Drug Cards Daily | Episode #52: tiotropium (Spiriva) | Treating COPD and Asthma with a LAMA

Tiotropium is a long acting muscarinic antagonist (LAMA) that is also known by the brand name Spiriva. It comes as an inhaled aerosol solution as well as an inhaled capsule. The main indications are for the treatment of COPD and Asthma. Tiotropium works through reversible competitive inhibition of acetylcholine at type 3 muscarinic receptors in bronchial smooth muscle which leads to bronchodilation. The medication is poorly absorbed in the GI and hepatically metabolized via the CYP2D6 and CYP3A4 pathways. The time to peak is between 5-7 minutes with the half-life elimination between 25-44 hours. Although the data in pregnancy and lactation is limited, the benefits for use likely outweighs the risk for a patient dealing with uncontrolled asthma. Some common side effects are cough, headache, GERD, rash and UTI. Some serious side effects are paradoxical bronchospasms and angle-closure glaucoma. It is important to note that tiotropium is not intended to be a quick-relief inhaler and should not be used for flare-ups or shortness of breath. It is recommended to clean the inhaler once monthly with water air dried. After usage the patient should rinse the mouth out; not due to the risk of thrush like with an ICS but to help prevent xerostomia and throat irritation.

Medical Edge: Breast cancer memory loss
Medical Edge: Breast cancer memory loss Side Effects 4 Views • 5 months ago

CINCINNATI (Liz Bonis) -- You may recall Local 12 News reported about a seminar for cancer and memory loss in September 2015. A young breast cancer survivor shared her story. She had a side effect and doctors at the UC Cancer Institute said more women need to know about it. Lisa Elliott said it was the little things she values more than ever right now. Not just going shopping again but actually remembering all the items on her shopping list. Just a few years ago Lisa was diagnosed December 2010 at age 29, I had taken off my bra one day and I saw a little bit of blood, and I thought, Oh, I must have had a bug bite that I scratched? It turned out she had breast cancer. After surgery, radiation and the medication for prevention, Tamoxifen, she did hear from health care providers about a lot of the expected side effects. But one of the things that they didn't talk about was a sort of foggy brain feeling. And she started noticing that. Her foggy brain however turned into a whole lot more, My friends would tease me. I would start to tell them a story and they would say, You just told me this yesterday! Full blown short and long-term memory loss, Dr. Luke Pater, a radiation oncologist, said his team has been looking into the after effect for years, It's seen with a lot of different therapy but most of the studies have resided with breast cancer and with patients with brain tumors. He said the treatments can definitely impact memory; sometimes short term and sometimes long term or even that foggy thinking. The good news is that it means that research shows patients are living longer. Lisa Elliott said people should consider these things and she wants other survivors to know the side effect is real. It doesn't make you feel better because you still forget things. But it makes you feel better that you are not crazy and it's important for me to share my story or experiences so other people don't feel like they are the only ones, said Lisa.  Today let’s talk about hyperbaric oxygen therapy (HBOT).

Hyperbaric oxygen therapy is one of the most incredible therapies. Oxygen therapy is very credible and has been thoroughly researched.

What is hyperbaric oxygen therapy?

When you breathe air, you’re only getting 21% actual oxygen. Oxygen therapy raises the oxygen to nearly 100%.

You’re put in a pressurized closed in environment so that the oxygen not only goes into your lungs but right into your tissues. It targets hypoxic areas. Basically, it drives a high amount of concentrated oxygen into injured tissues. It also has the capacity to stimulate stem cells by 800%.

During hyperbaric therapy, you’re in the hyperbaric chamber for 1-2 hours for maybe 3 times a week for several weeks.

There are a ton of benefits of hyperbaric oxygen therapy. Hyperbaric therapy targets:

• Dementia
• Stroke
• Heart
• Asthma
• Emphysema
• Autoimmune
• Brain trauma
• COPD
• Carbon dioxide
• Anemia
• Inflammation after injury
• Profound effect (DNA, Genes)
• Explode mitochondria in tumors
• PTSD
