FDA approves nivolumab plus cabozantinib for advanced renal cell carcinoma
FDA approves nivolumab plus cabozantinib for advanced renal cell carcinoma Side Effects 2 Views • 1 month ago

On January 22, 2021, the Food and Drug Administration approved the combination of nivolumab (Opdivo, Bristol-Myers Squibb Co.) and cabozantinib (Cabometyx, Exelixis) as first-line treatment for patients with advanced renal cell carcinoma (RCC).

Efficacy was evaluated in CHECKMATE-9ER (NCT03141177), a randomized, open-label trial in patients with previously untreated advanced RCC. Patients were randomized to receive either nivolumab 240 mg over 30 minutes every 2 weeks in combination with cabozantinib 40 mg orally once daily (n=323) or sunitinib 50 mg orally daily for the first 4 weeks of a 6-week cycle (4 weeks on treatment followed by 2 weeks off) (n=328).

The trial demonstrated a statistically significant improvement in progression-free survival (PFS), overall survival (OS) and confirmed overall response rate (ORR) for patients treated with nivolumab plus cabozantinib compared with those who received sunitinib. Median PFS per blinded independent central review (BICR) was 16.6 months versus 8.3 months; HR 0.51 (95% CI: 0.41, 0.64). Median OS was not reached in either arm; HR 0.60 (95% CI: 0.40, 0.89). Confirmed ORR per BICR was 55.7% and 27.1% in the nivolumab plus cabozantinib and sunitinib arms, respectively.

The most common adverse reactions (= 20%) in patients receiving the combination of nivolumab and cabozantinib were diarrhea, fatigue, hepatotoxicity, palmar-plantar erythrodysaesthesia syndrome, stomatitis, rash, hypertension, hypothyroidism, musculoskeletal pain, decreased appetite, nausea, dysgeusia, abdominal pain, cough, and upper respiratory tract infection.

The recommended dose is nivolumab 240 mg every 2 weeks (30-minute intravenous infusion) or 480 mg every 4 weeks (30-minute intravenous infusion) in combination with cabozantinib 40 mg orally once daily without food until disease progression or unacceptable toxicity.

The nivolumab review used the Real-Time Oncology Review (RTOR) pilot program, which streamlined data submission prior to the filing of the entire clinical application. Both the nivolumab and the cabozantinib applications used the Assessment Aid, a voluntary submission from the applicant to facilitate the FDA’s assessment. The FDA approved this application approximately one month ahead of the FDA goal date.

The nivolumab application was granted fast track review, and both the nivolumab and cabozantinib applications were granted priority review. A description of FDA expedited programs is in the Guidance for Industry: Expedited Programs for Serious Conditions-Drugs and Biologics.

Healthcare professionals should report all serious adverse events suspected to be associated with the use of any medicine and device to FDA’s MedWatch Reporting System or by calling 1-800-FDA-1088.

I don't normally talk a lot about studies on my channel, but I do think it is very important to sometimes for you to understand what the researchers are doing and the background that comes from your doctor giving you health advice.

So today I am going to talk about a great study that came out of rural China that focused on people who were at high risk for Cardiovascular disease, high blood pressure and stroke. They took 25% of their participants salt intake and replaced it with potassium chloride and then followed them for five years. The results were fascinating!

The study can be found here:

* If you have kidney disease you have to be very careful how much potassium you can have. So make sure to talk to your doctor about using a salt substitute.

Tiotropium: Benefits and side effects of this Powerful Medication for COPD and Asthma
Tiotropium: Benefits and side effects of this Powerful Medication for COPD and Asthma Side Effects 2 Views • 1 month ago

