Statins, also known as HMG-CoA reductase inhibitors, are a class of lipid-lowering medications that reduce illness and mortality in those who are at high risk of cardiovascular disease. They are the most common cholesterol-lowering drugs.

Low-density lipoprotein (LDL) carriers of cholesterol play a key role in the development of atherosclerosis and coronary heart disease via the mechanisms described by the lipid hypothesis. Statins are effective in lowering LDL cholesterol and so are widely used for primary prevention in people at high risk of cardiovascular disease, as well as in secondary prevention for those who have developed cardiovascular disease.

Side effects of statins include muscle pain, increased risk of diabetes mellitus, and abnormal blood levels of liver enzymes. Additionally, they have rare but severe adverse effects, particularly muscle damage. They inhibit the enzyme HMG-CoA reductase which plays a central role in the production of cholesterol. High cholesterol levels have been associated with cardiovascular disease.

There are various forms of statins, some of which include atorvastatin, fluvastatin, lovastatin, pitavastatin, pravastatin, rosuvastatin, and simvastatin. Combination preparations of a statin and another agent, such as ezetimibe/simvastatin, are also available. The class is on the World Health Organization's List of Essential Medicines with simvastatin being the listed medicine. In 2005, sales were estimated at US$18.7 billion in the United States. The best-selling statin is atorvastatin, also known as Lipitor, which in 2003 became the best-selling pharmaceutical in history.The manufacturer Pfizer reported sales of US$12.4 billion in 2008.Due to patent expirations, several statins became available in 2016 as less expensive generics.

Simvastatin belongs to a group of medicines called HMG-CoA
reductase inhibitors, also known as statins. It is most common
known by its brand name Zocor.

Simvastatin works by reducing the amount of bad cholesterol
made by the liver and raises the good cholesterol to reduce the risk
of you getting a heart attack or a stroke.

Before you start Simvastatin, you need to tell your doctor the
following
- if you are allergic to any statin that you might be prescribed in the
past, or to any ingredients listed on the packaging.

- if you have any liver disease, kidney disease and breathing
problems.

- if you are pregnant, intend of likely to be pregnant as it may affect
the baby.

- if you are breast feeding or intend to breast feed, simvastatin can
cross breast milk and affect the baby 

- if you have muscle pain, tenderness or weakness from other
medicines used to treat high cholesterol or triglycerides

- if you are drinking alcohol regularly. 

- If you are of an asian descendent. Doses of Asians and western are
different. Some doctor’s will have difficulty picking that up if you
are a mixed child. 

What Strength it comes in? 
Simvastatin comes in 10,20,40 and 80mg

How to take it?
Depending on your condition and your age, your doctor will decide the most appropriate dose for
you.
The doses of simvastatin is between 10-80 mg taken once a day
around the same time in the evening. Swallow the tablet(s) whole
with a glass of water. 

Things to be aware of: 
- Avoid drinking large quantities of alcohol.
- Drinking large quantities of alcohol may increase your chance
- liver problems when taking simvastatin
- Avoid drinking grapefruit juice.
- Be careful driving or operating machinery until you know how
this medicine affects you. Simvastatin generally does not cause any
problems with your ability to drive a car or operate machinery.
However, it may cause dizziness in some people so make are you
know how the medication affects you before you start driving or
operate machinates.

Side Effects: 

- muscle and joint pain, muscle weakness, especially in the
forearms, thighs, hips, shoulders, neck and back.  difficulty
climbing stairs or standing up from a chair. difficulty lifting
arms over the head, falling, and difficulty getting up from a

- fall
- constipation
- diarrhoea
- stomach pain
- nausea (feeling sick)

Overview.

Senna is an herb, and senna tea is made from the leaves of the Cassia senna plant. These plants thrive in tropical areas, though some can grow in more temperate climates.

Senna tea can be slightly sweet, but it has strong bitter undertones. Because of this, people sometimes mix senna tea with green tea or add honey to improve its flavor. It is also not incredibly aromatic.

Senna tea has been marketed as a constipation treatment and a “detox” tea. It’s also often used as a weight loss aid, although this is not recommended.

