Amphetamine is a stimulant drug that is used primarily in the treatment of ADHD. In this video, I discuss some of the proposed mechanisms by which amphetamine acts on the brain to produce its stimulant effects.

Welcome to 2 minute neuroscience, where I explain neuroscience topics in 2 minutes or less. In this installment I will discuss amphetamine.

Amphetamine is a stimulant drug used mainly in the treatment of ADHD. Several popular drugs, like Adderall, primarily contain amphetamine.

Although we don’t have a full understanding of the effects of amphetamine on the brain, there are some aspects of its pharmacology that are well established. Amphetamine binds to the transporter proteins for monoamines like dopamine, norepinephrine, and serotonin and then can be taken up into neurons via these transporter proteins. Once inside the neuron, amphetamine disrupts the storage of monoamines in synaptic vesicles. One way it does this is by inhibiting
a protein called vesicular monoamine transporter 2, or VMAT2, whose normal role is to transport monoamines into vesicles. Inhibition of VMAT2 leads to higher levels of these neurotransmitters in the neuron.

Through a mechanism or mechanisms that are still not fully understood, amphetamine is then able to cause the monoamine transporter proteins to run in reverse, leading to the increased release of monoamines---especially dopamine and norepinephrine---and increased levels of these neurotransmitters in the synaptic cleft. These elevated monoamine levels can have various effects on different parts of the central nervous system. Increased dopamine levels in the reward system, for example, may contribute to the reinforcing effects of amphetamine use.

Although the primary action of amphetamine is to cause the increased release of monoamines, there are various other mechanisms that may contribute to the effects of the drug. For example, amphetamine also competes with monoamines for reuptake into the neuron, in effect inhibiting reuptake. And it may inhibit the activity of monoamine oxidase, an enzyme that metabolizes monoamines. The effect of both of these actions is to additionally increase levels of monoamines in the neuron and the synaptic cleft.

Laryngopharyngeal Reflux (Silent Reflux) disease refers to abnormal reflux of gastric contents through the esophagus into the throat, which irritates and damages the pharyngeal mucosa, and causes corresponding symptoms.

Types of Proton Pump Inhibitors

Generally speaking, one PPI doesn't differ all that much from others. They all have similar mechanisms of action and similar rates of effectiveness. Currently approved PPIs include:

Prilosec (omeprazole)
Prevacid (lansoprazole)
Protonix (pantoprazole)
Aciphex (rabeprazole)
Nexium (esomeprazole)
Dexilant (dexlansoprazole)

When taken over the short term, most of the side effects associated with PPI use are mild and transient. The most common include constipation, diarrhea, flatulence, headache, upset stomach, nausea, and vomiting.

Increasing evidence suggests that long-term use may result in more serious problems. Among them:

It has been suggested that PPI use can interfere with the absorption of calcium, leading to bone fractures in certain cases. In response, the U.S. Food and Drug Administration issued warnings in 2011 advising that over-the-counter PPIs should be used for no longer than two weeks at a time for up to three treatments per year.

Long-term PPI use has also been associated with a slight increase in the risk of community-acquired pneumonia and Clostridium difficile infection.

A recent study shows a 20% to 50% higher risk of chronic kidney disease in PPI users.

How long do steroids stay in your system?
How long will it be detectable? If taken orally, steroids can show up in a urine test for up to 14 days. If injected, steroids can show up for up to 1 month.

How long is long term steroid use?
Treatment extending longer than three months is considered long term and results in the majority of severe side effects. When steroids are used for short durations of a few days or weeks, they are relatively safe.

What drugs should not be taken with steroids?
You should tell any doctor or dentist treating you that you are taking steroids. Some key drugs that interact with steroids include anticoagulants (such as warfarin), drugs for blood pressure, antiepileptics, antidiabetic drugs, antifungal drugs, bronchodilators (such as salbutamol) and diuretics.

Are steroids the same as corticosteroids?
Corticosteroids are a form of steroids used to treat swelling and inflammation from allergies, as well as allergic asthma. They're often referred to as steroids, but they're not the same types of products abused by some athletes. Corticosteroids may be used for a range of allergies.

Description
Neocilor is a preparation of Desloratadine. Desloratadine is a long-acting tricyclic histamine antagonist with selective H1-receptor antagonist activity. It is a non-sedating antihistamine. Desloratadine has effects on the chronic inflammatory response to allergens and lacks effects on the QT interval of ECG, unlike some other non-sedating antihistamines.

Indications
Allergic rhinitis: Neocilor is indicated for the relief of nasal and non-nasal symptoms of allergic rhinitis (seasonal and perennial).