How Tiotropium Works to Treat COPD and Asthma.
Tiotropium belongs to a class of medications known as long-acting anticholinergics. It works by blocking the action of acetylcholine, a neurotransmitter responsible for bronchoconstriction and mucus production in the airways. By inhibiting the effects of acetylcholine, Tiotropium helps relax the smooth muscles surrounding the airways, allowing for improved airflow and reduced symptoms of respiratory conditions.
Tiotropium is available in inhalation powder form, commonly administered through a handheld inhaler device. The medication is designed to provide long-lasting relief, with effects lasting up to 24 hours. This makes it an ideal choice for individuals who require sustained symptom control.
Benefits of Tiotropium for COPD Patients.
Tiotropium has been extensively studied for its efficacy in managing COPD symptoms. Clinical trials have consistently shown that Tiotropium improves lung function, reduces exacerbations, and enhances exercise tolerance in COPD patients. By improving airflow and reducing inflammation in the airways, Tiotropium helps individuals breathe more easily and experience fewer respiratory symptoms.
Moreover, Tiotropium has been shown to improve quality of life in COPD patients. It allows individuals to participate in daily activities without the constant fear of breathlessness and provides a sense of control over their condition. This improved quality of life can have a significant positive impact on mental well-being and overall satisfaction.
Benefits of Tiotropium for Asthma Patients.
While Tiotropium is primarily indicated for COPD, recent studies have also explored its potential benefits for asthma patients. Asthma is a complex condition with various subtypes, and not all patients respond to the same treatments. For individuals with severe or difficult-to-control asthma, Tiotropium may be considered as an add-on therapy to existing treatments.
Multiple studies have demonstrated that adding Tiotropium to standard asthma therapy can lead to improved lung function, reduced symptoms, and decreased exacerbations. It has been particularly effective in patients with asthma that is not well-controlled with inhaled corticosteroids alone. Tiotropium's long-lasting effects provide sustained relief and can help individuals achieve better asthma control.
Dosage and Administration of Tiotropium.
The recommended dosage of Tiotropium may vary depending on the individual's condition and the specific inhaler device used. It is important to follow the instructions provided by the healthcare professional or read the patient information leaflet carefully.
Tiotropium inhalation powder is typically administered once daily, at the same time each day, for optimal symptom control. The inhaler device should be primed before the first use and should be cleaned regularly to ensure proper functioning. It is important to inhale the medication deeply into the lungs to achieve the desired effects.
If a dose is missed, it should be taken as soon as remembered. However, if it is close to the next scheduled dose, the missed dose should be skipped. Double dosing should be avoided.
Potential Side Effects of Tiotropium.
Like any medication, Tiotropium may cause side effects in some individuals. The most common side effects include dry mouth, constipation, urinary retention, and sore throat. These side effects are generally mild and resolve with continued use. However, if they persist or worsen, it is important to consult a healthcare professional.
In rare cases, Tiotropium may cause more serious side effects such as allergic reactions, increased heart rate, or difficulty breathing. If any of these symptoms occur, immediate medical attention should be sought.
Precautions and Contraindications for Using Tiotropium.
Tiotropium should be used with caution in individuals with certain medical conditions, such as narrow-angle glaucoma, urinary retention, or an enlarged prostate. It is important to inform the healthcare professional about any pre-existing medical conditions or medications being taken before starting Tiotropium.

Tiotropium in adults with asthma - Video Abstract ID 177603
Tiotropium in adults with asthma - Video Abstract ID 177603 Side Effects 2 Views • 1 month ago

Video abstract of review paper “Barriers to achieving asthma control in adults: evidence for the role of tiotropium in current management strategies” published in the open access Therapeutics and Clinical Risk Management journal by Jenkins.

Abstract: Despite the availability of a range of treatment options and management guidelines, a high proportion of adults with asthma remain uncontrolled. The challenge of managing uncontrolled asthma includes providing efficacious treatment while limiting side effects, recognizing situations when a change in asthma therapy is required, and considering patient preferences and satisfaction. In line with the Global Initiative for Asthma report, asthma management is based on a backbone of inhaled corticosteroid (ICS) therapy and use of add-on therapies to achieve disease control. This review considers whether add-on options could be better utilized in clinical practice. A number of long-acting muscarinic antagonists are in development, but tiotropium is the most widely studied for use in asthma. Evidence demonstrating the efficacy of tiotropium as an add-on therapy to at least ICS in adults with symptomatic mild, moderate, and severe asthma is presented from randomized controlled trials and real-world evidence. In addition, the benefit of tiotropium therapy in a wide range of patient phenotypes and disease severities without the need for biomarker assessment is discussed. Additional strategies that complement this approach, such as recognizing and overcoming barriers to adherence, ensuring optimal device use, and education and support to enhance patient–physician communication, are discussed. Physician education can also help raise awareness that additional management options are available for patients with moderate-to-severe asthma who remain uncontrolled on ICS/long-acting β2-agonist treatment.
