Bevacizumab

Bevacizumab is a monoclonal antibody used to treat certain types of advanced cancers.

It gained attention due to increased usage in severe COVID-19.
Indications:
• Bevacizumab is indicated for treating colorectal cancer that spreads to other areas along with intravenous fluorouracil- based chemotherapy.
• Certain types of liver cancer
• Certain types of renal(kidney) cell cancer, cervical, fallopian tube, ovarian cancers, Glioblastoma of the brain.
• Severe COVID-19

Trade names:
Bevacizumab is popularly known as:
• Avastin
• Zybev
• Bryxta
• Abevmy
• Bevarest
• Cizumab etc.

How does Bevacizumab work?
Bevacizumab inhibits a protein named Vascular endothelial growth factor (VEGF). VEGF helps for promoting blood vessel growth, providing nutrition for cancer cells. By inhibiting VEGF, bevacizumab inhibits the growth of cancer cells.

Classification: Vascular endothelial growth factor (VEGF)

Dosage forms: Bevacizumab is available in the following forms:
• Injection
Bevacizumab injection is available in the following strengths:
• 100 mg/4 ml
• 400 mg/16 ml

How to take Bevacizumab?

This medication available in the form of injection, administered by health care provider, intravenously.

• It is taken in the dose of 5-10 mg/kg body weight every 2 weeks with other chemotherapeutic agents for colorectal cancer
• For lung cancer: 15 mg/kg every 3 weeks with carboplatin and paclitaxel
• For Renal Cancer - 10 mg/kg every 2 weeks
• The recommended dosage is 10 mg/kg intravenously every 2 weeks for glioblastoma.
• 15 mg/kg every 3 weeks with paclitaxel and cisplatin, or paclitaxel and topotecan for Stage III or IV ovarian, fallopian tube or peritoneal
cancer after surgical resection.

What are the common SIDE EFFECTS?

Bevacizumab is generally safe.
In case of severe or disturbing side effects consult the doctor immediately. The most common side effects are epistaxis or nose bleeding, headache, hypertension, rhinitis, proteinuria, taste alteration, dry skin, hemorrhage, lacrimation disorder, back pain and exfoliative dermatitis etc.

Other side effects include:

• Gastrointestinal- Constipation, diarrhea, increased thirst, indigestion, loss of appetite, mouth pain or sores, nausea, vomiting, black, tarry stools and dry mouth.
• Central Nervous System- Dizziness, tiredness
• Skin- Hair loss, swelling, or redness at the injection site, rash, hives, itching and swelling of the mouth, face, lips, or tongue.
• Eye and ENT- Voice changes.
• Heart- Difficulty breathing, tightness in the chest, congestive heart failure, calf pain or tenderness and chest pain.
• Miscellaneous- Muscle pain, sluggishness, weakness and weight loss.

PRECAUTIONS need to be taken.
Caution is required in patients with:

• Gastrointestinal perforations and Fistula
• Hemorrhage
• Blood clots
• Posterior Reversible Encephalopathy Syndrome (PRES)
• Renal Injury and Proteinuria
• Pregnant and lactating women
• Ovarian Failure
• Heart Failure

Special instructions:
• Discontinue bevacizumab in patients who develop wound healing complications that require medical intervention or necrotizing fasciitis.
• Do not administer or mix bevacizumab with dextrose solutions.

When should you NOT take Bevacizumab?
Bevacizumab should not be used in patients with-
• Allergy to Bevacizumab or any of the ingredients

Can Bevacizumab INTERACT with other drugs?
Bevacizumab can interact with the following drugs in various extents:
• Paclitaxel
• Carboplatin

Is palivizumab an antibody?
Palivizumab, a highly potent RSV-neutralizing monoclonal antibody (Mab), has been licensed in 1998 for prophylactic use to prevent RSV associated hospitalizations in high-risk infants.

How much does palivizumab cost?
Results: The mean cost of palivizumab per dose ranged from $1661 for infants younger than 6 months of age to $2584 for children in their second year of life.

