All about OXYBUTYNIN medicine: oxybutynin dosage (5 mg tablet, patch), oxybutynin uses, oxybutynin side effects, oxybutynin contraindications, oxybutynin interactions, oxybutynin mechanism of action (moa).

The brand names of Oxybutinin are: Ditropan, Ditropan XL, Dresplan, Nefryl, Tavor, Oxytrol
Mechanism of action of Oxybutinin:
Acetylcholine competitive antagonist at the post-ganglionic muscle receptor level, resulting in relaxation of the smooth muscles of the bladder.
Therapeutic Indications and Dosage of Oxybutynin:
Oral route:
Adults: 5 mg 3 times a day. In case of nocturnal urination disorders: 15 mg per day in a single dose at the end of the day. Increase to a maximum of 5 mg 4 times a day to obtain a clinical response, if side effects are well tolerated. After the initial control, a reduced maintenance dose can be introduced.
Children over 5 years old: 5 mg 2 times a day (spaced at least 4 hours apart).
Via Transdermal. Patch:
apply one patch to abdomen, hips or buttocks 2 times a week (every 3 or 4 days), do not apply 2 patches in the same place (1 patch contains 36 mg of oxybutynin, releases a nominal rate of 3.9 mg every 24 hours).

Oxybutynin belongs to the group of medicines called anti-spasmodic, and anti-cholinergic agents used to treat overactive bladder (OAB) with symptoms of urinary incontinence (urine leakage), urgent or frequent urination, increased night-time urination, dysuria (painful or difficult urination). Additionally, Oxybutynin is used to treat night-time bed-wetting in children above 5 years when other treatments have not worked.
Oxybutynin contains 'Oxybutynin', which works by blocking the release of a chemical transmitter called acetylcholine, responsible for bladder muscle contractions. Thereby relaxing the smooth muscles of the urinary bladder and stopping sudden muscle spasms (contractions). This helps control urine by delaying the initial desire to empty the bladder and increases the urine holding capacity of the bladder.
You are advised to take Oxybutynin for as long as your doctor has prescribed it for you, depending on your medical condition. In some cases, you may experience certain common side-effects such as dizziness, sleepiness, headache, facial flushing, constipation, dry mouth, dry skin, urinary retention, and diarrhoea.

In this video, we’re going to talk about breathing. Deep breathing does not increase blood oxygen levels. In fact, it can cause dizziness, make you tired, and even make you pass out.
Your red blood cells will not release oxygen unless enough carbon dioxide (CO2) is present. This is known as the Bohr effect. This means that the amount of oxygen released from your blood into your cells is dependent upon CO2.
If you’re breathing a lot or hyperventilating, you’ll get too much oxygen and not enough CO2. Too much oxygen can be dangerous to the body. If you give a baby too much oxygen, it can cause blindness, an epileptic seizure, or a coma. Too much oxygen also causes oxidation.
People in a panicked state experience acute hyperventilation. Chronic hyperventilation, also known as carbon dioxide syndrome, is a lack of carbon dioxide that occurs when a person is consuming too much air. This is often triggered by stress.
Mouth-breathing can deplete CO2 and inhibit oxygen absorption at the cellular level. This can affect the heart and cause hypoxia, fatigue, insomnia, muscle twitching, and increased histamine.
To fix this problem, you need to train your body to breathe properly to get oxygen deep into the cells by doing the following:
1. Relax your breathing.
Take soft, gentle breaths through your diaphragm and stomach, not your upper chest. Breathe in slowly for 5 seconds and exhale for 5 seconds. Practice this before you sleep at night, while driving, or while watching TV.
This simple breathing technique can help your body get out of fight-or-flight mode. You can even use a free app to help you time your breathing!
2. Breathe through your nose.
Your nose cleans, humidifies, and filters the air before it enters your lungs. Nose breathing increases your oxygen by 20%!
3. Use nose strips.
Nose strips can help open your airways and sinuses while you sleep to help you breathe better.
4. Use mouth tape.
Mouth tape can help you breathe through your nose while you sleep. You can filter the air all night, get more oxygen, and sleep better. You won’t wake up with a dry mouth, and you’ll wake up feeling refreshed. Mouth tape might even stop your snoring!

If you have any travel plans in the middle of your hyperbaric protocol, there are absolutely some things you need to know about when you can or cannot fly in an airplane in and around those sessions in hyperbaric medicine.
This whole conversation about flying after HBOT is all about nitrogen, specifically about the possibility of getting decompression illness (the bends).

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses the tapering or "deprescribing" of Psychiatric Medications including SSRIs (lexapro), SNRIs (effexor), Benzodiazepines (klonopin, xanax) and even buprenorphine. Patients who have taken medication for years, and failed at tapering or stopping these medications due to withdrawal ("brain zaps," muscle twitches, headaches, dizziness, anxiety, insomnia) may benefit from treatment with a provider specifically trained in tapering these medications. Additionally, Rittenhouse Psychiatric Associates providers can work with local compounding pharmacies to create doses in-between manufacturer made doses of these medications, to make the process of tapering or deprescribing as easy as possible. Sometimes when a patient undergoes this process, they find that the medication actually IS very helpful for their mood, anxiety or other symptoms, but they may not need the full dose that they have taken for years. This is NOT a failure, and can result in a decrease in side effects (and improved quality of life). Patients who have felt over-medicated, dull, cognitively slowed, or excessive sleepiness, may find that at a lower dose, they have an improved quality of life, and still have symptom control. Psychiatric medications can be incredibly important for an individual's care, and should never be decreased or stopped without consulting directly with a mental health professional. However, just because someone was started on a medication, or just because they have withdrawal when they try to decrease it, doesn't mean that they need to stay on it "forever."
