PCRI’s Executive Director, Mark Scholz, MD, discusses the use of hormone therapy for men with Azure (High-Risk) prostate cancer. He covers why it is necessary for men with high-risk disease and talks about the three categories of hormone therapy.

0:28 Men with Azure are generally given hormone therapy because there is a higher risk of spread outside the prostate. Hormone therapy is a systemic treatment that acts on the entire body, not just the prostate. It is effective, and its side effects are reversible. Compared to chemotherapy, another systemic treatment, it is more effective and has far fewer side effects.

1:23 The three categories of hormone therapy. Hormone therapy, also known as testosterone inactivating pharmaceuticals (TIP) comes in three different strengths. The first and least potent category of hormone therapy is the “anti-androgens.” Common examples of these drugs are Casodex and Nilutamide. The second category and the most commonly used type of hormone therapy are the “LHRH Agonists” drugs such as Lupron, Eligard, and Trelstar. The last category is the second generation hormonal agents that operate differently than the previous categories. Examples of these are Erleada, Xtandi, and Zytiga. You don’t want more hormone therapy than what is necessary because of added side effects.

2:43 Hormone therapy for the different subtypes of Azure. Each subtype of azure requires different amounts of hormone therapy. High-risk prostate cancer should be treated with a combination of radiation, seed implants, and hormone therapy, but the amount of hormone therapy can differ depending on the subtype. Low-Azure only requires 4-6 months of Lupron and Casodex. Basic-Azure needs between 18-24 months for optimal cure rates. And High azure needs 18-24 months possibly with the addition of a second generation TIP such as Erleada, Xtandi, or Zytiga.

4:12 Summary. The amount of hormone therapy depends on the subtype of prostate cancer. Many studies have shown that Low-Azure only requires a short course of hormone therapy and High-Azure can benefit from adding second generation hormone therapy.

Prozac opened a new era in psychiatry practiced principally by general practitioners rather than mental health professionals. Some refer to this as cosmetic psychopharmacology. Currently the top 25 drugs prescribed in America include 3 antidepressants. Once heralded as a great advance, professionals are increasingly questioning whether doctors initiate therapy for inappropriate reasons.

Of even more concern, many continue therapy for lengthy intervals. Attempts to cease treatment often result in a condition known as the discontinuation syndrome. Its symptoms are routinely mistaken for recurrence of the original depression. Unfortunately the response generally involves even more therapy.

Studies routinely fail to demonstrate a significant difference from placebo in mild to moderate depression. Prozac clearly improves severe depression, however this may be most appropriately managed by psychiatrists rather than general practitioners and internists. The way medicine is currently practiced dictates the overwhelming majority of people suffering from depression will never meet with a psychiatrist.

Unfortunately the vast majority of prescriptions for Prozac originate in brief interactions with non-specialists less than adequately schooled in the nuances of depression. A cursory evaluation during a hurried visit followed by a prescription with multiple refills seems almost guaranteed to miss the essentials of depression.

Prozac became the first modern antidepressant and belongs to the family of selective serotonin reuptake inhibitors or SSRIs. Other members of the class include Paxil, Zoloft, Celexa, and Lexapro. None demonstrate significant superiority in treating depression and all share a common side effect profile. For reasons unique to an individual, some people prefer or respond more favorably to one pill rather than another.

Common side effects include nausea, diarrhea, difficulty sleeping, anxiety, tremors and excessive perspiration. Sexual dysfunction affects more men than women with erectile difficulties, loss of libido and lack of orgasm occurring in anywhere between 1-70%. Obviously the lower numbers arise from drug company funded studies.

Bleeding difficulties may occur especially in those receiving aspirin or anticoagulants. Monitoring for weight change, either gain or loss, low blood sugar and falling levels of blood sodium remain essential.

Combination with triptans for migraine, other antidepressants, St. John’s wort, lithium or fentanyl may lead to the serotonin syndrome. Manifestations include dizziness, sweating, elevated temperature, increased heart rate and blood pressure, tremor, agitation, hallucinations and delirium.

Complaints suggestive of the discontinuation syndrome include headache, irritability, dizziness, sleeplessness, fatigue, crying spells, shakiness and a form of mania.

Prozac is not appropriate for pregnant or breast feeding women.

