Trade Name: Vasotec Indication hypertension, management of CHF

Actio:n block conversion of angiotensin I to angiotensin II, increases renin levels and decreases aldosterone leading to vasodilation

Therapeutic Class: antihypertensives

Pharmacologic Class: ACE Inhibitor

Nursing Considerations
 can cause neutropenia - check WBCs regularly
 use cautiously with potassium supplements and potassium sparing diuretics.
 use cautiously with diuretic therapy
 administer 1 hour before meals
 monitor blood pressure often
 monitor weight and fluid status
 monitor renal profile
 monitor CBC frequently
 dry cough

ENALAPRIL MALEATE TABLETS contains the active substance enalapril maleate.

‘ENALAPRIL’ belongs to a group of medicines known as ACE inhibitors (Angiotensin-Converting Enzyme Inhibitors). ‘ENALAPRIL’ works by causing your blood vessels to widen. This helps your blood pressure to fall. It also makes it easier for your heart to pump blood around your body.

The medicine usually starts to work within an hour, and the effect lasts for at least 24 hours. Some people will require several weeks of treatment until the best effect on their blood pressure is seen.

Do not take ‘ENALAPRIL’ if:

You are allergic to enalapril maleate, any other ACE inhibitors (a type of medicines similar to ‘ENALAPRIL’), or any of the other ingredients of this medicine.
Any member of your family has had an allergic reaction to these medicines or you have ever had swelling of your face, eyelids, lips, mouth, tongue, or throat which caused difficulty in swallowing or breathing (angioedema) when the reason why was not known or it was inherited.
You have diabetes or impaired kidney function and are treated with a blood pressure-lowering medicine containing aliskiren.
You are more than 3 months pregnant. (it is also better to avoid ‘ENALAPRIL’ in early pregnancy.
If you are being treated with sacubitril/valsartan, a medicine for heart failure.
Do not take ‘ENALAPRIL’ if any of the above apply to you. If you are not sure about taking ‘ENALAPRIL’, talk to your doctor.

Jonathan Spergel, MD, PhD, is Professor of Pediatrics at the University of Pennsylvania Perelman School of Medicine and Chief of the Allergy Section and Director of the Center for Pediatric Eosinophilic Disorders at Children’s Hospital of Philadelphia. Dr. Spergel is a fellow in the American Academy of Allergy, Asthma and Immunology, the American College of Allergy, Asthma and Immunology, and the American Association of Pediatrics. An expert in food allergy, atopic dermatitis, and eosinophilic esophagitis, he has published over 130 articles in the field. Action:
Bacteriostatic: suppresses bacterial protein synthesis

Therapeutic Class: anti-infective

Pharmacologic Class: macrolide

Nursing Considerations:
 Causes QT prolongation, ventricular arrhythmias
 diarrhea
 asses infection
 monitor liver function tests
 instruct patient to finish medication dosage even if they are feeling better
 medication should not be shared

Macrolides are drugs recognizable by their common ending of “-thromycin”, including erythromycin, azithromycin, and clarithromycin. Macrolides are antibiotics and can be used clinically to treat a wide spectrum of bacterial infections. Macrolides have the potential to cause QT prolongation, which could lead to the potentially fatal torsades de pointes, so keep an eye on the patient’s cardiac rhythm. Another side effect of taking macrolides is GI distress. Macrolides are hepatotoxic, meaning they can cause liver damage. Closely monitor the patient’s liver enzymes while they are taking the drug.

Escitalopram is commonly known for its brand name Lexapro or Cipralax and there are many brands out there in the market. It is a selective serotonin reuptake inhibitors (SSRIs) used to treat depression and generalized anxiety disorder

How to take escitalopram?

Follow the doctor's instructions carefully. The usual dose of Escitalopram is 10mg daily for adults.  Your doctor may start you initially on a lower dose and increase the dose up to a maximum of 20mg daily. (Dependent on your condition, I have seen cases where higher doses are given) If you have liver problems, the maximum recommended dose is 10mg a day.

Take escitalopram at roughly the same time each day. if it makes you sleepy, take it at night time before you go to bed, if it makes you alert take it in the day...more in the video

Do not stop taking Escitalopram or lower the dosage, without checking with your doctor. Do not let yourself run out of medicine over the weekend or on holidays. Continue to take citalopram even if you feel well. If you suddenly stop taking citalopram, you may experience withdrawal symptoms such as mood changes, irritability, more in the video...

When your doctor and you decide that it's time to stop the medication, your doctor and pharmacist will guide you into slowly stopping the medication. more in the video...

Like all medication, it comes with side effects. When a doctor decides to prescribe you a medication, he or she deems that the benefit outweighs the side effects. You can discuss with your doctor the benefit and the risk of taking escitalopram.

Escitalopram can take 1-6 weeks before you notice its full benefit, and before that, you may experience some side effects like dry mouth, nausea, more in the video...

Talk to your doctor immediately
- if you have thoughts of harming yourself or ending your life.
- unusual excitement

Call the ambulance or go to the emergency department of the hospital if develop an anaphylactic allergic reaction.

The above are not the only side effects of the medications, I will put a link down below so you have more information about it. Your pharmacist will give you more information in regards to this when you pick up your medication. Now Side effects that are being stated there, doesn't mean that you are going to get it. Many people do not experience them. You need to speak to your doctor to weigh out the risk and benefit of treating your condition. 

Notes of precautionary:
Escitalopram can interact with many other medications, check with your pharmacist before taking any medication or supplements including those you buy without a script in the supermarket.

One very important thing to take note of is anti-depressants acts as a crutch to help you overcome things a little easier. While you are on this medication, it's crucial that you work towards self-help,  work towards changing the pattern you think, changing the way you communicate with yourself, so when you are off it, you are able to stand on your own.  Listen to motivational speakers on topics of depressions, how to better yourself in life. Talk to your doctor about psychotherapy, go for counseling sessions, meditation, etc.
