Dr. Josef Witt-Doerring, a board-certified psychiatrist specializing in identifying and treating psychiatric adverse drug reactions, interviews Marchello and his mother Lisa. Marchello developed akathisia after starting psychiatric medications. Dr. Witt-Doerring brings a unique perspective from his experience as a Medical Officer at the FDA, where he analyzed adverse drug reactions and proposed modifications to drug labeling.
In this interview, Marchello recounts his journey to the psychiatrist’s office, his reaction to the medication, and how the subsequent mismanaged care led him to lose trust in the medical system. Later, Marchello and Lisa share invaluable insights into the caregiver-drug-injured person relationship, offering advice for other loved ones navigating similar challenges.

#dustasteride #Avodart #benignprostatichyperplasia
What is Dutasteride?
Dustasteride is a 5 alpha reductase enzyme inhibitor which is used to treat Benign Prostate Hyperplasia in men. Benign Prostate Hyperplasia is a non-cancerous enlargement of the prostate gland, which causes urinary symptoms such as
- the feeling your bladder being full all the time, even after passing urine
- the feeling of urgency to pass urine
- the need to pass urine often
- a weak urine flow
- leaking or dribbling and more
It can be very frustrating .
Benign Prostate Hyperplasia is very common as men age.
How does it work?
Prostate growth is caused by a hormone called dihydrotestosterone (DHT). Dutasteride lowers DHT production in the body, leading to shrinkage of the enlarged prostate thus reducing the urinary problems.
Before you start dutasteride, there are a few things you need to tell your doctor
1. if You are allergic to anything
2. if you suffer from any liver diseases or any other medical conditions.
3. if You are planning to have surgery or an anaesthetic.
more in video
Now, dutasteride is found in men's semen, and if exposed to a woman who is pregnant with a male baby, it may cause the male baby to be born with abnormal genital organs so Make sure Women (who are pregnant or may be pregnant) and children must not touch your dutasteride. Wash the affected area immediately with soap and water if there is any contact with the skin.
So the Frequently asked question, can I have sex while on dutasteride. you may, but you must not expose your partner with your semen, and you must use a condom.
Dutasteride can stay in the blood for a long time. You must not donate blood until 6 months after you've stopped taking dutasteride.
How to take dutasteride:
Your doctor will tell you how much to take. That will be dependent on your condition and also what other medication you are taking. DutasΩteride normally comes in capsule form. It must be swallowed whole and not be to chewed or opened otherwise it can make your mouth and throat sore. Take the medicine at the same time everyday, so you remember. Set an alarm to remind yourself if needed. Prostate becomes larger over a long period of time, So reducing the size of your prostate so your symptoms improve may take time as well, so if you are being prescribed this medication. take it as long as your doctor tells you to. Make sure you always have enough medication to last you over the weekends and holidays.
If you forget to take it, If it is almost time to take your next dose, skip the missed dose and take your next dose at the usual time. Otherwise, take it as soon as you remember and then go back to taking your medicine as you would normally. Do not take a double dose to make up for missed doses.
Go to your doctor regularly for a check-up, Your doctor may occasionally need to do tests to make sure that the medicine is working and to prevent side effects. Remember to Tell any other doctors, dentists and pharmacists who are treating you that you take this medicine.
Side Effects:
All medications come with side effects. It is always a risk-to-benefit ratio that the health professional is weighing when they prescribe you a treatment. Not everyone will experience side effects. So do not be alarmed by it. Side effects information It is there, so you know what to do if you do experience it. I will list some here, you can find more side effects information in the leaflet that your pharmacist hands you.
Let your doctor know, If you experience any of the following side effects,
- inability to achieve or maintain an erection
- decrease sex drive
- difficulty with ejaculation.
- breast swelling or tenderness.
If you experience an anaphylactic reaction to dutasteride where your face, mouth, throat or tongue swells or you have trouble breathing, send yourself to the hospital immediately.

Dutasteride or finasteride? Which is better? We have a detailed, point by point comparison coming right up. And in the end, we’re gonna give you our verdict. There is a winner, and you’ll find out who it is in today’s video.

