Transcript:

DR. RODGERS:

This drug Hydroxyurea – that was really the basis of our work in the late 80’s and early 90’s.

There was a group in the Heart, Lung, and Blood Institute who started working on this using a drug called 5-Azacytidine.

Just like all chemotherapy, it first causes some toxicity to the rapidly dividing cells, and then when you withdraw the drug and you give the cells a chance to recover maybe that’s what’s turning on the genes. And that led us to actually consider this drug Hydroxyurea because it’s a form of chemotherapy. It works on rapidly dividing cells. It doesn’t have the same toxicity profile that drugs like 5-Azacytidine does.

Using this drug Hydroxyurea in about 70 percent of hospitalized patients we could turn on their fetal hemoglobin to modest levels and the longer you gave it to them, the higher their values would go. Subsequently our work was replicated by groups in Seattle and Harvard and Johns Hopkins, and that led the NIH to fund this fairly large study, a cooperative randomized clinical trial, which tests whether the drug would in fact have a benefit.

And just to cut to the chase, the study was stopped early, a year early, because one group, which was blinded at the time, was doing much much, better than the other group, and that group was the Hydroxyurea group. In 1990, our paper came out. In 1995, this NIH-supported trial, this large randomized trial, showed that Hydroxyurea was quite beneficial. And in 1998, the FDA approved the drug as the first drug that was approved specifically to treat patients with sickle cell disease.

Febuxostat - how it lowers blood URIC ACID levels & precautions | Febuxostat tablet | Dr GPV Subbaiah | Spine surgeon

If you're taking Febuxostat tablets, there are a few precautions you should take.

First, Febuxostat tablets can cause serious allergic reactions. If you experience hives, difficulty breathing, or swelling of the face, throat, or tongue, stop taking it and contact a medical professional immediately.

Second, Febuxostat should be taken at the same time and in the same way each day. It's important to take Febuxostat exactly as prescribed by your doctor to prevent it from accumulating in your body.

Third, you should avoid drinking alcohol while taking Febuxostat since it can increase your risk of liver damage.

Fourth, Febuxostat can interact with other medicines, so it's important to tell your doctor about all the medications you're taking before starting it.

Finally, Febuxostat can cause severe joint pain and swelling. If you experience these symptoms, contact your healthcare provider right away.

These are just a few of the precautions to take when taking Febuxostat tablets. Be sure to talk to your healthcare provider to get the full list of precautions to keep in mind before starting a course of treatment.

Thanks for watching.

Long Term Effects of Taking GERD Medications | Dr. Joshua T. Taylor, MD, FACS
Long Term Effects of Taking GERD Medications | Dr. Joshua T. Taylor, MD, FACS Side Effects 2 Views • 2 months ago

Are there any long term effects of taking GERD medications?

With the use of PPIs and H2 blockers, typically a 2 week course is what is recommended. Certainly by the FDA in terms of treatment for GERD symptoms and then stopping that. Most of these medications now have become over-the-counter. So that is the regimen that would typically be recommended on these over-the-counter PPIs or H2 blockers - is a 2 week treatment & then coming off of them to see if the GERD symptoms have resolved. If they have not resolved, typically, that would then require a patient to be under a doctor's treatment for a prolonged treatment with these medications.

There certainly are risks with taking these medications long term. Women are more at risk for developing things like osteoporosis, because calcium absorption can be affected by that. There's a little bit of a higher risk of developing things like infection, especially lung infections. And there are some concerns about developing the risk of other cancers long term as well.

It's an individual treatment plan for each patient with the physician. But that should really be under a physician's care.

Ezetimibe - Cholesterol Levels, Side Effects, and Mechanism
Ezetimibe - Cholesterol Levels, Side Effects, and Mechanism Side Effects 2 Views • 2 months ago

Ezetimibe is a medication primarily used to lower blood cholesterol levels. It works by inhibiting the absorption of dietary cholesterol from the small intestine, which ultimately reduces the amount of cholesterol that enters the bloodstream. This makes it useful for managing high cholesterol levels, particularly when dietary and lifestyle changes alone are not sufficient to bring cholesterol levels under control.

