In addition to the huge variation in dose of B12 supplements, there are at least 4 different forms of the vitamin to choose from. The most common of these are cyanocobalamin and methylcobalamin.

But which of these is better? And why do some people argue that you should never take cyanocobalamin and that it is tantamount to poison?

Indeed, why is there poison – cyanide – in cyanocobalamin at all? Should we be concerned that this is an unnatural form of the vitamin?

Today we'll be exploring the different forms of B12 with a special focus on cyano- and methylcobalamin given their ubiquity, including manufacture, safety and conversion issues – and how I arrived at my recommendation for the form you should take.

Please note that the discussion in this video applies to healthy people with no known issues with B12 utilisation, and you should persist with the regimen prescribed by your health care provider.

Are you taking B12 injections and wondering if your cyanocobalamin B12 injection is poisonous? My name is Dr. Terranella, and in this video, we're going to look at different types of B12 and specifically the cyanocobalamin molecule, how it may be poisonous or problematic for you, and also the other side of it, when it's probably not a problem.
So, there are a few different types of B12.

But the most commonly used is cyanocobalamin. So, I wanted to look into this question because many people do use cyanocobalamin and ask if it is poisonous. Cyanocobalamin does contain a small amount of cyanide, and some people get worried when they find that out. And probably they should because cyanide is very toxic. Now, while cyanocobalamin is generally considered very safe, and many people do take it without any adverse problems, it does contain cyanide, which is a toxic substance. It is difficult to estimate how much cyanide is in each injection, but we can do some estimations to get a rough idea of how toxic or poisonous it could actually be.

All right, so the most common dose of B12, whether it's cyanocobalamin or any other cobalamin, is about 1,000 micrograms. Sometimes you'll do that once a week, maybe a little bit more in the beginning, and then as you go on, usually about once a month. So, if you're doing 1,000 micrograms, which is about 1 mL a week, how much cyanide are you actually getting?

First thing to note about cyanocobalamin is that it is a very stable structure. The cyanide molecule is bound to the cobalamin molecule very tightly. So, in order for you to get cyanide, it has to dissociate.

This will happen when that cyanocobalamin is converted into methylcobalamin, hydroxycobalamin, or adensylcobalamin, all of which are more functional forms of the vitamin B12 molecule. So, once that cyanocobalamin dissociates, you still have the cyanide molecule there, which can then attach to a potassium or a sodium molecule.

So, for every 1,000 micrograms of cyanocobalamin, you might yield about 1 microgram of cyanide. This is, again, something you might do once a month or on the high end, once a week. But cyanide is usually toxic in the several milligrams. So, you have a very small amount that you're going to yield from your injection, which is maybe 1 microgram versus several milligrams on the side of an actual toxic or major detrimental effect. If you get up to like 100 milligrams, that's what's going to be lethal.

Some estimates would be a little bit less, but of course it does depend on your size, the administration of it, and other susceptibility factors that you might have.

All this to say that it's actually pretty hard to get a toxic effect from cyanocobalamin. Part of that is because only small amounts are yielded of cyanide for each injection, and it's also a very stable molecule. Now, you might say to yourself, "Well, I don't really care if it's a small amount of poison. I don't want any poison in my body," and that's definitely a fair assessment as well, especially if you're in a more compromised health state. Most of the time when our health is compromised, it's coming from problems within the mitochondria. In particular, with cytochrome C and other parts of the electron transport chain. Well, cyanide is particularly toxic to cytochrome C and the whole electron transport chain.
So if you're having health problems, you're probably having mitochondrial problems, and this is not going to help that situation. In addition, most of us do have the ability to eliminate cyanide in small amounts when there's small amounts at a time, and therefore it gets eliminated before it causes any problems.

But in people that are in a more compromised health state, part of the problem is with toxic overload. detoxification systems are taxed or overrun. The cells, tissues and mitochondria are not able to eliminate even small amounts of toxins like cyanide when they're already overloaded.

So in a case like that, even small amount of cyanocobalamin might be problematic for someone. So now back to our question, Is cyanocobalamin poisonous. Well, I think it depends on what we mean by poisonous. There's certainly no instance where I can think of where cyanocobalamin has an injection to support your overall B vitamin status would actually be lethal or majorly problematic to your health. There are certainly cases in patients where I would think adding something like this could be more compromising to their health because they have problems with their mitochondria or electron transport chain.

The dosage needed to treat B12 deficiency, how oral is better than injectable, why the cheaper form (cyanocobalamin) is superior to the more expensive form (methylcobalamin), why you can’t just take it in a multivitamin because of the analogs, and you shouldn’t just take it in a pill you just swallow (because it needs to mix with that saliva protein), and then with the toothpaste thing totally coming out of left field.

This video explains how Rosuvastatin (CRESTOR) belongs to a a group of drugs called HMG CoA reductase inhibitors, or "statins." Rosuvastatin is used together with diet to lower blood levels of "bad" cholesterol (low-density lipoprotein, or LDL), to increase levels of "good" cholesterol (high-density lipoprotein, or HDL), and to lower triglycerides (a type of fat in the blood).
Rosuvastatin is used in adults to slow the progression of atherosclerosis (a build-up of plaque in blood vessels that can block blood flow). It is also used to lower the risk of stroke, heart attack, and certain other heart complications in men and women who have coronary heart disease or other risk factors.

