MEDICAL ANIMATION TRANSCRIPT: Fentanyl is a synthetic opioid drug. The pharmaceutical form is prescribed to treat severe pain, usually from cancer or after surgery. There’s also an illicit form that’s made and sold illegally on the drug market. People take it because it produces a strong “high” and intense feelings of well-being. It’s potency, or strength, is up to fifty times that of heroin, an illegal opioid, and one hundred times that of morphine, another opioid used to treat severe pain. As with any opioid, fentanyl addiction is a brain disease where you have an overwhelming craving for the drug. You can’t stop taking the drug despite the harm it may cause you. If you’re addicted and stop taking the drug, you can feel sick. This sickness is called withdrawal. An important treatment for addiction to opioids, such as fentanyl, is called medication-assisted treatment, or “MAT.” MAT is a “whole patient” approach that combines medication to help you stop using opioids, with counseling and behavioral therapy. Three common medications used to treat opioid addictions, including fentanyl, are methadone, buprenorphine, and naltrexone. In the brain, each drug attaches to the same receptors on cells as fentanyl does. Methadone activates the receptors, but it's a slower acting drug than fentanyl because it takes longer to reach the brain when used as prescribed. This reduces the “high” feeling and prevents withdrawal symptoms. Buprenorphine is a weaker drug than fentanyl because it doesn’t activate the receptors as strongly. This makes it much less likely to cause an overdose than a drug that fully activates the receptors. It also blocks fentanyl from attaching to the receptors. As a result, it reduces uncomfortable cravings for fentanyl without making you feel high. Naltrexone blocks the opioid receptors by attaching to them so fentanyl can’t. This prevents fentanyl from getting you “high”. It does reduce cravings, but does not stop withdrawal symptoms. Drug treatment is more effective when combined with behavioral therapy. Therapy can help you handle stress and life events that can trigger cravings for fentanyl. A fentanyl overdose is a condition where taking too much of the drug may cause life-threatening symptoms. An overdose of fentanyl can slow or stop your breathing. As a result, the lack of oxygen can lead to coma, permanent brain damage, and even death. Call nine-one-one immediately if you think a fentanyl overdose has happened. Start CPR if the person isn’t breathing or doesn’t have a pulse. The most effective and life-saving treatment for an overdose is naloxone. It’s an injectable drug or nasal spray that quickly blocks the effects of fentanyl and can stop an opioid-related overdose. Gabapentin, sold under the brand name Neurontin among others, is an anticonvulsant medication primarily used to treat partial seizures and neuropathic pain.[3][6] It is a first-line medication for the treatment of neuropathic pain caused by diabetic neuropathy, postherpetic neuralgia, and central pain.[7] It is moderately effective: about 30–40% of those given gabapentin for diabetic neuropathy or postherpetic neuralgia have a meaningful benefit.[8]

Sleepiness and dizziness are the most common side effects. Serious side effects include an increased risk of suicide, respiratory depression, and allergic reactions.[3] Lower doses are recommended in those with kidney disease.[3] Gabapentin acts by decreasing activity of a subset of calcium channels.[9][10][11]

Gabapentin was first approved for use in 1993.[12] It has been available as a generic medication in the United States since 2004.[13] In 2019, it was the tenth most commonly prescribed medication in the United States, with more than 47 million prescriptions.[14][15] During the 1990s, Parke-Davis, a subsidiary of Pfizer, used a number of illegal techniques to encourage physicians in the United States to prescribe gabapentin for unapproved uses.[16] They have paid out millions of dollars to settle lawsuits regarding these activities.[17] Medical uses

Gabapentin is recommended for use in focal seizures and neuropathic pain.[3][6] Gabapentin is widely prescribed off-label in the US and UK,[18][19] for example, for the treatment of non-neuropathic pain,[18] anxiety disorders and bipolar disorder.[20]
Seizures

Gabapentin is approved for the treatment of focal seizures;[21] however, it is not effective for generalized epilepsy.[22]
Neuropathic pain

Gabapentin is recommended as a first-line treatment for chronic neuropathic pain by various medical authorities.[6][7][23][24] This is a general recommendation applicable to all neuropathic pain syndromes except for trigeminal neuralgia.[7][24]

In regard to the specific diagnoses, the best evidence exists for gabapentin treatment of postherpetic neuralgia and diabetic neuropathy.[8] Gabapentin is approved for the former indication in the US.[3] In addition to these two neuropathies, European Federation of Neurological Societies guideline notes gabapentin effectiveness for central pain.[7] A combination of gabapentin with an opioid or nortriptyline may work better than either drug alone.[7][24]

Overall, gabapentin shows moderate effectiveness for neuropathic pain. Only a minority of patients obtain meaningful relief. Out of 10 persons treated with gabapentin, three to four benefit substantially as compared to one to two persons treated with placebo.[8]

