What is the science behind spironolactone?

Spironolactone is an anti-male hormone (anti-androgen) medication. It blocks the male hormone receptor and reduces the level of the male hormones, testosterone and DHEAS. This tablet was first used as a diuretic (“fluid tablet”) effect and increases urine production. This medication lowers blood pressure and reduces fluid retention.

Spironolactone is an oral medication & not a cream because androgens (the target for spironolactone) is found circulating in the bloodstream.

What skin conditions can be treated with spironolactone?

Hormonal acne: It’s the impact spironolactone has on the hormones that give it its acne fighting reputation. That’s because it blocks the effect that androgens have on oil glands. This medication is an alternative to Accutane or Oratane.

Hair loss in women: Due to its impact on hormone regulation, the drug has recently been prescribed for women experiencing hair loss, also known as androgenic alopecia.

Excessive unwanted hair: On the other hand excessive hair known as hirsutism, commonly seen in PCOS can be treated with spironolactone.

Oily skin: can also be effectively treated with this medication. Most patients only require a very small dose of between 50 to 75 mg.

"So you've been suggested to start a medication called Spironolactone for your acne. Now the interesting thing is that Spironolactone is not FDA-approved to treat acne. It's actually a blood pressure medication.

Now Spironolactone works by blocking the effects of testosterone, the male predominant hormone in your skin. Now if we think about it, testosterone is what gives us oil. Oil is what feeds the whole cycle of acne. So if we're blocking the effects of testosterone on the skin, then hopefully we're reducing the excess of oil that's produced in the skin, and then that's how your acne gets better. Now it's not too often prescribed for blood pressure because it really doesn't do that much for blood pressure, unless you increase, the dose is very high. But what they noticed when they did the studies decades ago on this, was that women who took Spironolactone and had acne, almost all of their acne went away. And so dermatologists, we've been writing Spironolactone for decades, and we've had very good luck with it. It's almost like a miracle medication for women who have acne, especially acne in areas where men have a beard, and when you get it on your forehead. And if you have that type of acne and it flares right around your menstrual cycle, then Spironolactone is probably going to be the home run for your acne..."

Videoconsejo sanitario sobre Espironolactona , para qué sirve este medicamento, cuándo y cómo debemos tomarlo. Videoconsejo impartido por el farmacéutico Jorge Gómez-Carpintero, elaborado por el Consejo General de Colegios Farmacéuticos y producido por Medicina TV.

Espironolactona es un fármaco diurético antagonista de la aldosterona, que actúa sobre los receptores de intercambio sodio/potasio del túbulo contorneado distal, provocando una mayor excreción de sodio y agua y la retención de potasio.

Por ello, espironolactona está indicada, sola o en asociación, en enfermedades o condiciones en las que el aumento de la diuresis resulte beneficioso, como el edema, la hipertensión arterial o la insuficiencia cardiaca.

Además, debido a su efecto ahorrador de potasio también está indicada en la hipopotasemia.

Y debido a su efecto antagonista de la aldosterona, está indicada en casos de hiperaldosteronismo.

Los medicamentos que contienen espironolactona requieren de receta para su dispensación.

Están disponibles en forma de comprimidos por vía oral con dosis de entre 25 y 100 mg de espironolactona.

Algunos medicamentos combinan espironolactona con altizida o clortalidona, que son también diuréticos, pero del grupo farmacológico de las tiazidas, por lo que complementan el efecto de espironolactona.

Por lo general, espironolactona es un fármaco bien tolerado y la frecuencia de efectos adversos es baja con dosis de hasta 100 mg al día.

El más común es la hiperpotasemia, pero también pueden aparecer otros como mareos, dolor de cabeza o somnolencia.

Además, debido al antagonismo que ejerce sobre los receptores de aldosterona pueden ocurrir otros efectos como menstruaciones irregulares en mujeres o dolor en las mamas.

Think you’ve tried everything for acne? That may not be true, especially if you live in the UK where Spironolactone has yet to be widely adopted as a mainstream treatment for acne. Allure magazine just did a piece on this under-the-radar option for ‘hormonal acne’, as did the Huffington Post. Now, you may ask, what do I mean by the term hormonal acne? In truth, the U-pattern of acne seen in grown-up girls is, by definition, hormonal - you don’t see this pattern in guys. A proportion will have an underlying hormonal issue, most commonly due to Polycystic Ovarian Syndrome, but most won’t. There’s simply a sebaceous gland interaction with normal hormone levels that leads to nodules and cysts, typically along the chin, jawline and upper neck.

Generic name: Spironolactone.
Brand names: Aldactone or CaroSpir.
Available as a tablet or oral suspension.

How to use it?
- Tablet: Just swallow it with half a glass of water.
- Oral suspension: first shake the bottle before use. Next up, measure the correct amount using a measuring spoon or measuring cup. Take this amount and afterwards rinse the cup or spoon with water and drink this water as well.