Cosentyx is an immunosuppressant that works by blocking the effects of a protein in the body called interleukin 17A that can cause inflammation. It is a monoclonal antibody and belongs to the drug class called interleukin inhibitors. Blocking the protein's activity reduces inflammation, pain, swelling, and any skin symptoms that you may have.

Cosentyx is given as an intravenous infusion by a healthcare provider, or as a subcutaneous (under-the-skin) injection. It is typically given monthly after the first starting doses.

The most common side effects of Cosentyx are diarrhea and cold symptoms, such as stuffy nose, sneezing, and sore throat. It also may increase your risk for infections.

What Is Cosentyx Used To Treat?

Cosentyx is one of the newer injectable medications on the market that's used to treat a number of different inflammatory-based conditions.

Those include Plaque Psoriasis, Psoriatic Arthritis, Ankylosing Spondylitis, and Axial Spondyloarthritis

Cosentyx MOA (Mechanism of Action)

I mentioned above that Cosentyx works by decreasing inflammation. Specifically, it does that by blocking interleukins.

The end result is a decrease in the release of cytokines and chemokines. These are substances that are involved in the inflammatory process.

But remember inflammation can be bad when there's too much of it. But it's also one of the first steps in your immune response which is a good thing.

So even though blocking interleukins sounds like a good thing in the late stages - it can also bring on some side effects which I’ll get to in a second.

How Is Cosentyx Dosed?

The dosing of Cosentyx changes a lot based off what you're treating.

But by and large you're going to see Cocentyx started with a loading dose and then moved to a maintenance dose.

It's a subcutaneous injection and the loading dose is usually 300 mg subcutaneously once a week for the first 5 weeks.

Then you move on to the maintenance dose which can vary from 150 mg to 300 mg every 4 weeks.

One of the downsides of Cosentyx is that to get the 300 mg dose requires two injections because the drug is only available in 150 milligram strength.

How Long Does It Take To Start Working?

This is a really good question and can vary from person to person.

But I like to use the 3 months rule of thumb. That means for most people you'll start noticing a difference by three months out.

In studies some people have noticed the difference is early as one month. Other people require more like 16 weeks before they see a difference it’s something you definitely have to be patient with.

What Are The Side Effects of Cosentyx?

The most common side effect you'll see with it by a long shot is increased infection rates.

This can happen in anywhere from 29% to 48% of people in studies. The types of infections can vary from the herpes virus, staph and upper respiratory tract infections.

Other less common side effects are going to be diarrhea in 3% - 4% of people. Headaches occur in around 2% of people.

It's important to remember that some people can actually see their cholesterol levels increase. But this only happens in about 2% of people.

The one thing I want to point out if you have Crohn's disease or ulcerative colitis Cosentyx can actually make those things worse. It's a very small chance that this will happen but it's something important for you and your doctor and all about.

What About Weight Gain or Hair Loss With Cosentyx?

In studies there weren’t any reports of weight gain or hair loss with Cosentyx. In fact, you're more likely to see weight loss because some of the side effects can be diarrhea and infections. So people are more likely to not be eating less when taking it.

Is It Safe to Take With My Other Medications?

A general rule of thumb here is that you don't want to take it with anything that can lower your immune system. Couple of examples would be cyclosporine or tacrolimus.

What About Monitoring?

Before your doctor starts you on this they're going to make sure to get a tuberculosis test done.

They're also going to keep an eye out for any signs or symptoms of irritable bowel disease.

Like a lot of other injectable medications Cosentyx can get pretty expensive - especially if you don't have good insurance.

Generic name: Methylphenidate.
Brand names: Ritalin and Concerta.
Available as a tablets, modified release tablets and capsules.

How to use it?
- Tablet: swallow it whole with half a glass of water. It is possible to chew on the tablet or break it.
- Modified release tablet or a capsule: also take it with water, but don’t break it and do not chew on it. This can decrease its effectivity.
In addition it is advised to take Methylphenidate during a meal, as this can decrease your chances on side effects.

When to use it?
Methylphenidate has a stimulating effect on the central nervous system. It has properties similar to amphetamines. It can be used to treat ADHD, narcolepsy and it can be used during palliative sedation.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience ,the severity of your symptoms and the brand of methylphenidate you might be using.Therefore always use the precise amount and frequency your doctor prescribed you.

- However, when treating ADHD symptoms with regular tablets in children older than 6 years or in adults: a usual starting dose would be 5mg 2x/day. If necessary this can be slowly increase up to a maximum dose of 60mg/day.
- Here it is important to note, that the dosages of modified release tablets and capsules may be different.
- When treating narcolepsy in adults, your doctor might prescribe a dose of 10mg/ 2-3 per day.

