Digoxin
Generic Name: digoxin
Trade Name: Lanoxin
Indication: CHF, A-fib, A-flutter Action Positive inotropic effect (increases force of myocardial contraction), prolongs refractory period, ↓ conduction through SA and AV nodes. Essentially digoxin is given to increase cardiac output and slow the rate.
Therapeutic Class: antiarrhythmic, inotropics
Pharmacologic Class: digitalis glycosides
Nursing Considerations:
Excreted by kidneys
Assess patient for hypersensitivity
Contraindicated with uncontrolled ventricular arryhtmias
Hypokalemia increase risk for toxcicity
Hypercalcemia ↑ risk for toxicity
Use caution with diuretic use as they may cause electrolyte abnormalities that can lead to toxicity
Assess patient for cardiac arrythmias including bradycardia
Signs of toxicity include vision changes (blurred vision, yellow, green vision disturbances)
Monitor pulse rate for 1 full minute prior to dosing patient (hold for pulse less than 60)

Cardiac glycosides like Digoxin are positive inotropic drugs that help give the heart a more forceful contraction, or in other words, they help increase the contractility of the heart muscle (the myocardium). Digoxin and other cardiac glycosides like Milrinone, help to increase cardiac output (C.O.), fancy words for helping the heart to pump out more oxygen rich blood to the body.

Calcium channel blocker like Nifedipine, Diltiazem, Verapamil are used to lower both blood pressure and heart rate for patient experiencing hypertension like in heart failure, and this is the main reason that CCB's are put in the class of antihypertensive drugs, since they decrease blood pressure. Calcium channel blockers are medications used to treat high blood pressure aka hypertension. Nurses, nursing students, and healthcare providers need to understand how these medications work in order to monitor and teach patients about their impact and side effects.

Why do you gain weight with antidepressants and mood stabilizers?
In this video I discuss two way. One way is by blocking histamine 1 receptors. We usually think of histamine as something that is associated with allergies and your immune response. If you’re exposed to something your body is sensitive to, you get a histamine response that can look like itching, runny nose or even shortness of breath. And if you take an antihistamine like Benadryl, it slows the allergic reaction and you feel better. That's how it works from the neck down. Histamine acts differently in your brain. In certain parts of your brain, it’s responsible for wakefulness or your ability to be alert. That's why if you take an anti-histamine it may reduce your allergic reaction, but if it crosses the blood brain barrier, it can also make you sleepy because it's blocking histamine’s stimulating effect in your brain. There are four histamine receptors, I’m going to talk about H1 receptors.
A second mechanism is blocking the 5HT 2c receptors. This is just a type of serotonin receptor. It’s been shown in mice that if you activate their 5HT2c receptors, they lose weight because they eat less. And if you block the receptor they became obese. Many of the newer antipsychotic medications block this receptor. Of all the antipsychotic medications, the worst offenders when it comes to weight gain are olanzapine, clozapine and quetiapine. And all three of these medications block 5HT2c.
In this video I also discuss what you can do to offset the weight gain.

Divalproex sodium is a 1:1 combination of valproic acid and sodium valproate.
It is well known under the brand Depakote. It is available as extended release or delayed release tablets. It releases valproic acid in to the body that produces antiepileptic action. Divalproex is slowly absorbed in to the body and it produces consistent plasma levels. It has less gastric side effects compared with valproic acid alone. However, it has somewhat less bioavailability that the pure acid.

Drugs: Docusate
Mechanism of action: Decrease surface tension of stool which allows mixing of water with stool. This makes the stool soft which is easy to pass It takes 1-3 days to work.
Use: prophylaxis of constipation and decrease straining at defecation.
Adverse events: nausea, cramp, abdominal pain, hepatotoxicity on prolonged use.

Presently, Alzheimer's Disease is incurable. However, the U.S. Food and Drug Administration (FDA) has approved six (6) prescription medications for treating some of the memory-loss symptoms associated with cognitive impairment and dementia.
These pharmaceuticals fall into three (3) broad categories.
Firstly, there are acetylcholinesterase inhibitors (AChEI). These are designed to boost the brain's levels of the neurotransmitter acetylcholine by hindering the body's ability to break it down. These drugs include donepezil (Aricept), galantamine (Razadyne/Reminyl), rivastigmine (Exelon), and the now-obsolete tacrine (Cognex).

Secondly, come the NMDA-receptor antagonists (NMDARA). These have to do with the funciton of the N-methyl-D-aspartate receptor and the neurotransmitter glutamate. At the present time, the main offering, here, is memantine (Namenda).
