1. Gout is an arthritis caused by the deposition of urate crystals into the joint space
2. Colchicine works by disrupting microtubules and prevents white blood cells from getting into the joint space which stops the inflammatory response
3. Colchicine has a high rate of adverse events, in particular explosive diarrhea
4. The drug also has a very narrow therapeutic index and overdose is nearly universally fatal, with no antidote or effective treatment option available
5. Alternative agents such as steroids, which reduce the inflammatory response to urate crystals, along with NSAIDs may be better options for treatment

PHARMACIST Fi shares 7 Top Tips to help you try to avoid colchicine toxicity side effects (such as diarrhea, nausea, vomiting, stomach or intestine area pain, throat pain, etc.) - which are thankfully much less common now that a lot of doctors are prescribing
*modern, smaller dose schedules for acute gout flare treatment,
*lower colchicine dose schedules for people with kidney or liver problems, and
*managing serious interactions with certain types of other medicines - by either stopping colchicine or significantly reducing the colchicine dose schedule (depending on which medicine is interacting with colchicine).

It is also important to remember to:
*avoid eating grapefruit and avoid drinking grapefruit juice while taking colchicine
*never take more colchicine than the doctor advises to take, and
*only take colchicine if it has been prescribed for you and don't give your colchicine to other people (because some people must not take colchicine due to some types of medical conditions, or potentially lethal medicine interactions with certain types of medicines).
If the doctor's instructions about how to take colchicine could be interpreted more than one way, or if you forget exactly how the doctor advised to take colchicine, as soon as possible, ask your doctor:
*how much colchicine to take per dose, and
*how much time to leave between colchicine doses, and
*whether or not there is a date that you need to stop taking colchicine (this will depend on whether the aim is to treat an acute gout flare, or to try to prevent future gout flares - because the colchicine dose schedules for these 2 different aims are likely to be quite different).

Michelle L. O'Donoghue, MD, MPH: Hi. This is Dr Michelle O'Donoghue, reporting for Medscape. Joining me today is Dr Paul Ridker. He's the Eugene Braunwald Professor of Medicine at Harvard Medical School. He's also the director of the cardiovascular prevention program at Brigham and Women's Hospital.
He's going to be joining me today to discuss a very important and emerging topic, which is the use of low-dose colchicine. I think there's much interest in the use of this drug, which now has a US Food and Drug Administration (FDA) indication, which we'll talk about further, and it's also been written into both European and American guidelines that have been recently released.
Many people are talking about where this fits into our current armamentarium, and I think there probably is no better person to discuss this than Paul Ridker, who's been at the forefront of research into anti-inflammatory therapeutics. Thank you for joining me, Paul.

Lifestyle Lipid-Lowering Paramount
O'Donoghue: As we think about the concept behind the use of colchicine, we've obviously done a large amount of research into lipid-lowering drugs, but where does colchicine now fit in?
Ridker: Let's make sure we get the basics down. Anti-inflammatory therapy is going to be added on top of quality other care. This is not a replacement for lipids; it's not a change in diet, exercise, and smoking cessation. The new data are really telling us that a patient who's aggressively treated to guideline-recommended levels can still do much better in terms of preventing heart attack, stroke, cardiovascular death, and revascularization by adding low-dose colchicine as the first proven anti-inflammatory therapy for atherosclerotic disease.
I have to say, Michelle, for me, it's been a wonderful end of a journey in many ways. This story starts almost 30 years ago for quite a few of us, thinking about inflammation and atherosclerosis. The whole C-reactive protein (CRP) story is still an ongoing one. We recently showed, for example, that residual inflammatory risk in some 30,000 patients, all taking a statin, was a far better predictor of the likelihood of more cardiovascular events, in particular cardiovascular death, than was residual cholesterol risk.
Think about that. We're all aggressively giving second lipid-lowering drugs in our very sick patients, but that means inflammation is really the untapped piece of this.
The two clinical trials we have in front of us, the COLCOT trial and the LoDoCo2 trial — both New England Journal of Medicine papers, both with roughly 5000 patients — provide very clear evidence that following a relatively recent myocardial infarction (that's COLCOT) in chronic stable atherosclerosis (that's LoDoCo2), we're getting 25%-30% relative risk reductions in major adverse cardiovascular events (MACEs) on top of aggressive statin therapy. That's a big deal. It's safe, it works, and it's fully consistent with all the information we have about inflammation being part and parcel of atherosclerosis. It's a pretty exciting time.
Inflammatory Pathway
O'Donoghue: It beautifully proves the inflammatory hypothesis in many ways. You led CANTOS, and that was a much more specific target. Here, in terms of the effects of colchicine, what do we know about how it may work on the inflammatory cascade?
Ridker: Our CANTOS trial was proof of principle that you could directly target, with a very specific monoclonal antibody, a specific piece of this innate immune cascade and lower cardiovascular event rates.
Colchicine is a more broad-spectrum drug. It does have a number of antineutrophil effects — that's important, by the way. Neutrophils are really becoming very important in atherosclerotic disease progression. It's an indirect inhibitor of the so-called NLRP3 inflammasome, which is where both interleukin (IL)-1 (that's the target for canakinumab) and IL-6 are upregulated. As you know, it's been used to treat gout and pericarditis in high doses in short, little bursts.
The change here is this use of low-dose colchicine, that's 0.5 mg once a day for years to treat chronic, stable atherosclerosis. It is very much like using a statin. The idea here is to prevent the progression of the disease by slowing down and maybe stabilizing the plaque so we have fewer heart attacks and strokes down the road.
It's entering the armamentarium — at least my armamentarium — as chronic, stable secondary prevention. That's where the new ACC/AHA guidelines also put it. It's really in as a treatment for chronic, stable atherosclerosis. I think that's where it belongs.

