Lyrica for Restless Legs Syndrome
Lyrica for Restless Legs Syndrome Side Effects 0 Views • 5 months ago

Lyrica® (pregabalin) is one of three medications in the class of drugs known as alpha-2-delta ligands along with its cousins gabapentin (Neurontin®) and gabapentin enarcarbil (Horizant®) that are the consensus first-line drugs for the treatment of restless legs syndrome (RLS). Andy Berkowski, MD of ReLACS Health analyzes the use of this medication in this video.

To read in more depth about RLS and other sleep disorders, follow A ReLACSing Blog:
https://www.relacshealth.com/blog

For more information on gabapentin and Horizant, this video explains the differences:
https://youtu.be/PPFw-W9unvw

Lyrica works by reducing the transmission of the abnormal signal from one brain cell to another that is telling the brain that one has to move the legs. It affects a calcium channel in these nerve cells and essentially quiets down that loud voice creating the urge to move and the discomfort caused by this disease. Originally and still occasionally used a drug to prevent seizures by this same mechanism, Lyrica has become popular for a variety of nerve or nerve pain conditions including Shingles, neuropathic pain in the feet, and fibromyalgia. It turns out to work fairly well for RLS in the majority of cases. For RLS, it can be dosed once or twice in the evening and/or at night when typical RLS symptoms take place. The main side effects can be dizziness and drowsiness, which people may adapt to after a few days. Rarely people can gain a few pounds on Lyrica, possible due to increase in body fluid rather than eating more or more fat accumulation, but this is unclear.

There is a Black Box warning about suicide risk with this medication in those with depression, and though this is very rare, it should be noted. Lyrica can slow down breathing in some circumstances when combined with other drugs that also affect breathing like opioids (narcotics, pain medications), hypnotics (benzodiazepines, prescription sleeping pills), and muscle relaxants. This is particularly important to know for those with untreated obstructive sleep apnea (OSA), significant obesity, COPD or lung disease, or anyone that may have reasons for difficulty breathing at night. Lyrica can be quite effective for RLS, but it is not always tolerated or effective. It is often less effective in those exposed to dopamine agonists (ropinirole/Requip®, pramipexole/Mirapex®, rotigotine/Neupro®) or those with the inevitable augmentation from these medications.

For more information on what is augmentation, see here:
https://youtu.be/gO0uQfILGQk

Arguably the most impression clinical trial for RLS in the New England Journal of Medicine was done on Lyrica a few years back so there is strong evidence in research and extensive experience among doctors treating RLS.
https://www.nejm.org/doi/full/....10.1056/nejmoa130364

Generally, clinicians start patients on either gabapentin or Lyrica, and if one does not work they try the other as there could be fewer side effects or better effects on RLS of one versus the other even if they are cousins from the same drug class. Lyrica seems to be better absorbed on an empty stomach than gabapentin or Horizant which are best taken with a meal.

These videos are for general medical information only, but for those who live in or near Michigan, Ohio, or Florida, you can hire @andyberkowskimd of ReLACS Health as your personal sleep and RLS specialist to determine whether pregabalin is a treatment for you. He has successfully treated several hundred individuals with the most challenging cases of RLS including those with augmentation from dopamine agonists and offers an RLS Augmentation Management Program for those who need more personalization, time, and expertise. Methylphenidate - Uses, Mechanism Of Action, Pharmacology, Adverse Effects & Contraindications
Methylphenidate - Uses, Mechanism Of Action, Pharmacology, Adverse Effects & Contraindications Side Effects 0 Views • 5 months ago