Key points about Ezetimibe include:

1. **Mechanism of Action:** Ezetimibe works by inhibiting the Niemann-Pick C1-Like 1 (NPC1L1) protein in the small intestine. This protein is responsible for the absorption of dietary cholesterol. By blocking this protein, Ezetimibe reduces the absorption of cholesterol from the diet, leading to decreased levels of cholesterol in the bloodstream.

2. **Combination Therapy:** Ezetimibe is often prescribed in combination with other cholesterol-lowering medications, such as statins. This combination can be more effective in reducing cholesterol levels compared to using either medication alone. Statins work by blocking an enzyme involved in cholesterol synthesis in the liver.

3. **Indications:** Ezetimibe is primarily used to treat hypercholesterolemia, a condition characterized by elevated levels of cholesterol in the blood. It can be used as a monotherapy or in combination with other medications to achieve better cholesterol control.

4. **Dosage and Administration:** Ezetimibe is typically taken orally in tablet form. The recommended dosage and frequency will depend on individual factors, including the patient's cholesterol levels and overall health. It can be taken with or without food.

5. **Side Effects:** Common side effects of Ezetimibe may include headache, diarrhea, abdominal pain, and fatigue. Serious side effects are rare but may include muscle pain or weakness (myopathy) and liver problems. It's important to monitor for any unusual symptoms and report them to a healthcare provider.

6. **Precautions and Contraindications:** Ezetimibe should be used with caution in individuals with liver disease or a history of liver problems. Pregnant or breastfeeding women should also consult their healthcare provider before taking this medication. Additionally, it's important to let your doctor know about any other medications or supplements you're taking to avoid potential interactions.

7. **Effectiveness:** Clinical studies have shown that Ezetimibe can effectively lower LDL cholesterol levels, which is the "bad" type of cholesterol associated with an increased risk of heart disease. By reducing LDL cholesterol levels, Ezetimibe can contribute to the overall reduction of cardiovascular risk.

As with any medication, it's essential to take Ezetimibe as prescribed by a healthcare professional and to follow their recommendations for cholesterol management, which may also include lifestyle changes such as a healthy diet, regular exercise, and smoking cessation. Always consult a medical professional for personalized advice and guidance on managing your cholesterol levels and overall cardiovascular health.

If you have been prescribed Ozempic, the brand name for semaglutide, you should have received training from your healthcare professional on how to give yourself the injection safely. In this video, we will review step-by–step how to give yourself an Ozempic injection using the pre-filled pen.

Before using Ozempic, review the Instructions for Use provided with your prescription. When you are ready to inject your dose, gather your materials. You will need: Your Ozempic pen, a new NovoFine Plus or NovoFine needle, a piece of gauze or a cotton ball, an alcohol swab, and a sharps container.

Now that you are ready to inject your dose, wash your hands and let’s review the steps on how to inject your Ozempic pen.

* Step 1: Check your pen to make sure the medication isn’t expired, frozen, or cloudy. It should look clear and colorless, and shouldn’t have particles in it.
* Step 2: Attach a new needle by removing the paper tab, pushing it onto the pen, and turning it until it is tight. Remove the outer cap and set it aside. Then, remove the inner cap and throw it away.
* Step 3: Before the first injection with each new pen, check the flow. Refer to the full instructions to do this.
* Step 4: Turn the dose selector until the dose counter shows the dose you have been prescribed.
* Step 5: Choose your injection site. You can inject it into your stomach, 2 inches away from your belly button, or into your thigh or upper arm. Make sure to change your injection site each week. Clean your chosen injection site with an alcohol swab and let it dry.
* Step 6: Insert the needle into your skin, making sure you can see the dose counter. Press and hold the dose button until the counter shows 0. You might hear or feel a click. Continue pressing the button and count for 6 seconds.
* Step 7: Remove the pen from your skin and release the dose button. Carefully remove the needle and place it directly into a sharps container. Place the cap on your pen.
* Step 8: If you have any bleeding at the injection site, you can press a cotton ball or gauze over the injection site until the bleeding has stopped.

