• Tizanidine is an alpha2-adrenergic agonist, used to treat spasticity associated with multiple sclerosis and spinal injury. Tizanidine is primarily metabolized by CYP1A2 and is considered a sensitive index substrate for this enzyme.
• The physiologically based pharmacokinetic (PBPK) modeling platform Simcyp® was used to evaluate the impact of CYP1A2 modulation on tizanidine exposure through drug-drug interactions (DDIs) and host-dependent habits (cigarette smoking).
• A PBPK model was developed to predict tizanidine disposition in healthy volunteers following oral administration. The model was verified based on agreement between model-simulated and clinically observed systemic exposure metrics (Cmax, AUC).
• The model was then used to carry-out DDI simulations to predict alterations in tizanidine systemic exposure when co-administered with various CYP1A2 perpetrators including competitive inhibitor (fluvoxamine), a mechanism-based inhibitor (rofecoxib), and an inducer (rifampin). Additional simulations were performed to evaluate the impact of cigarette smoking on systemic exposure.
• Under each scenario, the PBPK model was able to capture the observed fold changes in tizanidine Cmax and AUC when coadministered with CYP1A2 inhibitors or inducers.
• These results supplement the available research findings in the literature on PBPK predictions of drug-drug interactions and illustrate its potential application in drug development, specifically to support product labeling.

Potential Usefulness Of Topiramate In Crack Cocaine Treatment

Crack cocaine is powdered cocaine that has undergone an additional step of chemical processing. While the two forms of the drug have the same basic brain effects, crack only produces these effects for a relatively brief amount of time. This means that users of this form of cocaine may take the drug more frequently, and therefore may have greater odds of eventually experiencing the long-term brain changes associated with cocaine addiction.
In the study published in Drug and Alcohol Dependence, researchers from Parnassia Addiction Research Centre and the University of Amsterdam specifically investigated the potential usefulness of topiramate treatment in people addicted to crack cocaine. Thirty-seven participants in the study received a 12-week combination of topiramate and cognitive behavioral therapy, while another thirty-seven participants only received cognitive behavioral therapy.
The researchers mainly measured the effectiveness of topiramate treatment by comparing the rate of continued program involvement among recipients of the medication to the rate of continued involvement among those who did not receive the medication.

Clinical trials have been analyzed and found there is no clinical evidence that supports the use of Topamax for treating cocaine use disorder (cocaine addiction).
A person may have been prescribed topiramate in an attempt to help with an addiction to something else, such as cocaine. This practice is not recommended because of the lack of efficacy, and the number of adverse effects linked to taking the drug. Withdrawal from Topamax could be a very good strategy for improved health, especially where it was not found useful in treating addiction to other substances. Treating the biological and emotional triggers and precursors for addictive behaviors would be alternative choices for the best pathway forward.
The protocols we use at Alternative to Meds Center aim toward several significant outcomes, aside from safe tapering from prescription drugs. One of the prime areas we investigate is finding root causes for the conditions that preceded the medication. If migraines linked to specific toxic accumulations, as an example, we can expect to see improvements after removing the toxicity. It is not always possible to reduce medications to zero. However many improvements become possible after changing diet, arming oneself with de-stressing therapies including counseling, and lifestyle changes.

I started to get aura headaches about 3 years after I was diagnosed with MS. I was experiencing vertigo and severe fatigue with them to the point where I was spending most of my time in bed recovering. I was prescribed Topiramate or Topamax as it is also known and this has reduced my headaches hugely.
I started taking 25mg a day, increased to 50 mgs and I am now on 125mg a day of Topiramate.
Aura headaches can start with symptoms of blurred vison, wavy lines, dots or flashing lights before you actually get the migraine attack which can be a long lasting painful headache. I never really used to get the migraines, but now I do and they are painful on head movement. It also leaves me overwhelming fatigue.
Since being on this Topiramate medication I can live a relatively normal life, it does still happy occasionally but no where near to the same extent.
Topirmate did cause weight loss quickly for me, this is not a medication that will be helpful to everyone as it does have some very strong side effects, so please always ask your medical team.
*Please also only use aspirin if you are not allergic to it.