Colchicine is a medication that is primarily used to treat gout and other inflammatory conditions. It works by reducing the inflammation and pain associated with these conditions.
Gout is a type of arthritis that occurs when there is a buildup of uric acid crystals in the joints, leading to inflammation and severe pain. Colchicine works by inhibiting the movement of certain white blood cells, called neutrophils, into the affected joint. This reduces the release of inflammatory substances and decreases the inflammatory response in the joint, thereby relieving pain and swelling.
Colchicine is also used to prevent gout attacks in individuals who have frequent episodes. It is typically taken orally, either as a tablet or capsule, and is most effective when taken at the first sign of a gout attack.
In addition to treating gout, colchicine is also used to manage other inflammatory conditions such as familial Mediterranean fever and Behçet's disease. It may also be used off-label for other conditions, such as pericarditis.
It is important to note that colchicine can have side effects, and it may interact with other medications. Common side effects include diarrhea, nausea, vomiting, and abdominal pain. In rare cases, it can cause more serious side effects, such as muscle weakness or damage, bone marrow suppression, or allergic reactions. It is important to discuss any potential risks or concerns with a healthcare professional before starting colchicine treatment.

Colchicine also has other types of interactions. Here are some examples of other interactions with colchicine (from Colgout brand colchicine product information for doctors):
*Alcohol increases risk of colchicine toxicity side effects like nausea, vomiting, diarrhoea, stomach or intestine pain, and increases gout flare risk by raising uric acid level
*Colchicine may increase risk of potential muscle side effects (like muscle tenderness or weakness) of some statin or fibrate medicines for cholesterol (e.g. simvastatin, atorvastatin, lovastatin, gemfibrozil, fenofibrate)
*Colchicine may increase risk of low blood cell production if taken with other medicines with this potential side effect
*Acidifying substances (like ammonium chloride, ascorbic acid (vitamin C), acid phosphates) may make colchicine less effective
*Alkalinizing substances (like sodium bicarbonate, potassium citrate) may increase the effect of colchicine
*Colchicine may increase the effect of medicines that make us sleepy, and alcohol
*Colchicine may increase the effect of adrenaline, dopamine, dobutamine, isoprenaline or ephedrine
*If colchicine is taken long-term, it may decrease absorption of vitamin B12, so ask your doctor for a blood test for vitamin B12 - to find out if you may need a vitamin B12 supplement
*The herbal medicine St John's Wort (Hypericum perforatum) may make colchicine less effective, by making our body get rid of colchicine quicker than usual; some types of medicines may interact with colchicine in this way

It is marketed as a stool softener, but according to The American Journal of Gastroenterology, docusate has been shown to be no better at treating hard stool in constipation than placebo.

- Docusate is a stool softener. It draws water into the stool to make it softer, which helps relieve constipation. As it is an anionic surfactant that lowers the surface tension of the oil and water interface of the stool, allowing fecal material to be penetrated by water and fat, thereby softening the stool.
- It is available as capsules and tablets, syrup and enema.
- Docusate is used in the short-term treatment of constipation; prophylaxis in patients who should not strain during defecation, such as after anorectal surgery, myocardial infarction, also used to evacuate the colon for rectal and bowel examinations, and for prevention of dry hard stools.