Is palivizumab effective?
Palivizumab is an RSV-specific monoclonal antibody licensed for the prevention of serious lower respiratory tract infection (LRTI) caused by RSV in high-risk children. 1:01 Why is Casodex added to everything?

Originally Casodex was added to Lupron (or another hormonal agent) to counteract the testosterone flare that occurs in the first month of hormonal treatment. Additionally, studies showed a slight survival advantage—without significant additional side effects—in men who continued to use the Casodex and the Lupron together for the entire duration of the treatment. However, in the past, Casodex's high cost made it impractical to use for longer than the first month to suppress the Lupron flare.

3:01 Is Casodex Chemotherapy?

While Casodex is often designated at a chemotherapy for billing reasons, it is in a totally different class of medications than chemotherapies like Taxotere and Jevtana.

The side effects of Casodex are similar to Lupron, except usually milder; when added to Lupron, some men will experience increased fatigue. When used alone, Casodex can cause breast tenderness and enlargement. This can be counteracted by radiation to the nipples prior to treatment, the use of an aromatase inhibitor, or both.

Although increasingly rare as technology and practices improve, radiation proctitis can be disastrous if it occurs, and so the use of SpaceOAR is a wise choice.

You can find doctors that use SpaceOAR on the SpaceOAR website, or you can ask your radiation oncologist if they are familiar with the technology. If they are not, then that may be a good indication that this physician is not up to speed with the latest developments in the field.

Biopsy reports are given for 12-core random biopsies. Each core ought to be labeled with the location within the prostate from which it was taken. This is not universal, unfortunately, but it is good practice. Physicians are looking for the number of cores that contain cancer cells. If a core has cancer cells, then the biopsy report will say what percentage of the core involved cancer cells. The Gleason score is the final significant component of the biopsy report. Essentially, your doctor wants to know "How big is the tumor?" and "What grade is the tumor?"

15:03 How accurate are biopsy reports? Should I get a second opinion?

There is oftentimes disagreement among pathologists as to the exact grade of a given sample. It's a good idea, then, to get a second opinion on a pathology report. There are many good options. Jonathan Epstein of John Hopkins, for example, has a good reputation and a convenient system for sending in reports for a second opinion.

Gleason score, among other factors, determines the best treatment (or non-treatment) for a man to balance achieving a cure (if necessary) and avoiding the side effects of overtreatment.

Bi-calutamide and feminizing hormone therapy

Bicalutamide is an anti-androgen drug which means that it blocks the effect of androgenic hormones such as testosterone and DHT. The important thing to understand is that bicalutamide does not lower testosterone levels, it simply blocks the effect of testosterone on tissues.

In many cases, Testosterone levels initially increase after starting this medication however, that is nothing to be concerned about. Testosterone levels start declining over time, this change happens much more rapidly if you're also taking estrogen and progesterone in addition to bicalutamide.

Bicalutamide is most commonly used for treating men with prostate cancer

Off-label uses:
Puberty blocker
Feminization therapy
Androgenic Alopecia in females
Acne Reduction

Today primarily will be focusing on bicalutamide use in transgender females
Other anti-androgen medications often prescribed are spironolactone and cyproterone acetate. However, these drugs are much weaker than bicalutamide. Cyproterone is also not available in the US.

Bicalutamide also helps the aromatization of testosterone to estrogen which is why it is especially helpful for breast development when compared to other anti-androgen medications. Some small studies have shown better breast growth when compared to Aldactone and cyproterone. A possible mechanism is that Aldactone causes early maturation of the breast bud when compared with bicalutamide.

Bicalutamide is known to cause a reduction in vitamin C levels and glutathione levels in animal studies. Taking glutathione and vitamin C may reduce the risk of liver toxicity however I have no data to support this strategy.

