On this episode I breakdown nitrofurantoin pharmacology. Nitrofurantoin is an antibiotic that is used in the treatment of UTI’s.
Nitrofurantoin has activity by creating reactive metabolites that can disrupt protein synthesis and ultimately kills the bacteria causing the infection.
It is important to remember with nitrofurantoin, that in the setting of systemic infection and/or kidney involvement, the drug may not be very effective.
Nitrofurantoin can potentially reduce the effectiveness of certain vaccines. Be sure to look this up prior to administration.
Nitrofurantoin can cause pulmonary symptoms as well as neuropathy. This is very important to monitor for.

- Nifedipine is a calcium channel blocker. It inhibits movement of calcium ions across cell membrane in systemic and coronary vascular smooth muscle, leading to vasodilatation and reduction in peripheral vascular resistance. So it reduces myocardial oxygen demand, and relaxes and prevents coronary artery spasm.
- It is available as 10, 20 mg immediate-release capsules, and 30, 60, and 90 mg extended-release tablets.

Today we’re going to talk about niacin (vitamin B3) and schizophrenia. With schizophrenia, a person may have hallucinations, delusions, paranoia, feelings of disconnection, a loss of interest in life, and may feel emotionless.
The medical theory of what causes schizophrenia is dopamine excess. A person is then given certain drugs to help regulate this. The potential problem with these drugs is that they could have side effects that cause metabolic syndrome and insulin resistance. This has been shown to worsen symptoms of schizophrenia because of what it could do to the brain.
However, Dr. Abram Hoffer had another theory about schizophrenia. It was called the adrenochrome theory. Adrenochrome is basically the byproducts of adrenaline. In his research, he found that the chemical structure of adrenaline is similar to LSD and that in higher amounts, it could have certain effects on the brain. His theory was that schizophrenia comes from excessive amounts of adrenochrome in the brain, which is toxic.
Normally, a person has an enzyme that clears out that chemical. But, people with schizophrenia have a defect with this enzyme. He found that large doses of vitamin B3 every day (straight niacin) may help prevent the conversion of the precursor of adrenaline to adrenaline, making less adrenaline. This could potentially help clear out adrenaline as well as decrease the production of adrenaline.
Other potential benefits of vitamin B3:
• It could help reduce liver toxicity
• It could help reduce oxidative stress in the mitochondria
• It could help the DNA repair

In this video, we’re going to talk about niacin deficiency symptoms. If you have a niacin deficiency, you might experience the “4 D's”: dermatitis, diarrhea, dementia, and even death.
Subclinical symptoms of a niacin deficiency include the following:
• Loss of taste
• Metallic/rancid taste in the mouth
• Loss of smell/change in smell
• Face moves when you look in the mirror
• Words move when you read
• Unstable or moving ground when walking
• Fatigue
• Inability to relax
You need large amounts of niacin to address a deficiency. You can take either niacin or niacinamide.
Niacin is a precursor to NAD, which helps extract electrons from foods. This allows you to get more energy from the food that you eat.
Vitamin B3 helps support something known as sirtuins, which support longevity and DNA repair. L-tryptophan can convert to niacin, serotonin, and melatonin!
Niacin is very beneficial for skin health and conditions like acne and psoriasis. A niacin deficiency can cause very dry, scaly skin.
The form of niacin known as niacinamide can help with inflammatory conditions like arthritis. Niacin also supports healthy levels of LDL and HDL cholesterol. It was very popular before the development of statins.
Animal protein and organ meats are the best sources of niacin. You can also get niacin from spirulina and nutritional yeast, but look for nutritional yeast that does not have added synthetic vitamins.
There is a tremendous amount of research on niacin and cholesterol, the heart, and mood disorders like schizophrenia, depression, and bipolar disorder.
A diet high in ultra-processed foods can lead to a niacin deficiency. Pregnancy, alcoholism, Covid-19, and certain medications can also contribute to a niacin deficiency.

