Pharmacology of Oxcarbazepine (Trileptal) ; Mechanism of action, Pharmacokinetics, Uses, Effects
Pharmacology of Oxcarbazepine (Trileptal) ; Mechanism of action, Pharmacokinetics, Uses, Effects Side Effects 1 Views • 1 month ago

Oxcarbazepine, marketed under the brand name Trileptal, is an antiepileptic medication used to treat partial-onset seizures. Here's an overview of its pharmacology:

Mechanism of Action
Voltage-Sensitive Sodium Channel Blockade: Oxcarbazepine and its active metabolite, 10-monohydroxy metabolite (MHD), block voltage-sensitive sodium channels. This action stabilizes hyperexcited neuronal membranes, inhibits repetitive neuronal firing, and reduces the propagation of synaptic impulses, which helps prevent the spread of seizures2.

Indications
Partial-Onset Seizures: Oxcarbazepine is indicated for use as monotherapy or adjunctive therapy in the treatment of partial-onset seizures in adults and pediatric patients.

Dosage and Administration
Immediate-Release Formulation: The typical starting dose for adults is 300 mg twice daily, with gradual increases based on clinical response and tolerability.

Extended-Release Formulation: The starting dose for adults is 600 mg once daily, with gradual increases based on clinical response and tolerability.

Adverse Effects
Common Side Effects: Include dizziness, drowsiness, headache, nausea, and vomiting2.

Serious Side Effects: Include hyponatremia (low sodium levels), hypersensitivity reactions, and increased risk of suicidal thoughts and behaviors3.

Pharmacokinetics
Absorption: Oxcarbazepine is rapidly absorbed after oral administration and extensively metabolized to its active metabolite, MHD3.

Metabolism: Oxcarbazepine is metabolized in the liver to MHD, which is responsible for most of its antiepileptic activity2.

Excretion: The metabolites are primarily excreted in the urine.

Half-Life: The elimination half-life of oxcarbazepine is approximately 2 hours, while the half-life of MHD is about 9 hours

A: Trileptal, otherwise known as oxcarbazepine is one of the newer anti-seizure medications. It's based off an old medication, carbamazepine or Tegretol, as you probably heard in the past. It's another fast sodium channel drug and very good for seizures with partial-complex seizures.

B: Seizures that start in one area of the brain and spread. It's well tolerated with fewer side effects than Tegretol or Carbatrol, which it's derived from. The biggest issue with that, the biggest side effect is, you can get some drowsiness when you start taking it. That usually goes away within a week or so.

B: You start the dose slowly and gradually increase it so there are fewer side effects. There is a risk of an allergic reaction, as there is with any medication. This presents, usually, as a red rash all over the trunk and chest. If that happens, just stop the medication. Take Benadryl, and then, slowly increase it again. There is the most sodium issues, but more in adults than in children.

A: It's also called an enzyme inducer. You have to watch what medications you mix this with, because it could affect how the other medications perform. When you start the medication, you might have to increase the dose after a few weeks, to adjust for how the liver is metabolizing it. It's a medication that you need to watch what you're using it with, and watch those rare side affects.

B: It can also decrease the efficacy of birth control pills. You need to be careful if you're on birth control pills, that you use some other means of contraception.

A: Trileptal or oxcarbazepine is a first line indicated drug, very good for partial-complex seizures, not to be used in generalized epilepsy, with very few side effects and very well tolerated.

How long does it take for triamcinolone cream to work
How long does it take for triamcinolone cream to work Side Effects 1 Views • 1 month ago

How long does it take for #triamcinolone cream to work?

A course of treatment for seven days is usually sufficient.

If your symptoms have not improved after this time (or if they get worse), speak again with your doctor for further advice.

Topical corticosteroids like triamcinolone should not be used for long periods of time or on large areas of the body.

If you are using a moisturiser along with this preparation, apply the moisturiser first.

Then wait 10-15 minutes before applying Aureocort® ointment.

This allows time for the moisturiser to be absorbed before the topical corticosteroid is applied.

Your skin should be moist but not slippery when you apply Aureocort®.

Do not use Aureocort® on your face unless a skin specialist doctor has said you should.

