suicide.

Chapters:
00:00 Introduction

00:39 Careful, Mental Health Monitoring
00:50 Other Medications for Interactions
01:01 Antidepressants (Fluoxetine and Paroxetine)
01:13 Continuous Drug Safety Monitoring

Additionally, certain drug interactions can increase the risk of these adverse effects. Calcium channel blockers (verapamil, diltiazem), alpha-blockers (prazosin, tamsulosin), CNS depressants (benzodiazepines, opioids), and SSRIs (fluoxetine, paroxetine) can exacerbate metoprolol side effects, potentially leading to bradycardia, hypotension, and worsened mental health outcomes.

These findings highlight the critical importance of post-marketing drug surveillance and the need for healthcare professionals to monitor patients closely. If you experience unusual symptoms while taking metoprolol, consult your healthcare provider immediately.

How to safely come off beta blockers
How to safely come off beta blockers Side Effects 1 Views • 5 months ago

A lot of people have contacted me and asked me as to how to safely stop beta blockers and therefore I thought I would do a quick video around this subject.

Before I start it is important to say that please do not try and alter your dosage without speaking with you health care provider as he will know about your unique clinical situation and all I am doing in this video is just giving general information.

Beta Blockers are one of the commonest classes of medications that we use in cardiology and often serves as a jack of all trades. They basically blunt sympathetic activity and by doing so, have a multitude of potentially beneficial effects such as reducing heart rate, reducing blood pressure, making the heart less irritable, reducing the demands on the heart and increasing the time for the heart to fill with blood do that the heart is able to contract more effectively. In some situations they make people feel better I.e improve quality of life. In other situations, they may actually have a prognosis modifying role.

Let me give you some examples.

Beta blockers can be used to control blood pressure. Here they don’t necessarily make patients feel better may have a prognosis modifying role (albeit a weak one).

Beta blockers can be used as a treatment for heart failure. Here they make people feel better and have a very significant impact on improving lifespan

Beta blockers can be used for heart rhythm disturbances (ectopics/AF/SVT/VT) and her they may make the patient feel better and in some heart rhythm disturbances may also have a prognosis modifying role

Beta blockers can be used to treat angina and prolong life after a heart attack so against they may improve quality and quantity of life.

Sometimes beta blockers are used in a purely symptom control role such as for controlling anxiety or benign ectopics in people with normal hearts

So if you are taking beta blockers it is always worth asking why you take them. Is it because they make you feel better or is it because they in some way are being used for their prognosis modifying effects? This is important because if they have a prognosis modifying role you may decide you just want to remain on them

Acute, abrupt withdrawal of beta blockers has been shown in many case reports to be associated with an increased risk of morbidity and mortality. This is mainly seen in patients who were taking the beta blocker for angina due to significant coronary disease and in a few of these patients, stopping the beta blocker was associated with more angina and even development of heart attacks. Similarly there have been case reports of patients who were taking beta blockers to control dangerous heart rhythm disturbncaces and when the beta blocker was abruptly stopped it led to precipitation of ventricular dysrhythmias and even in some cases death. There have also been reports of heart failure status worsening and blood pressure going up excessively. This is termed as Acute beta blocker withdrawal syndrome.

Why does it happen?
It is believed that withdrawal effects happen because if increased sympathetic activity which is probably because of Beta receptor up regulation during the period of beta blockade. Upregulation is a well recognised phenomenon where the receptors that are being blocked become more sensitive and responsive to circulating adrenaline. It is believed that receptors take 24-36 hours to down regulate when the blockade is removed. So if you stop the beta blocker abruptly and it doesn’t stay in the body long (and how long it hangs around in the body depends on that beta blockers half life) then there may be a period where you have receptors which are significantly more sensitive to the effects of the adrenaline and this can then trigger this withdrawal syndrome. What this means is that the withdrawal syndrome is far more likely with short acting beta blockers or those beta blockers where you have to take 2 or 3 doses in day because they will have a much shorter half life. Beta blockers that have a lon half life will stay in the body for much longer and therefore will allow that time that is needed for the beta receipts to down regulate.

So if you are taking a short acting beta blocker I.e propranolol, metoprolol, carvedilol etc, it is best to be more cautious about reducing the dose.In such cases I would suggest taking the usual dose once daily for a week, then every other day for a week and then stop.