If you have any questions about how to inject Ozempic, contact your healthcare provider or the manufacturer, Novo Nordisk.

The Science Behind Bumetanide: Exploring Its Mechanism of Action and Therapeutic Applications
The Science Behind Bumetanide: Exploring Its Mechanism of Action and Therapeutic Applications Side Effects 2 Views • 2 months ago

How does bumetanide work?
Bumetanide belongs to a class of drugs known as loop diuretics, which work by inhibiting the sodium-potassium-chloride (NKCC2) co-transporter in the thick ascending limb of the loop of Henle in the kidneys. This mechanism of action prevents the reabsorption of sodium, chloride, and potassium, leading to increased urinary excretion of these ions. By promoting diuresis, bumetanide effectively reduces the volume of extracellular fluid, making it a valuable tool in the management of edema and hypertension.
Furthermore, bumetanide's ability to inhibit the NKCC2 co-transporter has also been found to have implications beyond its diuretic effects. Research has shown that this mechanism of action can impact various physiological processes, including brain function and development. This has opened up new avenues for exploring the therapeutic potential of bumetanide in the treatment of neurological disorders.
Bumetanide's dual role as a diuretic and a modulator of brain function has sparked interest in its applications beyond traditional uses. Let's explore some of the exciting therapeutic possibilities that have emerged.
The role of bumetanide in treating edema
As a potent diuretic, bumetanide is highly effective in the treatment of edema, a condition characterized by the accumulation of fluid in the body's tissues. Edema can result from various factors, including heart failure, liver disease, or kidney dysfunction. By increasing the excretion of sodium and water, bumetanide helps reduce the swelling and discomfort associated with edema.
Bumetanide's diuretic effect is achieved by blocking the NKCC2 co-transporter in the loop of Henle, preventing the reabsorption of sodium and chloride ions. This results in increased urinary output and subsequent reduction in extracellular fluid volume. The rapid onset of action and strong diuretic properties make bumetanide a valuable tool in managing edema in patients with diverse underlying conditions.
In addition to its diuretic effects, research has uncovered potential benefits of bumetanide in the treatment of neurological disorders. Let's delve into the exciting findings that have emerged in this field.
Bumetanide as a diuretic
Bumetanide's diuretic properties have been well-established for decades, making it a reliable choice for managing conditions such as edema and hypertension. The drug's ability to inhibit the NKCC2 co-transporter in the loop of Henle leads to increased urinary excretion of sodium and chloride, resulting in reduced extracellular fluid volume.
The efficacy of bumetanide as a diuretic has been demonstrated in numerous studies. For instance, a randomized controlled trial involving patients with congestive heart failure showed that bumetanide was superior to other diuretics in terms of weight loss and symptom improvement. Similarly, in the management of hypertension, bumetanide has been shown to effectively reduce blood pressure levels.
However, it is important to note that bumetanide, like other diuretics, can have side effects. These include electrolyte imbalances, dehydration, and ototoxicity. Therefore, careful monitoring and appropriate dosage adjustments are crucial in ensuring the safe and effective use of bumetanide as a diuretic.
Bumetanide's potential in epilepsy treatment
In recent years, researchers have discovered that bumetanide's ability to modulate ion transport in the brain may have profound implications for the treatment of epilepsy. Epilepsy is a neurological disorder characterized by recurrent seizures, which result from abnormal electrical activity in the brain. Current antiepileptic drugs primarily target synaptic transmission, but their efficacy is limited in many patients.
Bumetanide offers a novel approach to epilepsy treatment by targeting the imbalance of chloride ions in the brain. In normal brain development, there is a switch in the concentration of chloride ions from high to low inside neurons, which is crucial for the inhibitory actions of neurotransmitters like gamma-aminobutyric acid (GABA).