If you have been told to use it on your face, be careful not to get any preparation near your eyes and do not use it for longer periods of time than you have been advised.

Unless advised to do so by your doctor, do not apply a bandage or dressing to the area being treated, as this will increase absorption of the preparation and increase the risk of side-effects.

Continue to use Aureocort® until the flare-up has gone and then stop it.

Triamcinolone, Triamcinolone Acetonide
Triamcinolone, Triamcinolone Acetonide Side Effects 1 Views • 1 month ago

Triamcinolone, Triamcinolone Acetonide .

Why is this medication prescribed?

Triamcinolone, a corticosteroid, is similar to a natural hormone produced by your adrenal glands. It often is used to replace this chemical when your body does not make enough of it. It relieves inflammation (swelling, heat, redness, and pain) and is used to treat certain forms of arthritis; skin, blood, kidney, eye, thyroid, and intestinal disorders (e. g. , colitis); severe allergies; and asthma. Triamcinolone is also used to treat certain types of cancer.

This medication is sometimes prescribed for other uses; ask your doctor or pharmacist for more information.

How should this medicine be used?

Triamcinolone comes as a tablet and syrup to be taken by mouth. Your doctor will prescribe a dosing schedule that is best for you. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand.

Do not stop taking triamcinolone without talking to your doctor.
Stopping the drug abruptly can cause loss of appetite, upset stomach, vomiting, drowsiness, confusion, headache, fever, joint and muscle pain, peeling skin, and weight loss. If you take large doses for a long time, your doctor probably will decrease your dose gradually to allow your body to adjust before stopping the drug completely.
Watch for these side effects if you are gradually decreasing your dose and after you stop taking the tablets or oral liquid, even if you switch to an inhalation. If these problems occur, call your doctor immediately.
You may need to increase your dose of tablets or liquid temporarily or start taking them again.

Take triamcinolone exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

What special precautions should I follow?

Before taking triamcinolone,

tell your doctor and pharmacist if you are allergic to triamcinolone, aspirin, tartrazine (a yellow dye in some processed foods and drugs), or any other drugs.

tell your doctor and pharmacist what prescription and nonprescription medications you are taking, especially anticoagulants ('blood thinners') such as warfarin (Coumadin), arthritis medications, aspirin, cyclosporine (Neoral, Sandimmune), digoxin (Lanoxin), diuretics ('water pills'), estrogen (Premarin), ketoconazole (Nizoral), oral contraceptives, phenobarbital, phenytoin (Dilantin), rifampin (Rifadin), theophylline (Theo-Dur), and vitamins.

if you have a fungal infection (other than on your skin), do not take triamcinolone without talking to your doctor.
tell your doctor if you have or have ever had liver, kidney, intestinal, or heart disease; diabetes; an underactive thyroid gland; high blood pressure; mental illness; myasthenia gravis; osteoporosis; herpes eye infection; seizures; tuberculosis (TB); or ulcers.

tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking triamcinolone, call your doctor.
if you are having surgery, including dental surgery, tell the doctor or dentist that you are taking triamcinolone.

if you have a history of ulcers or take large doses of aspirin or other arthritis medication, limit your consumption of alcoholic beverages while taking this drug. Triamcinolone makes your stomach and intestines more susceptible to the irritating effects of alcohol, aspirin, and certain arthritis medications. This effect increases your risk of ulcers.

What special dietary instructions should I follow?

Your doctor may instruct you to follow a low-sodium, low-salt, potassium-rich, or high-protein diet. Follow these directions.

Triamcinolone may cause an upset stomach. Take triamcinolone with food or milk.

What should I do if I forget a dose?

When you start to take triamcinolone, ask your doctor what to do if you forget a dose. Write down these instructions so that you can refer to them later.

If you take triamcinolone once a day, take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

What side effects can this medication cause?

Triamcinolone may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:

upset stomach.
stomach irritation.
vomiting.
headache.
dizziness.
insomnia.
restlessness.
depression.
anxiety.
acne.
increased hair growth.
easy bruising.
irregular or absent menstrual periods.

If you experience any of the following symptoms, call your doctor immediately:

skin rash.
swollen face, lower legs, or ankles.
vision problems.
cold or infection that lasts a long time.
muscle weakness.
black or tarry stool.