If you are taking a long acting beta blocker eg Atenolol, Bisoprolol etc , then take half the usual dose for a week, then half the the usual dose every other day for a week and then stop.

As I say it is very important that you only alter your dosages if sanctioned and supervised by your own doctor as it may in some patients have dangerous consequences.

Metoprolol side effects. What are the side effects of metoprolol and how to take metoprolol.

Blood Pressure Machine
https://amzn.to/3khdOGq

Metoprolol interactions
https://www.rxlist.com/lopress....or-drug.htm#interact

What is metoprolol used for

Metoprolol is a heart medication that works by blocking the effects of epinephrine on beta-1 receptors within the heart, this causes the heart to pump more slowly and with less force. Metoprolol is available as two different salts: You got: metoprolol tartrate brand name Lopressor and metoprolol succinate, brand name Toprol XL. They are both used to lower high blood pressure and reduce chest pain, otherwise known as angina.

How does metoprolol work

Both versions of metoprolol belong to a class of drugs you may already heard of: called beta-blockers. Like other beta blockers, metoprolol works by changing the way your body responds to some nerve impulses, especially in the heart. And when this happens, It slows down your heart rate and makes it easier for your heart to pump blood around your body and decreases blood pressure. When the blood pressure is lowered, the amount of blood and oxygen is increased to the heart and this resolves any chest pain and makes the flow of blood around your body run more smoothly.

How to take metoprolol

The best way to take metoprolol, is how your doctor has told you to take it. There are so many different doses and variations that can be made with metoprolol based on different conditions, so the you have been prescribed to take it, is going to be the best way for you to get the most benefits from it. Strengths for the immediate release form range from: 25 mg, 37.5, 50 mg, 75 and 100. And strengths for the extended release range from: 5 mg, 50, 100, and 200 mg. Always consult with your doctor before making any changes. Especially when it comes to stopping metoprolol abruptly.

Metoprolol side effects

Now its totally normal for in the beginning of starting metoprolol to have it feel a little rough. If youre experiencing headaches. make sure you rest and drink plenty of fluids. Do not drink too much alcohol as well and Ask your pharmacist to recommend a painkiller. Headaches usually go away after the first week of taking metoprolol. But if youre feeling tired, dizzy or weak. stop what you're doing, and sit or lie down until you feel better. Dont drive or use tools or machinery if you're feeling tired and again, avoid drinking too much alcohol as this can make you feel worse. Another side effect that may be surprising at first is cold hands and feet. So my tips for this is: put your hands or feet under warm running water, massage them, and wiggle your fingers and toes. You want to avoid smoking or have drinks with caffeine in it as these can make your blood vessels narrower and restrict your blood flow. Mittens and warm socks can help too. Now if youre feeling nauseous: stick to simple meals and avoid rich or spicy foods. It might also help to take your metoprolol after a meal. Some even find this medicine to cause stomach pain: try to rest and relax. It can help to eat and drink slowly and have smaller and more frequent meals. Putting a heat pad or covered hot water bottle on your stomach may also help. But If you're in a lot of pain, speak to your pharmacist or doctor.

The Truth About Metoprolol Usage Side Effects 2 Views • 5 months ago

This is a pharmacist review of the medication Metoprolol (Toprol XL, Lopressor).

Metoprolol, sold under the brand names Toprol XL and Lopressor, is a medication that belongs to the class of beta-blockers. It is commonly prescribed to treat high blood pressure, angina, and heart failure and to prevent heart attacks. Metoprolol works by reducing the workload of the heart and slowing down the heart rate, which in turn reduces the blood pressure.

While Metoprolol is generally well-tolerated, like all medications, it can cause side effects. Some common side effects include fatigue, dizziness, lightheadedness, shortness of breath, and slow heartbeat. More serious side effects, such as swelling in the legs or ankles, chest pain, and difficulty breathing, require immediate medical attention.

Before taking Metoprolol, it is important to inform your doctor about your medical history, especially if you have a history of asthma, diabetes, liver or kidney disease, or any other heart condition. It is also important to let your doctor know about any other medications you are taking, as Metoprolol can interact with certain drugs, including over-the-counter medications.

When taking Metoprolol, it is important to follow your doctor's instructions carefully. This includes taking the medication at the same time every day, not skipping doses, and not abruptly stopping the medication without consulting your doctor.