Always take spironolactone with some food. This could decrease your chance on side effects affecting your stomach and intestines.

When to use it?
Spironolactone is a diuretic, which prevents your body from absorbing too much salt. A diuretic, also called a water pill, helps you to get rid of salt (sodium) and water. The result is an increased production of urine, which will make you pee more often. Through this effect, Spironolactone is used to treat heart failure, high blood pressure or low blood potassium levels.
It also treats fluid retention, in medical terms called edema, in people with heart failure or certain severe kidney problems.
Furthermore, Spironolactone is also used to diagnose and treat a condition where you have too much aldosterone in your body. Aldosterone is a hormone produced by your adrenal glands, which helps to regulate the salt and water balance in your body.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, in most use cases your doctor will prescribe a dose of 25-200 mg/day. Your doctor usually starts with a low dose, which will be increased if necessary.

What are the side-effects?
- Very common (10%): headache, poor digestion, diarrhea, fatigue, drowsiness, decreased libido, erectile dysfunction, painful breasts, breast disorders, menstrual disorders and (irreversible) lowering of the voice (women).
- Common (1-10%): nausea, vomiting, weakness, paresthesia, change in vaginal discharge and post-menopausal bleeding.
- Uncommon (0,1-1%): rash, skin reactions, muscle cramps and confusion.
- Rare (0,01-0,1%): blood cell disorders, paralysis, dehydration, eczema and hypersensitivity.
- Very rare (0,01%): acute renal failure, stomach inflammation, gastrointestinal cramps, intestinal bleeding, alopecia (hair loss), breast cancer and hepatitis.

Safety:
- You may feel a little dizzy the first few days of using this medicine. If you continue to feel dizzy, do not participate in traffic.
- Furthermore, combining spironolactone with alcohol could increase the side effects you are experiencing.
- Next, Spironolactone could have interactions with other drugs you are using. Therefore always discuss this with your personal doctor.
- Lastly, Spironolactone could be dangerous for pregnant women and women who are breastfeeding. Therefore always discuss this with your personal doctor.

My skincare hot take: Spironolactone is a BAND AID solution and your acne is gonna come back when you stop taking it 🤷🏻‍♀️ If you talk with a derm about recurring hormonal cystic acne, the recommendation is usually always Spironolactone which is a high level way to regulate your hormones. It can also impact your fertility and sometimes it's required to take birth control alongside Spiro. But Spiro + BC are both only band aid solutions (and a lot of admin tbh) that are covering up the true problem. I definitely had some sort of internal hormonal imbalance that kept causing recurring acne that both skincare and a healthy diet weren't fixing. So I wanted to get labwork done and that's when I found that Skincare , which is a skincare brand born out of struggles with acne, offered an Acne Lab Panel test which is a blood test via Health where holistic practitioners take a look at your bloodwork to see if there is anything off internally. And depending on what's going internally, it may take multiple rounds of labwork + supplements to clear any imbalances. I got recommended a regimen of supplements to address my underlying imbalances and my breakouts aren't completely gone but they are SO much smaller and more infrequent! I'm taking a break to focus on freezing my eggs, then I am going to evaluate how my skin is after and may do another round of labwork. Clearstem gifted me the Acne Lab Panel (retails for $500) but I paid $200 myself to speak with a Clearstem Practitioner to understand my results more. The Acne Lab panel is 100% worth it as this isn't even something I could get from my dermatologist and after egg freezing, I'll probably do a second round of the acne lab panel (and purchase it myself). It is structurally related to GABA, which is why we can observe the term GABA within its name. But interestingly, pregabalin does not act as GABA. Even though it is structurally related to GABA, it still doesn’t bind to GABA receptors. Then how can pregabalin affect neuronal conduction? Pregabalin reduces the opening of calcium channels, thereby reducing impulse conduction.

One of the important indications for pregabalin is the treatment of neuropathic pain. Elevated levels of glucose in diabetic patients may result in damage to neurons, leading to a loss of control and excessive firing of nociceptive neurons, resulting in neuropathic pain. Pregabalin can reduce neuropathic pain by reducing the propagation of pain signals to the brain.

Your Medication Pregabalin

Pregabalin is also known by the brand name: Lyrica.

Pregabalin comes in 25, 50, 75, 100, 150, 200, 225 and 300 mg capsules and 20 mg/mL oral liquid.

Pregabalin is most commonly used to help control certain kinds of seizures, painful nerve disease, and fibromyalgia. No matter what your doctor has you taking it for, pregabalin is usually taken by mouth with or without food most commonly 2 to 3 times a day. If upset stomach occurs take pregabalin with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses.

