PCRI’s Executive Director, Mark Scholz, MD, discusses the use of hormone therapy for men with Azure (High-Risk) prostate cancer. He covers why it is necessary for men with high-risk disease and talks about the three categories of hormone therapy.

0:28 Men with Azure are generally given hormone therapy because there is a higher risk of spread outside the prostate. Hormone therapy is a systemic treatment that acts on the entire body, not just the prostate. It is effective, and its side effects are reversible. Compared to chemotherapy, another systemic treatment, it is more effective and has far fewer side effects.

1:23 The three categories of hormone therapy. Hormone therapy, also known as testosterone inactivating pharmaceuticals (TIP) comes in three different strengths. The first and least potent category of hormone therapy is the “anti-androgens.” Common examples of these drugs are Casodex and Nilutamide. The second category and the most commonly used type of hormone therapy are the “LHRH Agonists” drugs such as Lupron, Eligard, and Trelstar. The last category is the second generation hormonal agents that operate differently than the previous categories. Examples of these are Erleada, Xtandi, and Zytiga. You don’t want more hormone therapy than what is necessary because of added side effects.

2:43 Hormone therapy for the different subtypes of Azure. Each subtype of azure requires different amounts of hormone therapy. High-risk prostate cancer should be treated with a combination of radiation, seed implants, and hormone therapy, but the amount of hormone therapy can differ depending on the subtype. Low-Azure only requires 4-6 months of Lupron and Casodex. Basic-Azure needs between 18-24 months for optimal cure rates. And High azure needs 18-24 months possibly with the addition of a second generation TIP such as Erleada, Xtandi, or Zytiga.

4:12 Summary. The amount of hormone therapy depends on the subtype of prostate cancer. Many studies have shown that Low-Azure only requires a short course of hormone therapy and High-Azure can benefit from adding second generation hormone therapy.

¿Qué es la metformina?
La metformina es el medicamento más recetado para el manejo de la resistencia a la insulina, prediabetes, diabetes gestacional y diabetes tipo 2. También se puede recetar metformina para síndrome de ovario poliquistico y bajar de peso. Se lleva utilizando más de 50 años y tiene muchos estudios sobre su eficacia, seguridad y efectos a largo plazo. Es uno de los medicamentos más seguros ya que por sí mismo es muy raro que causa una hipoglucemia (bajón de azúcar en sangre).

¿Cómo actúa la metformina en el cuerpo?
Su principal función va a ser disminuir la glucosa en sangre con tres mecanismos diferentes:

- Disminuye la cantidad de glucosa que produce y libera el hígado hacia el torrente sanguíneo.
- Disminuye la absorción de glucosa en el intestino.
- Sensibiliza a las células (principalmente musculares) a la insulina.

Como ves, los 3 mecanismos funcionan por un lado disminuyendo la cantidad de glucosa que va a entrar a la sangre y aprovechando la insulina que tu cuerpo ya secreta.

Beneficios adicionales de la metformina
- Se ha visto un incremento en la perdida de peso y de % de grasa.
- Ayuda a disminuir los niveles de triglicéridos en sangre cuando estos se encuentran elevados.
- Bajo costo y alta disponibilidad.

Efectos secundarios de la metformina
En realidad la metformina es un medicamento bastante gentil. Sus efectos secundarios son leves y generalmente bien tolerados. Los efectos secundarios principales están asociados a síntomas gastrointestinales como:
- Diarrea
- Gases
- Nausea
- Dolor abdominal

Metformina antes o después de los alimentos
Para reducir los efectos secundarios gastrointestinales, se recomienda tomar la metformina durante o después de los alimentos.

Dosis escalonada
Si los síntomas aún persisten, puedes consultar con tu especialista cómo tomar la metformina de forma escalonada. Esto quiere decir empezar con una dosis más baja e ir incrementandola poco a poco. Probablemente si para este momento los síntomas aún persisten, lo ideal será cambiar de medicamento.

Uso a largo plazo de metformina
El consumo a largo plazo de metformina esta asociado a deficiencia de Vitamina B12. Esto pasa ya que se disminuye la absoricón de esta vitamina en el intestino. Es importante orientarte para ver en que momento requieres comenzar con suplementación de esta vitamina.

Insuficiencia renal y metformina
Aunque la metformina por si misma no causa daño renal, en pacientes que presentan daño, se aumenta el riesgo de generar acidosis láctica. La acidosis láctica es una emergencia médica que requiere de atención inmediata y por lo mismo la metformina está contraindicada en pacientes que tengan daño renal.

¿Cómo se mi dosis de metformina?
La dosis y frecuencia varían entre persona y persona. Inclusive una misma persona probablemente va a ir ajustando su dosis a traves del tiempo. Lo que funciona a una persona puede no funcionarle o inclusive perjudicarle a otra. Consulta con tu especialista para recibir las recomendaciones ideales para tu caso.