How To Use Propranolol? - Use, Dose, Side Effects - Doctor Explains Side Effects 2 Views • 2 months ago

In this video we will cover everything you need to know about Propranolol. We will discuss how and when to use it, we will cover its side effects, dosage and much, much more.

You can find more useful videos on cardiovascular diseases and their treatments here:
https://www.youtube.com/playli....st?list=PLaG3Bo3L0gX

Generic name: Propranolol.
Brand names: Propranolol.
Available as a tablet, delayed release capsule and as a drink.

How to use it?
- Tablet: Just swallow it with half a glass of water.
- Capsule: make sure to swallow it whole with water. Don’t chew on the capsule and don’t break it, as this could decrease its effectivity.
- Drink: measure the precise amount you need to take with measuring cup. Take this amount. Afterwards rinse the cup with some water and drink this water as well.

When to use it?
Propranolol is part of a group of drugs called beta blockers. These reduce the workload on your heart and widen your blood vessels, causing your heart to beat slower and with less force. Therefore Propranolol can be used to treat; a high blood pressure; several types of heart problems (like arrhytmias and angina pectoris), thyroid problems, migraine, tremor and anxiety.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating a high blood pressure in adults, most doctors will start with an initial dose of 40mg 2-3x/day (when taking tablets) or 80mg 1x per/day (when taking capsules). Which if necessary can be slowly increased up to a max. dose of 320 mg/day.

What are the side-effects?
- Common (1-10%): slow heart rate, shortness of breath, nightmares, cold hands/feet and fatigue.
- Uncommon (0.1-1%): diarrhoea, nausea and vomiting.
- Rare (0.01-0.1%): changes in your blood values, worsening of heart failure, low blood pressure, hallucinations, fainting, psychoses, mood swings, confusion, amnesia, dizziness, visual disturbances, dry eyes and skin rash.
- Very rare (0.01%): aggravation of myasthenia gravis, myasthenia gravis-like syndrome, alteration in fat metabolism and low blood sugar.

Safety:
- The first days of using propranolol it can lead to dizziness or fatigue. Please do not drive while experiencing this.
- Furthermore, alcohol widens the blood vessels. It can therefore increase the side effect of dizziness at the beginning of treatment. Try drinking alcohol in moderation first so you can estimate its effects.
- In addition smoking speeds up the breakdown of this drug. If you stop smoking, the amount of this drug in the blood may increase. This can make it stronger or cause side effects.
- Propranolol can also have interactions with other drugs your are taking. Always discuss this with your prescribing doctor.
- Lastly, propranolol could be dangerous for pregnant women and women who are breastfeeding. Therefore always discuss this with your personal doctor.

Amoxicillin and Clavulanate Potassium Augmentin Understanding the Uses and Potential Side Effects
Amoxicillin and Clavulanate Potassium Augmentin Understanding the Uses and Potential Side Effects Side Effects 2 Views • 2 months ago

Amoxicillin and clavulanate potassium, commonly known as Augmentin, is a combination antibiotic medication that is widely used to treat a broad range of bacterial infections. The combination of amoxicillin, a penicillin-type antibiotic, and clavulanate potassium, a beta-lactamase inhibitor, provide a stronger and broader spectrum of coverage than amoxicillin alone, making it a very effective treatment option for certain types of infections that may be resistant to other antibiotics.
One of the main indications of amoxicillin and clavulanate potassium is for the treatment of respiratory tract infections, such as bronchitis and pneumonia. It is also commonly used to treat urinary tract infections, skin infections, and some dental infections. Additionally, it is often prescribed as a prophylactic treatment for people who are at a high risk of developing certain types of bacterial infections.
Amoxicillin and clavulanate potassium is usually administered orally, as a tablet or a liquid suspension. The typical adult dose ranges from 250mg to 500mg, taken two to three times per day, depending on the severity of the infection. The duration of treatment depends on the type and severity of the infection, but typically lasts for 7 to 10 days.
Like all medications, amoxicillin and clavulanate potassium also has some side effects. The most common side effects include gastrointestinal distress, such as nausea, vomiting, and diarrhea. Allergic reactions, such as hives, difficulty breathing, or swelling of the face, lips, tongue, or throat, can also occur in rare cases. It's important to speak with your healthcare provider if you experience any unusual symptoms while taking the medication.
It's also important to note that amoxicillin and clavulanate potassium can interact with certain other medications, such as blood thinners, and some pain relievers, so it's crucial to inform your healthcare provider of all medications you are taking prior to starting treatment with amoxicillin and clavulanate potassium.
In conclusion, amoxicillin and clavulanate potassium is a safe and effective antibiotic combination that can be used to treat a wide range of bacterial infections, particularly those that may be resistant to other antibiotics. However, as with any medication, it is important to be aware of the potential side effects and to speak with your healthcare provider if you experience any unusual symptoms while taking the medication. It is also important to take the medication as prescribed by your healthcare provider and to keep all appointments with them to monitor your progress.