Methylphenidate is a medication used mainly in the treatment of attention deficit hyperactivity disorder in children and adults.
In addition, it is used to treat narcolepsy in adults as a second-line option.
Off-label uses of methylphenidate include resistant depression in the elderly population, treatment for fatigue in patients with cancer, apathy in Alzheimer's disease, and enhancing cognitive performance, including memory.
Neurotransmitters are chemical molecules that allow nerve signal conduction through a synapse.
A synapse consists of pre-synaptic and post-synaptic terminals and the synaptic cleft.
When the nerve impulse reaches the pre-synaptic terminal, it releases neurotransmitters into the synaptic cleft, and they bind to the post synaptic membrane to transmit the nerve impulse.
Once the nerve impulse is transmitted thought the synapse, released neurotransmitters are taken up by the pre-synaptic terminal.
This is called the re-uptake of neurotransmitters.
Methylphenidate blocks the re-uptake of two neurotransmitters, norepinephrine, and dopamine, in presynaptic neurons.
This will increase the concentrations of dopamine and norepinephrine in the synaptic cleft, which creates a stimulant effect within the central nervous system, especially in the pre-frontal cortex.
Methylphenidate also slightly increases the activity of 5HT1A receptor, which also increases the levels of dopamine.
Since it increases the levels of dopamine in the brain, methylphenidate can provide neuroprotection in conditions like Parkinson disease.
Methylphenidate is taken per orally and it is readily absorbed into the blood.
A transdermal patch is also available, but it is not used frequently.
Oral tablet is available in immediate release, extended release, and sustained release forms.
Once in blood, methylphenidate is transported by plasma proteins.
It is metabolized in the liver by a process called de-esterification to ritalinic acid.
More than 75% of the drug is eliminated by the kidneys.
Insomnia and agitation are the commonest adverse effects of methylphenidate.
Central nervous system side effects such as dizziness and headache may also occur.
Gastro intestinal side effects include nausea and vomiting, diarrhea, abdominal pain, and dry mouth.
Increased heart rate and palpitations can also occur with methylphenidate.
Some patients experience excessive sweating and ulceration of the digits.
Less frequently, patients may develop blurred vision and decreased libido.
Many of these adverse effects can be minimized by dose adjustments and by avoiding an afternoon or evening dose, as advised by a healthcare professional.
Methylphenidate inhibits the metabolism of warfarin, phenytoin, tricyclic antidepressants, and selective serotonin re uptake inhibitors and increases their concentrations.
Therefore, caution should be taken when taking these medications and methylphenidate concurrently.
Methylphenidate should not be given to any patient who has hypersensitivity to it.
Other contraindications include the following.
Glaucoma.
Severe hypertension.
Motor tics.
Personal or family history of Tourette syndrome.
Age less than 6 years.
Structural cardiac abnormalities, cardiomyopathy, and arrhythmias.
In addition, patients should not be given methylphenidate if they are taking monoamine oxidase inhibitors.
There should be a minimum of at least 14 days after discontinuation of monoamine oxidase inhibitors before methylphenidate is given due to the risk of hypertensive crisis.

Moxifloxacin Eye Drops: Uses, Dosage, Mechanism, Side Effects, and Important Advice | MediInsights
Moxifloxacin Eye Drops: Uses, Dosage, Mechanism, Side Effects, and Important Advice | MediInsights Side Effects 0 Views • 5 months ago

Welcome to MediInsights, where we unlock the wonders of wellness! In today's video, we delve into the world of Moxifloxacin Eye Drops. Join us as we explore everything you need to know about this powerful medication, including its uses, dosage guidelines, mechanism of action, potential side effects, and essential advice for safe usage.

What are Moxifloxacin Eye Drops?
Moxifloxacin Eye Drops are a broad-spectrum antibiotic used to treat bacterial infections of the eye, such as bacterial conjunctivitis. This medication is highly effective due to its ability to target a wide range of bacteria, making it a go-to choice for eye infections.

Uses of Moxifloxacin Eye Drops:
These eye drops are primarily prescribed for bacterial eye infections. By inhibiting bacterial growth, Moxifloxacin helps alleviate symptoms like redness, irritation, and discharge, providing relief and promoting healing.

Dosage of Moxifloxacin Eye Drops:
Typically, the dosage is one drop in the affected eye(s) three times a day for seven days. It's crucial to follow your healthcare provider's instructions for the best results and to ensure the infection is completely eradicated.

Mechanism of Action:
Moxifloxacin works by blocking bacterial topoisomerase II (DNA gyrase) and topoisomerase IV, enzymes necessary for bacterial DNA processes. This disruption halts bacterial replication and repair, leading to the death of the bacteria and resolution of the infection.

Side Effects:
While Moxifloxacin is generally well-tolerated, some common side effects include:

Eye irritation
Dryness
Redness
Tearing
Rare but serious side effects can include severe allergic reactions, eye pain, vision changes, and severe eye irritation. Seek medical attention if you experience any of these severe symptoms.

Important Advice for Using Moxifloxacin Eye Drops:
Complete the Course: Ensure you finish the full course of the medication, even if symptoms improve, to prevent antibiotic resistance.
Avoid Contamination: Do not touch the dropper tip to any surface, including your eye, to maintain sterility.

