Major Drug Interaction of Levofloxacin with the following Medicines:
Amiodarone
Using amiodarone together with levoFLOXacin is not recommended.
Combining these drugs can increase the risk of an irregular heart rhythm that may be serious. You should seek immediate medical attention if you develop sudden dizziness, lightheadedness, fainting, or fast or pounding heartbeats during treatment with amiodarone or levoFLOXacin.
It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Betamethasone
LevoFLOXacin and other medications in its class can cause tendinitis and tendon rupture, and the risk may be increased when combined with a steroid such as betamethasone.
Older adults over 60 years of age and those who have received a kidney, heart, and/or lung transplant may be particularly susceptible.
Tendon rupture can occur during or up to several months after finishing levoFLOXacin treatment and may require surgery or result in prolonged disability. Talk to your doctor if you have any questions or concerns.
Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications.
Stop taking levoFLOXacin and call your doctor immediately if you experience pain, swelling, or inflammation of a tendon area such as the back of the ankle, shoulder, biceps, hand, or thumb.
You should also avoid exercise or use of the affected area until further instruction from your doctor.
Bupropion
BuPROPion may rarely cause seizures, and combining it with other medications that can also cause seizures such as levoFLOXacin may increase that risk.
You may be more susceptible if you are elderly, undergoing alcohol or drug withdrawal, have a history of seizures, or have a condition affecting the central nervous system such as a brain tumor or head trauma.
Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications.

Cytomel is a T3 only thyroid medication (the brand version of liothyronine) that is far more active than levothyroxine. Because it's so much more active, it also happens to be much better at helping thyroid patients with Hashimoto's and hypothyroidism lose weight.

Before you run out and grab it, though, or try to switch medications, there are a few things you need to know.

When using it you will want to keep an eye on certain lab tests including free t3, total t3, and reverse T3.

In addition, you will also want to keep an eye on your basal body temperature and resting heart rate.

Because of its power, it's important to keep an eye on these metrics to make sure you don't accidentally take too much.

As far as dosing for weight loss is concerned, most patients do best taking a dose somewhere between 5 and 20 mcg per day.

Instead of thinking as Cytomel as an alternative to levothyroxine, I think it's best thought of as a complementary option to it.

Many thyroid patients see the best results when using both of these thyroid medications together.

Changing the time of day that to take your thyroid medication can improve its effectiveness and reduce your thyroid symptoms. If you are like most thyroid patients then you are probably taking your thyroid medication first thing in the morning.

But what if I told you there's nothing magical about this time of day? While that's the standard advice, there are emerging data that suggests that not only are other times of day an option, but they also might be better.

One study followed patients who took their thyroid medication in the morning and then at night and evaluated their thyroid lab tests both times. They found that patients taking their thyroid medication in the evening had higher levels of free thyroid hormones compared to those taking it in the morning.

Does this mean you should take your medication at night? Not necessarily, but it may be worth considering.

The biggest potential downside to taking your medication at night is that it may interfere with your sleep. This isn't actually a problem for MOST people but it can be a problem for some. If taking your thyroid medication in the evening does cause sleeping issues then the benefits of taking it at that time likely do not outweigh the consequences of decreased sleep quality.

But, no problem, you can always go back to taking it in the morning.

This strategy has the potential to work with ALL thyroid medications, including those with T3. But note that T3-containing medications are more likely than T3-containing medications to cause sleep issues (but only slightly).

Why Tirosint beats levothyroxine for treating low thyroid in hypothyroid and low thyroid patients.

Levothyroxine is one of the most commonly prescribed medications to treat low thyroid conditions in the United States and around the world.

Unfortunately, it isn't the best medication for the job!

In fact, if you are reading this then you probably know what I am talking about. Are you taking levothyroxine and still dealing with issues such as brain fog, weight gain, constipation, hair loss, etc.?

If so, you may be one of the many thyroid patients out there who simply don't respond well to levothyroxine.

Does this mean hope is lost? No!

This is where Tirosint steps in.

Tirosint is VERY similar to levothyroxine in that it contains the same ACTIVE ingredients but it differs in its inactive ingredients.

The fewer inactive ingredients found in Tirosint mean it's easier for it to get into your body.

#3. It's easier to take.
Because it's in a gel cap (it's liquid-based) it is not only easier for your gut to absorb it, it can also be taken via the sublingual route.

Taking your thyroid medication sublingually increases absorption and helps you feel better.

In addition, it can also potentially be taken with food which may work better for many people with tight schedules.

Levothyroxine is a medicine used to treat an underactive thyroid gland (hypothyroidism) which affects 1 in 70 women and 1 in 1,000 men in the UK.