Thanks to its ability to treat constipation, it’s sometimes used short-term to treat hemorrhoids. However, there is insufficient evidence to support the use of senna for hemorrhoids or weight loss.

Senna is approved by the FDA as a nonprescription laxative. The recommended intake for adults age 12 and older is 17.2 milligrams (mg) per day. Experts caution not to exceed 34.4 mg per day. For constipation in children, 8.5 mg per day is recommended. For constipation following pregnancy, 28 mg divided into 2 doses has been used.

Side effects.

Senna tea can cause a number of different immediate side effects. These include:

mild abdominal cramps.
severe diarrhea, especially with long-term usage.
nausea.
faintness, which can often happen as a result of diarrhea that leads to water loss.

Health benefits.

Senna tea is a viable short-term treatment for constipation. Because senna tea can treat constipation, it may also help treat hemorrhoids that occur as a result of chronic constipation, but you should consult your doctor before use.

Senna is also thought to have both anti-inflammatory and antiparasitic properties.

Risks, complications, and interactions.

While senna tea seems to be effective as a short-term constipation treatment, it should not be used long-term. The American Herbal Products Association actually recommended that it not be used long-term due to the potential risks.

Long-term senna usage may cause laxative dependence and liver damage.

You should consult your doctor before taking senna if you have:

colon disorders.
heart disease.
liver disease. Senna tea may also interact with medications like blood thinners and diuretics.

Scopolamine, also known as hyoscine, is an anticholinergic drug primarily used to prevent nausea and vomiting from motion sickness and postoperative recovery. It is available in transdermal, oral, and injectable forms. Its mechanism involves blocking muscarinic acetylcholine receptors in the central and peripheral nervous systems, thereby inhibiting signals that trigger vomiting. Commonly, a transdermal patch (1.5 mg) is applied behind the ear to provide relief for up to 72 hours, while oral or injectable doses range from 0.3 to 0.6 mg every 6–8 hours. Aside from motion sickness, it’s also used for postoperative nausea, irritable bowel syndrome, Parkinson’s-related drooling, and to reduce secretions in palliative care. An estimated 90% of patients use their metered dose inhaler incorrectly. Inadequate inhaler technique lowers drug deposition to the lungs, wastes medication and may lead to poor disease control, reduced quality of life, increased emergency hospital admissions and higher treatment costs.

HOW TO USE A SPACER WITH INHALER
1) Remove the inhaler and spacer cap. Carefully examine the products for damage, missing parts, or foreign objects. Remove any foreign objects prior to use. The products should be replaced immediately if there are any damaged or missing parts.
2) Firmly fit the two halves of the spacer together (If necessary).
3) Stand or sit upright when using your inhaler and spacer.
4) If you are starting a new inhaler or have not used the inhaler for more than one week you must prime your inhaler. Please read the information leaflet of your inhaler for instructions.
5) Hold the inhaler upright, insert into the spacer and shake 4 or 5 times.
6) Breathe out gently.

7) (Mouthpiece Spacer - Adults) Place the mouthpiece between your teeth without biting and form a good seal around it with your lips.

7) (Masked Spacer - Adults & Children) Apply mask to face and ensure an effective seal.

8) (Mouthpiece Spacer - Single Breath Technique - Adults) Press the inhaler at the beginning of a slow inhalation. Breathe in slowly and deeply through the mouth until a full breath has been taken. Hold breath for 5-10 seconds or however much comfortable.

8) (Mouthpiece Spacer - Tidal Breathing Technique - Adults) Press the inhaler at the beginning of a slow inhalation. Breathe in slowly and deeply through the mouth until a full breath has been taken. Keep lips tight on the mouthpiece breathing normally 2-3 times through the chamber.

8) (Masked Spacer - Tidal Breathing Technique - Adults & Children) Press the inhaler at the beginning of a slow inhalation. Breathe in slowly and deeply through the mask until a full breath has been taken. Keep mask tight on the face breathing normally 5-6 times through the chamber.

9) If you have done the SINGLE BREATHING TECHNIQUE remove mouthpiece from you mouth and hold your breath for 10 seconds or as long as is comfortable.

