Mechanism of Action
GABA-A Receptor Agonism: Temazepam enhances the activity of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA) by binding to the benzodiazepine site of the GABA-A receptor complex. Indications
Insomnia: Temazepam is indicated for the short-term management of insomnia characterized by difficulty in falling asleep, frequent nocturnal awakenings, and/or early morning awakenings2.

Dosage and Administration
Initial Dose: The typical starting dose for adults is 7.5 to 30 mg taken orally at bedtime.

Maximum Dose: The maximum recommended dose is 30 mg per day.

Adverse Effects
Common Side Effects: Include dizziness, drowsiness, headache, and gastrointestinal disturbances.

Serious Side Effects: Include complex sleep behaviors (e.g., sleep-walking, sleep-driving), which may result in serious injuries, including death.

Pharmacokinetics
Absorption: Temazepam is well absorbed from the gastrointestinal tract, with minimal first-pass metabolism.

Telmikind-CT 40/6.25 Tablet 10's belongs to the class of anti-hypertensives primarily taken to treat hypertension (high blood pressure). Hypertension or high blood pressure is a life-long or chronic condition in which the blood pressure against the artery wall becomes too high. It can lead to the risk of heart diseases, like heart attack and stroke.

Telmikind-CT 40/6.25 Tablet 10's is a combination of Telmisartan (angiotensin II receptor antagonist) and Chlorthalidone (thiazide diuretic or water pill). Telmisartan is an angiotensin receptor blocker that helps to relax and widen the blood vessels (arteries). Chlorthalidone is a diuretic that prevents absorption of excess salt in the body, preventing fluid retention. Video abstract of original research paper “CYP2D6 polymorphisms may predict occurrence of adverse effects to tamoxifen: a preliminary retrospective study” published in the open access journal Breast Cancer - Targets and Therapy by authors Wickramage I, Tennekoon KH, Ariyaratne MAY.

Introduction and aims: Tamoxifen is an adjuvant drug effective in treating hormone receptor – positive breast cancer. However, 30%–50% of patients relapse and many develop adverse
effects, such as hot flashes and fatty liver. Allelic variations altering the activity of cytochrome P450-2D6 enzyme affect response to tamoxifen by modulating metabolism of tamoxifen into its pharmacologically active metabolite endoxifen. Although association between CYP2D6 polymorphisms and recurrence of breast cancer in patients on tamoxifen had been reported, little
evidence exists on association between these polymorphisms and adverse effects to tamoxifen. This study explored the association between CYP2D6 polymorphisms and tamoxifen effects,
hitherto not studied in Sri Lanka.
Methods: A retrospective preliminary study was carried out on 24 breast cancer patients on tamoxifen for minimally 3 months attending National Cancer Institute, Maharagama, Sri Lanka.
They were not on CYP2D6-inhibiting drugs, chemotherapy or other endocrine therapy, and had no conditions that could occur as adverse effects to tamoxifen before starting the therapy. Their
blood samples were collected, DNA was extracted and genotyped using SNaPshot Multiplex sequencing based single-nucleotide polymorphism (SNP) assay.
Results: SNP/allele frequencies detected: 1846G greater than A (confirmatory of *4 null allele)=8.3%; 2549delA (confirmatory of *3 null allele)=50%; 100C greater than T (suggestive of *10 reduced functional allele, in addition to other alleles)=0%; combination of 2988G greater than A, -1584C and 2850C greater than T (strongly suggestive of *41 or other reduced functional allele)=4.8%. Occurrence of heterozygous 2988G greater than A SNP with -1584C and 2850C greater than T was significantly higher among those with ultrasound-diagnosed fatty liver following the commencement of tamoxifen therapy (P=0.029). Adverse effects occurred at a significantly higher frequency among postmenopausal women (P=0.041). Three patients who developed recurrence of breast cancer had no association with SNPs tested.
Conclusions: CYP2D6 SNP combination 2988G greater than A, -1584C and 2850C greater than T, strongly suggestive of *41 reduced functional allele, is likely to be useful in predicting occurrence of adverse effect fatty liver in breast cancer patients on tamoxifen, thereby alternative treatment can be considered and lifestyle modifications implemented. Larger sample studies are recommended with the measurement of tamoxifen and metabolite levels. Alternative therapy should be considered for postmenopausal patients.

Generic name: Tacrolimus.
Brand names: Protopic, Advagraf and Prograf.
Available as a capsule, tablet, suspension, injection and ointment.

How to use it?
- Ointment: dry your skin and apply it in a thin layer. Wash your hands thoroughly with water and soap afterwards. Avoid contact with water the firsts 30 minutes after applying it.
- Capsules or tablets: Swallow it completely with half a glass of water. Make sure you don’t break them and you don’t chew on them, because this can decrease the effectivity. Capsules or tablets should also be taken on an empty stomach, 1 hour before a meal or 2 hours after a meal. This improves the effectivity.

Duration of treatment?
The duration of treatment depends on many factors, mostly the indication of use and the severity of this indication. Therefore the duration of use can be weeks, years and in some cases even a life time.

When to use it?
Tacrolimus is an immune suppressive drug.
As an ointment it can be used to treat mild to severe eczema.
As a capsule, tablet, suspension or injection, it can be used to prevent an organ rejection after an organ transplant or to treat several autoimmune bowel diseases.

What is the correct dosage?
Always use the dosage and frequency prescribed by your own doctor.
Ointment starting dose for adults: twice a day for a maximum of 6 weeks.
Ointment maintenance dose for adults: twice a week on affected areas.
Capsules, tablets, injections or suspensions: dosage is based on your blood levels.

What are the side-effects?
Ointment:
- Very common (10%): itching and burning sensation.
- Common (1-10%): sensation of warmth, pain, irritation, rash, itching and local skin infection.

Tablet/Capsule/Injection/Suspension:
Very common (10%): high blood pressure, headache, insomnia, nausea, diarrhea, kidney problems and diabetes.
Common (1-10%): fast heart rate, heart problems, cough, fever, anxiety, confusion, mood swings, vomiting, abdominal pain, rash, acne and sweating.

Safety:
Do not eat grapefruits.
Do not use alcohol.
Avoid sun exposure and apply sunscreen.

Pregnancy and Lactation:
Tacrolimus can be dangerous when pregnant and while breastfeeding. Therefore always discuss this with your prescribing physician.