Half-life is 6 days so the drug stays in the body for several days so only twice a weekly dose is adequate. Compare that to spironolactone which has a half-life of only 2 hours
Do not use or use caution if you

If you drink heavily
Over the age of 35
Get a liver panel done 2-4 weeks after starting the medication. For any sign of liver failure such as jaundice, the medication must be immediately stopped.
Dosing:
Bicalutamide stays in the body for several days and has a half-life of 6 days which means half of the drug is broken down and removed by the body in 6 days. Twice a weekly dose of 50 mg is adequate. Higher doses increase the risk of liver toxicity and should be avoided
In summary, this is a very good medication for feminization therapy especially if high breast growth is strongly desired. Liver toxicity is a major concern and can be life-threatening so do not consume alcohol, and avoid hepatotoxic medications such as Tylenol.

Pepto-Bismol can be used to relieve diarrhea and upset stomach in adults and children 12 years and older. In children, 2 years to 12 years old, an alternative product can be used to help with heartburn and stomach upset, called Children’s Pepto Antacid. Pepto-Bismol is effective at relieving common symptoms of upset stomach, such as heartburn, nausea, gas, or indigestion.

You should talk to your doctor or pharmacist before taking this medication if you are pregnant or if you drink alcohol. Pepto Bismol is available as a liquid, caplet, and chewable tablet form. Take as recommended by the product packaging, unless your doctor or pharmacist said otherwise.

Typical Dosing:
Oral: ~524 mg every 30 to 60 minutes or 1,050 mg every 60 minutes as needed for up to 2 days (maximum: ~4,200 mg [8 doses (regular strength); 4 doses (maximum strength)]/24 hours)

IMPORTANT SAFETY INFORMATION
BREO contains vilanterol. LABA medicines such as vilanterol when used alone increase the risk of hospitalizations and death from asthma problems. BREO contains an ICS and a LABA. When an ICS and LABA are used together, there is not a significant increased risk in hospitalizations and death from asthma problems.
Do not use BREO to relieve sudden breathing problems. Always have a rescue inhaler with you to treat sudden symptoms.
Do not use BREO if you have a severe allergy to milk proteins or are allergic to any of the ingredients in BREO. Ask your healthcare provider if you are not sure.
Do not use BREO more often than prescribed.
Do not take BREO with other medicines that contain a LABA for any reason. Tell your healthcare provider about all the medicines you take and about all of your health conditions.
Call your healthcare provider or get medical care right away if your breathing problems get worse, if you need your rescue inhaler more often than usual or it does not work as well to relieve your symptoms.
BREO can cause serious side effects, including:
fungal infection in your mouth or throat (thrush). Rinse your mouth with water without swallowing after using BREO to help reduce your chance of getting thrush.
pneumonia. People with COPD have a higher chance of getting pneumonia. BREO may increase the chance of getting pneumonia. Call your healthcare provider if you notice any of the following symptoms: increase in mucus (sputum) production; change in mucus color; fever; chills; increased cough; increased breathing problems.
weakened immune system and increased chance of getting infections (immunosuppression). You should avoid exposure to chickenpox and measles, and, if exposed, consult your healthcare provider without delay. Worsening of existing tuberculosis, fungal, bacterial, viral, or parasitic infections, or herpes infection of the eye may occur.
reduced adrenal function (adrenal insufficiency). This can happen when you stop taking an oral corticosteroid (such as prednisone) and start taking a medicine containing an inhaled corticosteroid (such as BREO). During this transition period, when your body is under stress such as from fever, trauma (such as a car accident), infection, surgery, or worse COPD symptoms, adrenal insufficiency can get worse and may cause death. Symptoms include: feeling tired; lack of energy; weakness; nausea and vomiting; low blood pressure (hypotension).
sudden breathing problems immediately after inhaling your medicine. If you have sudden breathing problems immediately after inhaling your medicine, stop taking BREO and call your healthcare provider right away.
serious allergic reactions. Call your healthcare provider or get emergency medical care if you get any of the following symptoms of a serious allergic reaction: rash; hives; swelling of your face, mouth, and tongue; breathing problems.
effects on heart: increased blood pressure; a fast or irregular heartbeat, awareness of heartbeat; chest pain.
effects on nervous system: tremor; nervousness.
bone thinning or weakness (osteoporosis).
eye problems including glaucoma and cataracts. You should have regular eye exams while using BREO.
changes in laboratory blood values, high blood sugar (hyperglycemia) and low potassium (hypokalemia).