The thyroid gland makes thyroid hormone which helps to control energy levels and growth. Levothyroxine is taken to replace the missing thyroid hormone.

KEY FACTS:
• Levothyroxine starts working straight away, but it may be several weeks before your symptoms start to improve.
• The most common side effects of levothyroxine are caused by taking a bigger dose than you need. Your doctor can lower your dose to help reduce any side effects.
• Before you start taking levothyroxine, your healthcare professional will do a blood test to see what dose you need. Once you start taking the medicine you'll have regular blood tests to see how well it’s working.

HOW AND WHEN TO TAKE:
• Unless told otherwise by your prescriber or healthcare professional take levothyroxine once a day in the morning, ideally at least 30-60 minutes before having breakfast or a drink containing caffeine like tea or coffee.
• Both breakfast and caffeinated drinks can stop your body taking in levothyroxine properly so it doesn't work as well.
• If you can't take it first thing in the morning on an empty stomach take levothyroxine at least 2 hours after eating food.
• If you buy any medicines, check with a pharmacist that they are safe to take with levothyroxine. Levothyroxine should not be taken at the same time as indigestion remedies or preparations containing calcium or iron (which are contained in some vitamin products). These types of medicines reduce the amount of levothyroxine absorbed by your body. Leave at least 2-4 hours between taking your dose of levothyroxine and any such preparation.
• Soya interferes with thyroxine absorption, therefore if you are taking thyroxine you should try to avoid soya. If you wish to take soya, there should be as long a time interval as possible between eating the soya and taking the thyroxine.
• Avoid products such as kelp, as they may interfere with thyroid function & wellbeing. Kelp is derived from seaweed and is naturally high in iodine. Because of this it is sometimes marketed as a "thyroid booster" and can be purchased in dry preparations and tablets. As with iodine itself, it is of no health benefit to those with thyroid disease.

BLOOD TESTS:
Your healthcare professional will do regular blood tests to check the levels of thyroid hormones in your body before and after starting levothyroxine. These will allow your doctor to adjust your dose to suit you.

At the start of treatment you can expect to have blood tests often. Once your hormone levels are stable, you’ll usually have a blood test after 4 to 6 months, and after that once a year.

You may need blood tests more often if you:

• Are pregnant
• Start or stop a medicine that can interfere with levothyroxine
• Have any symptoms that could mean your dose is not quite right

MISSED DOSES:
If you forget to take a dose, take it as soon as you remember, unless it's almost time for your next dose. In which case just skip the forgotten dose. Do not take 2 doses together to make up for a missed dose.

If you forget doses often, it may help to set an alarm to remind you. You could also ask your pharmacist for advice on other ways to help you remember to take your medicine.

The side effects of stopping thyroid medication abruptly and why I never recommend this route. If you want to stop taking your thyroid medication then make sure to do it correctly and safely.

Is it safe to stop taking your thyroid medication? The answer is no for several reasons which I will explain here.

But first, we need to talk about why you are taking thyroid medication, to begin with.

Thyroid medication is prescribed to those people who are deficient in thyroid hormone (meaning their body isn't producing enough).

So if you were to stop taking your thyroid medication then your body would revert back to whatever it was capable of producing before you started taking thyroid medication.

This process usually results in certain symptoms including:

- Hair loss
- Weight gain
- Depression
- Increased pain
- Increased fatigue
- And other symptoms of hypothyroidism

But why do you experience these symptoms?

These 4 reasons will help you understand why you will most likely feel worse if you do:

#1. Your body may be reliant upon thyroid medication.
If you've had your thyroid removed or if it's been destroyed by radioactive iodine then you may not be able to produce thyroid hormone on your own. In this case, you will be required to take thyroid hormone.

#2. Your HPT axis may be blunted.
Taking thyroid medication blunts the normal HPT axis in your body and it can take weeks to months for this system to recover. During this period you will most likely feel very poorly.

#3. Your body may not be able to produce thyroid hormone by itself.
Conditions such as Hashimoto's damage the thyroid gland and even though you may be able to produce some thyroid hormone you may not be able to produce enough to sustain yourself.

#4. Thyroid supplements do not replace thyroid medication.
Lastly, you want to make sure that you use supplements to compliment your medication but not to replace it. Supplements can help improve natural conversion and production of thyroid hormone but they aren't as powerful as thyroid medications.

3. Carotenoderma is the yellow-orange discolouration of the skin due
to carotenaemia a condition in which the blood level of β-carotene are high.
4. Sparse and brittle hair, which comes out in handfuls.
5. Loss of hair in the outer third of the eyebrows
6. Slow growing, ridged and brittle nails.
7. Myxoedema can occur in long standing cases of hypothyroidism resulting in puffy face, eyelids, hands and feet.
8. Slow wound healing.

