Dangerous Side Effects
Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Amitriptyline (Elavil) a tricyclic antidepressant (TCA) that has been used in psychiatry to treat depression since about 1960. Tricyclic antidepressants are not as commonly used as some other antidepressant medications (like SSRIs and SNRIs) as they tend to carry increased risks for side effects (including, but not limited to, cardiac arrhythmias, increased intra-ocular pressure, dry mouth, constipation, urinary retention, sedation, orthostatic hypotension, tachycardia and others). Amitriptyline is also FDA approved for use in neuropathic pain, post-herpetic neuralgia, fibromyalgia and migraine headache prophylaxis. If an individual develops suicidal thoughts while taking an antidepressant (or at any time, whether they take a medication or not) they need to seek immediate medical attention. TCAs, like amitriptyline or Elavil) can be extremely effective (with all of this being said) and for some individuals who do not respond to other treatment modalities, they can be an important next step for the treatment of their depression.

Amitriptyline is a tricyclic antidepressant used to treat various conditions, including depression, chronic pain (like neuropathic pain), and migraine prevention. While it can be effective, amitriptyline can cause a range of side effects, both short-term and long-term. These side effects can vary in intensity from mild to severe.
Common Side Effects:
Drowsiness and Fatigue: Amitriptyline is known for causing sedation, making users feel extremely tired or sleepy.
Dry Mouth: Many users experience persistent dry mouth while taking amitriptyline.
Dizziness: Feeling lightheaded, especially when standing up quickly (due to orthostatic hypotension), is a frequent side effect.
Weight Gain: Long-term use of amitriptyline is often associated with significant weight gain.
Constipation: This medication can slow down digestion, leading to constipation.
Blurred Vision: Visual disturbances, including blurred vision or trouble focusing, are possible.
Increased Appetite: Amitriptyline can lead to increased hunger, contributing to weight gain.
Urinary Retention: Difficulty urinating or inability to empty the bladder completely is a possible side effect.
Increased Sweating: Some individuals may notice excessive sweating, especially at night.
Confusion or Cognitive Impairment: Some people report memory problems, difficulty concentrating, or confusion, especially in older adults.
Serious Side Effects:
Cardiovascular Issues:
Irregular Heartbeat (Arrhythmia): Amitriptyline can affect the heart’s electrical signals, leading to arrhythmias, especially in individuals with pre-existing heart conditions.
Increased Heart Rate (Tachycardia): A faster-than-normal heart rate is another possible issue.
Suicidal Thoughts: As with other antidepressants, amitriptyline can increase the risk of suicidal ideation, particularly in young adults and teenagers during the early stages of treatment.
Seizures: Though rare, amitriptyline may lower the seizure threshold in some individuals, increasing the risk of seizures.
Mania: In people with bipolar disorder, amitriptyline can trigger manic episodes.
Severe Allergic Reactions:
Swelling of the face, lips, tongue, or throat, and difficulty breathing, which may indicate an allergic reaction (anaphylaxis).
Orthostatic Hypotension: Severe drops in blood pressure upon standing, which can lead to fainting or falls, particularly in older adults.
Tardive Dyskinesia: A rare side effect involving involuntary movements, often of the face or limbs, after prolonged use.

Lesson on the antibiotic amoxicillin, what bacteria amoxicillin targets, the mechanism of action of amoxicillin, contraindications of amoxicillin usage, and adverse effects of amoxicillin. Amoxicillin is an aminopenicillin antibiotic that has a broad spectrum of targets including gram positive cocci, enterococci, and gram negative bacteria including E. coli, shigella and salmonella. However, recent antibiotic resistance has rendered amoxicillin less effective. In this lesson, we also discuss how bacteria become resistant to amoxicillin and what can be used to increase the effectiveness of amoxicillin against bacteria that have become resistant to amoxicillin. We discuss infections that amoxicillin can be used for (urinary tract infections, middle ear infections).
NOTE: The Gram Negative “Aerobes” discussed in the video are actually “Facultative Anaerobes”, although they act as “aerobes” in the presence of oxygen.

▪ Amoxicillin is a penicillin antibiotic. Clavulanate potassium helps prevent certain bacteria from becoming resistant to amoxicillin.
▪ Amoxicillin and clavulanate potassium is a combination medicine used to treat many different infections caused by bacteria, such as sinusitis, pneumonia, ear infections, bronchitis, urinary tract infections, and infections of the skin.
▪ Common side effects may include: severe stomach pain, diarrhea that is watery or bloody, nausea, vomiting; diarrhea; rash, itching; vaginal itching or discharge; or diaper rash.
▪ Amoxicillin and clavulanate potassium can make birth control pills less effective. Ask your doctor about using a non-hormonal birth control (condom, diaphragm, cervical cap, or contraceptive sponge) to prevent pregnancy.
▪ Avoid taking this medicine together with or just after eating a high-fat meal. This will make it harder for your body to absorb the medication. Antibiotic medicines can cause diarrhea, which may be a sign of a new infection. If you have diarrhea that is watery or bloody, call your doctor before using anti-diarrhea medicine.

Clotrimazole: Drug Treats Jock Itch, Athlete's Foot, Oral Thrush, and Other Fungal Infections

Clotrimazole belongs to a class of medications called broad-spectrum antifungal drugs.
Clotrimazole’s mechanism of action is to alter the permeability of the fungal cell wall leading to loss of rigidity, thus resulting in fungal cell death.
Clotrimazole is prescribed for treating diaper rash, oral thrush or oropharyngeal candidiasis, athlete’s foot (tinea pedis), tinea corporis or ringworm infections, jock itch or tinea cruris, vaginal yeast infections or vulvovaginal candidiasis, pityriasis versicolor (skin eruption on the trunk and proximal extremities), and skin diseases with associated fungal infections.
Clotrimazole throat paints must be applied to the lesions by using a clean cotton bud after the meals.
Clotrimazole vaginal cream or suppositories can be placed by using the applicator provided.

Benzodiazepines are a class of medication, often used to treat short-term anxiety conditions, including panic attacks and specific phobias. Some examples of Benzodiazepines include diazepam, lorazepam, and alprazolam. Valium (Diazepam) is a benzodiazepine which is commonly used to treat anxiety disorders. It is also frequently used to treat alcohol and sedative withdrawal (in detox facilities) as it has a very long half-life. There are significant risks of side effects, including abuse, dependence, tolerance and withdrawal.
Dr. Pagnani discusses tapering and stopping this medication in patients as well. Because it has a very long half-life (taking up to 100 hours for half of its metabolites to leave someone's system) patients often do not wait long enough after a decrease in their dose (to allow themselves to come to a new baseline), before making an additional decrease. It can take up to 3 weeks, to fully leave someone's system (when they stop the medication)! This long half-life can lead to a compounding effect of withdrawal, and patients believing that they need to stay on the medication "forever," when in fact, they can often taper off of it, with a more calculated and slow approach. Providers can even use compounding pharmacies to make incredibly small decreases overtime.
Starting, stopping or changing doses of this medication needs to be done by an individual's psychiatric provider, or patients can have serious adverse outcomes.