9) If you have done the TIDAL BREATHING TECHNIQUE, holding your breath is not necessary simply remove spacer.

10) If you have been advised to take another dose repeat the process again after 30-60 seconds.
11) Remove inhaler from spacer and replace the caps
straight away to keep out dust.

USEFUL TIPS
• Ensure the spacer is the correct one to fit your inhaler.
• If your inhaler contains a corticosteroid rinse your mouth
out with water after your dose.
• If you're ever unsure about your inhaler technique always double check with your pharmacist, nurse practitioner or GP.
• Always read the patient leaflet provided with your
inhaler and spacer for any specific instructions.

📍 Ceftriaxone antibiotics, brand name, Rocephin, is a third generation cephalosporin, and beta lactam antibiotic. That inhibits cell wall synthesis, in susceptible bacteria. Ceftriaxone has activity versus Gram negatives, and Gram positives bacteria, but lacks any substantial activity against anaerobic bacteria.
📍 Ceftriaxone is used for infections, like community-acquired pneumonia, bacterial meningitis, and otitis media caused by certain bacteria. Commonly used in combination with azithromycin, for community acquired pneumonia, requiring hospitalization.
📍 Dosing of ceftriaxone. It has a long half-life, so typically dosed, once daily in adults. But in more severe infections. For example, meningitis, may require twice daily dosing. Usual adult’s doses are, 1gram daily. 2gram daily, or 2gram twice a day. No dose adjustments are needed for renal or hepatic dysfunction. But it is recommend, not to exceed 2 gram per day with concurrent renal and hepatic dysfunction.
📍 Adverse effects of ceftriaxone. Ceftriaxone is secreted in the bile, there’s a risk for pseudo cholithiasis, and pancreatitis. This drug also interferes, with vitamin K metabolism, leading to increased INR, and thus bleeding risk. Hypersensitivity reaction can also occur. Do not administer via I V line, along with calcium. Because this can cause potentially fatal precipitates, to form in lungs and kidneys, which ultimately leads to death.

Triptane sind die wichtigsten Migräne-Medikamente. Es gibt 7 verschiedene Triptane mit unterschiedlicher starker Wirkung, unterschiedlichem Wirkeintritt und Halbwertszeiten.
Triptane sollten nicht bei Aura genommen werden und können auch zu einem sog. Recurrence führen. Auch gibt es zwischen 10-20% Non-Responder.
Folgende Nebenwirkungen können auftreten:
1. Müdigkeit

3. Leichte Übelkeit, selten bis zum Erbrechen
4. Missempfindungen, Kribbeln, Wärme- oder Hitzegefühl

Dr. Newman explains that there are various approaches for treating migraine headaches, including ones that deal with the actual headache itself and preventative measures before a migraine attack.

Lawrence C. Newman, MD: There are many different approaches that we used to treat people who suffer from migraine. In general, we divide the therapies into acute medications and preventative medications.

The acute medications are medications that taken at the onset of the headache to shorten the attack of pain, and hopefully to get rid of the associated symptoms, the nausea and vomiting for instance.

We divide the acute medications into two broad categories, symptomatic medications and specific medications. A symptomatic medication is not just for migraine.

It can be used for pain or for nausea and vomiting for any reason. It can include over the counter medications like Aspirin or Tylenol or medications combined like with caffeine like Excedrin for instance or certain prescription medications.

The prescription forms of antiinflammatory medications as well. Specific agents on the other hand worked just for migraine and the beauty of these agents is that they target not only the head pain, but the associated features, the nausea, the vomiting, the lightness, the sound and the smell sensitivity.

They work quickly. They do not cause sedation in general, and they are not addicted. Agents like those include the Triptans, Sumatriptan or Imitrex, Rizatriptan or Maxalt were various forms, those medications, Ergotamines and Dihydroergotamine like Migranal nasal spray and several others.

Again, these agents target the headaches specifically not generally. For people who had very frequent attacks of migraine, frequent is defined as more than one headache per month will often use preventative agents.

Preventative agents fall in several different categories. High blood pressure medications, antidepressant medications, and antiseizure medications. These agents have been shown to decrease both the frequency and severity of attacks of migraine letting patients go on to live a normal life.