Transcription:
Today we’ll be comparing finasteride to dutasteride across various domains, like their efficacy and side effects.
We’ll give you the winner in each area.
And in the end we’ll give you our verdict on the overall winner.
So let’s get into it.
Mechanism of action.
So both finasteride and dutasteride block DHT.
And they do this in a similar way: by inhibiting the enzyme that converts testosterone into DHT.
This enzyme is called five-alpha-reductase.
By inhibiting this enzyme, our body is unable to convert the circulating testosterone into DHT, and the levels of DHT drop.
Dramatically.
So both drugs are so-called 5-alpha-reductase inhibitors.
But there is a big difference between them.
There are versions of the 5-alpha-reductase enzyme.
These are simply called type 1 and type 2. They have a slightly different chemical structure but exactly the same function: they both convert testosterone to DHT.
Now finasteride only inhibits the type 2 enzyme.
Whereas dutasteride inhibits the type 1 and type 2.
And because our body makes DHT from both enzymes, dutasteride results in a near-total suppression of DHT levels, over 90%.
This compares to around 70% reduction from finasteride.
So, clearly, the winner here is dutasteride, right?
Well, actually no.
You see, it’s only the DHT made from the type 2 enzyme that causes hair loss.
We know this from a rare genetic mutation that makes some people lack a functional type 2 enzyme.
These people still have DHT in their system from the type 1 enzyme, but they’re immune to hair loss.
So even though dutasteride is the most efficient DHT blocker of the two, you judge a medication by its selectiveness, not by its potency.
If it’s only the type 2 enzyme that causes hair loss, the superior medication is the one that only blocks the type 2 enzyme, not both.
The more targeted the medication, and the less it interferes with other aspects of our biochemistry, the better.
WINNER: FINASTERIDE
Efficacy
So both drugs are efficient at blocking the progression of hair loss, at least for the majority of users.
There is so much research at this point that this is beyond doubt.
But which of the two is better? With which one are you more likely to regrow hair?
Now guys I want to be clear: the differences between the two drugs aren’t that large.

An interesting study was done on 500 patients with the flu that compared echinacea and the drug Tamiflu. Tamiflu is mainly given to people with the flu or to someone who has been exposed to the flu but hasn’t shown symptoms yet.
The study found that echinacea had the same efficacy as Tamiflu in decreasing flu symptoms, with reduced risk of complications. Always check with your doctor before you try any natural remedy.
Side effects of Tamiflu:
• Nausea
• Vomiting
• Diarrhea
• Dizziness
• Headaches
• Nose bleeds
• Insomnia
• Respiratory problems
• The flu
Potential benefits of echinacea:
• It has antiviral effects
• It may improve cold symptoms (sneezing and sinus pressure)
• It has anti-inflammatory effects (especially when using the roots)
• It has anti-migraine effects
• It may enhance the immune system (increases monocytes and stimulates neutrophils and natural killer cells)

A person struggling with the side effects of Effexor or experiencing a lack of efficacy of the medication may not have been informed of safer alternatives. It is well-established that SNRI antidepressants like Effexor can cause side effects that outweigh the benefits of continuing to take them.
Common Side Effects During Venlafaxine Titration
Venlafaxine is associated with many side effects which can start to emerge during the period of time taking the drug.
These can be found and explored more extensively in several informative articles that can be found on our site. But here, we want to focus on the side effects when a person begins to withdraw from venlafaxine. These reactions can emerge when a person either misses a dose, including being late by a few hours, or when a person is trying to withdraw from venlafaxine, even when venlafaxine tapering is being done gradually. It is often the sudden change that is associated with intense withdrawals during venlafaxine tapering. Venlafaxine tapering should be done under medical oversight and very gradually.
Each person is a unique individual, and naturally, some people will react to withdrawal from venlafaxine differently than others. However, there are some symptoms that appear to be statistically common to many, when large numbers of consumers are surveyed. These include a sensation often referred to as “brain zaps,” shivers, or electric shocks through the head area. Commonly, tinnitus (ringing in the ears) and dizziness accompany the brain zaps. These sensations can be quite severe and as they can come on randomly, suddenly, and can persist for a time, brain zaps can be very unsettling.
The withdrawal effects that occur when stopping a serotonin-norepinephrine re-uptake inhibiting drug like Effexor can be daunting, and one should never stop taking this drug abruptly. However, with guidance, a person can gradually come off the drug safely and may benefit by transitioning to Effexor alternatives. Based on our research and direct clinical experience, we know other possibilities exist besides living a lifetime medicated on antidepressant drugs.
Do you wish to start living life in a way that resembles your desired self without Effexor side effects?
In many cases, Effexor may not adequately treat symptoms. Effexor (venlafaxine) has a very short half-life, approximately 5 hours. This means a late or missed dose can cause grievous problems, resulting in time lost at work, injuries and accidents, and other negative consequences. As mentioned above, serious harm to health can occur because of potentially life-threatening Effexor side effects that many persons experience.4 For all of these reasons, one may begin to consider safe and effective Effexor alternatives for improvements in mental wellness.
The bottom line is if this antidepressant medication is not working as it should or if you are experiencing unwanted side effects, talk with your doctor, and talk to us, about alternatives to Effexor.