Today we're going to talk about the niacin flush and if it's dangerous or not. The answer is no. I do not believe the niacin flush is harmful or dangerous.
With a niacin flush, you're getting massive relaxation of the muscles inside your blood vessels (vasodilation). This occurs because niacin releases something called prostaglandins.
Prostaglandins are lipids or fat compounds that have a hormone effect. Prostaglandins increase vasodilation, help reduce blood pressure, and help blood clotting. Prostaglandins are also involved in headaches and fevers.
Niacin has been used therapeutically for over 50 years, and it is the most effective natural agent for increasing your HDL (the good cholesterol). It can also decrease LDL (the damaging LDL) and triglycerides. The keto diet and intermittent fasting may also help with a person's cholesterol.
A person concerned about their cholesterol may need to take niacin in higher amounts (1000-2000mg per day). Keep in mind the type of niacin that is non-flushing doesn't have cholesterol-lowering benefits. Niacin may also potentially help reduce schizophrenia.

You mentioned that you've been using the Implanon for contraception and after having it removed you're wondering what sort of side effects are there and how long is it going to take to get pregnant? Studies have shown that 90% of women actually ovulate within three to four weeks after it's removed. Once that device is out of your body you're not receiving the hormone anymore and you can start ovulating and thus get pregnant. Some women notice that they have a little bit of irregular spotting or bleeding for a few weeks and then have a normal period. That will help you to time things just right if you're actually trying to conceive. Once you're having regular cycles you can usually figure out that you're ovulating about mid-cycle and try to time intercourse with ovulation and increase your chances of having a baby. Overall it takes most couples up to six months to get pregnant and this is whether you had the Implanon or not. Your chances aren't greater of having infertility problems just because you've had that form of contraception. The birth control implant is a small plastic rod that gets placed into your arm, and it lasts for three years.
One of the most common side effects of this form of birth control is breakthrough bleeding. You may notice that when you have this implant placed that your periods will no longer be predictable. In fact, you can have irregular bleeding and spotting for the duration of this implant. So, after you've had the implant placed, if you notice irregular bleeding, don't worry. If you are concerned, you can always call your physician or your nurse practitioner. However, the birth control is still effective despite the bleeding.
Another side effect that patients may be concerned with are headaches. If you do get headaches with this form of birth control, you can take Tylenol and Motrin to relieve them. Nexium Side Effects - In this video, we discuss the effects of Nexium, which include both the short term side effects and long term health concerns. Nexium is a proton pump inhibitor (PPI) often prescribed to treat heartburn and acid reflux, and more specifically, gastroesophageal reflux disease (GERD). To provide relief, the drug effectively stops the stomach from producing acid. While this drug does have the ability to provide short term relief, it also has the potential to lead to long term health problems.
Most people are familiar with the side effects that are displayed on the commercial and box label because, quite simply, this is what is advertised. These side effects of Nexium include headaches, nausea, diarrhea, dry mouth, stomach pain and constipation. In addition, there are some even more severe initial Nexium side effects: dizziness, uneven heart rate, seizures, choking, and cramping. However, what is less known about the side effects of Nexium is what it can do to your body a year, 2 years, or even longer down the road.
Stomach acid can cause pain, hence acid reflux and why you are probably considering or already prescribed Nexium. However, stomach acid is a natural occurrence in the body. When you stop something in your body that is supposed to happen naturally, some nasty stuff can happen, especially if you take Nexium for too long (ie. more than 8 weeks).
In 40% of people who take Nexium for more than 8 weeks, they actually become addicted. And if you do end up taking Nexium for a year or more, it can increase the chance of Osteporosis by 45% and the chance of dementia by 18%. That is just startling!
In addition, with the lack of acid production, natural absorption of minerals is hindered, your stomach is more prone to infection and bacteria growth, which in turn, increases the chances of stomach cancer.
Like most prescription drugs, Nexium obviously serves a purpose and can rightfully be prescribed by a doctor to treat your heartburn or acid reflux. However, please note, that Nexium just provides short term relief instead of a long term solution to prevent you from having those uncomfortable symptoms in the future. Drug Cards Daily | Episode #38: esomeprazole (Nexium) | Get Stomach Acid and H. pylori Infection Under Control
Esomeprazole is available over the counter and by prescription. The commonly known brand name is Nexium. Esomeprazole is a substituted benzimidazole proton pump inhibitor (PPI). It comes in a delayed release capsule and as delayed release granule packets. When treating GERD and other hypersecretory conditions a typical dosing range is between 20-40 mg po qd-bid. Esomeprazole is also used as part of a multi-drug regimen for treating H. pylori infection. The mechanism of action behind esomeprazole is through the suppression of acid secretion by inhibiting hydrogen-potassium ATPase. Esomeprazole is the S-enantiomer of omeprazole (Prilosec). Since esomeprazole affects stomach acid it may alter how well other drugs work. For example, absorption of drugs may decrease which may lead to a decrease in efficacy of those drugs. Esomeprazole is best when taken 1 hour before a meal and best if taken before the first meal of the day. If needed, the capsule can be opened.

