Action:
inhibits calcium transport resulting in inhibition of excitation and contraction, leads to depression of AV and SA node leading to decreased HR, leads to vasodilatation and decreased blood pressure.
Therapeutic Class: antianginals, antiarrhythmics, antihypertensive
Pharmacologic Class: Ca Channel Blocker (Bezothiazepine)
Nursing Considerations:
contraindicated in 2nd and 3rd AV block
may cause arrhythmias, CHF, bradycardia, peripheral edema, gingival hyperplasia
increases digoxin levels
don't drink grapefruit juice
assess for signs of CHF
monitor EKG continuously
tell patient to change positions slowly
monitor serum potassium
instruct pt on how to take blood pressure

Actio:n alters the perception and reaction to pain by binding to opiate receptors in the CNS, also suppresses the cough reflex
Therapeutic Class: Opioid Analgesic, allergy, cold and cough remedies, antitussive
Pharmacologic Class: opioid agonist
Nursing Considerations:
Assess BP, respirations, and pulse before and during administration - medication causes general CNS depression
Narcan (nalaxone) is the antidote for overdose
use caution with concurrent use of MAOI - avoid use within 14 days of each other
may be used as an antitussive
advised to dilute with NS prior to administration and to administer slowly to decrease CNS depression

Heart failure is when the heart is unable to pump effectively, called systolic heart failure; or unable to fill properly, called diastolic heart failure. In both cases, blood output is reduced. Ejection fraction is reduced in systolic heart failure, but typically preserved in diastolic heart failure.
The pathophysiology of heart failure involves a vicious cycle in which reduced cardiac output, as a compensatory response, activates the renin-angiotensin-aldosterone system (RAAS) and sympathetic system. However, these systems cause vasoconstriction, increase heart rate and blood pressure, making it even harder for the heart to pump. Increased aldosterone level also promotes ventricular remodeling, myocardial scarring, and vascular injury, worsening the disease.
On the other hand, the natriuretic peptide system is also activated. This system is protective to the heart. It promotes vasodilation, sodium and water excretion, and inhibits cardiac remodeling.
Most drugs used in heart failure therapy aim to inhibit RAAS and sympathetic activities, and/or promote the natriuretic system. Other drugs increase ventricular contractility or reduce water retention – a major heart failure symptom.
First-line therapy for patients with reduced ejection fraction typically includes an angiotensin-converting enzyme (ACE) inhibitor, and a beta-blocker.
- ACE inhibitors block the conversion of angiotensin-I to angiotensin-II in RAAS, thereby inhibiting RAAS activity. Common side effects include dry cough, headache, and hypotension. Rarely, ACE inhibitors may cause a swelling reaction known as angioedema.
- Angiotensin receptor blockers (ARBs) inhibit the effects of angiotensin-II. Their mechanism of action is similar to that of ACE inhibitors, but they do not usually cause a cough.
- Beta-blockers decrease heart rate by binding to β1-adrenergic receptor in the heart and blocking the sympathetic influences that act through these receptors. Adverse effects: hypotension, bradycardia and AV blocks.
- Vasodilators reduce blood pressure and are usually used for patients who cannot tolerate ACE inhibitors or ARBs.
- Diuretics are often prescribed to relieve fluid retention. Loop diuretics are most powerful and typically used for most patients. Thiazides are less effective, but they also have a vasodilation effect, and are thus preferred for patients with hypertension but only mild fluid retention.
- Aldosterone receptor antagonists block the action of aldosterone. Because aldosterone’s primary function is to promote sodium and water retention, and potassium excretion; aldosterone antagonists act as potassium-sparing diuretics. However, their effect in heart failure treatment is also attributed to the inhibition of aldosterone’s damaging impact on the heart and blood vessels.
- Digoxin increases cardiac contractility by inhibiting the sodium-potassium pump, causing intracellular sodium concentration to rise. Digoxin is a medication used to manage and treat heart failure and certain arrhythmias, and abortion. It is in the cardiac glycoside class of drugs. This article outlines the indications, action, and contraindications for digoxin as a valuable agent in managing heart failure and certain arrhythmias
Digoxin is used to treat heart failure, usually along with other medications. It is also used to treat certain types of irregular heartbeat (such as chronic atrial fibrillation). Treating heart failure may help maintain your ability to walk and exercise and may improve the strength of your heart. Treating an irregular heartbeat can also improve your ability to exercise.Digoxin belongs to a class of medications called cardiac glycosides. It works by affecting certain minerals (sodium and potassium) inside heart cells. This reduces strain on the heart and helps it maintain a normal, steady, and strong heartbeat.
How to use Digoxin
Take this medication by mouth with or without food as directed by your doctor, usually once daily. If you are using the liquid form of this medication, carefully measure the dose using the dropper provided by the manufacturer. Do not use a household spoon because you may not get the correct dose.
Your body may not absorb this drug as well if you also eat foods that are high in fiber or if you take certain medications. Take this medication at least 2 hours before or after eating food products that are high in fiber (such as bran). If you are also taking cholestyramine, colestipol, or psyllium, wait at least 2 hours after taking your digoxin dose before taking any of these products. If you are taking antacids, kaolin-pectin, milk of magnesia, metoclopramide, sulfasalazine, or aminosalicylic acid, take them as far apart from your digoxin dose as possible. Ask your pharmacist if you are not sure when to take any of your medications.
The dosage of this medication is based on your medical condition, age, body weight, laboratory tests, and response to treatment.
Use this medication regularly to get the most benefit from it. To help you remember, use it at the same time each day. Do not stop taking this medication without consulting your doctor. Some conditions may become worse when the drug is suddenly stopped.

