- In less than 3 minutes you'll learn:

- What is Rabeprazole (AcipHex)?

- Rabeprazole belongs to a category of drugs called proton pump inhibitors. It suppresses the secretion of gastric acid by combining with an enzyme on the gastric parietal cells called H+/K+-ATPase (proton pump), which is the final pathway for acid secretion. And this decreases hydrogen ion transport into gastric lumen.

- It is used in the treatment of erosive esophagitis. duodenal ulcers and gastro-esophageal reflux disease.
- Rabeprazole is used to treat conditions involving excessive stomach acid, such as Zollinger-Ellison syndrome.
- And also used for the treatment of Helicobacter pylori in combination with amoxicillin and clarithromycin.

It is nearly completely absorbed after oral administration and undergoes essentially no first-pass hepatic metabolism, resulting in its absolute bioavailability at 90-100%.
It is primarily eliminated by renal excretion, with approximately 80-90% of a dose being excreted unchanged in the urine, and a small amount is excreted unchanged in the feces.
Calculate creatinine clearance to determine appropriate dosing.

Climax control condom:

In general, condoms can decrease sensitivity and keep you from ejaculating early. But there are also climax control condoms available over the counter that are either made of a thicker latex material or contain a numbing agent meant to delay climax.

Penn Medicine's Trauma Center uses all means necessary to quickly find and stop bleeding, as uncontrolled bleeding is the leading cause of death in patients who have been severely injured. Depending on the location and severity of bleeding, a patient may require specific medications, red blood cell transfusion, invasive catheter procedures, plasma or platelets, or surgery.

Patrick Kim, MD, trauma surgeon and the Trauma Program Medical Director at Penn Medicine's Level 1 Trauma Center, discusses life-threatening bleeding and the use of tranexamic acid (TXA), a medication that helps control bleeding after trauma which has been used to treat bleeding on a wide varity of patients in the operating room and Emergency Department.

"It has the greatest impact of survival on the patients who are bleeding the most," Dr. Kim explains. "In one study, TXA improved survival by 14% -- which is remarkable for any single medical treatment. TXA is an important part of our Trauma Center strategy to stop bleeding and save lives. Although it's not a replacement for blood transfusion or prompt surgery, it is an important addition to these treatments." Venlafaxine XL is an antidepressant that belongs to a group of medicines called serotonin and norepinephrine reuptake inhibitors (SNRIs). This group of medicines is used to treat depression and other conditions such as anxiety disorders. It is thought that people who are depressed and/or anxious have lower levels of serotonin and norepinephrine in the brain. It is not fully understood how antidepressants work, but they may help by increasing the levels of serotonin and norepinephrine in the brain. Venlafaxine XL is a treatment for adults with depression. It is also a treatment for adults with the following anxiety disorders: generalized anxiety disorder, social anxiety disorder (fear or avoidance of social situations), and panic disorder (panic attacks). Treating depression or anxiety disorders properly is important to help you get better. If it is not treated, your condition may not go away and may become more serious and more difficult to treat.

Do not take Venlafaxine XL :

- If you are allergic to venlafaxine or any of the other ingredients of this medicine.
- If you are also taking, or have taken within the last 14 days, any medicines known as irreversible monoamine oxidase inhibitors (MAOIs), used to treat depression or Parkinson’s disease. Taking an irreversible MAOI together with Venlafaxine XL , can cause serious or even life-threatening side effects. Also, you must wait at least 7 days after you stop taking Venlafaxine XL before you take any MAOI.

Today primarily will be focusing on bicalutamide use in transgender females
Other anti-androgen medications often prescribed are spironolactone and cyproterone acetate. However, these drugs are much weaker than bicalutamide. Cyproterone is also not available in the US.

Bicalutamide also helps the aromatization of testosterone to estrogen which is why it is especially helpful for breast development when compared to other anti-androgen medications. Some small studies have shown better breast growth when compared to Aldactone and cyproterone. A possible mechanism is that Aldactone causes early maturation of the breast bud when compared with bicalutamide.

Bicalutamide is known to cause a reduction in vitamin C levels and glutathione levels in animal studies. Taking glutathione and vitamin C may reduce the risk of liver toxicity however I have no data to support this strategy.

Half-life is 6 days so the drug stays in the body for several days so only twice a weekly dose is adequate. Compare that to spironolactone which has a half-life of only 2 hours

Have Liver problems
If you drink heavily
Over the age of 35
Get a liver panel done 2-4 weeks after starting the medication. For any sign of liver failure such as jaundice, the medication must be immediately stopped.
Dosing:
Bicalutamide stays in the body for several days and has a half-life of 6 days which means half of the drug is broken down and removed by the body in 6 days. Twice a weekly dose of 50 mg is adequate. Higher doses increase the risk of liver toxicity and should be avoided
In summary, this is a very good medication for feminization therapy especially if high breast growth is strongly desired. Liver toxicity is a major concern and can be life-threatening so do not consume alcohol, and avoid hepatotoxic medications such as Tylenol.