What are common the side-effects?
- Very common (10%): insomnia, nervousness, headache, palpitations, decreased appetite, nausea and a dry mouth.
- Common (1-10%): upper respiratory tract infection, shortness of breath, decreased appetite, weight loss, aggression, agitation, irritability, restlessness, anxiety, panic attack, stress, depression, decreased libido, erectile dysfunction, tics, tremor, mood swings, fast heart rate, high blood pressure , upset stomach, vomiting, diarrhoea, cough, sore throat, rash, itching, fever and fatigue.
Uncommon (0.1-1%): Hypersensitivity reactions, psychotic disorders, suicidal ideas, hallucinations, blurred vision, dry eyes, chest pain, hot flush, constipation, muscle twitching and muscle tension.
- Rare (0.01-0.1%): mania, libido disorder, disorientation, chest pain, visual impairment and menstrual disorder.

Safety:
First of all be careful to combine methylphenidate with alcohol. As this might increase its dazing effects.
In addition, it can be dangerous to drive while using this drug. As it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects than please do not drive. This is especially true for the first few weeks you are using it.
Furthermore, methylphenidate can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
Unfortunately, the effects of methylphenidate during pregnancy or while breastfeeding are not studied properly. Therefore it could be dangerous and its usages should be avoided. Always discuss this with your personal doctor, as there might be some exceptions.

PHARMACIST Fi shares 7 Top Tips to help you try to avoid colchicine toxicity side effects (such as diarrhea, nausea, vomiting, stomach or intestine area pain, throat pain, etc.) - which are thankfully much less common now that a lot of doctors are prescribing
*modern, smaller dose schedules for acute gout flare treatment,
*lower colchicine dose schedules for people with kidney or liver problems, and
*managing serious interactions with certain types of other medicines - by either stopping colchicine or significantly reducing the colchicine dose schedule (depending on which medicine is interacting with colchicine).

It is also important to remember to:
*avoid eating grapefruit and avoid drinking grapefruit juice while taking colchicine
*never take more colchicine than the doctor advises to take, and
*only take colchicine if it has been prescribed for you and don't give your colchicine to other people (because some people must not take colchicine due to some types of medical conditions, or potentially lethal medicine interactions with certain types of medicines).

If the doctor's instructions about how to take colchicine could be interpreted more than one way, or if you forget exactly how the doctor advised to take colchicine, as soon as possible, ask your doctor:
*how much colchicine to take per dose, and
*how much time to leave between colchicine doses, and
*whether or not there is a date that you need to stop taking colchicine (this will depend on whether the aim is to treat an acute gout flare, or to try to prevent future gout flares - because the colchicine dose schedules for these 2 different aims are likely to be quite different).

If you take colchicine for a medical condition other than gout, ask a doctor for advice about the info in this video or description section, as it may or may not be suitable for your medical condition.

so, if you take any of the following medicines, ask your doctor for advice before taking colchicine for gout, as you may need a colchicine dose reduction to manage the interaction
*while being treated with one of the following medicines, and
*also for 2 weeks after being treated with one of the following medicines
(needing a colchicine dose reduction to manage an interaction with one of the following medicines is even more likely if you have a kidney or liver problem; doctors can find info about colchicine dose reduction in medicine prescribing information or the Australian Medicines Handbook 2023):
azithromycin
boceprevir
carvedilol
everolimus
fluconazole (commonly used antifungal medicine)
fluvoxamine
glecaprevir with pibrentasvir
isavuconazole
ledipasvir
letermovir
osimertinib
propafenone
quinidine
ranolazine

Please note: medicines on the above list are not the only medicines which interact with colchicine in this way, so ask your doctor or pharmacist for a medicine interactions check.

Colchicine also has other types of interactions. Here are some examples of other interactions with colchicine (from Colgout brand colchicine product information for doctors):
*Alcohol increases risk of colchicine toxicity side effects like nausea, vomiting, diarrhoea, stomach or intestine pain, and increases gout flare risk by raising uric acid level
*Colchicine may increase risk of potential muscle side effects (like muscle tenderness or weakness) of some statin or fibrate medicines for cholesterol (e.g. simvastatin, atorvastatin, lovastatin, gemfibrozil, fenofibrate)
*Colchicine may increase risk of low blood cell production if taken with other medicines with this potential side effect
*Acidifying substances (like ammonium chloride, ascorbic acid (vitamin C), acid phosphates) may make colchicine less effective
*Alkalinizing substances (like sodium bicarbonate, potassium citrate) may increase the effect of colchicine
*Colchicine may increase the effect of medicines that make us sleepy, and alcohol
*Colchicine may increase the effect of adrenaline, dopamine, dobutamine, isoprenaline or ephedrine
*If colchicine is taken long-term, it may decrease absorption of vitamin B12, so ask your doctor for a blood test for vitamin B12 - to find out if you may need a vitamin B12 supplement
*The herbal medicine St John's Wort (Hypericum perforatum) may make colchicine less effective, by making our body get rid of colchicine quicker than usual; some types of medicines may interact with colchicine in this way

If you think you might have one of these interactions, ask a doctor for advice as soon as possible about whether any medicine changes may or may not be needed.