DIGOXIN SIDE EFFECTS
Digoxin is derived from foxglove (a flowering plant of the plant genus digitalis). According to the National Capital Poison Center, “all parts of the plant are poisonous, possibly even deadly, if swallowed.”
MECHANISM OF ACTION
Fundamentally, digoxin is a poison. It exerts its therapeutic effect by poisoning a cell membrane protein known as the sodium-potassium ATPase (Na+/K+ pump). When this pump is poisoned, sodium accumulates within the cell (the therapeutic target being cardiac muscle cells). Another cell membrane protein called the sodium-calcium exchanger normally exchanges intracellular calcium for extracellular sodium. However, when the concentration of sodium inside the cell is high (as is the case for someone taking digoxin), the sodium-calcium exchanger shuts down. The consequence? Calcium accumulates within the cardiac muscle cell, which causes the heart to contract more forcefully. This can be useful for patients with heart failure.
Digoxin also stimulates the vagus nerve, which has the effect of slowing the conduction of electrical impulses. For this reason, digoxin can also be used to treat atrial fibrillation.
SIDE EFFECTS
The probability that an individual will experience side effects from digoxin is roughly 1 in 5 (20%). Side effects tend to affect one of three systems: #1 cardiac conduction system (e.g., bradycardia, tachycardia, atrioventricular (AV) block, ventricular arrhythmia); #2 central nervous system (e.g., headache, weakness, dizziness, apathy, confusion, depression, anxiety, delirium, hallucinations); #3 gastrointestinal system (e.g., diarrhea, nausea/vomiting, abdominal pain, intestinal ischemia, intestinal hemorrhagic necrosis).
DIGOXIN TOXICITY
Digoxin is a “narrow therapeutic index drug,” meaning that the difference between a dose that will result in therapeutic failure and a dose that will cause life-threatening toxicity is very small. Somewhere in the middle (a very narrow window) is the therapeutic window. That’s what doctors are aiming for, and the thing is… it is slightly different in every patient. So, the doctor will consider the clinical response of the patient in conjunction with the serum concentration of digoxin to optimize the dose and avoid overt toxicity.
“Digoxin toxicity” is a condition characterized by nausea/vomiting, cardiovascular symptoms (e.g., cardiac arrhythmias), yellow-green visual color disturbances/halo effect, dizziness, and fatigue (Cummings et al.). The symptoms can vary from one patient to another. For instance, many elderly patients exhibit vague symptoms such as dizziness and fatigue, making the diagnosis more difficult. If toxicity is suspected, the physician will order a blood test to check the serum digoxin level. Digoxin toxicity is most likely to occur when the serum digoxin concentration exceeds 2.0 ng/mL.
DIGOXIN TOXICITY TREATMENTS
If digoxin toxicity does occur, treatment do exist. For instance, in the case of a large overdose, an antidote can be given (digoxin immune Fab), which is 80-90% effective in reversing digoxin toxicity. Despite the availability of this and other treatments, digoxin toxicity is a very serious condition. Consider the following quote: “When treated promptly, the outcomes are good, but any delay in treatment can lead to death” (Cummings et al.).
PROBABILITY OF DIGOXIN TOXICITY: 1-4%
In any given patient taking digoxin, the risk of digoxin toxicity is somewhere between 1 in 100 (1%) and 1 in 25 (4%). The risk rises with age. For instance, in those age greater than 40, the risk of toxicity is 1%; however, in those age greater than 85, the risk is closer to 4% (David et al.).