Evidence finds little or no benefit and significant risk in those with chronic low back pain or sciatica.[25][26] Gabapentin is not effective in HIV-associated sensory neuropathy[27] and neuropathic pain due to cancer.[28]
Anxiety

There is a small amount research on the use of gabapentin for the treatment of anxiety disorders.[29][30] In a controlled trial of breast cancer survivors with anxiety,[30] and in a trial for social phobia,[29] gabapentin significantly reduced anxiety levels. For panic disorder, gabapentin is ineffective.[30][29] Conversely, some psychiatric textbooks see "a possible role for gabapentin in anxiety disorders, particularly social phobia and panic disorder" based on "case reports and double-blind studies"[31] or state that controlled studies have not shown gabapentin to be very effective for psychiatric indications; "however, clinically it is effective".[20]
Sleep

Gabapentin is effective in treating sleep disorders such as insomnia and restless legs syndrome. Gabapentin enhances slow-wave sleep in patients with primary insomnia. It also improves sleep quality by elevating sleep efficiency and decreasing spontaneous arousal. The results suggest that gabapentin may be beneficial in the treatment of primary insomnia.[32]
Drug dependence

Gabapentin is moderately effective in reducing the symptoms of alcohol withdrawal and associated craving.[33][34][35] The evidence in favor of gabapentin is weak in the treatment of alcoholism: it does not contribute to the achievement of abstinence, and the data on the relapse of heavy drinking and percent of days abstinent do not robustly favor gabapentin; it only decreases the percent days of heavy drinking.[36]

Gabapentin is ineffective in cocaine dependence and methamphetamine use,[37] and it does not increase the rate of smoking cessation.[38] Gabapentin does not significantly reduce the symptoms of opiate withdrawal.[37] There is insufficient evidence for its use in cannabis dependence.[39]

Hydroxyzine will be prescribed more than 7 million times this year. Originally known as Atarax and Vistaril, hydroxyzine is an antihistamine in the same family as Benadryl or diphenhydramine. Unlike the newer non-sedating antihistamines, hydroxyzine readily passes into the brain and causes sleepiness.

Hydroxyzine remains available by prescription only. It assists in treatment of a veritable menu of conditions ranging from allergies to itch and motion sickness to anxiety. Hydroxyzine provides sedation and may be useful as a non-addictive sleeping pill. It may also boost the analgesic effect of opioids.

Side effects associated with hydroxyzine appear similar to the other antihistamines. Recent concerns in Europe suggest some individuals appear susceptible to electrocardiographic abnormalities that may lead to cardiac arrhythmias. For this reason the EMA, the European equivalent of the FDA, suggests as low a dose as possible for as short a period as feasible. Those concerns do not seem to be mirrored in the United States.

Due to hydroxyzine’s likelihood of causing sedation, caution seems appropriate when engaging in any activities that require mental alertness. It might pose a danger to drive a motor vehicle or operate heavy equipment while under the effects of the drug. The half life of hydroxyzine rises from 14 hours in an adult to nearly 30 hours in senior citizens.

One of the metabolic by-products of hydroxyzine is cetirizine. This later compound is the generic of Zyrtec, a popular non-sedating antihistamine. Interestingly of all of the non-sedating antihistamines, Zyrtec seems most likely to cross into the central nervous system and promote sleepiness.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Benadryl (diphenhydramine) and its potential to be utilized for insomnia, anxiety and other medical conditions. He discusses that diphenhydramine is the active ingredient in many over-the-counter sleep aids (like unisom) and it may be preferred to controlled medications like benzodiazepines, as it does not have the same level of abuse potential. With that said, many individuals fail to recognize the possible side effects of using over-the-counter medications like benadry, which include dry mouth, constipation and bowl obstruction, urinary retention, cardiac arrhythmias (like torsades de points and QT prolongation), seizure, renal failure and others. Finally, benadryl DOES have abuse potential (every medication that causes sedation has abuse potential) and it is recommended that taking any medication (even over-the-counter) is under the direction and supervision of a medical professional.

Indication: Mild to moderate pain, inflammatory states

Action: Decreases pain and inflammation by inhibiting prostaglandins

Therapeutic Class: antipyretics, antirheumatics, nonopioid analgesics, nonsteroidal anti-inflammatory agents

Pharmacologic Class: nonopioid analgesics

Nursing Considerations:
 may cause GI bleeding, hepatitis, Stevens-Johnson Syndrome
 may cause anaphylaxis
 monitor for headache, nausea, vomiting, constipation
 therapy should be discontinued after first sign of rash
 monitor renal and liver labs
 patient should avoid using alcohol
