Anticoagulants are drugs used to reduce blood clotting, or coagulation.
Coagulation is critical for the control of bleeding, which has 4 stages:
- Stage 1: formation of platelet plugs.
- Stage 2: formation of blood clots. Coagulation cascade produces thrombin and fibrin.
- Stage 3: termination of clot formation. Antithrombin binds to thrombin, as well as several other clotting factors, and inhibits their function.
- Stage 4: dissolution of blood clots.
Medications that prevent formation of platelet plugs are antiplatelets. Medications that interfere with the function or synthesis of clotting factors are anticoagulants. Anticoagulants and antiplatelets are the 2 classes of antithrombotic drugs.
Heparin binds to antithrombin and increases its affinity for thrombin and other clotting factors, thereby enhancing antithrombin’s inhibitory effect on coagulation, typically up to thousand-fold.
Low-molecular-weight heparin is produced by chemically splitting heparin into pieces of about one-third of its original size. The smaller size reduces its non-specific binding to plasma proteins and blood components, resulting in fewer side effects, longer half-life, and a more predictable dose-dependent response, so monitoring is not usually required. However, the shorter chain length also reduces its ability to bridge between antithrombin and thrombin, producing a reduced inhibitory effect on thrombin. Its anticoagulation effect relies on inhibition of factor 10a, for which a specific five-saccharide motif is sufficient.
Fondaparinux is a synthetic drug based on this five-saccharide sequence. It has a longer half-life and is associated with less bleeding complication than low-molecular-weight heparin in some studies.
Vitamin K Antagonists, such as warfarin, act by inhibiting the enzyme required for activation of vitamin K. Vitamin K is an essential cofactor for the synthesis of many clotting factors.
Direct oral anticoagulants are a new class of synthetic drugs that includes direct thrombin inhibitors and direct factor 10a inhibitors.
The major adverse effect of all anticoagulants is uncontrolled bleeding. Reversal of anticoagulation: hemodialysis, red blood cells transfusion, platelet transfusion, activated charcoal to prevent absorption of the last dose of an oral anticoagulant. Certain anticoagulants have specific reversal agents that can counteract their effect. Heparin’s action can be reversed with protamine. Vitamin K, given orally or intravenously, can counteract warfarin. There are also specific antidotes for direct oral anticoagulants.

On our first look at anticoagulants part 1, we looked at coumadin, the most commonly used oral anticoagulant. Now we will look at heparin, a commonly used injectable anticoagulant.

Heparin is a pharmaceutical preparation of the naturally occurring anticoagulant produced by white blood cells in various body tissues, obtained from pig intestines or the lungs of cattle. It is an injectable blood thinner used to treat & prevent the formation of clots and the extension of existing clots within the blood. It does not break down clots that have already formed, but it allows the body’s natural clot lysis mechanisms to work normally to break down clots that have formed.

Humira is mostly used to treat inflammation. This can come in handy when someone has arthritis or another example of it's use is with chrone's disease. This is where there is inflammation in the digestive system.

Inflammation is very bad, that is where the immune system attacks itself, it's the main cause of pain with arthritis, it accelerates the aging process and causes health problems. In fact some health experts feel that without inflammation most diseases would not even exist.

The bad news with humira is the side effects. Some people using the drug develop a fast growing type of cancer. It also can lower your ability to fight off infections and weaken the immune system.

The immune system is something you really don't want weak. It also weakens someone's ability to clot blood. This makes it easier to bleed and bruise. The side effects keep adding up, including liver problems, nervous system problems and heart problems. s and I haven't even talked about the minor side effects like allergic reactions or free radicals.

There are much better options.

You could try an anti-inflammatory diet and this includes lots of food with antioxidants and cutting back on processed meals and fast foods.

Reducing stress, mild exercise, reducing toxins, mercury and pesticides all help.

Next would be omega 3. A supplement is a great choice because you don't get the mercury of toxins from fish. Different brands give different results.

You could also try something known as green lipid mussel power.

When it comes to herbal remedies there are tons like ginger, garlic and turmeric.

I was diagnosed with an autoimmune condition called psoriatic arthritis in April 2023 and have been taking methotrexate since May 2023. A month ago I had my first humira/adalimumab injection and have now taken 3 shots on total. The methotrxate has helped a bit but I'm hoping with the new medication I'll go into remission and I wanted to document my experience from the start, giving 2 week updates to help other understand what to expect.

Humira: The Most Financially Successful Drug Ever!

Humira has made $200 Billion over the course of its pharmaceutical life. The original Humira patent expired, but the makers of Humira--AbbVie--filed an additional 132 patents and extended its patent-life to 2023.

During that time, AbbVie raised the price of Humira 30 times from $522 per syringe to $2,984 per syringe. It now costs $78,000 per year to treat one patient with Humira.

Consequently, AbbVie rose to be worth $270 Billion and is the 23rd most valuable company on the stock market... $80 Billion larger than the Walt Disney Company.

Express Scripts, Optum and Prime Therapeutics will offer biologic alternatives to Humira in 2023, but CVS Caremark has yet to announce any alternative to the expensive medication.

Humira shows how completely ineffective PBMs are in controlling specialty pharmacy costs.

Alternative solutions include: 1) Travel to Mexico/Canada to buy specialty drugs for less, 2) Hire own specialists (e.g. rheumatologists) who will be better stewards of the employers resources while still following clinical best-practices, 3) Have lawyers try to get plan members that take specialty medications on Disability so that Medicare will cover their medication costs, 4) Legislative Advocacy to counter the Pharma lobbying that allowed for confiscatory pricing of specialty pharmacy medications.

I was diagnosed with an autoimmune condition called psoriatic arthritis in April 2023 and have been taking methotrexate since May 2023. 8 months ago I had my first humira/adalimumab injection and have now taken around 16 shots in total. The methotrexate helped a bit but with the humira/adalimumab I've made great progress I wanted to document my experience from the start, giving monthly updates to help other understand what to expect. Humira has been a very positive experience for me so far and I go into more detail in this video