By stopping these chemicals being released, fluticasone reduces the inflammation in lungs that tightens the airways and makes it difficult for air to get in and out.

Vilanterol is a type of medicine called a long-acting beta 2 agonist. It works by stimulating receptors in the lungs called beta 2 receptors, which causes the muscles in the airways to relax and the airways to open up.

Breztri Aerosphere is a prescription inhaler used to treat chronic obstructive pulmonary disease (COPD) , a chronic lung disease that includes chronic bronchitis, emphysema, or both. It contains three active ingredients:
Budesonide – A corticosteroid that reduces inflammation in the lungs.
Glycopyrrolate – A long-acting muscarinic antagonist (LAMA) that helps relax airway muscles.
Formoterol fumarate – A long-acting beta agonist (LABA) that helps open the airways.
Breztri is used for the long-term maintenance treatment of chronic obstructive pulmonary disease (COPD), including:
Chronic bronchitis – Persistent inflammation of the airways.
Emphysema – Damage to the air sacs in the lungs.
It reduces inflammation in the airways (Budesonide).
Relaxes airway muscles to improve airflow (Glycopyrrolate).
Opens airways for easier breathing (Formoterol).
Mechanism of action
Each of the 3 ingredients in Breztri has a different mechanism of action. Budesonide helps to decrease inflammation in the lungs which can lead to breathing problems. Glycopyrrolate blocks the effects of acetylcholine (a neurotransmitter) in the lungs, which allows the airways to relax and widen, making breathing easier. Formoterol binds to and activates beta2-adrenergic receptors on the smooth muscle cells in the airways. This activation also allows the airways to relax and widen, making breathing easier. Formoterol has a long-lasting effect, typically lasting 12 hours or more, which provides sustained symptom relief throughout the day for COPD patients.

Breztri Aerosphere Dosage

Standard Dosage for COPD

Recommended dose: Two inhalations (puffs) twice daily

Time of use: Morning and evening (approximately 12 hours apart)

Inhaler strength: Each puff contains:

Budesonide 160 mcg.

Glycopyrrolate 9 mcg.

Formoterol fumarate 4.8 mcg.

Like all medications, Breztri Aerosphere can cause side effects. These can be classified as common, serious, and rare.

Common Side Effects

What is aclidinium? and What is aclidinium used for?

Aclidinium is used as a long term treatment to prevent wheezing, shortness of breath, coughing, and chest tightness in patients with chronic obstructive pulmonary disease (COPD, a group of diseases that affect the lungs and airways) such as chronic bronchitis (swelling of the air passages that lead to the lungs) and emphysema (damage to air sacs in the lungs).
Aclidinium is in a class of medications called bronchodilators.
It works by relaxing and opening the air passages to the lungs to make breathing easier.


How to use aclidinium?

Aclidinium comes as a dry powder in an inhalation device to inhale by mouth. It is usually inhaled twice a day, once every 12 hours. Inhale aclidinium at around the same times every day.
Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand.
Inhale aclidinium exactly as directed.
Do not inhale more or less of it or inhale it more often than prescribed by your doctor.

Do not use aclidinium to treat a sudden attack of wheezing or shortness of breath.
Your doctor will prescribe a rescue medication to treat sudden attacks of symptoms.
Keep this rescue medication with you at all times in case you have sudden difficulty breathing.

Your condition may worsen over time during your treatment with aclidinium. Do not take extra doses of aclidinium if this happens.
Call your doctor or get emergency medical help if your breathing problems worsen, you need to use your rescue medication to treat sudden attacks more often, or your rescue medication does not relieve your symptoms as well as it did in the past.

Aclidinium can help control your symptoms but does not cure COPD.
You may notice some improvement in your symptoms the first day that you use aclidinium, but it may take longer for you to feel the full benefit of the medication.
Continue to use aclidinium even if you feel well. Do not stop using aclidinium without talking to your doctor.

Before you use your aclidinium inhalation device for the first time, read the manufacturer's directions for use carefully.
Ask your doctor or pharmacist to show you how to use the device and practice using it while he or she watches.

