There is a new sleep medication approved and licensed for the treatment of chronic insomnia. This insomnia drug is called Daridorexant or Quviviq
Daridorexant is an orexin receptor antagonist and it blocks the orexin type 1 and type 2 receptors which is what causes you to keep awake. so it helps you to fall asleep faster and stay asleep for longer.
Daridorexant dose is 25mg or 50mg taken in the evening and 30 minutes before bedtime. It is advised not to take with a large meal as this can reduce the effectiveness of the sleep pill.
Side effects of Daridorexant include sleep paralysis (not able to move or speak for several minutes), hallucinations, worsening of depression and suicidal thoughts, headache, dizziness, fatigue and feeling drowsy during the day.
Daridorexant (Quiviviq) has a better safety profile than other sleeping pills such as Mirtazapine, Seroquel, Trazodone, Amitriptyline, diazepam and zopiclone, with no addictive, tolerance or withdrawal symptoms seen after stopping the drug.

There are various sleep medications that help to treat insomnia but the questions is which is the best sleeping pill or the best sleep medication for insomnia?
Some sleeping tablets such as diazepam or zopiclone are only available on prescription while some such as Nytol or kalms are over the counter.
Some sleeping pills include:
Diazepam
Zopiclone, Zolpidem
Promethazine (Phenergan), Hydroxyzine (Atarax)
Amitriptyline
Mirtazapine
Nytol, Kalms
I have left out melatonin as it is only licensed in the over 50s in the uk. However, that will be covered in another video.
If you have sleeping difficulties, rather than rely on sleeping tablets, you should speak to your Dr as there are other treatment options such as lifestyle changes, sleep hygiene, sleep restriction therapy, CBT and others.
There might also be underlying problems causing sleep disorder that need to be tackled such as mental health problems like depression and anxiety, stress, or medical conditions like ADHD, sleep apnoea, arthritis, dementia and even menopause in women as well as aide effects of medications

Zopiclone and Eszopiclone are drugs used to combat insomnia. Zopiclone has existed for a few decades and Eszopiclone (often called Lunesta) entered the market in 2005. They are sedating substances and can cause hallucinations in some settings.

Zolpidem is a widely used hypnotic that operates at the GABAa receptor. Compared to benzodiazepines, it produces less anxiolysis, muscle relaxation, and anticonvulsant action.
At medical doses, it produces little beyond sedation and the negative effects are mild. Due to personal variation, effects like euphoria, CEVs/OEVs, and anxiolysis are rarely noted at only 10 mg. Those effects appear more often (though still not reliably) at common to strong doses.
It is effective at improving sleep onset, though the instant-release version isn't very effective at maintaining sleep throughout the night.

What is Zolpidem?
Zolpidem is a medication used to start and maintain sleep. It is a doctor's prescription medication only.
Zolpidem comes in different forms such as immediate release, extended release, sublingual tablet where you place it under the tongue and spray
.
How to use it?
Depending on your age and condition, your doctor will prescribe you the most suitable dosage and form. It is usually taken just before bed. Follow the doctor's instructions carefully
on how to take it.
Usually, Zolpidem or any other medicines used to treat sleeping disorders are only for short-term use which is anywhere from 2 - 4 weeks. Continuous long-term use is not recommended unless advised by the doctor
Do not drink alcohol while you are on this medication. It can cause extra drowsiness. and excessive alcohol drinking actually causes sleeplessness in the first place.
Zolpidem may cause drowsiness the next day, if affected, do not drive or operate machinery.
Before you take Zolpidem, Tell your doctor if:
- you are allergic to anything.
- you drink alcohol
- you have any other medical conditions.
- you are on any other medication
- you are going to have surgery, dental treatment or anaesthetics
- if you are pregnant, likely to be pregnant or breastfeeding
Side Effects
All medications come with side effects. It is always a risk-to-benefit ratio that the doctor weighs when they prescribe you a treatment. Not everyone will experience side effects. So do not be alarmed by
it. You can find more side effects information in the leaflet that your pharmacist hands you. If anything worries you, speak to your doctor.
If you experience any sleepwalking which is very rare, you need to contact your doctor straight away.
Side effects information is there for your reference, so you know what to do if you do experience it. It is not there to scare you or to stop you from getting appropriate treatment.

Men with prostate cancer are much more likely to die of cardiac disease and not from prostate cancer, unless the diagnosis is made at a very late stage. For that reason, a manโs cardiac history, his cardiovascular risk factors, and the choice of which drug to use for ADT are very critical issues.
Androgen deprivation therapy, ADT, the mainstay of treatment for years, is associated with a 10-20% increased risk of cardiac complications, such as a heart attack, stroke, or sudden death. If a man has already had two important cardiac events before starting ADT the risk of another cardiac event is greatly increased. Surprisingly these serious problems can occur within the first 6 โ 12 months.
Degarelix (Firmagon), the newer drug for ADT, is less toxic to the heart than leuprolide (Lupron), and, for many clinicians, is becoming the preferred agent for hormone therapy.
Every man before starting ADT needs to have a thorough evaluation for cardiac risk factors. This exam should focus on past history of cardiovascular disease, hypertension, diabetes, elevated cholesterol and triglycerides, and the life-style factors diet and exercise habits.
In some men this pre-ADT evaluation will lead to choosing Firmagon over Lupron, or receiving the shortest period of drug exposure as possible, or beginning a program for modification of cardiac risk factors โ or possibly avoiding the use of these drugs entirely.