Chronic idiopathic urticaria: Neocilor is indicated for the symptomatic relief of pruritus, reduction in the number of hives and size of hives, in patients with chronic idiopathic urticaria.

Dosage & Administration
As there are no food effects on the bioavailability, Neocilor may be taken without regard to meal.
Adults and adolescent of 12 years or over : 5 mg once daily.
Child 6-11 years : 2.5 mg once daily.
Child 12 months-5 years : 1.25 mg (2.5 ml Neocilor syrup) once daily.
Child 6 months-11 months : 1 mg (2 ml Neocilor syrup) once daily.
Patients with liver or renal impairment: A starting dose of one Neocilor tablet (Desloratadine 5 mg) every other day is recommended.
Pediatric use: The safety and effectiveness of Desloratadine in pediatric patients under 6 months of age has not been established.

Side Effects
Desloratadine is generally well tolerated. However, dry mouth, fatigue, somnolence and myalgia are commonly reported side-effects. Less common side-effects may include dizziness, headache and nausea. Rarely rash, pruritus and urticaria may occur.

Use in Pregnancy & Lactation
Pregnancy: There are no adequate and well-controlled studies in pregnant women. Because animal reproduction studies are not always predictive of human response, Desloratadine should be used in pregnancy only if clearly needed.
Lactation: Desloratadine passes into breast milk. Therefore, a decision should be made whether to discontinue nursing or to discontinue Desloratadine, taking into account the importance of the drug to the mother.

Drug Interaction
No clinically important drug interactions have been reported.

Over Dose
No clinically relevant adverse events have been reported in case of overdosage. However, in the event of overdosage, symptomatic and supportive treatment is recommended.

Generic name: Desloratadine.
Brand names: Aerius, Neoclarityn and Clarinex
Available as a tablet and drink.

How to use it?
Tablet: Swallow the tablet completely with half a glass of water. You could break the tablet if necessary.
Drink: Use a measuring cup or measuring spoon to measure the correct amount you need to take.
It is also best to use this medication on set times. This will make it less likely you will forget a dose.

Duration of treatment?
The usual treatment takes weeks to years. However if you are not experiencing any improvements after 3 days of use then you should contact your prescribing doctor. An other drug might be more suitable for you.

When to use it?
Desloratadine is an antihistamine. It can be used to treat an allergic reaction, like a rhinitis or hives.

What is the correct dosage?
Always use the dosage and frequency prescribed by your own doctor.
Usually doctors prescribe 5 mg a day for adults or a child older than 12 years.

What are the side-effects?
- Common (1-10%): headache, dry mouth and fatigue.
- Very rare (0.01%): hypersensitivity reaction, fast heart rate, dizziness, somnolence, nausea, vomiting and diarrhea.

Safety:
You can combine Desloratadine with any type of food, however be careful when you combine it with alcohol. As this might increase the dazing effect of Desloratadine.
In addition it is usually safe to drive while using this drug, however it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
Lastly Desloratadine can have interactions with other drugs. Therefore always discuss this with your treating physician.

Pregnancy and Lactation:
No harmful effects where found for the usage of Desloratadine while pregnant or breastfeeding. Which means Desloratadine can probably be used safely. To be sure always discuss this with your prescribing doctor.

My name is Melinda Curle. I was on Depakote throughout my teen age years. I would say that the side effects of Depakote were subtle. However, it is important to realize that I started slowly on anticonvulsants and increased my dosage over about two years. My body adjusted and the side effect of Depakote on me was mainly lethargy and responding slowly. My family noticed that I responded slower than the other siblings, but never said anything to me until my medication was switched and they thought I could handle the opinion that I was "slower." Side effects of Depakote on other people will vary and it is good to check with your doctors about them. Not everyone will want to deal with the side effects of Depakote. I certainly didn't. There were some things that improved my quality of life while I was on Depakote. I was a swimmer and getting extra exercise did seem to lessen the side effects of Depakote. Another thing that helped was that I didn't eat a ton of junk food. My parents just didn't have much around the house. The Depakote side effects were minimized by a healthier lifestyle.

_Benign_ rashes are very common (10%) in patients treated with Lamictal, particularly children and those patients on concomitant Depakote therapy, or those for whom the titration schedule was too aggressive, but the more deadly variety is actually quite rare, akin to the incidence of the same syndromes in response to other medications, medications which clinicians frequently use instead of Lamictal, ironically due to a concern about Stevens-Johnson.

Lamictal may actually be the best-tolerated mood stabilizer and, while it is not particularly effective at stabilizing acute mania, it has been approved by the FDA for bipolar maintenance treatment, and is particularly helpful in bipolar I depression (it has not been shown to be especially effective for other forms of depression, including unipolar major depression).
