What is guaifenesin and codeine?


Guaifenesin and codeine cough syrup is a commonly prescribed medication to treat cough that combines the opioid painkiller codeine with the expectorant guaifenesin.


What is Codeine?

Guaifenesin-Codeine cough syrup comes in many brand names, including Cheracol, Cheratussin and Robitussin AC. Codeine-guaifenesin cough syrup is taken by mouth every four to six hours, and people are encouraged to drink plenty of water when taking it to deal with constipation or nausea side effects.

This medication is a short-term treatment. People who take codeine guaifenesin cough syrup over the long term may experience withdrawal symptoms if they stop taking it suddenly.

Griseofulvin

 Griseofulvin is a Heterocyclic Benzofuran antifungal Drug.

 Griseofulvin is fungistatic for most dermatophytes, including Epidermophyton, Trichophyton and Microsporum, etc., but not against Candida And other fungi causing deep mycosis.

It was one of the early antibiotics extracted from Penicillium griseofulvum. However, because of lack of antibacterial activity, little attention was paid to it: clinical utility in dermatophytosis was demonstrated only around 1960.

 

Mechanism

Griseofulvin binds to Tubulin (Protein in nucleus) interfering With microtubules function.

Griseofulvin inhibits fungal cell mitosis.

Griseofulvin gets deposited in Keratin forming cells of skin hairs, nails and protect Skin, hairs, nails from getting newly Infection.

The absorption of Griseofulvin from Gastrointestinal Tract it’s somewhat irregular because of its very low water solubility.

Absorption is improved by using Microfined particles and taking it with fatty food.

Route of administration – Orally only ( Tablet and Syrup )

Half life – 9-20 hours

Doses –

125–250 mg QID with meals; duration depends on the Site of infection (turnover rate of keratin).

Scalp 4 weeks

Palm, soles 6 to 8 weeks

Finger nails 6 to 8 months

Toe nails 10 to 12 months

 Superficial Dermatophytosis (Superficial Skin fungal Infections)

Tinea Corporis / Ringworm

-          Infection of the skin that can affect any part of the body excluding scale, face, beard and nail.

Tinea Barbae 

-         Fungal Infection of the beard and moustache areas.

Tinea Capitis

-         Fungal Infection of the skin of scalp, eyebrows.

Tinea Unguium / Onychomycosis

-         Fungal Infection of Nail

Tinea Pedis / Athlete’s foot

-         Fungal Infection between the toes

Tinea Cruris / Jock itch

-         Fungal Infection in the skin of the genital, inner, underarm and buttocks.

Amphotericin B and Nystatin work by binding to the fungal cell membrane component ergosterol, leading to increased fungal cell membrane permeability and the loss of intracellular constituents.
Amphotericin has a lesser affinity for the mammalian cell membrane component cholesterol, but this interaction does account for most adverse toxic effects.
Amphotericin B has activity against Candida spp., Cryptococcus neoformans, Blastomyces dermatitidis, Histoplasma capsulatum, Sporothrix schenckii, Coccidioides immitis, Paracoccidioides braziliensis, Aspergillus spp., Penicillium marneffei, etc.


Amphotericin is used intravenously for treatment of Candida esophagitis, rapidly progressive mucormycosis or invasive aspergillosis.
Intrathecal infusion of amphotericin B is useful in patients with meningitis caused by Coccidioides.
Intravenous administration of amphotericin B is the treatment of choice for mucormycosis and is used for the initial treatment of cryptococcal meningitis, severe or rapidly progressing histoplasmosis, blastomycosis, and coccidioidomycosis.
Intraocular injection is used successfully for fungal endophthalmitis.
