Educational Pearls:

1. Bactrim (TMP-SMX) is a sulfa antibiotic used for a number of infections but can have untoward effects.
2. Hypoglycemia can be induced by the sulfa component of Bactrim, which binds receptors in the pancreas causing insulin to be secreted, dropping blood sugar levels.
- Oral diabetic medications, specifically sulfonylureas, when taken with Bactrim most often lead to these hypoglycemic episodes.
3. Hyperkalemia can be induced by Trimethoprim by blocking potassium excretion in the kidney, so Bactrim in combination with ACE inhibitors/ARBs/spironolactone can induce high potassium.
4. When prescribing Bactrim, remember to check home medications and if a patient is taking an above medication, check a baseline BGL and potassium.

Over 6 million adults in America take a prescription sleep aid and another unknown but potentially double or triple that number take an over the counter sleep aid.

One of the most common is diphenhydramine (also known as Benadryl or Unisom brand names). Diphenhydramine works by blocking histamine activity as well as acetylcholine activity which provokes a drowsy state.

Acetylcholine is like a nerve cell manager and when its activity is shut off the coordination between neurons and the ability to file memory’s and information in an orderly fashion to be retrieved easily at a moments notice is broken.

Now consuming a diphenhydramine-based drug occasionally every 3-6 months isn’t likely to have any negative long term implications.

However, taken regularly like the majority of the patients I see coming in using them, can spell disaster for your cognition and memory long term.