Patty McGoldrick: Neurontin is also known as gabapentin and it's another medication that can be used for control of seizures and epilepsy. It's well tolerated, it's used for generalized, impartial seizures. It's available both in pills that can be crushed or it's available in an oral formulation, so a liquid for small children.
Dr. Steve Wolf: The major side effects is sedation.
Patty: Right.
Dr. Wolf: Sleepiness. It can cause a little bit of an increased appetite and weight gain, so you need to monitor that very closely. Though it's also useful for patients who are having trouble sleeping at night. It already helps to sort of increase the sleep control.
Patty: And actually we use it a lot for people who are having difficulties sleeping. It's also used for pain, for neuropathic pain.
Dr. Wolf: So it's a good medication Another option in our armory of people with difficult to control seizures but not one of your first line kind of drugs.
Patty: So Neurontin or gabapentin, third or fourth line drug for control of generalized, impartial seizures. A big side effect is drowsiness. It's also used for pain and it's well tolerated for the most part.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Gabapentin (neurontin) and its mechanism of action. He discusses its FDA approved indications including partial seizures, post-herpetic neuralgia, neuropathic pain, fibromyalgia and alcohol dependence and some off-label uses (such as the treatment of anxiety, and alcohol withdrawal). Gabapentin (Neurontin) is a controlled medication in some states, and does have abuse potential (albeit, it is thought of as having less abuse potential than benzodiazepines or other pain medications like opiates). As this medication has various side effects (including abuse potential) it must be taken under the direction and supervision of a medical provider.

Generic name: Gabapentin.
Brand names: Neurontin or Horizant.
Available as a capsule and as a tablet.
How to use it?
Make sure to swallow the capsule or tablet whole with half a glass of water.
In addition, it is recommended to take gabapentin at fixed times. This will make it less likely that you forget a dose.
Furthermore, after a few days to months your doctor will evaluate if gabapentin is sufficiently effective for you. If necessary your doctor will adapt your dose or will switch to another drug.
When to use it?
Gabapentin is part of a group of drugs called anti-epileptic drugs. However it can be used to treat several medical conditions, like:
- Epilepsy
- Neuropathic pain
What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.
- Most commonly your doctor will slowly increase your dose, up to a maintenance dose of 900-3600mg per day divided over 3 dosages. Therefore always use the precise amount and frequency your doctor prescribed you.
What are common the side-effects?
- Very common (10%): somnolence, dizziness, fatigue, increased risk for viral infections and fever.
- Common (1-10%): pneumonia, infections, aggression, fatigue, confusion, anxiety, depression, weight gain, appetite increase, amnesia, headache, insomnia, tremor, convulsions, altered reflexes, high blood pressure, cough, rash, itching, muscle and back pain, muscle twitching and erectile dysfunction.
- Uncommon (0.1-1%): allergic reactions, high blood sugar, agitation, palpitations, swallowing disorder and oedema.
- Rare (0.1%): palpitations, low blood sugar, loss of consciousness and respiratory depression.
Safety:
First of all be careful to combine gabapentin with alcohol. As this might increase its side effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
Furthermore, gabapentin can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