Despite its suggested activity in boosting serotonin, the drug’s mechanism of action remains unsettled. Neither elevating nor lowering serotonin levels guarantees a change in mood. Emerging concepts clearly indicate abnormalities in many brain chemicals underlie depression. Serotonin’s role in the process is under review. The recently released Spravato – esketamine – treats depression by targeting a different area, the NMDA receptor.

1. Anxiolytic Effects: Lorazepam is primarily used as an anxiolytic (anxiety-reducing) medication. It works by enhancing the effects of a neurotransmitter called gamma-aminobutyric acid (GABA) in the brain. GABA inhibits the activity of nerve cells, leading to a calming and sedative effect.

2. Dosage Forms: Lorazepam is available in various forms, including tablets, oral solutions, and injectable solutions. The injectable form is often used in hospital settings to manage acute anxiety or agitation.

3. Short-Term Use: Due to the risk of tolerance, dependence, and withdrawal symptoms, lorazepam is usually prescribed for short-term use, typically not exceeding a few weeks. Prolonged or excessive use can lead to physical and psychological dependence.

4. Withdrawal: Abruptly stopping lorazepam after regular use can lead to withdrawal symptoms, including anxiety, restlessness, insomnia, muscle tension, and in severe cases, seizures. Tapering off the medication under medical supervision is usually recommended to avoid withdrawal complications.

5. Side Effects: Common side effects of lorazepam include drowsiness, dizziness, muscle weakness, and impaired coordination. Less common side effects may include memory problems, confusion, and changes in mood.

6. Precautions: Lorazepam should be used with caution in individuals with a history of substance abuse, as well as those with a history of depression or other mental health disorders. It's important to inform your healthcare provider about any medications you are taking, as lorazepam can interact with other drugs and substances, including alcohol.

7. Elderly and Special Populations: The effects of lorazepam can be more pronounced in the elderly population, leading to increased sedation and risk of falls. Dosage adjustments may be necessary for older adults. Pregnant and breastfeeding women should consult their healthcare providers before taking lorazepam, as it can potentially affect the developing fetus or infant.

8. Medical Supervision: Lorazepam should be taken under the supervision of a medical professional who can monitor its effects, adjust the dosage, and guide the discontinuation process when necessary.

It's important to note that lorazepam and other benzodiazepines are not recommended for long-term use due to the potential for dependence and other adverse effects. If you have concerns about anxiety or any other medical condition, it's advisable to consult a healthcare provider who can recommend appropriate treatment options based on your individual needs.

A Memantina é um dos medicamentos utilizados no manejo da doença, e é fundamental que vocês, familiares e cuidadores, compreendam como ela age, para quem é indicada e o que esperar do tratamento.

Para entender como a Memantina funciona, precisamos falar um pouco sobre o que acontece no cérebro de uma pessoa com Alzheimer. Nosso cérebro se comunica através de impulsos elétricos e substâncias químicas chamadas neurotransmissores. Um desses neurotransmissores é o glutamato. O glutamato é essencial para o aprendizado e a memória, ele ajuda as células cerebrais, ou neurônios, a se comunicar e a formar novas memórias.

O problema é que, na Doença de Alzheimer, há um desequilíbrio nesse sistema. Existem níveis excessivos de glutamato que ficam constantemente "ativos", sobrecarregando os neurônios. Pensem nisso como um "ruído" excessivo ou uma "superestimulação" constante. Essa superestimulação, a longo prazo, pode danificar e até matar os neurônios.

É aí que a Memantina entra em ação. Ela funciona como um "protetor" dos neurônios. A Memantina se liga a um tipo específico de receptor, chamado receptor NMDA, onde o glutamato atua. Ao fazer isso, ela modula a atividade do glutamato. Ela não bloqueia completamente o glutamato, o que seria prejudicial, mas sim impede que o glutamato em excesso cause danos. É como se ela desligasse o "volume" do ruído excessivo, permitindo que os sinais importantes (aqueles necessários para a memória e o aprendizado) ainda passem, mas protegendo os neurônios do estresse constante.

Ao reduzir essa superestimulação, a Memantina ajuda a preservar a função dos neurônios por mais tempo, o que pode resultar em uma melhora ou estabilização dos sintomas cognitivos e comportamentais, como a memória, a atenção, o raciocínio e até a redução de agitação.

A Memantina é geralmente indicada para pacientes que estão em estágios moderados a graves da Doença de Alzheimer.
