Melatonin - What Is Melatonin Explained - Benefits, Side Effects Of Melatonin
Melatonin - What Is Melatonin Explained - Benefits, Side Effects Of Melatonin Side Effects 2 Views • 2 months ago

In this video we discuss what is melatonin, how our body uses it in different ways, and melatonin as a supplement. We also cover how it affects sleep.

Transcript/notes
What is melatonin? Well, melatonin is actually a hormone that is increased by the brain in response to darkness, and decreases in the morning as it gets lighter outside.

Melatonin works with the body’s natural circadian rhythm. The circadian rhythm is the body’s internal clock, which signals when it is time to sleep, time to wake, and time to eat.

In some people, low melatonin levels can be caused by alcohol or caffeine consumption, smoking, aging, some medications and exposure to high levels of light at night from certain electronic devices.

Taking a melatonin supplement on a short term basis, 30 minutes to an hour before bedtime can help some people fall asleep and can also help increase sleep duration. 1 to 5 milligrams generally appears to be effective but varies greatly from person to person and it is typically smarter to start out with a lower dosage, but it is always best to consult your doctor before taking any type of sleep aide.

Melatonin also plays a role in body temperature regulation, blood pressure regulation, and blood glucose regulation.

Some of the reported side effects of melatonin include headache, dizziness, nausea and sleepiness. And many sources recommend not taking a melatonin supplement if you are pregnant or breastfeeding, have an autoimmune disorder, have diabetes or high blood pressure and if you are taking a medication, check with your pharmacist.

The bottom line is that for some people, taking a low dose of melatonin, over a short period of time to help with sleep may be beneficial.

Antidepressant mechanism - One hypothesis for the pathophysiology of depression is that it is due to low levels of monoamine neurotransmitters (mainly serotonin, norepinephrine and dopamine). That is why antidepressants aim to increase the levels of these neurotransmitters in the synaptic cleft. They do this by slowing the reuptake of the neurotransmitters so that they stay in the cleft longer and interact with post synaptic receptors more often. The first drugs in this group were non-specific and increased all of the monoamines, which lead to lots of side effects and safety issues related to toxicity. Newer antidepressants are more selective and mostly only effect 1 or 2 monoamines.

General principles:
Unfortunately, antidepressants take at least a month to start working. Good patient education about the delayed onset of effect and close monitoring of the patient during this initial period is extremely important. Patients can become hopeless if they expect the drug to start working right away. This may be one reason why antidepressants are associated with suicide, especially in patients 25 years old and younger. Another proposed mechanism is that a depressed person may have the energy to carry out their suicide once the medications start to work. There is now a black box warning for suicide on antidepressants. Some psychiatrists argue that they don’t actually see this association with suicide in clinical practice, and that the thing that really increases the risk for suicide is not treating a depressed person with the proper medications. However, it is still standard practice to have a close follow up with patients you are starting on antidepressants. Usually this will involve a follow up visit about 2 weeks after the medication is started. At this visit the drug will not have started working yet so you can’t evaluate efficacy, but you can monitor for side effects like suicidality. Another serious side effect you have to be on the lookout for soon after initiating treatment is mania. If a bipolar individual is incorrectly diagnosed as having depression, an antidepressant may induce a manic episode.

Another very serious side effect that has to be considered for antidepressants is Serotonin Syndrome. This usually occurs when you combined multiple antidepressants at the same time or combine an antidepressant with another medication that increases serotonin such as dextromethorphan or an opioid. It presents with tremor, diaphoresis, tachycardia, flushing and hypertension. If not corrected it can progress to delirium, AMS and death. Treatment includes medication cessation and the use of Cyproheptadine (a serotonin antagonist). In order to prevent this from happening you should have about a month “Wash Out” period when you are switching between antidepressants. So you taper the 1st medication down and then stop it, give the patient at least a month with no antidepressant and then start adding the new medication slowly.

Most side effects begin immediately after starting the medication, but diminish over the course of a month. This is another reason why patient compliance is poor with these meds. It makes them sick and the drug doesn’t work during the first few weeks. However, if they can stick with it the medications will likely start working and the side effects will diminish over time. A principle that applies to all of the antidepressants is “start low and go slow.” This means that you start with a lower dose and slowly increase it in order to decrease side effects and increase patient compliance. The dose you start the patient on may not even be at a therapeutic level, but every month or so you can increase the dose a bit.