Oseltamivir is also known by the brand name Tamiflu

Tamiflu comes in capsule and oral suspension dosage forms

Tamiflu is taken for treatment or prevention of influenza infection. For treatment, Tamiflu is taken twice daily, but for prevention it is taken once daily. Tamiflu works best if taken within two days of the start of symptoms, or exposure to the flu. How often and how long you take Tamiflu can be different depending on what you are taking it for, so make sure you take it as instructed. It is important to finish this medication, even if symptoms disappear. Tamiflu can be taken with or without food, but if stomach upset occurs, take it with food. Tamiflu works best if you take it on schedule. Tamiflu cannot be used to treat a cold, and be aware that you can still spread the flu while taking Tamiflu.

If you miss a dose:
• Take the missed dose as soon as you think about it.
• If it is close to your next dose, skip the missed dose and go back to your normal dosing schedule.
• Do not take two doses at the same time, or extra doses.
Some common side effects of Tamiflu are headache, upset stomach, diarrhea, and vomiting
Some less common side effects of Tamiflu are pain and nausea.
Some rare side effects of Tamiflu include abnormal thinking or behavior, swelling, anxiety,
Feeling cold, trouble thinking, nightmares, seizures, and skin rash.

Do not take Tamiflu if:
• You are taking Probenecid,
• Or within two weeks of getting a flu shot.

While taking Tamiflu, remember, it is important to tell your doctor or pharmacist if you have:
• signs of an allergic reaction, such as rash, hives, itching, tightness in the chest, trouble breathing, swallowing, or talking, swelling of the face, tongue, or throat
• change in thinking
• changes in behavior
• problems speaking
• Shakiness
• Seizures
• hallucinations
• If you have signs of Stevens-Johnson syndrome, such as red, blistered, swollen or peeling skin, with or without fever, red or irritated eyes,
• sores in the mouth, throat, nose, or eyes.
• If your condition does not improve, or you have new or worsening symptoms
• Or if you start taking new medications, vitamins, or supplements.

Tamiflu liquid should be stored in the refrigerator. Shake well before giving a dose, and measure doses carefully using a measured spoon, cup, or oral syringe.

Oxycontin is a powerful opioid pain medication and is the brand name for a timed-release formula of oxycodone.

According to results from the 2010 National Survey on Drug Use and Health (NSDUH), an estimated 2.4 million Americans used prescription drugs nonmedically for the first time within the past year, which averages to approximately 6,600 initiates per day. More than one-half were females and about a third were aged 12 to 17. Although prescription drug abuse affects many Americans, certain populations, such as youth, older adults, and women, may be at particular risk.

Antidepressants for anxiety?
Antidepressants for anxiety? Side Effects 0 Views • 5 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses the use of antidepressants for anxiety disorders. The term "anti-depressants" is really a misnomer, and may cause individuals to believe that their only option for treating anxiety disorders with medication, are to use medications with abuse potential (which is NOT TRUE). SSRI's and SNRI's can be some of the most-effective forms of treatment for anxiety disorders, for certain individuals.

Dr. Pagnani is the founder and medical director of Rittenhouse Psychiatric Associates, an Instructor at The Johns Hopkins Hospital for the Department of Psychiatry, he supervises psychiatric residents at Jefferson Hospital, he serves on the Young Professional Board of Uplift, the Center for Grieving Children and he maintains a private practice of Psychiatry in Philadelphia where he sees patients for one-time psychiatric evaluations and second opinions. He is a licensed physician and surgeon in NJ, PA, NY, MD, DE, FL and CA and can provide consultations to individuals via HIPAA compliant Telehealth visits living in these locations.

Rittenhouse Psychiatric Associates is a private practice of psychiatry consisting of about 25 psychiatrists, psychiatric nurse practitioners and psychotherapists with physical locations in Philadelphia and Paoli, PA and serving all of PA, NJ, NY and FL virtually via HIPAA compliant tele-psychiatry. Individuals seeking not only evaluations but long-term psychiatric care, can contact Rittenhouse Psychiatric Associates directly.

These videos aim to increase awareness and discussion of mental health topics and to encourage struggling individuals to schedule an appointment directly with a mental health provider. Mental health needs to be normalized, and simply speaking about mental health topics openly, is an important first step.

Dr. Pagnani and Rittenhouse Psychiatric Associates are NOT advocating for or against the use of any mentioned psychiatric medications for any specific individual. Additionally, any side effects mentioned are not a complete list of potential side effects or contraindications to taking any mentioned medications, and all risks and benefits of a medication should be discussed with an individual’s provider at an appointment, before starting, stopping or making changes to a medication regimen.
