Mirtazapine has a unique mechanism of action that gives it an interesting side effect profile - drowsiness and weight gain are common while sexual dysfunction and nausea are rare. A large network meta analysis found that mirtazapine stacks up favourably compared to other antidepressants in terms of efficacy and acceptability. In my opinion, Mirtazapine is best suited for those with depression and insomnia who do not mind gaining a bit of weight.

Indication: short term treatment of active ulcer, GERD, treatment of heartburn, indigestion, management of Zollinger Ellison syndrome, prevention of GI bleeding in critically ill patients, management of symptoms associated with overuse of NSAIDs

Action: blocks action of histamine located in gastric parietal cells, inhibits gastric acid secretion

Nursing Considerations:
 may cause arrythmias, agranulocytosis, aplastic anemias
 assess for abdominal pain and occult blood
 monitor CBC