Dulaglutide: The Game-Changing Diabetes Treatment - How it Works, Uses, and Side Effects Explained
Dulaglutide: The Game-Changing Diabetes Treatment - How it Works, Uses, and Side Effects Explained Side Effects 1 Views • 1 month ago

What is Dulaglutide and how does it work?.
Dulaglutide belongs to a class of medications known as GLP-1 receptor agonists. It mimics the action of a hormone called glucagon-like peptide-1 (GLP-1) in the body. GLP-1 is produced naturally in the intestines and plays a crucial role in regulating blood sugar levels.
When you inject dulaglutide, it activates GLP-1 receptors in the pancreas, leading to increased insulin release. Insulin is a hormone that helps regulate blood sugar levels by allowing glucose to enter cells and be used as energy. By stimulating the release of insulin, dulaglutide helps lower blood sugar levels.
In addition to its effects on insulin release, dulaglutide also reduces the production of glucose in the liver. This helps prevent excessive glucose buildup in the bloodstream and further contributes to improved glycemic control.
Furthermore, dulaglutide slows down the emptying of the stomach and reduces the absorption of sugar from the gut. By doing so, it helps prevent sudden spikes in blood sugar levels after meals, promoting more stable glucose control throughout the day.
Uses of Dulaglutide in diabetes management.
Dulaglutide is primarily used for the management of type 2 diabetes. It can be prescribed as a monotherapy, meaning it is used alone, or in combination with other diabetes medications such as metformin or insulin.
The goal of using dulaglutide is to achieve and maintain optimal blood sugar control. By keeping blood sugar levels within a target range, individuals with diabetes can reduce the risk of long-term complications such as heart disease, kidney damage, and nerve damage.
In addition to its blood sugar-lowering effects, dulaglutide has also been shown to promote weight loss in some individuals. This can be particularly beneficial for individuals with type 2 diabetes who are overweight or obese, as weight loss can further improve glycemic control and reduce the risk of complications.
It's important to note that dulaglutide is not recommended for the treatment of type 1 diabetes or diabetic ketoacidosis. It is specifically designed for individuals with type 2 diabetes who have not achieved adequate blood sugar control with diet and exercise alone.
Effectiveness of Dulaglutide in controlling blood sugar levels.
Numerous clinical trials have demonstrated the effectiveness of dulaglutide in controlling blood sugar levels in individuals with type 2 diabetes.
In a study published in the New England Journal of Medicine, researchers compared the effects of dulaglutide to placebo in over 9,000 patients with type 2 diabetes. The study found that dulaglutide significantly reduced hemoglobin A1c levels, a measure of average blood sugar control over a period of time, compared to placebo. Additionally, more patients in the dulaglutide group achieved the recommended target A1c levels compared to the placebo group.
Another study published in Diabetes Care compared dulaglutide to exenatide, another GLP-1 receptor agonist, in individuals with type 2 diabetes. The study found that dulaglutide was more effective at reducing A1c levels and promoting weight loss compared to exenatide.
Overall, these studies and others have consistently shown that dulaglutide is an effective treatment option for individuals with type 2 diabetes, providing significant improvements in blood sugar control and weight management.
Potential side effects of Dulaglutide.
Like any medication, dulaglutide can cause side effects in some individuals. It's important to be aware of these potential side effects and discuss them with your healthcare provider before starting treatment.
Common side effects of dulaglutide may include nausea, vomiting, diarrhea, and stomach pain. These side effects are usually mild and tend to improve over time as your body adjusts to the medication. If these side effects persist or worsen, it's important to contact your healthcare provider for further guidance.
In rare cases, dulaglutide can cause more serious side effects such as pancreatitis, a condition characterized by inflammation of the pancreas. Symptoms of pancreatitis may include severe abdominal pain, nausea, vomiting, and fever. If you experience these symptoms while taking dulaglutide, it's important to seek immediate medical attention.

Common side effects of Depakote. Depakote, also known as divalproex sodium, is used to manage various medical conditions. While it can be an effective treatment, it is important to be aware of the potential side effects. Some of the most common side effects associated with Depakote include:.
Nausea and gastrointestinal discomfort:. Many individuals experience mild to moderate nausea when starting Depakote. This side effect typically subsides after a few weeks as the body adjusts to the medication. It is recommended to take Depakote with food to reduce the likelihood of stomach upset.
Dizziness and drowsiness:. Depakote can cause dizziness and drowsiness, especially in the initial stages of treatment. It is important to be cautious when engaging in activities that require mental alertness, such as driving or operating machinery. If these side effects persist or worsen, it is advisable to consult with your healthcare provider.
Weight gain:. Weight gain is another common side effect of Depakote. This can be particularly concerning for individuals who already struggle with weight management. It is important to maintain a healthy lifestyle and discuss any concerns about weight gain with your healthcare provider. They may be able to recommend strategies to mitigate this side effect.
It is essential to note that not everyone will experience these side effects, and their severity can vary from person to person. If you are starting Depakote and notice any side effects, it is crucial to inform your healthcare provider for proper guidance.
Serious side effects and risks of Depakote.
While most individuals tolerate Depakote well, there are some potential serious side effects and risks that need to be considered. It is important to be aware of these risks and discuss them thoroughly with your healthcare provider before starting treatment. Some of the more serious side effects and risks associated with Depakote include:.
Liver damage:. Depakote has the potential to cause liver damage, including liver failure. Symptoms of liver damage may include jaundice (yellowing of the skin and eyes), abdominal pain, dark urine, and unusual fatigue. Regular liver function tests are typically recommended while on Depakote to monitor liver health.
Pancreatitis:. Depakote usage has been associated with an increased risk of pancreatitis, which is inflammation of the pancreas. Symptoms of pancreatitis may include severe abdominal pain, nausea, vomiting, and fever. If you experience these symptoms, it is essential to seek immediate medical attention.
Birth defects:. Depakote has been linked to an increased risk of birth defects if taken during pregnancy. The risk of birth defects is highest when Depakote is taken during the first trimester. It is crucial for women of childbearing age to discuss the potential risks with their healthcare provider and explore alternative treatment options if planning to become pregnant.
These are just a few examples of the serious side effects and risks associated with Depakote. It is important to remember that the benefits of the medication may outweigh the risks for some individuals. However, it is crucial to have open and honest discussions with your healthcare provider to make an informed decision about treatment.