Losartan Side Effects (& Why They Occur)

Losartan (also known as Cozaar) is an angiotensin-receptor blocker medication used to treat hypertension (high blood pressure) and diabetic nephropathy. Losartan-induced blockade of angiotensin receptors leads to reduced angiotensin II (Ang II) effects, to include a reduction in vasoconstriction, and Ang II-mediated release of catecholamines from the adrenal glands among other effects. However, Losartan can lead to mild and severe side effects, including issues with the kidneys, blood and blood sugar levels.

LISINOPRIL SIDE EFFECTS
Lisinopril, marketed under the brand names Prinivil® and Zestril®, belongs to a group of drugs called “ACE inhibitors,” which work by inhibiting angiotensin-converting enzyme (ACE). ACE is responsible for converting the hormone angiotensin I to angiotensin II. Reducing the production of angiotensin II lowers blood pressure because angiotensin II is a potent vasoconstrictor. In other words, angiotensin II causes blood vessels to constrict. Much like pinching a garden hose increases the pressure of the water inside the hose, constricting a blood vessel increases blood pressure. Continue reading to learn more about lisinopril side effects from a pharmacist.

Furthermore, angiotensin II has the downstream effect of triggering aldosterone secretion from the adrenal glands. Aldosterone, a mineralocorticoid, promotes the retention of sodium, which ultimately leads to fluid retention and increases blood pressure. Therefore, by inhibiting ACE, we reduce blood pressure by two mechanisms (a reduction in the production of both angiotensin II and aldosterone).

PROBABILITY OF SIDE EFFECTS
The probability that a person will experience side effects while taking lisinopril is approximately 15.5%. In other words, one person will experience one or more side effects for every six people taking lisinopril.

DRY COUGH
Most of the side effects of lisinopril stem directly from lisinopril’s mechanism of action. For example, the most common side effect of lisinopril, a dry cough (also sometimes called a “nonproductive cough”), stems from the fact that lisinopril inhibits another enzyme called kininase II which is identical to ACE. Since kininase II is responsible for the breakdown of bradykinin, the consequence of inhibiting kininase II is the accumulation of bradykinin, which triggers a dry cough in around 4% of people who take lisinopril.

HYPERKALEMIA
A potentially serious and yet relatively common side effect of lisinopril is hyperkalemia (high potassium in the blood). Much like a dry cough, hyperkalemia is also a direct consequence of lisinopril’s mechanism of action. When the production of angiotensin II is reduced, as is the case for those taking lisinopril, the amount of aldosterone secreted by the adrenal glands is also reduced. Aldosterone is like a signal to the kidneys to retain sodium in the blood at the expense of potassium. In other words, potassium is removed from the blood and placed into the urine in exchange for sodium, which is removed from the fluid that will become urine and directed back into the bloodstream. Since lisinopril reduces aldosterone secretion, there is a relative decrease in blood sodium levels and a corresponding increase in blood potassium levels.

A normal serum potassium level would be something in the ballpark of 3.5-5.0 mEq/L. The average patient who takes lisinopril will experience an increase in serum potassium of only 0.1 mEq/L; however, 6% of patients with normal kidney function will experience an exaggerated rise in serum potassium levels (i.e., greater than 0.5 mEq/L). For this reason, healthcare providers must monitor electrolyte levels (including potassium levels) in patients taking lisinopril.