Be careful not to get aclidinium powder in your eyes. If you get the powder in your eyes, you may experience blurred vision and sensitivity to light.

The aclidinium inhalation device does not need to be cleaned. If you want to clean the device, you may wipe the outside of the mouthpiece with a dry tissue or paper towel.
Never use water to clean the device because you might damage the medication.


When to use aclidinium?

Before using aclidinium:

tell your doctor and pharmacist if you are allergic to aclidinium, atropine (Atropen, in Lomotil, in Lonox, in Motofen), any other medications, any of the ingredients in aclidinium inhalation powder, or milk proteins.

tell your doctor and pharmacist what prescription and nonprescription medications, vitamins, nutritional supplements, and herbal products you are taking or plan to take.
Be sure to mention any of the following: antihistamines; atropine (Atropen, in Lomotil, in Lonox, in Motofen); glycopyrrolate (Lonhala Magnair, Seebri, in Bevespi Aerosphere, in Utibron); ipratropium (Atrovent); medications for irritable bowel disease, motion sickness, Parkinson's disease, ulcers, and urinary problems; tiotropium (Spiriva); and umeclidinium (Incruse Ellipta, in Anoro Ellipta, in Trelegy Ellipta).
Your doctor may need to change the doses of your medications or monitor you carefully for side effects.

tell your doctor if you have or have ever had glaucoma (increased pressure in the eye that may cause vision loss), benign prostatic hypertrophy (BPH; enlargement of a male reproductive gland), a bladder condition, or any other condition that makes it difficult for you to empty your bladder completely.

tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking aclidinium, call your doctor.


What should I do if I forget a dose?

Skip the missed dose and continue your regular dosing schedule.
Do not inhale a double dose to make up for a missed one.


What side effects can aclidinium cause?

Aclidinium may cause side effects.
Tell your doctor if any of these symptoms are severe or do not go away:

headache.
runny nose and other cold symptoms.

Briumvi vs Ocrevus: How do they compare? - Key Points..Briumvi (ublituximab) and Ocrevus (ocrelizumab) are two injectable medications that may be used to treat multiple sclerosis in adults..Briumvi and Ocrevus are both B-cell therapies and work the same way..Head-to-head trials directly comparing the Briumvi and Ocrevus have not been conducted, making it difficult to definitively determine which is more effective..Both Briumvi and Ocrevus carry a risk of infusion reactions and infections...Briumvi (ublituximab) and Ocrevus (ocrelizumab) are prescription intravenous (IV) infusions given to treat relapsing forms of multiple sclerosis (MS). Ocrevus is also used to treat primary progressive MS..While there have not been head-to-head comparison studies in patients with MS, both medications work well and have similar side effects. Both medications attach to and decrease B cells expressing CD20 on their surface. Both medications are given as an IV infusion. The dose is different depending on which medication you are taking..Briumvi is first given as a 150 mg IV infusion, followed by 450 mg 2 weeks later. Then, 450 mg is infused once every 6 months..Ocrevus is first given as a 300 mg IV infusion, followed by another 300 mg infusion 2 weeks later. Then, 600 mg is infused once every 6 months..One notable difference in dosing between the two medications is that Briumvi has shorter infusion times compared to Ocrevus. For the first infusion, it takes about 4 hours to infuse Briumvi. The remaining doses are infused over about 1 hour. This is compared to around 2.5 hours for the first two infusions of Ocrevus, and 3.5 hours or longer for any additional infusions..Briumvi and Ocrevus have similar side effects.The most common side effects of both Briumvi and Ocrevus include infections and infusion reactions. In clinical studies of Briumvi, 48% of patients taking it experienced infusion reactions. Additionally, 45% of patients taking Briumvi reported an upper respiratory tract infection. In comparison, 40% of patients taking Ocrevus for relapsing forms of MS in clinical studies reported an upper respiratory tract infection, and 34% had an infusion reaction..Other side effects reported by patients taking Briumvi or Ocrevus were lower respiratory tract infections and pain in arms or legs. These are not all side effects possible with these medications.