Sacubitril / Valsartan (neprilysin inhibitor)
Sacubitril / Valsartan (neprilysin inhibitor) Side Effects 1 Views • 1 month ago

Sacubitril/valsartan is a medication that combines two active ingredients, sacubitril and valsartan, and is primarily used to treat heart failure. It's sold under the brand name Entresto and has been approved by various regulatory agencies, including the U.S. Food and Drug Administration (FDA) and the European Medicines Agency (EMA), for the treatment of chronic heart failure in certain patients.

Here's some information about each of the components and how they work together:

1. **Sacubitril**: Sacubitril is an angiotensin receptor-neprilysin inhibitor (ARNI). Neprilysin is an enzyme responsible for breaking down certain hormones, including natriuretic peptides, which play a role in promoting vasodilation (relaxation of blood vessels) and reducing fluid retention. By inhibiting neprilysin, sacubitril helps increase the levels of these beneficial hormones, leading to improved cardiovascular function.

2. **Valsartan**: Valsartan is an angiotensin II receptor blocker (ARB). Angiotensin II is a hormone that causes blood vessels to constrict and promotes the release of another hormone, aldosterone, which leads to fluid retention. Valsartan blocks the receptors that angiotensin II binds to, thus preventing its effects on blood vessels and reducing aldosterone release. This results in vasodilation and decreased fluid retention, which are beneficial for heart failure patients.

The combination of sacubitril and valsartan in a single medication like sacubitril/valsartan (Entresto) provides a dual mechanism of action. By simultaneously inhibiting neprilysin and blocking angiotensin II receptors, this medication helps improve the heart's pumping ability, reduce fluid accumulation, and alleviate the symptoms of heart failure.

Entresto is typically prescribed for patients with chronic heart failure with reduced ejection fraction (HFrEF). Ejection fraction is a measure of the heart's ability to pump blood. HFrEF indicates that the heart's pumping function is impaired, and this condition can lead to symptoms like shortness of breath, fatigue, and fluid retention.

It's important to note that sacubitril/valsartan can have side effects, including low blood pressure, elevated levels of potassium in the blood (hyperkalemia), and an increased risk of angioedema (a swelling of the deeper layers of the skin). Therefore, it's prescribed under the guidance of a healthcare professional who will monitor the patient's response and adjust the dosage as needed.

As with any medication, individuals should consult their healthcare provider before starting sacubitril/valsartan to ensure that it's appropriate for their specific condition and medical history.

Drug Cards Daily | Episode #33: valsartan (Diovan) | Treating NSTEMI, STEMI, Heart Failure and Hypertension

Valsartan, also known as Diovan, is an angiotensin II receptor blocker (ARB). It is used for treating NYHA class II to IV heart failure along with both treating and managing hypertension. Another common indication is for both STEMI and NSTEMI patients. Typical initiation dosing for heart failure, STEMI and NSTEMI is 20 mg twice daily with titrations up to 160 mg twice daily. When initiating treatment for hypertension the range begins at 80-160 mg po every day up to a max dose of 320 mg per day. Valsartan works by blocking AT2 from the AT1 receptor and is considered to be more efficient than ACEs along with having less side effects such a cough. Some common side effects are orthostatic hypotension, dizziness, and lightheadedness. Common monitoring parameters for valsartan are blood pressure, blood pressure urea, pregnancy, and electrolytes. Patients should avoid salt substitutes containing potassium. There is a black box warning for fetal toxicity because drugs that affect the renin-angiotensin system can cause injury/death to the fetus.