Pregabalin should not be discontinued abruptly because of the possibility of increasing seizure frequency. Therapy should be withdrawn gradually. Abrupt discontinuation has been associated with anxiety, diarrhea, headache, excessive sweating, sleeplessness, and nausea.

Some common side effects with pregabalin include swelling in the arms and legs, dizziness, tiredness, headache, weight gain, blurry vision and dry mouth.
Some less common side effects include drunken feeling, sleep disorder, confusion, loss of coordination, and constipation.

While taking pregabalin, remember it is important to tell your doctor or pharmacist if you:
• Have signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.)
• Have signs of depression, loss of strength of energy
• Experience vision changes, bruising, burning, or numbness feeling
• Have signs of Steven’s-Johnson syndrome, such as red, swollen, or blistered skin
• Start taking new medications, vitamins, or supplements

All forms of pregabalin including the liquid needs to be stored at room temperature in a dry place. Use the measuring device received from the pharmacist or doctor for the oral liquid.

Generic name: Pregabalin.
Brand names: Lyrica.
Available as a capsule and as a drink.

How to use it?
Capsule: make sure to swallow it whole with halve a glass of water. Do not break it and don’t chew on it, as this might decrease its effectivity.
Drink: make sure to measure the correct amount with a measuring spoon and drink it.

In addition, it is recommended to take Pregabalin at fixed times. This will make you less likely to forget a dose.

When to use it?
Pregabalin is part of a group of drugs called anti-epileptic drugs. However it can be used to treat several medical conditions, like:
- Epilepsy
- Neuropathic pain
- Generalized Anxiety Disorder

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.

- When treating adults, the initial starting dose is usually 150mg/7,5ml per day divided over 2-3 doses. Based on individual response and tolerance, your doctor might slowly increase the dose up to 300 mg/15 ml per day. If needed this can be further increased up to a maximum dose of 600 mg/30 ml per day.

What are common the side-effects?
- Very common (10%): somnolence, dizziness and headache.
- Common (1-10%): euphoria, confusion, irritability, disorientation, insomnia, decreased libido, coordination problems, tremor, memory impairment, impaired concentration, balance disorder, blurred vision, dizziness, nausea, vomiting, diarrhoea, dry mouth, constipation, back pain, pain in limbs, erectile dysfunction, feeling drunk, fatigue and weight gain.
- Uncommon (0.1-1%): Hypersensitivity, decreased hunger, low blood sugar, restlessness, depression, agitation, aggression, mood swings, hallucinations, abnormal dreams, increased libido, panic attacks, loss of consciousness, cognitive impairment, speech disorder, eye pain, heart failure , low blood pressure, shortness of breath, itching, rash, muscle stiffness, fever and weight loss.
- Rare (0.01-0.1%): Allergic reactions, loss of vision, squint, palpitations, swallowing disorder, pancreatic inflammation, jaundice, Stevens-Johnson syndrome, renal failure and breast enlargement.
- Very rare (0.01%): liver failure and inflammation of the liver.

Safety:
First of all be careful to combine Pregabalin with alcohol. As this might increase its side effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
Furthermore, Pregabalin can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.

Pregnancy and Lactation:
Research shows that use of Pregabalin can be dangerous when pregnant or when breastfeeding. However, there are some exceptions. Therefore always discuss this with your personal doctor.

Pregabalin (Lyrica®) is one of three medications in the class of seizure drugs known as alpha-2-delta ligands along with its cousins gabapentin (Neurontin®) and gabapentin enarcarbil (Horizant®) that are the consensus first-line drugs for the treatment of restless legs syndrome (RLS). @andyberkowskimd of ReLACS Health dissects the concerns about weight gain when it comes to taking pregabalin.

Pregabalin has known side effects that can including dizziness or drowsiness, but weight gain is another important concern of RLS patients at ReLACS Health when discussing this treatment. Very little is known about how pregabalin causes weight gain. It could be increased appetite, negative effects on the body's metabolism, or even simply a gain in water weight. Regardless, the risks for some patients seem to be consistently present in studies, but the degree of the effect on those who have weight gain may also be small.

Much of the research on pregabalin is in those with epilepsy (seizure disorder) as this was one of the original uses for the drug.

This is important to those with RLS who may be taking 75-150 mg for RLS rather than as much as 600 mg/day for epilepsy.

Lastly, Hamandi et al. (2006) showed an average weight gain of 1-2 pounds and about 9% greater chance of weight gain over placebo in a few studies of pregabalin in epilepsy.

The bottom line is that weight gain is common but may not be severe. It also may not be bothersome at lower doses of pregabalin. As with all treatment interventions, one must weigh (pun intended) the risks of the unwanted effects with the treatment benefits. With increase in body mass from medications, small adjustments to lifestyle whether improved sleep, more exercise, reducing processed carbs and added sugars, etc. can go a long way to offsetting weight gain from pregabalin.