Today our video is regarding an oral medicine which is used to treat fungal infection. There are antifungal creams availaible in the market for fungal infection, but some time, creams are not effective so you have to take tablets in order to eliminate the infection
The medicine which we are talking about is Fluconazole, which comes under the brand name of diflucan, which is used to prevent and treat a variety of fungal and yeast infections. It belongs to a class of drugs called azole antifungals. It works by stopping the growth of certain types of fungus.
Fluconazole is used to treat many fungal infections caused by candida including:
Vaginal yeast infections caused by Candida, skin irritation on the head of the penis (balanitis), and fungal infection of the mouth (oral thrush)
infections in your blood or elsewhere in your body
Fluconazole is also used to treat a brain infection called cryptococcal meningitis. This is caused by a fungus called cryptococcus.

Fluconazole is available as capsules or a liquid .
for adults Fluconazole comes in 50mg, 150mg or 200mg. the doctor will prescribe you daily or on weekly basis based on your medical condition.
The liquid usually comes in 2 different strengths:
50mg fluconazole in a 5ml spoonful (50mg/5ml)
200mg fluconazole in a 5ml spoonful (200mg/5ml)
The dose for different condtion is given in chart, which dr will prescribed you after seeing your condtion.
oral (mouth) thrush – 50mg a day, taken for 7 to 14 days
vaginal thrush or balanitis – 150mg, taken as a single dose
vaginal thrush that keeps coming back – 150mg, taken once every 72 hours for the first 3 doses, then take 150mg once a week for 6 months
candida infections (in your blood or elsewhere in your body) – 200mg to 800mg a day for several weeks
cryptococcal meningitis – 200mg to 800mg a day for several weeks
to stop cryptococcal meningitis coming back – 200mg a day, taken long term
to prevent fungal infections if you have a weakened immune system (a low white blood cell count) – 50mg to 400mg a day, until your white blood cell count improves
side effects usually include nausea, diarrhea.
Also remember Fluconazole is usually not recommended in pregnancy

Guaifenesin (Mucinex, Robitussin) – Side Effects, Uses, Mechanism of Action, Dosage, Warnings
Guaifenesin, found in over-the-counter products like Mucinex and Robitussin, is an expectorant used to relieve chest congestion in conditions like colds and bronchitis. It works by increasing the volume and reducing the viscosity of secretions in the respiratory tract, aiding in mucus clearance. Common side effects include nausea, vomiting, drowsiness, dizziness, and rash. Dosages vary by age, with adults typically taking 200-400 mg every 4 hours, not exceeding 2.4 grams daily. Guaifenesin can interact with other cough medications, antihistamines, decongestants, and alcohol, which may increase the risk of side effects. Warnings include avoiding use if allergic, and consulting a healthcare provider if pregnant, breastfeeding, or having chronic respiratory conditions.

So what is DXM, or Dextromethorphan? DXM is commonly found in Robitussin, Dayquil and other over the counter cold and flu medicine. DXM can come in both gel tablet and syrup form. It is a cough suppressant ingredient made to help get over the cough. However, it also has hallucinogenic effects if you consume too much of it.

The effects of DXM last anywhere from one to two hours but feels a lot longer. Some of the effects or what you experience can include:
- Enhanced perception of colors
- Enhanced sound quality
- Hallucinogenic feelings similar to Psilocybin mushrooms
While DXM is not physically addictive, it can become mentally addictive much like other non physically addictive drugs and behaviors.
Dextromethorphan Addiction Symptoms:
- Frequently being high with limited motor skills
- Frequently purchasing or taking cold medicine
- Drinking large volumes of cold medicine all at once
- Coordination and balance issues that would be expected of someone drunk
- Consistent lethargy or slurred speech
- Rashes and skin problems