The probability of hyperkalemia in patients taking lisinopril ranges from 2.2-4.8% on average.

HYPOTENSION
Hypotension (low blood pressure) is defined as a blood pressure reading of less than 90/60 mmHg. The lack of adequate blood flow that results from hypotension can damage vital organs. Additionally, fainting can also occur, which can lead to head injuries and broken bones. In uncomplicated patients taking lisinopril alone for blood pressure control, the risk of hypotension is about 0.8%. Meanwhile, complicated patients (e.g., those with other heart disease and kidney disease) taking lisinopril, especially when taking lisinopril combined with other blood pressure-lowering medication(s), have a roughly 11% risk of developing hypotension.

Understanding Tramadol : Uses, Dosage, Side Effects, and More

Hello friends, in this video, we will be discussing #tramadol a type of pain-killer. We'll cover how Tramadol works, the types of pain it can be used for, whether it can cure my problem, if it can potentially harm my kidneys or liver, the correct dosage, and the potential side effects.
Tramadol belongs to the opioid class of drugs. This medication is suitable for various types of pain, including acute pain, chronic pain, arthritis, cancer pain, and nerve pain. It is primarily used for moderate pain and is comparatively milder than morphine. Tramadol assists in relieving pain. When taken in the recommended dosage, it does not adversely affect the kidneys or liver. In fact, the dosage can be adjusted in case of kidney or liver disease. The maximum allowable dosage of Tramadol is 400 mg/day. The dosage also depends on whether it's short-acting, long-acting, or an extended-release preparation.

Common side effects of Tramadol include nausea, vomiting, constipation, dizziness, drowsiness, restlessness, poor concentration, itching, dry mouth, and headaches. Tramadol should be used with caution in patients with epilepsy. With regular use, Tramadol can lead to dependence, tolerance, or withdrawal symptoms.

The Science behind Spironolactone | Dr Davin Lim
The Science behind Spironolactone | Dr Davin Lim Side Effects 2 Views • 2 months ago

Big thankyou to my colleague, Eshini, for helping me out with this video!
Find her on instagram - @dr.esh.perera

What is the science behind spironolactone?

Spironolactone is an anti-male hormone (anti-androgen) medication. It blocks the male hormone receptor and reduces the level of the male hormones, testosterone and DHEAS. This tablet was first used as a diuretic (“fluid tablet”) effect and increases urine production. This medication lowers blood pressure and reduces fluid retention.

Spironolactone is an oral medication & not a cream because androgens (the target for spironolactone) is found circulating in the bloodstream.
.
.
What skin conditions can be treated with spironolactone?

Hormonal acne: It’s the impact spironolactone has on the hormones that give it its acne fighting reputation. That’s because it blocks the effect that androgens have on oil glands. This medication is an alternative to Accutane or Oratane.

Hair loss in women: Due to its impact on hormone regulation, the drug has recently been prescribed for women experiencing hair loss, also known as androgenic alopecia.

Excessive unwanted hair: On the other hand excessive hair known as hirsutism, commonly seen in PCOS can be treated with spironolactone.

Oily skin: can also be effectively treated with this medication. Most patients only require a very small dose of between 50 to 75 mg.

To see if you are a good candidate for this medication, book in with one of the medical dermatologists @cutis_dermatology.

*Disclaimer: I am a procedural dermatologist, I do not treat medical conditions such as hair loss, acne or other conditions that require prescriptions/tablets.

Nonetheless, I am sure every single one of us has experienced nausea, and/or vomiting on many occasions over the course of our lives. The problem with nausea is it is like fatigue, it is nondescript and could have countless causes, everything from early pregnancy to a viral infection and everything in between.

Nausea is also a common side effect of the GLP-1 RAs medications used for diabetes and obesity.

⚠️ Are you experiencing nausea due to your medication? Let me know your thoughts in the comments!
