Irbesartan (Avapro). Side effects of this commonly used medication. Side Effects 2 Views • 5 months ago

Understanding the Common Side Effects of Irbesartan (sold under the brand name: Avapro, which is registered trademark),

Welcome to our educational video by Dr. Lukasz Kiljanek, a Board Certified Nephrologist, Hypertension Specialist, and Internal Medicine physician.

Irbesartan a widely prescribed medication for high blood pressure (hypertension), heart failure or kidney disease. While it is highly effective, it's important to be aware of potential side effects. In this video, we'll discuss common side effects.

Cough: Some people might experience a persistent dry cough while taking Irbesartan. If you develop a cough, let your healthcare provider know. There are other medications that provide almost the same benefits as Irbesartan but may not cause a cough.

Dizziness: Dizziness can happen, especially if your blood pressure lowers too much or if you are dehydrated.

High Potassium level: Irbesartan can cause your potassium levels to become high. Your healthcare provider will likely monitor your potassium levels, usually 3-4 weeks after starting Irbesartan or increasing the dose of Irbesartan to make sure potassium stays within a safe range.

Increased creatinine level: Creatinine level can go up, and this sometimes indicates that the medication is actually working. Blood work will be needed to monitor creatinine levels to ensure it remains within safe ranges, usually 3-4 weeks after starting Irbesartan or increasing the dose of Irbesartan.

Always notify your healthcare provider if you develop any side effects or have any concerns or questions about your medication. They can provide the best advice and support for your specific situation.

Pharmacology of Ivabradine ; Overview, Pharmacokinetics, Mechanism of action, Uses, Effects
Pharmacology of Ivabradine ; Overview, Pharmacokinetics, Mechanism of action, Uses, Effects Side Effects 2 Views • 5 months ago

Ivabradine is a heart rate-lowering medication used to treat chronic heart failure and stable angina pectoris. Here's a brief overview of its pharmacology:

Mechanism of Action
Ivabradine selectively inhibits the funny current (If) in the sinoatrial (SA) node of the heart. This current is responsible for regulating the heart rate2. By inhibiting the If current, ivabradine reduces the heart rate without affecting myocardial contractility or ventricular repolarization.

Administration Routes
Ivabradine is taken orally, usually in tablet form.

Pharmacokinetics
Absorption: Ivabradine is well absorbed from the gastrointestinal tract, with a bioavailability of about 40%.

Distribution: It has a volume of distribution of approximately 100 L and is about 70% protein-bound.

Metabolism: Ivabradine is metabolized in the liver by the enzyme CYP3A4.

Excretion: It is excreted mainly through urine (4% as unchanged drug) and feces.

Half-Life: The elimination half-life is about 2 hours, with an effective half-life of around 6 hours.

Indications
Ivabradine is indicated for the treatment of:

Chronic heart failure in patients with a left ventricular ejection fraction (LVEF) of 35% or less, who are in sinus rhythm with a resting heart rate of 70 beats per minute (bpm) or higher, and who are either on maximally tolerated doses of beta-blockers or have a contraindication to beta-blocker use.

Stable angina pectoris in patients who cannot be treated with beta-blockers or in combination with beta-blockers when they are insufficiently controlled.

Adverse Effects
Common side effects include:

Visual disturbances (e.g., phosphenes)

Bradycardia (slow heart rate)

Dizziness

Headache

More serious side effects can include:

Severe bradycardia

Heart block

Does this cover what you were looking for, or is there something more specific you'd like to know about ivabradine?

----------------------------------------------------------------------------------------------------------------
Ivabradine is a unique medication used in the management of chronic congestive heart failure (CHF), specifically in patients with reduced ejection fraction (HFrEF) and persistent sinus tachycardia (heart rate ≥70 bpm) despite optimal beta-blocker therapy 🩺💓. Ivabradine selectively inhibits the funny current (If) in the sinoatrial node, reducing heart rate without affecting myocardial contractility or blood pressure 🌬️💊. By lowering the heart rate, it improves myocardial efficiency, reduces oxygen demand, and allows for better ventricular filling during diastole, ultimately alleviating symptoms and reducing the risk of hospitalization in CHF patients.

Ivabradine is particularly useful in patients who cannot tolerate higher doses of beta-blockers or those contraindicated for beta-blocker therapy. Common side effects include bradycardia, visual disturbances (phosphenes), and hypotension, which necessitate careful patient monitoring. It is not recommended for patients with atrial fibrillation or severe liver impairment. Ivabradine complements standard CHF therapies like ACE inhibitors, ARBs, beta-blockers, and aldosterone antagonists, contributing to better outcomes and improved quality of life. 🌿✨

Anti anginal Drugs: Ivabradine & Fasudil: CVS Pharmacology
-----------------------------------------------------------------------------------------------------
Ivabradine and Fasudil are unique anti-anginal drugs that provide symptomatic relief in ischemic heart disease through distinct mechanisms, making them valuable alternatives to traditional agents like nitrates, beta-blockers, and calcium channel blockers 💓💊. Ivabradine, a selective If (funny current) channel inhibitor, acts on the sinoatrial (SA) node, reducing heart rate (negative chronotropy) without affecting myocardial contractility or blood pressure, thus lowering oxygen demand while preserving cardiac output 🫀⚡.

This makes it especially useful in chronic stable angina and heart failure patients who cannot tolerate beta-blockers due to hypotension or bradycardia ⏳💉. It is contraindicated in severe bradycardia, acute decompensated heart failure, and pregnancy ⚠️🚫.

Fasudil, on the other hand, is a Rho-kinase inhibitor that works by inhibiting vascular smooth muscle contraction, promoting vasodilation, and preventing coronary artery spasm, making it highly effective in vasospastic (Prinzmetal’s) angina 🌊🩸.
Additionally, Fasudil has neuroprotective effects and is being explored for conditions like stroke, pulmonary hypertension, and Alzheimer's disease 🧠🔬. Side effects of Ivabradine include visual disturbances (phosphenes), bradycardia, and atrial fibrillation, while Fasudil may cause hypotension and dizziness 🚨🛑.

Unlike nitrates, both Ivabradine and Fasudil do not cause tolerance, making them suitable for long-term angina management 🔄💡. These drugs are often used as adjuncts alongside traditional therapies, providing better symptom control and improving exercise tolerance in chronic stable and vasospastic angina 🌍❤️. #Ivabradine #Fasudil