Lidocaine brings about negative inotropic effects and antiarrhythmic actions in the heart which weaken the force of muscular contractions and can calm erratic and uncoordinated electro-myocardial activity. Its primary use is for cardiac arrest from ventricular fibrillation (VF) and pulseless ventricular tachycardia (VT). So, to be concise and to the point, YES it is safe to inject lidocaine into the blood stream.

But, let’s dive deeper into this question as it is one of out most importance. Since I was a medical student it was driven into my head to never inject lidocaine into a vein or artery, I guess it ultimately could cause a severe or even deadly reaction.

So every time I am to inject lidocaine you are supposed to pull back on the plunger and check for any blood, and if you see any you are to redirect that the needle repeat the process and then you are safe to inject the lidocaine. I believe this is why Robin Hood asked the question here, and why it is important to know why this is really more of a modern day medical myth.

But first…what is Lidocaine?

Lidocaine is the most commonly used anesthetic for local infiltration. Local anesthetics reversibly block sodium channels within the nerve fibers, which prevents transmission of pain signals by disrupting depolarization of the nerve. Thus, infiltration of dilute solutions of local anesthetics near the nerve fibers produces relief of pain with less interruption in the sense of touch or temperature and without muscle paralysis in most children and adults.

Lidocaine is usually given as a 1 percent solution (10 mg/mL). If large volumes are needed or a smaller dose is desired, the clinician may use a 0.5 percent solution. Higher concentrations of lidocaine beyond 1 percent do not promote improved onset or duration of analgesia and may increase the risk of toxicity.

The dose of lidocaine should not exceed the following:

•Lidocaine WITHOUT epinephrine – 4 mg/kg (0.4 mL/kg of lidocaine 1 %, maximum total dose: 300 mg [30 mL of lidocaine 1 percent])

•Lidocaine WITH epinephrine – 7 mg/kg (0.7 mL/kg of lidocaine 1 percent, maximum total dose: 500 mg [50 mL of lidocaine 1 percent with epinephrine]).

For an added margin of safety, 80% of the maximum allowable dose should be used in children under eight years of age. In addition, because anesthetics in the amide class are metabolized by the liver and excreted by the kidneys, the total dose of lidocaine should be decreased by approximately 50 percent in elderly patients with severe liver or kidney disease.

The information just discussed is of OUTMOST importance and I recommend you memorize these numbers. 4-30 / 7-50 / 80-8

80% reduction in children under 8

The onset of anesthesia typically occurs within 2 – 5 minutes, and the duration of anesthesia varies from 30 minutes to two hours for plain lidocaine. Adding epinephrine will extend the duration of anesthesia one more hour, it will also decrease local bleeding and reduce systemic absorption of lidocaine (this is why you can inject more lidocaine when it is mixed with epinephrine).

It also is commonly used as an antiarrhythmic agent to depress ventricular arrhythmias.
In addition to direct nerve toxicity, systemic toxicity affecting the brain and/or cardiac muscle can lead to sudden and dramatic changes in the patient’s vital signs.

Finally, there are the side effects of a relative overdose at the site of injection, which can be quite dramatic. Examples include total spinal anesthesia or subdural injection of the drug that can cause severe hemodynamic compromise such as hypotension or bradycardia up to a cardiac and respiratory arrest.

Inadvertent intra-arterial injections may cause local anesthetic toxicity in the tissue beds supplied by that artery even well under the systemic toxic concentration. This complication is seen mostly with injections into the neck, causing central nervous system (CNS) symptoms often during the injection or shortly after that without progressing to the feared cardiac toxicity.

SO… So the routine aspiration while doing local infiltration of areas of the body away from major vessels is just not necessary. This recommendation does change when in close proximity to a large vessel like the groin or in the neck area.


Having said all this, lets consider hematoma blocks. A common procedure of anesthesia performed in the reduction of broken and displaced bones in the Emergency Department. In this technique a needle is inserted into the tissue and aimed towards the site of most swelling, presumably the site of hematoma (a large blood collection in the tissue). Once blood is aspirated then 8-10 mL are injected for anesthesia at that site. Now…based on what we have been discussing this should seem like a crazy thing to do! How do you know you are in the hematoma and not directly into a vein?

You don’t…

Remember that lidocaine is an antiarrhythmic medication, and when treating cardiac issues this medication is injected directly into the intravenous route. This of course is to be done under close cardiac monitoring.

Furthermore, Lidocaine is currently being use in the IV form in the treatment and management of acute renal colic pain.
